진정법을 시행하는 치과의사는 반드시 발생할 수 있는 응급상황에 적절히 대처할 수 있는 능력과 환경을 갖추어야 한다. 2010년 치의학회에서 발간한 진정법 가이드라인에서 진정법을 준비하고 책임지는 치과의사는 기본생명구조술과 전문심장구조술 자격을 유지할 것을 요구하고 있다. 국내에서는 대한심폐소생협회가 2004년부터 기본생명구조술 및 전문심장구조술을 위한 Provider, Instructor 양성 과정을 진행하고 있다. 저자는2014년에 이 과정들을 이수하면서 과정에 대한 소개와 경험한 내용을 공유하고자 하였다. 기존 교육이 응급의료를 담당하는 사람들에게 초점이 맞추어 졌기에, 향후 치과의사의 특수성에 보다 적합한 교육 과정 개발이 필요하다고 사료된다.
심미는 시대적 관심과 트렌드에 따라 영향을 받아왔다고 여겨지며 그에 따라 요구되어지는 장비와 소재의 한계 또한 있는데 심미증례는 또한 이를 극복해야 할 필요가 있다고 생각한다. 그리하여 환자들의 요구와 환경에 맞춘 다음의 증례를 소개하고자 하며 아날로그에서 행해져온 것들을 이어가면서 디지털이 더욱 발전하기를 희망하며 최신의 기술들을 사용함으로써 현재의 한계에 도전하는 것 그 자체로 의의가 있다고 생각한다. 디지털은 여러 방면에서 장점들이 있는데 특히 디지털 커뮤니케이션에 관련하여 관심이 있으며 구강내 인상 장치와 소프트웨어를 통해서 치과의사와 기공사간의 보철물에 관한 의사소통이 보다 직접적으로 가능해졌으며 이제는 디지털 커뮤니케이션을 통해서 보철과 심미의 새로운 시대를 열것이라 기대한다. 이번 증례를 통해서 다음의 2가지 요소를 통해서 직접적이고 실제적인 디지털 커뮤니케이션 과정을 소개 하고자 한다.
맹출 중인 치아는 오랜 기간 동안 이동하는 과정에서 주위의 여러 요인들, 즉 유치의 조기 상실, 과잉치, 국소적 병소, 혀, 입술, 저작근, 외상 등에 의해서 다양한 영향을 받을 수 있다. 이러한 여러 요인들이 작용할 경우 치아의 정상적인 맹출 과정이 방해를 받게 되어 치아의 맹출 장애를 초래할 수 있다. 본 연구에서는 서울대학교 치과병원 소아치과에 2004년 1월 2일부터 2005년 12월 31일까지 신환으로 내원한 환자들, 총 8,010명의 주소를 분석하였고, 맹출 장애를 보이는 환자는 전체의 8.79%를 보였다. 맹출 장애의 유형은 맹출 지연이나 조기 맹출, 이소 맹출로 분류할 수 있었고, 맹출 지연이 가장 많이 나타난 치아는 상악 중절치이며, 조기 맹출이 호발한 치아는 하악 유중절치이고, 이소 맹출이 가장 많이 나타난 치아는 상악 제1대구치였다.
Objectives: By introduced reciprocation motion file in dentistry, dentists benefit simple canal shaping procedure and time-saving. But, reciprocation motion generates uncomfortable vibration to doctors and patients. Because there was no study about this consideration, this study compared vibration pattern and power generated from reciprocation motion motor and conventional rotary motor. Materials & Methods: One conventional rotary motor; X-Smart (Dentsply Maillefer, Ballaigues, Switzerland); and two reciprocating motors; WaveOne Motor (Dentsply Maillefer, Ballaigues, Switzerland) and X-SMART PLUS (Dentsply Maillefer, Ballaigues, Switzerland); were used in this study. Triaxial $ICP^{(R)}$ Accelerometer (Model 356A12, PCB piezotronics, New York, USA) was attached on motor's handpiece head, and was measured tri-axial vibratory acceleration with NI Sound and Vibration Assistant 2009 software (National Instruments, Texas, USA). Mean vibratory acceleration and maximum vibratory acceleration was measured on fixed position and handed position. The results of vibratory acceleration were statistically analyzed using ANOVA and multiple comparisons are made using Turkey's test at p<0.05 level. Results: Reciprocating motors showed higher mean vibratory acceleration and maximum vibratory acceleration than conventional rotary motor (p<0.05). Between reciprocating motors, X-SMART PLUS had lower mean vibratory acceleration and maximum vibratory acceleration than WaveOne Motor (p<0.05). Conclusion: Reciprocating motors generate more vibration than conventional rotary motor. Further study about effect of vibration to dentist and patient is needed. And it seems to be necessary to make a standard about vibration level in endodontic motors.
Purpose : The purpose of this study was to survey the present status of the regional oral health centers for the disabled. Material and methods : Face to face interviews with oral healthcare professionals in eight regional oral health centers and literature reviews were conducted among eight regional oral health centers for the disabled during 2, July to 30, August 2017. Results : It is necessary to establish more regional oral health centers for the disabled in area where the number of disabled person as percentage of whole region population is high. Also, all the centers appeared a lack of professional manpower including dentist, dental hygienist and anesthesiologist, equipment and facilities for the demands of patients and their guardians. It is essential for the government to improve the support system to regional oral health centers for the disabled. Conclusions : This study can be used as a reference for the countermeasure to make a new policy and activate the accessibility of dental care services in the public for the disabled.
Purpose: This study aimed to compare the accuracy of 3-dimensional(3D) printed models derived from multidetector computed tomography (MDCT) and cone-beam computed tomography (CBCT) systems with different fields of view (FOVs). Materials and Methods: Five human dry mandibles were used to assess the accuracy of reconstructions of anatomical landmarks, bone defects, and intra-socket dimensions by 3D printers. The measurements were made on dry mandibles using a digital caliper (gold standard). The mandibles then underwent MDCT imaging. In addition, CBCT images were obtained using Cranex 3D and NewTom 3G scanners with 2 different FOVs. The images were transferred to two 3D printers, and the digital light processing (DLP) and fused deposition modeling (FDM) techniques were used to fabricate the 3D models, respectively. The same measurements were also made on the fabricated prototypes. The values measured on the 3D models were compared with the actual values, and the differences were analyzed using the paired t-test. Results: The landmarks measured on prototypes fabricated using the FDM and DLP techniques based on all 4 imaging systems showed differences from the gold standard. No significant differences were noted between the FDM and DLP techniques. Conclusion: The 3D printers were reliable systems for maxillofacial reconstruction. In this study, scanners with smaller voxels had the highest precision, and the DLP printer showed higher accuracy in reconstructing the maxillofacial landmarks. It seemed that 3D reconstructions of the anterior region were overestimated, while the reconstructions of intra-socket dimensions and implant holes were slightly underestimated.
The purpose of this study is to provide the general history of fostering dentists in Japan and introducing their new roles. This research was conducted based on the government policy report on dentists and the information published by each educational institution. Based on the collected data, the official websites were used to represent the latest statistics of the institutions. The number of dentists in Japan has increased. The government established the National Examination for Dentists to guarantee the quality of dentists. After the standards for developing questions for the national examination were established in 1985, the contents of the examination have been appropriately improved by revising the standards every four years. This improvement has required dental students to study a variety of subjects for six years at dental school. Since dentists in Japan are required to respond to various demands from the nation; the Model Core Curriculum for Dental Education was developed to teach medical ethics and abilities to ensure that dentists conduct themselves professionally. Recently, the roles of dentists have been changing in Japan. When providing dental services to older patients over the age of 65, dentists and other dental professions focus on maintaining oral functions, such as saliva secretion, bite force, tongue movement, and masticatory/swallowing functions. However, oral function-related services for children are different. In addition to providing essential dental services, dental practitioners also provide special treatment, such as oral muscle training, myofunctional therapy, health guidance, and space retainers to the child patients with developmental insufficiency in oral functions. Dentistry in Japan has undergone numerous changes over the years and has continued to offer high-quality dental health services. Thus, information gained from the Japanese experience may be helpful to dental professions in other developed countries for planning oral health measures.

This study aimed to assess the competency of artificial intelligence (AI) in pediatric dentistry and compare it with that of dentists. We used open-source data obtained from the Korea Health Personnel Licensing Examination Institute. A total of 32 item multiple-choice pediatric dentistry exam questions were included. Two AI-based chatbots (ChatGPT 3.5 and Gemini) were evaluated. Each chatbot received the same questions seven times in separate chat sessions initiated on April 25, 2024. The accuracy was assessed by measuring the percentage of correct answers, and consistency was evaluated using Cronbach's alpha coefficient. Both ChatGPT 3.5 and Gemini demonstrated similar accuracy, with no significant differences observed between them. However, neither chatbot achieved the minimum passing score set by the Pediatric Dentistry National Examination. However, both chatbots exhibited acceptable consistency in their responses. Within the limits of this study, both AI-based chatbots did not sufficiently answer the pediatric dentistry exam questions. This finding suggests that pediatric dentists should be aware of the advantages and limitations of this new tool and effectively utilize it to promote patient health.
본 연구는 초기우식 진단장비인 QLF-D와 새로운 치아우식활성 검사법인 Cariview를 우식예측 연구에 활용할 수 있는지를 평가하기 위하여, 만 3세 아동 57명을 대상으로 구강검사를 실시한 후 우식경험도를 분석하고 이들 간의 상관성을 평가하여 다음과 같은 결과를 얻었다. 1. 연구대상자의 우식경험도 조사 결과, 우식경험유치지수는 평균 1.84였으며, QLF-D를 이용한 분석 결과는 치면 세균막 부착에 대한 점수가 1.47이었고, 연구대상자 당 평균 1개 정도의 초기우식병소($0.96{\pm}1.21$)가 있는 것으로 나타났다. 또한 Cariview 검사에서 산생성능은 평균 27.52로 나타났고, 연구대상자 중 68%가 저위험군에 속하는 것으로 파악되었다. 2. 우식경험유치지수와 초기우식병소 개수의 상관계수가 0.617, 치면세균막 부착정도(SPS)와는 상관계수가 0.500으로 비교적 강한 상관관계를 확인할 수 있었고(p<0.01), 우식경험유치지수와 Cariview 결과(0.286) 간에는 유의한 양의 상관관계가 나타났다(p<0.05). 그러나 우식유치지수, 우식경험충전유치지수에서는 Cariview 점수와의 상관관계를 확인할 수 없었다. 이상의 결과를 종합해보면, QLF-D를 이용하여 분석한 치면세균막 점수와 초기우식병소는 우식예측에 가장 중요한 영향을 주는 과거 우식경험에 높은 상관성을 보였으므로, 초기우식병소 탐지와 치면세균막 부착 정도를 동시에 파악할 수 있는 QLF-D는 우식예측 연구에 유용한 장비로 활용할 수 있을 것이다.
상악 전치부 임플란트 같이 심미성이 요구되는 부위의 보철수복은 임플란트 몸체의 식립각도에 영향을 많이 받는다. 순측으로 식립된 임플란트의 각도 때문에, 비심미적인 보철물의 결과가 예상되는 경우 치료에 앞서 환자와 임플란트 보철수복의 한계점을 논의하는 것은 매우 중요하다. 환자에게 치료의 한계점을 시각화하여 보여주기 위해 얼굴스캔 데이터와, CT 파일, 그리고 구내스캔을 중첩하여 디지털진단왁스업을 시행하여 환자에게 제시하였다. 환자는 순측으로 돌출된 보철물의 디자인에 불만족하였다. 환자의 의견을 반영하기 위해 3D 데이터를 이용하여 사고로 인해 이미 근관치료된 인접치를 포함한 새로운 디자인을 제시하고, 기공실팀과 함께 환자의 의견이 반영된 보철물을 제작하였다. 환자는 자신의 의견이 반영된 보철물에 심미적 및 기능적으로 만족하였다.
