Since the 1980s, the western welfare states have experienced a wide spectrum of socio-economic changes; changes in population composition, the economic globalization, the post-industrialization, an increasing flexibility in the labor market. etc. This study examines the trend of poverty in welfare states, and analyzes how those socio-economic changes are related to it. For these purposes, this study first calculates the poverty indices for several years in 10 welfare states using the Luxembourg Income Study database, and then decomposes the index by subpopulation and income sources. Major findings of this study can be summarized as follows. First of all, the welfare state in general has experienced an increasing trend in the degree of poverty since the 1980s. In particular, poverty has greatly intensified in the United Kingdom and the Netherlands. Many other welfare states including Canada, Germany, Sweden, and Norway have also experienced substantial increases in poverty. The increasing trend of poverty is not wholly due to changes in population composition such as increases in the aging population and one-parent(mother) families. Contrary to the traditional belief, these population groups are not as much poor as the working-age population. In particular, the degree of poverty in the elderly is less severe than in the working-age group. Furthermore, since the 1980s the market income poverty in the aging population has shown a decreasing trend in many welfare states. The degree and trend of poverty in one-parent families vary greatly across countries, owing to the labor market and income transfer policies. The most important reason for the increasing poverty trend in the welfare state is that the degree of poverty has been deepening in the working-age population. Especially, the market income poverty of the working-age population has considerably increased in every country except the Netherlands. Structural changes in the economy and the labor market may drive the increasing trend of poverty. Further studies and deliberate anti-poverty policies are needed to tackle the factors relating to the increase in the market income poverty.
Objectives The goal of this review is to investigate clinical and experimental studies on external application treatment for atopic dermatitis in Korean literature and to propose for the better method of clinical studies in order to seek more effective treatment. Methods Electronic researches were performed with KTKP, OASIS, National Assembly Library, Korean Medicine Database, KISS, DBpia, and KISTI. Results and Conclusions In Twenty six studies, the numbers of clinical and experimental studies are respectively 10 (38.46%) and 16 (61.54%). The numbers of studies that used herbal complex were 20 (76.92%), and out of that, 6 studies had used a single herbal medicine (23.08%). The external application with oriental medicine for the atopic dermatitis used with Phellodendri Cortex (7), Sophorae Radix (6), Scutellariae Radix (6), Lonicerae Flos (5), Coptidis Rhizoma (5) and so on. Among the clinical studies, the 5 studies were double-blind and randomized-controlled study (50%). The numbers of studies that used Hanifin and Rajka Diagnostic Criteria (1980) were 6 (60%), and that used the Diagnostic Criteria in Korean Atopic Dermatitis (2005) were 4 (40%). Among the clinical studies, categories to evaluate of atopic dermatitis are respectively SCORAD Index (80%), Total IgE (80%), Eosinophil count (70%) and so on. All of the clinical studies (100%) showed a statistically significant decline in atopic dermatitis according to the SCORAD Index, Modified SCORAD Index, the Clinic index score. Among the experimental studies, the numbers of studies that used NC/Nga mice were 9 (56.25%), and out of that 5 studies used BALB/c mice (31.25%). Most of the studies (68.75%) used DNCB as allergy inducing materials. The scales for evaluation of atopic dermatitis were Clinical skin severity score, Histopathologic examination, Immunohematologic examination, safety test and so on. In 12 cases (75%) of experimental studies, the IgE level of experimental group showed a statistically significant decline after using external application. In 8 study cases (50%), Clinical skin severity score of experimental group showed a statistically significant decline after using external application.

Objectives: The study was designed to provide clinical evidence of homogeneity in analysis of the herbal medicine, Guipi decoction in treating insomnia, using a t-test of randomized controlled trials (RCTs). Methods: This study included RCTs that verified homogeneity of Guipi decoction in treating insomnia. Literature studies of CNKI (Chinese National Knowledge Infrastructure Database) were performed. The basic components group was made to the Guipi decoction of the Yixuerumen?s text. The excluded group was created with the number of herbs excluded from the basic components group. T-test performed that the effective rate in the basic components group and excluded groups. Results: The basic components of Guipi decoction contain Atractylodis Rhizoma Alba, Poria Sclerotium (Poria Sclerotium Cum Pini Radix), Polygalae Radix, Astragali Radix, Zizyphi Semen, Angelicae Gigantis Radix, Aucklandiae Radix, Glycyrrhiza uralensis Fischer, Ginseng Radix (Codonopsis Pilosulae Radix), Longanae Arillus. Herbs excluded from the basic components group were Longanae Arillus, Ginseng Radix (Codonopsis Pilosulae Radix), Glycyrrhiza uralensis Fischer and herbs added to the basic components group were Polygonum multiflorum Thunberg, Schisandrae Fructus, Salviae Militiorrhizae Radix, Paeoniae Radix Alba (in order from the most used to the least). T-test revealed the effective rate in the basic components group was not statistically significant compared to the excluded groups (p=0.161/p=0.162/p=0.103). Conclusions: Effectiveness in treatment of basic components of Guipi decoction and excluded groups were revealed to be homogeneous in treating insomnia. As the number of herbs in the basic components group decreased, mean value in effectiveness of treatment also decreased consequentially. However, there were no statistical significances.
Recently, the interest in the orthodontic treatment for children is increased by a rise in national income level. The number of cephalometric radiography that could diagnose a malocclusion and malposition between teeth and jawbone increased. It required attention to radiation exposure, because the subject of dental examination is children which are more sensitive to radiation and the head and neck, the object of that include radiation sensitive organ such as the thyroid, bone marrow, eyes, salivary gland, and so on. In this study, we measured two-dimensional dose distribution in cephalometric radiography system (VATEC Pax-400C) using Agfa CP-G Plus film and MagicMax Dosimeter, and calculated radiation organ dose of head and neck through MCNPX simulation. And then we designed a radiation protective device to decrease radiation dose. The dose distribution of the cephalometric radiography system irradiated the head and neck overall as well as the oral and maxillofacial parts. The radiation organ dose calculated that thyroid, oesophagus and eyes are irradiated high, and the radiation organ dose decreased about 70 ~ 80% by the application of the radiation protective device. The results of this study will be used construction of database for dental radiation exposure and research of reducing radiation dose.

Distal myopathy is a clinically and genetically heterogeneous group of degenerative diseases of the distal muscle. Glycogen storage disease type IXD (GSD9D) is a metabolic distal myopathy characterized by muscle deficiency of phosphorylase kinase, a key regulatory enzyme in glycogen metabolism. Affected individuals may develop muscle weakness, degeneration, and cramps, as well as abnormal muscle pain and stiffness after exercise. It has been reported that mutations in the PHKA1 gene which encodes the alpha subunit of muscle phosphorylase kinase cause GSD9D. In this study, we examined a Korean GSD9D family with a c.3314T>C (p.I1105T) mutation in the PHKA1 gene. This mutation has not been previously reported in any mutation database nor was it found in 500 healthy controls. The mutation region is well conserved in various other species, and in silico analysis predicts that it is likely to be pathogenic. To date, only seven mutations in the PHKA1 gene have been documented, and this is the first report of Korean GSD9D patients. This study also describes and compares the clinical symptoms and pathological conditions of previously reported cases and these Korean patients. We believe that our findings will be useful for the molecular diagnosis of GSD9D.
Background: Previous studies generally indicate that synchronous bilateral breast cancers (SBBC) have an equivalent or moderately poorer survival compared with unilateral cases. The prognostic characteristics of SBBC would be relevant when planning adjuvant therapies and follow-up medical surveillance. The frequency of SBBC among early breast cancers in clinical settings in Australia and New Zealand was investigated, plus their prognostic significance, using the Breast Cancer Audit Database of the Society of Breast Surgeons of Australia and New Zealand, which covered an estimated 60% of early invasive lesions in those countries. Design: Rate ratios (95% confidence limits) of SBBC were investigated among 35,370 female breast cancer cases by age of woman, histology type, grade, tumour diameter, nodal status, lymphatic/vascular invasion and oestrogen receptor status. Univariate and multivariable disease-specific survival analyses were undertaken. Results: 2.3% of cases were found to be SBBC (i.e., diagnoses occurring within 3 months). The figure increased from 1.4% in women less than 40 years to 4.1% in those aged 80 years or more. Disease-specific survivals did not vary by SBBC status (p=0.206). After adjusting for age, histology type, diameter, grade, nodal status, lymphatic/vascular invasion, and oestrogen receptor status, the relative risk of breast cancer death for SBBC was 1.17 (95% CL: 0.91, 1.51). After adjusting for favourable prognostic factors more common in SBBC cases (i.e., histology type, grade, lymphatic/vascular invasion, and oestrogen receptor status), the relative risk of breast cancer death for SBBC was 1.42 (95% CL: 1.10, 1.82). After adjusting for unfavourable prognostic factors more common in SBBC cases (i.e., older age and large tumour diameter), the relative risk of breast cancer death for SBBC was 0.98 (95% CL: 0.76, 1.26). Conclusions: Results confirm previous findings of an equivalent or moderately poorer survival for SBBC but indicate that SBBC status is likely to be an important prognostic indicator for some cases.

This paper proposes the combined image retrieval system that gives the same relevance as exhaustive search method while its performance can be considerably improved. This system is combined with two different retrieval methods and each gives the same results that full exhaustive search method does. Both of them are two-stage method. One uses condensation of feature vectors, and the other uses binary-tree clustering. These two methods extract the candidate images that always include correct answers at the first stage, and then filter out the incorrect images at the second stage. Inasmuch as these methods use equal algorithm, they can get the same result as full exhaustive search. The first method condenses the dimension of feature vectors, and it uses these condensed feature vectors to compute similarity of query and images in database. It can be found that there is an optimal condensation ratio which minimizes the overall retrieval time. The optimal ratio is applied to first stage of this method. Binary-tree clustering method, searching with recursive 2-means clustering, classifies each cluster dynamically with the same radius. For preserving relevance, its range of query has to be compensated at first stage. After candidate clusters were selected, final results are retrieved by computing similarities again at second stage. The proposed method is combined with above two methods. Because they are not dependent on each other, combined retrieval system can make a remarkable progress in performance.
The purpose of this study is to identify key factors of Co-opetition through examining the environmental and structural changes in the ports industry and suggest a few Co-opetition strategies for enhancing our ports capabilities in the long-term. In recent years, the trend toward market liberalization and globalization in ports industry has made rapid progress and it made the competition fiercer. To survive in this rapid changes, the ports have traditionally pursed a competitive strategy. However, an alternative strategy replacing the traditional strategy was introduced: namely, 'Co-opetition' a buzzword combining competition and co operation. Policy proposals this study suggests are the construction of a database regarding costs and benefits of ports for more effective analysis and improving the competitive power of a shipping company operating national flag carriers for achieving the purpose of Co-opetition and adoption of selection and concentration strategy for increasing the possibility of Co-opetition success.
More than 70% of Korean Peninsula is consisted of mountains, so that lots of roads, rail-roads and tunnel,which play a pivotal role in the industry activity, are existed along the rock-slope and in the rock-mass. Thus,it is urgent that tegration of management system through the optimum survey and design of rock-slope excavation, proper stabilization method and database of rock-slope. However, conventional methods have shortcoming with the economy of survey time and human resources, and the overcome of difficulties of approach to the in-situ rock-slope. To overcome the limitation of conventional method, this paper proposed the development of remote measurement system using Terrestrial Laser Scanning System. The method using Terrestrial 3D Laser Scanning System, which can get 3D spatial information on the rock-slope and2)Dept. Geosystem Engineering, Kangwon National University, Korea tunnel, has an advantage of reduction of measurement time and the overcome of difficulties of approach to the in-situ rock-slope/dam/tunnel. In the case of rock-slope, through the analysis of 3D modeling of point-cloud by Terrestrial Laser Scanning System, orientation of discontinuity, roughness of joint surface, failure shape and volume were successively achieved. in the case of tunnel face, through reverse-engineering, monitoring of displacement was possible.
Purpose: Although trauma is the most common cause of death under age 18, Korean national pediatric trauma data has lack of clinical data. This study is to prepare manpower resources, equipment, and make a correct policy decision on pediatric trauma victims Methods: The study enrolled 528 patients under age 16 with traumatic injury visited Wonju Severance Christian Hostpital Trauma Center, from February 12, 2015 to December 31, 2016. We analyzed the distribution of gender, age, place and time of the accident, injury mechanism, injury severity, and injured organ by medical record. Results: The major injury mechanisms were blunt injury in 485 (91.90%), penetrating injury in 27 (5.10%), burn in 13 (2.50%), near drowning in 2 (0.40%), and foreign body ingestion in 1 (0.20%). Ninety-seven (18.4%) patients were injured at home and 67 (12.7%) patients were injured at school. The overall mortality rate was 1.13% (n=6). 5 mortalities were related to automobile accident and one was fall down. Mean Injury Severity Score (ISS) was 4 (2, 8). No statistical significance was observed in the mean ISS between each age group. The peak time of accident occurrence was between 16 and 17 o'clock. The mean ISS was higher in blunt injury group than penetrating injury with statistical significance ($6.50{\pm}7.60$ vs. $3.00{\pm}8.10$; p<0.05). The most common injury site was upper extremity. Mean ISS was highest in thorax injury. However, mean ISS of thorax injury was higher with statistical significance only compared with face, neck and upper extremity injury. Conclusions: We reported our pediatric trauma patients data of our hospital level I trauma center, which is the only one level I trauma center of Gangwon Province. These data is useful to prevent and prepare for pediatric trauma.
