서울.경기지역 신샌아의 수면/활동 양상에 관한 연구 (A Study on the Sleep pattern of normal neonates in Seoul and Kyungki area)

  • The sleep is the most important problem for new parents. However, there were very rare the study about sleep of normal neonates and early infants in Korea. So we attempted pilot survey study of neonates' sleep pattern. The purpose of this study was to investigate the sleep pattern of normal neonates in Seoul and Kyungki area and to provide for basis for nursing intervention for parents of them. The subjects of this study were 37 normal full-term neonates of 2~3weeks of age who were visited by researcher or assistant at postpartum care unit, health care clinic, outpatient clinic located in Seoul and Kyungki area from April 20, 1999 to March 15, 2000. This study was descriptive survey study under the convenient sampling method. The instrument of this study was the NCASA translated by Korean Parent Child Health Academic Association. The collected data were coded as 1/4hour unit, and analyzed by MS Office Excel program. The result of this study were as followed: 1. The mean amount of total sleep was 15.1hours. The mean amount of daytime sleep was 9.5hours. The mean amount of nighttime sleep was 5.7 hours. The mean amount of longest sleep was 2.9hours. The mean amount of shortest sleep was 0.6hour. 2. The mean amount of total activity was 8.9hours. The mean amount of daytime activity was 6.5 hours. The mean amount of nighttime activity was 2.3hours. The frequency of feeding was 10.4 times a day. The frequency of daytime feeding was 7.3 times. The frequency of nighttime feeding was 3.1 times. It is suggested that various subjects and more data according to age, and comparative study among different cultures are needed for further study in the future.

의료기관과 시장특성이 간호사 확보수준에 미치는 영향 (The Effects of Institutional and Market Factors on Nurse Staffing in Acute Care Hospitals)

  • Nurse staffing level is an important factor that influences the quality of health service and patient outcomes. This study was carried out to examine the current state of acute hospital nurse staffing and find out factors that affect the nurse staffing level. Nurse staffing of individual hospitals was measured using the number of registered nurses per 100 beds. Descriptive and multiple regression analyses were conducted using 592 acute care hospitals' data. Regression model included structure factors such as referral level, ownership, medical and general staffing, and financial outcome factors such as occupancy rate, inpatient and outpatient revenues. Market characteristics included strength of competition, supply of nurses, and income and health status level of consumers. The average number of nurses per 100 beds was 28 and showed a great variation according to the referral level. Regression model explained this variation as much as 76.87%. Hospital structure variables which affecting the hospital nurse staffing level positively were ICU bed ratio, the staffing level of specialist, training doctor and employees except doctor and nursing personnel, while the negative factor was nurse aid staffing level. General hospitals employed more nurses than hospitals. Among outcome characteristics, occupancy rate and the amount of health insurance inpatient revenue affected positively on the hospital nurse staffing level. The more supply of the new nurse and the higher consumer income and health status in the medical service markets, the more nurses were employed by the medical institutes. According to the study result, hospitals employed more nurses when they had more financial incentive by increasing nurses. This means appropriate hospital incentive policy and regulation policy, which hospital violate nurse staffing level have to pay penality, should be needed. Clarifying job description between nurses and nurse aids and the reentry program for unemployed experienced nurses will be helpful to increase nurse staffing level.

Epidemiological Trends of GI Cancers in Patients Visiting a Tertiary Care Hospital in Chandigarh, North India

  • Background: Cancer has become an epidemic disease. Nearly ten million new cancer cases are diagnosed annually in the world and out of these about half are from the developing world. To appropriately plan for treatment, management and prevention of the disease, it becomes necessary to study the trends about morbidity caused by cancers. Materials and Methods: Data for patients diagnosed with any form of gastrointestinal (GI) cancers was extracted from records maintained in the outpatient department registers of the Oncology Department of Government Medical College and Hospital in Chandigarh from 1999 to 2012. Trends were analysed for different categories of GI cancers for the period of 12 years. Results: In present study GI cancers accounted for 23 % of all registered cases (n-9603) of carcinomas. Males predominated for all GI cancers except in the gall bladder. Gastrointestinal cancers as a proportion of total cancers increased from 21% in 1999 to 25.9% in 2012 with a significant increasing trend in our series (${\chi}^2$ for linear trend=9.36, p<0.003). Cancers of the tonsil, oral cavity and pharynx taken together showed an increasing trend over the years (${\chi}^2$ for trend=55.2, p<0.001) whereas cancers of the lower GI (${\chi}^2=19.6$, p<0.0001) and gall bladder (${\chi}^2=19.5$, p<0.0001) showed a declining trend in our series. Conclusions: GI cancers form a significant proportion of all cancers reporting to our data. In depth studies to ascertain the reasons for the changing trends are required to design intervention programs. Further information is necessary from cancer registries and from the hospital records of oncology departments.

한방소아·청소년과 외래 환자 주소증의 최근 경향에 대한 연구 (A Study on the Recent Trend of Chief Complaint of Korean Pediatric and Adolescent Outpatients)

  • Objectives The purpose of this study is to investigate about chief complaints of Korean pediatrics and adolescent outpatients in the ${\bigcirc}{\bigcirc}$ university hospital for their recent trend. Methods The study was composed of 4,677 new patients aged between 1 month and 20 years who had been visited pediatrics in ${\bigcirc}{\bigcirc}$ university hospital from 2012 January to 2015 December. Results 1. Among those patients, majority was early childhood as 51.4%, followed by early childhood, late childhood, infancy, and adolescence. 2. Chief complaints in this population have been recorded in the fall the most as 29.6%, but the distribution was inconsistent. 3. To categorize, digestive-related chief complaints were the highest as 30.6%, followed by respiratory, supplemental use, growth, psychiatrics. Respiratory-related chief complaints increased dramatically in the fall. 4. In infancy and early childhood, digestive/respiratory complaints were common, whereas, growth complaints were common in late childhood and adolescence. 5. Digestive/respiratory complaints have been over 50% out of all chief complaints annually. Psychiatric and growth-related complaints have been trending up, whereas, supplemental/skin complaints have decreased every year. 6. Specifically, anorexia were the most common in digestive complaints, and rhinitis/sinusitis were top among respiratory complaints. Precocious puberty and developmental disability increased every year. Conclusions The most common chief complaints were digestive and respiratory-related, and precocious puberty and growth issues have been trending up lately. Based on this result, more studies that targets precocious puberty and growth problems are needed.

경희의료원 한방소아.청소년과 외래 환자의 주소증에 대한 연구(2009년 3월~2010년 2월) (A Study of the Chief Complaint of Pediatric Outpatients in the Kyung Hee Oriental Medicine Hospital)

  • Objectives: The purpose of this study is investigate about chief complaints of pediatric outpatients in the oriental medicine hospital and search for a latest of patients and their symptom. Methods: The study was composed of 3024 new patients aged between 0 and 20 who had been visited pediatrics in Kyung Hee oriental medicine hospital from 2009 March to 2010 February. Results: In age distribution, 1 year old child have most visited, and the number of the patients have fallen sharply as age have increased. The most visited month is August, and the least is November. In systemic division of chief complaints, in order of respiratory part, growth part, restorative herb medicine part, digestive part, neurologic & psychologic part, dermatologic part visited in patients number, and in single chief complaints, in order of growth, restorative herb medicine, anorexia, rhinosinusitis, cold, atopic dermatitis, seizure & epilepsy have visited. As for age and systemic division, between 1 and 6 years old, the patients of respiratory division have most visited, more than 7 years old, growth division. As for age and single chief complaints, in 0 year old, seizure & epilepsy have most visited, between 4 and 6 years old, rhinosinusitis, more than 7 years old, growth. Growth and respiratory division have increased the most in August, restorative herb medicine and digestive division have increased the most in September. In single chief complaints, patients of growth have most visited in most of time, but in September and November patients of restorative herb medicine have visited the most. Conclusions: The most visited division was respiratory part, and the most visited chief complaints was growth. More data of patients should be accumulated for further study, and we expect more specific study would be done about many kinds of disease in basis of this study.

2015년 국민보건계정과 경상의료비 (2015 National Health Accounts and Current Health Expenditures in Korea)

  • Background: This paper aims to demonstrate current health expenditure (CHE) and National Health Accounts of the years 2015 constructed according to the SHA2011, which is a new manual of System of Health Accounts (SHA) that was published jointly by the Organization for Economic Cooperation and Development (OECD), Eurostat, and World Health Organization in 2011. Comparison is made with international trends by collecting and analysing health accounts of OECD member countries. Particularly, financing public-private mix is parsed in depth using SHA data of both HF as financing schemes as well as FS (financing source) as their revenue types. Methods: Data sources such as Health Insurance Review and Assessment Service's publications of both motor insurance and drugs are newly used to construct the 2015 National Health Accounts. In the case of private financing, an estimation of total expenditures for revenues by provider groups is made from the Economic Census data; and the household income and expenditure survey, Korean healthcare panel study, etc. are used to allocate those totals into functional classifications. Results: CHE was 115.2 trillion won in 2015, which accounts for 7.4 percent of Korea's gross domestic product. It was a big increase of 9.3 trillion won, 8.8 percent, from the previous year. Government and compulsory schemes's share (or public share) of 56.4% of the CHE in 2015 was much lower than the OECD average of 72.6%. 'Transfers from government domestic revenue' share of total revenue of HF was 17.8% in Korea, lower than the other contribution-based countries. When it comes to 'compulsory contributory health financing schemes,' 'Transfers from government domestic revenue' share of 14.9% was again much lower compared to Japan (44.7%) and Belgium (34.8%) as contribution-based countries. Conclusion: Considering relatively lower public financing share in the inpatient care as well as overall low public financing share of total CHE, priorities in health insurance coverage need to be repositioned among inpatient care, outpatient care and drugs.

모유수유 어머니의 양육 스트레스와 남편의 지지가 모유수유 적응에 미치는 영향 (Impact of Parenting Stress and Husband's Support on Breastfeeding Adaptation among Breastfeeding Mothers)

  • Purpose: This study investigated the relationships among parenting stress, husband's support, and breastfeeding adaptation in mothers. Methods: A correlational survey design was utilized in this study. Data were collected from 181 breastfeeding mothers with infants under 12 months of age. Participants were recruited from the outpatient clinic, a child photo studio, or from the kid's cafe from July 22 to September 13, 2016. Instruments for collecting data included Childcare Stress Inventory (CSI), Husband's Support scale, and the Breastfeeding Adaptation scale. Data were analyzed using SPSS 21.0 by frequency, descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient and multiple regression. Results: Of the 181 subjects, 54.7% responded they only breastfed with 75.7% who received breastfeeding education. Seventy-two percent of mothers made a decision to breastfeed by themselves. The mean score was $2.39{\pm}0.53$ for parenting stress, $3.07{\pm}0.43$ for husband's support, and $3.85{\pm}0.49$ for breastfeeding adaptation. Factors influencing breastfeeding adaptation were identified as lower parenting stress (${\beta}=-.54$), greater husband's support (${\beta}=.31$), breastfeeding (${\beta}=.31$), and older age of baby (${\beta}=.14$), with 56.0% that explained variance. Conclusion: This study reports that parenting stress has negative effect on breastfeeding adaptation, while husband's support has positive effect. Results may be useful in developing new strategies for enhancing breastfeeding adaptation.

간호일터영성이 병원간호사의 직무만족, 소진 및 이직의도에 미치는 영향 (Effects of Nursing Workplace Spirituality on Job Satisfaction, Burnout and Turnover Intention among General Hospital Nurses)

  • Purpose: Nursing workplace spirituality (NWS) has received attention as a new and meaningful subject for nursing to consider, but little is known about the relation of NWS to nursing. The purpose of this study was to identify the effect of NWS on job satisfaction, burnout and turnover intention. Methods: Participants were 145 clinical nurses, who had worked for over 6 months in one of four general hospitals in B city. Data were collected using self-reported questionnaires and analyzed using descriptive statistics, independent t-test, ANOVA, Pearson coefficient and hierarchical multiple regression. Results: In the second hierarchy controlling general characteristics, significant predictors of job satisfaction were burnout and NWS, which explained 45.0% of the variance. NWS had more influence on job satisfaction than burn out, and the model was suitable. NWS showed no statistically significant effect on burnout and turnover intention, when general characteristics and job related factors were controlled. Conclusion: Findings indicate that nursing workplace spirituality has a positive influence on job satisfaction, but no direct influence on burnout or turnover intention, which may indicate an indirect influence. Nurse managers need to develop the NWS enhancement program and provide them to nurse to improve job satisfaction.

대전.금산 지역 당뇨.고혈압 질환자의 인삼에 대한 인식도 조사 (Recognition of Ginseng by Diebetic and Hypertensive Patients in Daejeon and Geumsan Areas)

  • Research on recognition, preference, and consumption of ginseng was conducted with the goal of enhancing the consumption of ginseng, and commercializing local ginseng dishes in Geumsan. With help from the Eulji University hospitals in Daejeon and Geumsan, a survey was completed by a group of 200 patients, including 50 diabetic and 50 hypertensive patients from each center. Results based on a patient's residence (p<0.001), and illness (p<0.001), showed that the percentage of outpatient treatments was very high at the center in Geumsan. Based on a patient's residence (p<0.001) and illness, a preference for ginseng was very high in Geumsan. Based on a patient's residence (p<0.01) and illness (p<0.05), the willingness to try new ginseng dishes was stronger at the center in Geumsan compared to the center at Daejeon. Based on residence and illness (p<0.001), hypertensive patients were more enthusiastic towards taking ginseng herbs. Based on residence (p<0.001) and illness (p<0.001), percipience concerning the effects of ginseng was more positive in Geumsan. Based on residence (p<0.001) and illness, the frequency of ginseng herb intake was higher in Geumsan. The research provides the basis for establishing changes in the perception of ginseng, providing accurate information, and promoting ginseng in local economies.

골관절염을 가진 여성의 균형감, 유연성, 심폐기능 및 근력에 대한 타이치 운동과 자조관리 프로그램의 효과비교 (Effects of Tai Chi or Self-help Program on Balance, Flexibility, Oxygen Consumption, and Muscle Strength in Women with Osteoarthritis)

  • Purpose: This randomized study was done to compare the effects of a 6 month Tai Chi exercise and a self-help program on balance, flexibility and muscle strength in women with osteoarthritis (OA). Method: In this experimental study, 82 women with OA recruited from outpatient clinics or community health centers were randomly assigned either to a Tai Chi group or a self-help group. Thirty women (mean age = 62 years) in the Tai Chi group and 39 (mean age = 59 years) in the self-help group completed posttest measures (balance, flexibility, oxygen consumption, abdominal muscle strength, back muscle strength, and grip strength) at 6 months. Results: After the 6 month, Tai Chi participants had significantly greater balance (mean difference = 2.9 vs. 0.9 for the self-help), grip strength (mean difference = 4.6 vs. 0.9 for the self-help), and back muscle strength (mean difference = 4.1 vs. -0.3 for the self-help). However, flexibility, oxygen consumption, and abdominal muscle strength were not significantly different between the groups. Conclusion: Tai Chi increased balance, grip strength and back muscle strength in older women with osteoarthritis compared to the self-help program. Whether these changes improve physical functioning and fall prevention requires further study.
