Association of Sasang Constitution with Bone Mineral Density in Postmenopausal Korean Women (폐경 후 한국 여성에서 사상체질과 골밀도의 연관성 분석)

  • Purpose: To evaluate the association of sasang constitution with bone mineral density (BMD) in postmenopausal Korean women. Methods: Sasang constitution was analyzed by Phonetic System for Sasang Constitution in 92 postmenopausal Korean. The BMD was checked using dual-energy x-ray absorptiometer(DEXA) at lumbar spine, femur neck. Results: Bone mineral density of lumbar spine(T-score) according to Sasang constitution was Taeumin($-1.12{\pm}1.26$), Soyangin($-1.30{\pm}1.06$), Soeumin($-1.51{\pm}1.21$). Bone mineral density of femur neck(T-score) according to Sasang constitution was Taeumin($-0.76{\pm}1.07$), Soyangin($-0.92{\pm}0.81$), Soeumin($-0.84{\pm}0.86$). Conclusion: There was significant relationship between sasang constitution and BMD of femur neck. There was no significant relationship between sasang constitution and BMD of lumbar spine.

  • This study was developed the metallic plate for fixation in the femur fracture for the orthopedic region and rigid fixation with plates has a firm place in fracture treatment. Most plates can be used for rigid as well as biological and dynamical fracture fixation. The device's designation and sizing has a specific with bending structural stiffness and strength, known meaning that is reliable regardless of the plate by the short type and long type. Short plate have a wrapping of femur and long plate have to preserve a pole of femur. The bending strength of the curved metallic long plate has to evaluate a 11,000N and The bending strength of the curved metallic short plate has to evaluate a 6,525N. The tensile stress through to press a plate is $1573N/m^2\;and\;1539N/m^2$. The device can be used to support Revision case of Hip Implant and to use a case of Hip screw compression of Hip Neck Fracture.

Maternal and lifestyle effect on bone mineral density in Korean children and adolescents aged 8-19 (어머니의 골밀도와 생활습관이 소아청소년의 골밀도에 미치는 영향)

  • Higher bone mineral density (BMD) at a young age, calcium intake, and exercise are important for prevention of osteoporosis later in life. We examined familial effects of BMD between mothers and children and adolescents aged 8-19 in Cheonan, Korea and the relationships between BMD and lifestyle parameters, including: food and nutrient intake and exercise. For daughters and sons, significant differences in BMD were observed at the three bone sites (total femur, femur neck, and lumbar spine) according to age, gender, body mass index, exercise, and milk consumption, compared to the reference value for each classification category. Mean differences in children's BMD were observed according to maternal BMD. Energy and calcium intake were lower in both children and mothers in comparison to the estimated daily energy requirement; however, their protein intake was much greater than the daily recommended intake. After adjusting for age and gender and for mother's age, body mass index, and total calorie intake, results of the food frequency test showed an association of a higher intake of meat, meat products, milk and milk products with greater BMD of total femur, femur neck, and lumbar spine of children. In addition, exercise was positively associated with higher BMD. Regression analysis showed a positive association of BMD with age, male gender, exercise, and mother's BMD. In conclusion, after adjustment for environmental parameters, maternal BMD had a positive influence on BMD in daughters and sons. This finding suggests that parents need to check their BMD in order to determine whether their children are at increased risk of low BMD.

Usefulness of Fracture Risk Assessment Tool Using Lumbar Bone Mineral Density in Prediction of Osteoporotic Vertebral Fracture

  • Objective : To investigate the value of lumbar bone mineral density (BMD) in fracture risk assessment (FRAX) tool. Methods : One hundred and ten patients aged over 60 years were enrolled and divided into 2 groups as non-osteoporotic vertebral fracture (OVF) and OVF groups. The 10-year-risk of major osteoporotic vertebral fracture of each group was calculated by FRAX tool with femoral and lumbar spine BMDs to compare the usefulness of lumbar spine BMD in prediction of OVF. The blood level of osteocalcin and C-terminal telopeptide (CTX) as markers of activities of osteoblast and osteoclast, respectively were analyzed using the institutional database. Results : In the OVF group, the ratio of patients with previous fracture history or use of glucocorticoid was higher than those in non-OVF group (p=0.000 and 0.030, respectively). The levels of T-score of femur neck and lumbar spine in OVF group were significantly lower than those in non-OVF group (p=0.001 and 0.000, respectively). The risk of OVF in FRAX using femur BMD in non-OVF and OVF groups was $6.7{\pm}6.13$ and $11.4{\pm}10.06$, respectively (p=0.007). The risk of using lumbar BMD in the 2 groups was $6.9{\pm}8.91$ and $15.1{\pm}15.08$, respectively (p=0.002). The areas under the receiver operator characteristic curve in the FRAX risk with lumbar and femur neck BMD were 0.726 and 0.684, respectively. The comparison of osteocalcin and CTX was not significant (p=0.162 and 0.369, respectively). Conclusion : In our study, the 10-year risk of major osteoporotic fracture in the OVF group of our study was lower than the recommended threshold of intervention for osteoporosis. Hence, a lower threshold for the treatment of osteoporosis may be set for the Korean population to prevent OVF. In the prediction of symptomatic OVF, FRAX tool using lumbar spine BMD may be more useful than that using femur neck BMD.

The Converge Effects of Long-term Weight-bearing Exercise on Lumbar, Femur Neck BMD and Body Compositions in Person with Intellectual Disabilities and Normal Men (지적장애와 일반인 남성의 장기간 체중부하운동 참여가 요추와 대퇴골경부 골밀도 및 신체구성에 미치는 융합적 효과)

  • The purpose of this study was to determined the converge effects of weight-bearing exercise on lumbar, femur neck BMD and body compositions in intellectual disabilities and normal men. Nineteen men were participated for this study, they are divided into two groups(intellectual disabilities, n=9, and normal men, n=10). All subjects are accomplished on weight-bearing exercise (music rope-jumping) program for 60 minutes a day, 3 days a week throughout 12 weeks. Two-way ANOVA with repeated measures was used to determine differences between and within the intellectual and normal groups for dependent variables. The statistically significant level was set at 0.05. In this study, there were a significant differences on the bone mineral density of lumbar and femur neck level after exercise training in both groups (respectively p<.01, p<.001). Also, I found that there were effectively decreased on body weight, percent of body fat, and BMI levels after exercise training for 12 weeks in both groups (respectively p<.001). Therefore, music rope-jump exercise program may suggested to be one of the ideal training methods for enhancing and the convergence positive effect of bone mineral density and body compositions in intellectual and normal person.

  • A 49-year-old male patient with a carcinoma of the right pyriform sinus had a whole-body bone scan and gamma camera based F-18 FDG-PET for staging. Tc-99m MDP bone scan depicted diffuse increased uptake in the left femur due to chronic osteomyelitis but no skeletal metastasis. F-18-FDG-PET revealed increased focal bone uptake and uptake in the draining sinus due to chronic osteomyelitis in addition to visualization of the right pyriform sinus carcinoma and right neck nodal uptake. Fluorine-18 fluorodeoxyglucose-positron emission tomography is significantly more accurate than the bone scan in pinpointing chronic osteomyelitis focus and draining soft tissue infection.

Influence of Impact Angle on Deformation in Proximal Femur during Slide Falling (측방 낙상시의 충격 각도가 대퇴골 근위부의 변형에 미치는 영향)

  • Falling related injuries are categorized as the most serious and common medical problems experienced by the elderly, hip joint fracture, one of the most serious consequences of falling in the elderly, occurs in only about 1% of falling. Nevertheless, hip fracture accounts for a considerable part of the disability, death, and medical costs associated with falling. In this study, we considered the impact angle and displacement rate in falling as another factor affecting femoral strength. Using a fresh-frozen human femur, we developed system to simulate the falling condition and then conducted the experiments changing the impact angle (0$^{\circ}$, 15$^{\circ}$, 30$^{\circ}$) of proximal femur. Also, in order to analyze the relative risk due to falling to normal situation in proximal femur, we did the static test simulating the two-legged stance condition. The results showed that the change in impact angle affected the strain distribution in proximal femur, and that a large deformation in femoral neck than in other sites. Furthermore despite low impact velocity, a large deformation in proximal femur occurred in the impact test and different strain distribution was observed compare to the static case.

  • Bone is a living material with a complex hierarchical structure that gives it remarkable mechanical properties. Bone constantly undergoes mechanical. Its quality and resistance to fracture is constantly changing over time through the process of bone remodeling. Numerical modeling allows the study of the bone mechanical behavior and the prediction of different trauma caused by accidents without expose humans to real tests. The aim of this work is the modeling of the femur fracture under static solicitation to create a numerical model to simulate this element fracture. This modeling will contribute to improve the design of the indoor environment to be better safe for the passengers' transportation means. Results show that vertical loading leads to the femur neck fracture and horizontal loading leads to the fracture of the femur diaphysis. The isotropic consideration of the bone leads to bone fracture by crack propagation but the orthotropic consideration leads to the fragmentation of the bone.

Comparative Study for Osteosynthesis of Femoral Neck Fractures: Cannulated Screws versus Femoral Neck System

  • Purpose: The purpose of this study is to compare the radiological results of fixation using the femoral neck system (FNS) and cannulated screw (CS) for treatment of femoral neck fractures. Materials and Methods: A retrospective study of patients with femoral neck fractures who underwent internal fixation and had follow-up of more than six months from 2010 to 2020 was conducted. A total of 87 patients were enrolled in the study. The FNS group included 20 patients and the CS group included 67 patients. Classification of fractures was performed according to Garden and Pauwels classification. Operation time, intraoperative blood loss, sliding distance of the implant, lateral soft tissue irritation caused by implants, and complications were evaluated. Results: The mean operation time was 40.30 minutes in the FNS group and 46.84 minutes in the CS group. The mean intraoperative bleeding volume was 51.25 mL in the FNS group and 72.16 mL in the CS group. Bone union was achieved in 18 patients in the FNS group (90.0%) and in 61 patients in the CS group (91.0%). The mean sliding distance of the implant was 4.06 mm in the FNS group and 3.92 mm in the CS group. No patients in the FNS group and 12 patients in the CS group complained of soft tissue irritation. Conclusion: A shorter operative time, less intraoperative bleeding, and less irritation of soft tissue were observed in the FNS group. FNS could be an alternative to CS for fixation of femoral neck fractures.
