Evaluating Correlation between Geometrical Relationship and Dose Difference Caused by Respiratory Motion Using Statistical Analysis

  • Dose differences between three-dimensional (3D) and four-dimensional (4D) doses could be varied according to the geometrical relationship between a planning target volume (PTV) and an organ at risk (OAR). The purpose of this study is to evaluate the correlation between the overlap volume histogram (OVH), which quantitatively shows the geometrical relationship between the PTV and OAR, and the dose differences. 4D computed tomography (4DCT) images were acquired for 10 liver cancer patients. Internal target volume-based treatment planning was performed. A 3D dose was calculated on a reference phase (end-exhalation). A 4D dose was accumulated using deformation vector fields between the reference and other phase images of 4DCT from deformable image registration, and dose differences between the 3D and 4D doses were calculated. An OVH between the PTV and selected OAR (duodenum) was calculated and quantified on the basis of specific overlap volumes that corresponded to 10%, 20%, 30%, 40%, and 50% of the OAR volume overlapped with the expanded PTV. Statistical analysis was performed to verify the correlation with the OVH and dose difference for the OAR. The minimum mean dose difference was 0.50 Gy from case 3, and the maximum mean dose difference was 4.96 Gy from case 2. The calculated range of the correlation coefficients between the OVH and dose difference was from -0.720 to -0.712, and the R-square range for regression analysis was from 0.506 to 0.518 (p-value <0.05). However, when the 10% overlap volume was applied in the six cases that had OVH value ${\leq}2$, the average percent mean dose differences were $34.80{\pm}12.42%$. Cases with quantified OVH values of 2 or more had mean dose differences of $29.16{\pm}11.36%$. In conclusion, no significant statistical correlation was found between the OVH and dose differences. However, it was confirmed that a higher difference between the 3D and 4D doses could occur in cases that have smaller OVH value.

Correlation between body alignment and foot pressure in high School Baseball Players (고등학교 야구선수들의 신체정렬과 족저압 분포 간의 상관관계 분석)

  • This study was undertaken to provide correct posture guidance and reference material for sports injury and injury prevention programs, by analyzing body alignment and plantar pressure distribution in high school baseball players. Totally, 32 subjects were enrolled for the study. Body alignment was measured from the trunk imbalance, kyphotic angle, lordotic angle, pelvic tilt, and pelvic torsion. Foot pressure was measured from the left maximum pressure, right maximum pressure, left weight, and right weight. The mean and standard deviation of the measured factors were calculated and tabulated, and the correlation between body alignment and foot pressure distribution was analyzed using Pearson's correlation analysis. All statistical significance levels were set at 0.05. Body alignment and foot pressure distribution show a positive correlation between the lordotic angle of lumbar spine and the left maximum pressure. There was no correlation between trunk imbalance, kyphotic angle, lordotic angle, pelvic tilt, pelvic torsion, and the left·right weight distribution and the left·right maximum pressure of each foot. Results of this study determined that the increase of the lordotic angle of lumbar spine results in more force on one side of the foot, thereby increasing the maximum pressure. We believe our data could be a reference for exercise programs on physical alignment and plantar pressure distribution of athletes.

Changes in Distributive Equity of Health Insurance Contribution Burden (건강보험료 부담의 형평성 변화)

  • Objectives : We analyzed the changes from 1996 to 2002 in distributive equity of the contribution burden in the Korean National Health Insurance. Methods : The study subjects were a total of 8,923 employee households and a total of 7,296 self-employed households over the period from 1996 to 2002. Those were the households meeting the two criteria as completing each annual survey and having no change in the job of head of the household during that period from the raw data of the Household Income and Expenditure Survey annually conducted by the Korean National Statistical Office. The unit of analysis was a household, and this was the standard for assessing the contribution that is now applied on a monthly basis. Deciles Distribution Ratio, Contribution Concentration Curve and Contribution Concentration Index were estimated as the index of inequality. Multiple regression analysis was conducted to compare the annual ability-to-pay elasticity of the contribution to the reference year of 1996 for three groups (all households, the employee households, and the self-employed households). Results : For the index of inequality, the distributive equity of contribution was improved in all three groups. In particular, the employee group experienced a substantial improvement. Using multiple regression analysis, the ability-to-pay elasticity of the contribution in the employee group significantly increased ($\beta$=0.232, p<0.0001) in the year 2002 as compared to the reference year of 1996. The elasticity in the self-employed group also significantly increased ($\beta$=0.186, p<0.05), although its change was smaller than that in the employee group. Conclusions : The employee group had a greater improvement for the distributive equity of the contribution burden than the self-employed group. Within the observation period, there were two important integration reforms: one was the integration of 227 self-employed societies in 1998 and the other was the integration of 139 employee societies in 2000. We expected that the equity of the contribution burden would be improved for the self-employed group since the integration reform of 1998. However, it was not improved for the self-employed group until the year 2000. This result suggests that capturing exactly the beneficiaries' ability-to-pay such as income is the precedent for distributive equity of the contribution burden, although a more sophisticated imposition standard of contribution is needed.

A Study on the Cooperative Program Planning and Teaching (학교도서관의 협동교수프로그램에 관한 연구)

  • Cooperative program planning and teaching is a strategy for developing and implementing resource based learning. This approach combines the classroom teacher's subject expertise and knowledge of the students with the teacher­librarian's specialized knowledge of the availability and use of learning materials. And the purpose of cooperative program planning is to develop learning experiences or units of study that effectively integrate the student's resource center activities with other learning experiences. There are also some conditions that are conducive to effective planning sessions. First, the teacher-librarian must be prepared to initiate planning with teachers, rather than waiting for teachers to come to them. Second, the teacher-librarian must be prepared to present suggestions in such a way that the teacher can respond. The purpose of this study is (1) to investigate the planning process of cooperative program planning and teaching in the previous studies and (2) to find general problems when the cooperative program planning and teaching apply in actual situation. For these purposes, I chose a school library and a teacher-librarian in Seoul to observe how this cooperative program planning and teaching carry out in the school library. Main findings are summarized as follows: (1) A teacher who teaches alone for one grade is more proper to carry out the cooperative program planning and teaching. Young teachers are usually more active to change their teaching methods. (2) The cooperative program planning and teaching is a program what needs a lot of materials. When there is no right reference book in a school library, it would be good to access DB through PC telecommunication. It is also possile to contact lirarians who work in large public libraries or university libraries. (3) The cooerative program planning and teaching needs cooperative working between a teacher-librarian and a teacher. Thus a teacher-librarian should be in his school in the day time like the other teachers for the program's planning process. ( 4) There has to be a guide to change into resource based learning in teaching method. Thus the Korean Library Association or Korean Library and Information Science Society should offer a seminar or a workshop about cooperative program planning and teaching for the teacher-librarians. (5) It needs a system that a teacher-librarian can know about student's assignments so that he can prepare reference books for them. (6) The school library can be able to offer excellent service like cooperative program planning and teaching to the teachers and students according to a teacher-librarian's enthusiasm.

AGAR-ALGINATE COMBINED IMPRESSION TECHNIQUE AND DIMENSIONAL CHANGE RESULTING FROM IDOPHOR DISINFECTION (아기-알지네이트 연합 인상체의 보관방법 및 idophor 침적소독에 의한 체적 변화에 관한 연구)

  • Statement of problem : Because dental impression can be a cross-infection source, disinfection, such as immersion disinfection is nessecary. However, the disinfection process may change the volume of the impression cast. Purpose : The purpose of this study is to find out the effects on dimensional change of different storage times and methods, of agar-alginate impression when immersed in a 5% idophor disinfectant. Material and Methods : An agar-alginate impression was made from a mandible model and then was disinfected and stored according the experimental conditions and a stone model was produced. Measurements were taken between reference points on the original mandible model and they were compared to measurements taken between reference points on the stone model. The study was divided into 4 groups. In group 1, the impression was stored in a 100% humidor for 30 minutes, 1 hour, 2 hours, 3 hours, 4 hours, 6 hours, 12 hours and 24 hours and stone models were made at each time. In group 2, the impression was immersed in 5% idophor disinfectant and then stored in a 100% humidor for the same length of times as group 1, and stone models were made at each time. In group 3, the impression was stored in extend-A-pourR, a special storage solution for 30 minutes, 1 hour, 2 hours, 3 hours, 4 hours, 6 hours, 12 hours, 24 hours 3 days, and 7 days and stone models were made at each time. In group 4, the impression was immersed in 5% idophor disinfectant and then stored in stored in extend-A-pourR, a special storage solution for the same length of times as group 3, and stone models were made at each time. 5 impressions and stone models were made at each time to make a total of 180. The Student-t test (P < .05) was used to do a statistical analysis of the measurements of the mandible model and stone models. The repeated-measure 2-way analysis of variance (P < .05) was used to do a statistical analysis of the difference in the 4 groups. Results : The percent liner dimensional change was from $0.25{\pm}0.03%$ (group 1, 30 minutes) to $0.34{\pm}0.06%$ (group 4, 7 days). No significant change was noticed between the 4 groups. Conclusion : According to the above study, in both methods, least dimensional change was recorded when the storage times were short and in both methods, immersion in 5% idophor disinfectant did not effect dimensional change in the agar-alginate impression.

Characteristic Evaluation of Exposed Dose with NORM added Consumer Product based on ICRP Reference Phantom (ICRP 기준팬텀 기반의 천연방사성핵종이 포함된 가공제품 사용으로 인한 피폭선량 특성 평가)

  • In Korea, July 2012, the law as called 'Act on Safety Control of Radioactive Rays Around Living Environment' was implemented to control the consumer product containing Naturally Occurring Radioactive Material (NORM), but, there are no appropriate database and effective dose calculation system. The aim of this study was to develop evaluation technique of the exposure dose with the use of the consumer products containing NORM and to understand the characteristics of the exposed dose according to the radiation type and energy. For the evaluate of exposure dose, the ICRP reference phantom was simulated by the MCNPX code based on Monte Carlo method, and the minimum, medium, maximum energy of alphas, betas, gammas from the representative NORM of Uranium decay series were used as the source term in the simulation. The annual effective doses were calculated by the exposure scenario of the consumer product usage time and position. Short range of the alpha and beta rays are mostly delivered the dose to the skin. On the other hand, the gamma rays mostly delivered the similar dose to all of the organs. The results of the annual effective dose with $1Bq{\cdot}g^{-1}$ radioactive stone-bed and 10% radioactive concentration were employed with the usage time of 7 hours 50 minute per day, the maximum annual effective dose of alphas, betas, gammas were calculated 0.0222, 0.0836, $0.0101mSv{\cdot}y^{-1}$, respectively.

  • This paper presents a 6~18 GHz 7-bit digital-controlled attenuator. The proposed attenuator is based on switched-T architecture, but no inductor is used for minimum chip size. The designed attenuator was fabricated using $0.18{\mu}m$ CMOS process, and characterized using on-wafer testing setup. The resolution(minimum attenuation step) and the maximum attenuation range of the attenuator were measured to be 0.22 dB and 28 dB, respectively. The measured RMS attenuation error and the RMS phase error for 6~18 GHz were less than 0.26 dB and $3.2^{\circ}$, respectively. The reference state insertion loss was less than 12.4 dB at 6~18 GHz. The measured input and output return losses were better than 9.4 dB over all frequencies and attenuation states. The chip size is $0.11mm^2$ excluding pads.

Comparison of Upper Extremity Muscle Activity With Transverse Plane Angle Changes During Vertical Keyboard Typing (타이핑 작업 시 수직형 키보드의 수평면 끼인각 변화에 따른 상지의 근활성도 비교)

  • In order to prevent upper extremity musculoskeletal disorders, effective keyboard selection is an important consideration. The aim of this study was to compare upper extremity muscle activity according to transverse plane angle changes during vertical keyboard typing. Sixteen healthy men were recruited. All subjects had a similar typing ability (rate of more than 300 keystrokes per minute) and biacromion and forearm-fingertip lengths. Four different types of keyboard (vertical keyboard with a transverse plane angle of $60^{\circ}$, $96^{\circ}$, or $120^{\circ}$, and a standard keyboard) were used with a wrist support. The test order was selected randomly for each subject. Surface electromyography (EMG) was used to measure upper extremity muscle activity during a keyboard typing task. The collected EMG data were normalized using the reference contraction and expressed as a percentage of the reference voluntary contraction (%RVC). In order to analyze the differences in EMG data, a repeated one-way analysis of variance, with a significance level of .05, was used. Bonferroni correction was used for multiple comparisons. There were significant differences in the EMG amplitude of all seven muscles (upper trapezius, middle deltoid, anterior deltoid, extensor carpi radialis, extensor carpi ulnaris, flexor carpi radialis, and flexor carpi ulnaris) assessed during the keyboard typing task. The mean activity of each muscle had a tendency to increase as the transverse plane angle increased. The mean activity recorded during all vertical keyboard typing was lower than that recorded during standard keyboard typing. There was no significant difference in accuracy and error scores; however, there was a significant difference between transverse plane angles of $60^{\circ}$ and $120^{\circ}$ with regard to comfort. In conclusion, a vertical keyboard with a transverse plane angle of $60^{\circ}$ would be effective in reducing muscle activity compared with vertical keyboards with other transverse plane angles.

Bioequivalence of Neuracetam Tablet to Neuromed Tablet (Oxiracetam 800 mg) (뉴로메드정(옥시라세탐 800 mg)에 대한 뉴라세탐정의 생물학적동등성)

  • The purpose of the present study was designed to evaluate the bioequivalence of two oxiracetam tablets, Neuromed tablet (Korea Drug Co., reference drug) and Neuracetam tablet (Sam Jin Pharmaceutical Co., test drug), according to the guidelines of Korea Food and Drug Administration (KFDA). Release of oxiracetam from the tablet in vitro was tested using KP VIII Apparatus II method with various dissolution media (pH 1.2, 4.0, 6.8 buffer solution and water). Twenty-four healthy volunteers, $23.7\;{\pm}\;2.4$ year in age and $68.9\;{\pm}\;6.2$ kg in body weight, were divided into two groups and a randomized $2{\times}2$ cross-over study was performed. After oral administration of a tablet containing 800 mg of oxiracetam, blood samples were taken at predetermined time intervals and concentrations of oxiracetam in plasma were determined using HPLC-MS-MS. The dissolution profiles of two formulations were very similar at all dissolution media. In addition, pharmacokinetic parameters such as $AUC_t$, $C_{max}$ and $T_{max}$ were calculated and ANOVA test was utilized for the statistical analysis of the parameters using logarithmically transformed $AUC_t$ and $C_{max}$ untransformed $T_{max}$. The results showed that the differences between two formulations based on the reference drug were 0.42%, 0.45% and -12.58% for $AUC_t$, $C_{max}$ and $T_{max}$, respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals for the log transformed data were within the acceptance range of log 0.8 to log 1.25 (e.g., $log0.94{\sim}log1.06$ and $log0.90{\sim}log1.07$ for $AUC_t$, and $C_{max}$, respectively), indicating that Neuracetam tablet is bioequivalent to Neuromed tablet. The major pharmacokinetic parameters, $AUC_t$, and $C_{max}$ met the criteria set by KFDA for bioequivalence indicating that Neuracetam tablet is bioequivalent to Neuromed tablet.

  • Lovastatin is a lipid lowering agent for the treatment of hypercholesterolemia and belongs to a new class of pharmacologic compounds called the 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase inhibitors. By competitively inhibiting HMG CoA reductase, lovastatin disrupts the biosynthesis of cholesterol in hepatic and peripheral cells and increases the synthesis of high-density-lipoprotein HDL) receptors. Following oral administration, the lactone ring of lovastatin is hydrolysed to the active inhibitor of HMG CoA reductase, lovastatin acid. Lovastatin is known to have poor oral absorption and wide individual variation. In this study, bioequivalence test of two lovastatin formulations, the test drug ($Lovaload^{TM}$, Chong Kun Dang Pharmaceutical Co.) and the reference drug ($Mevacor^{TM}$, Chung Wae Pharmaceutical Co.) were conducted according to the guidelines of Korea Food and Drug Administration (KFDA). A total of 18 healthy male volunteers, $31.90\pm3.60$ years old and $72.17\;7.88$ kg of body weight in average, were evaluated in a randomized crossover manner with a 2-week washout period. Concentrations of lovastatin acid in plasma were measured upto 12 hours following a single oral administration of eight tablets (20 mg of lovastatin per tablet) by high-performance liquid chromatography with UV detection at 238 nm. The area under the concentration-vs-time curve from 0 to 12 hours $(AUC_{0-12h})$ was calculated by the trapezoidal summation method. The statistical analysis showed that there are no significant differences in $AUC_{0-12h),\;C_{max}\;and\;T_{max}$ between the two formulations ($6.72\%,\;1.52\%,\;and\;0.88\$, respectively). The least significant differences between the formulations at $\alpha$=0.05 were less than $20\%\;(11.65\%,\;19.73\%,\;and\;14.81\%\;for\;AUC_{0-12h},\;C_{max}\;and\;T_{max}$, respectively). The $90\%$ confidence intervals for these parameters were also within $\pm20\%\;(-1.50{\leq}{\delta}{\leq}15.00$, $-12.50{\leq}{\delta}{\leq}15.50,\;and\;-9.64{\leq}{\delta]{\leq}11.40{\leq}\;for\;\;AUC_{0-12h}$ ,$C_{max}\;and\;T_{max}$, respectively). In conclusion, the new generic product $Lovaload^{TM}$ was proven to be bioequivalent with the reference drug.
