기능성 음성장애의 병태생리 (Pathophysiology of Functional Dysphonia)

  • 기능성 음성장애는 후두에 구조적 혹은 신경학적 병변이 없이 음성발성에 장애가 있는 질환으로, 의학적인 부분만으로 설명하기가 어려운 장애 요소들을 내포하고 있기 때문에 진단에 많은 혼란이 있을 수 있는 논쟁의 여지가 많은 음성질환이다. 기능성 음성장애의 중요한 부분 중의 하나인 muscle tension dysphonia는 후두 근육과 후두 주변 근육의 지나친 긴장에 의해서 발생하는 음성장애로, 비록 후두 내/외근의 운동이 제대로 조절되지 않는 것이 muscle tension dysphonia의 가장 첫 번째 원인이라고 알려져 있고, 특정의 개인적인 인성의 특징(personality traits)들도 발생에 있어서 상당히 중요한 부분을 차지 하는 것으로 연구되고 있지만, 이들 근육이 도대체 어떻게 문제가 되는 지에 대해서는 사실 아직까지 완전하게 설명하기에는 곤란한 점이 많다. 또한 최근 들어서는 muscle tension dysphonia를 하나의 질환으로 보기보다는 기능성 음성장애의 문제점들을 설명하는 진단적 지표(diagnostic label)로 보는 경향이 많으며, 기능성 음성장애와 기질적 음성장애(organic voice disorder)를 아우르는 위치에 있는 것으로 해석하는 경향도 있다. 따라서 기능성 음성장애에 대한 분류나 발생기전에 대한 의견들은 아직까지 논란이 되는 부분들이 많은 상태이고, 기능성 음성장애를 더 잘 이해하고 이 질환을 성공적으로 치료해 나갈 수 있는 중요한 요인들을 찾아낼 수 있도록 더 많은 연구들이 이루어 져야 할 것으로 생각된다.

보툴리눔독소 주입에 의한 음성장애 및 언어장애의 치료 (Botulinum Toxin Injection for the Treatment of Voice and Speech Disorders)

  • Botulinum toxin, a neurotoxin derived from Clostridia Botulinum, has been injected into the target muscle(s) for the treatment of several kinds of voice and speech disorders at the Voice Clinic, Yonsei Institute of Logopedics and Phoniatrics since December 1995. Criteria for the diagnosis and method of injection for spasmodic dysphonia, mutational dysphonia, muscle tension dysphonia, dysphonia after total laryngectomy, and stuttering were summarized. Among 144 patients with adductor type spasmodic dysphonia, who were injected one time to maximum 8 times during the 27 months, 90% were recognized as having better than slight improvement. Even though the injected cases were small, not only the abductor type spasmodic dysphonia, but also the intractable mutational dysphonia or muscle tension dysphonia resistant to voice therapy revealed that botulinum toxin injection would be another options for treatment. Patients who cannot phonate after total laryngectomy and some forms of adulthood stutterers can also be candidates for the injection of botulinum toxin.

광배근 근피판 거상 후 점진적 긴장 봉합술의 유용성 (The Effectiveness of Progressive Tension Suture at Latissimus Dorsi Muscle Harvest)

  • Purpose: The latissimus dorsi muscle flap is a versatile flap used in a variety of reconstructive procedures. The most common complication of LD muscle flap is donor site seroma, reported to occur in 20 to 79 percent of cases. The formation of dead space under the flap is intimately associated with seroma formation. The authors think that the use of progressive tension suture at closing donor site can decrease the formation of dead space and ultimately reduce the incidence of donor site seroma. Methods: A retrospective review was performed with 38 patients who underwent latissimus dorsi muscle harvest for breast reconstruction from March 2003 to September 2004. Progressive tension sutures were used during donor site closure in 22 patients. This group was compared with controls group(16 patients) who underwent latissimus dorsi muscle harvest without using this technique. Operation time, length of hospital stay, period of drainage, complication, and satisfaction about postoperative scar of donor site were examined. Results: The average length of hospital stay was 10.2 days and 12.7 days, and the mean duration of drainage were 7.3 days and 11.7 days in each progressive suture group and control group. These results were statistically significant (p<0.05). In the 22 patients who underwent progressive tension suture, none had seroma, hematoma or skin necrosis. In control group(16 patients), there happened one seroma formation and one partial skin necrosis. These complications were healed by aspiration of seroma and wound revision. The patients' satisfaction was not statistically significant, but the higher points were given by the patients who underwent progressive tension suture. Conclusion: This technique, progressive tension suture, is an effective method to reduce or eliminate donor site seroma, which is the most common complication associated with latissimus dorsi muscle harvest.

Forskolin Changes the Relationship between Cytosolic $Ca^{2+}$ and Contraction in Guinea Pig Ileum

  • This study was designed to clarify the mechanism of the inhibitory effect of forskolin on contraction, cytosolic $Ca^{2+}$ level $([Ca^{2+}]_i)$, and $Ca^{2+}$ sensitivity in guinea pig ileum. Forskolin (0.1 nM ${\sim}$ 10 ${\mu}M$) inhibited high $K^+$ (25 mM and 40 mM)- or histamine (3 ${\mu}M$)-evoked contractions in a concentration-dependent manner. Histamine-evoked contractions were more sensitive to forskolin than high $K^+$-evoked contractions. Spontaneous changes in $[Ca^{2+}]_i$ and contractions were inhibited by forskolin (1 ${\mu}M$) without changing the resting $[Ca^{2+}]_i$. Forskoln (10 ${\mu}M$ ) inhibited muscle tension more strongly than $[Ca^{2+}]_i$ stimulated by high $K^+$, and thus shifted the $[Ca^{2+}]_i$-tension relationship to the lower-right. In histamine-stimulated contractions, forskolin (1 ${\mu}M$) inhibited both $[Ca^{2+}]_i$ and muscle tension without changing the $[Ca^{2+}]_i$-tension relationship. In ${\alpha}$-toxin-permeabilized tissues, forskolin (10 ${\mu}M$) inhibited the 0.3 ${\mu}M$ $Ca^{2+}$-evoked contractions in the presence of 0.1 mM GTP, but showed no effect on the $Ca^{2+}$-tension relationship. We conclude that forskolin inhibits smooth muscle contractions by the following two mechanisms: a decrease in $Ca^{2+}$ sensitivity of contractile elements in high $K^+$-stimulated muscle and a decrease in $[Ca^{2+}]_i$ in histamine-stimulated muscle.

$17\beta$-estradiol의 고혈압 유도반응 억제와 인체적용 전기자극의 $17\beta$-estradiol 활성 증가 (The inhibition of Hypertension-related Response by $17\beta$-estradiol and the Increase of $17\beta$-estradiol Activity by Electrical Stimulation)

  • Purpose: $17\beta$-estradiol is the most active endogenous estrogen, which is related to favorable changes in the plasma lipid profile, to relaxation of the coronary vessels, and to a decrease in platelet aggregation and vascular smooth muscle cell migration. However, although the beneficial effect of estrogens on plasma lipoproteins (ie, lowering low-density lipoprotein and increasing high-density lipoprotein cholesterol) contributes to cardiovascular protection, it does not fully account for the protective effect, particularly in the application of physical therapy, including low frequency electrical stimulation. Methods: The aim of this study was to demonstrate the inhibition of stressors, such as endothelin-1 (ET-1), serotonin (5-hydroxytryptamine, 5-HT), prostaglandin $F2\alpha$ ($PGF2\alpha$), and a protein kinase C (PKC) activator 12-deoxyphorbol 13-isobutyrate (DPB), induced isometric tension by $17\beta$-estradiol in vascular smooth muscle strips, respectively. In addition, the effects of low frequency electrical stimulation at the meridian points (CV-3, -4, Ki-12, SP-6, LR-3, BL-25, -28, -32, -52) on the indirect antihypertensive effect were examined by monitoring the changes in the serum $17\beta$-estradiol concentration in healthy volunteers. Results: Isometric tension analysis showed that the responses of inhibited tension by $17\beta$-estradiol were similar to the same stressors in rat aortic smooth muscle strips. Furthermore, although the continued amplitude modulation (AM) type of electrical stimulation was not increased significantly by electrical stimulation, the current of the frequency modulation (FM) type of low frequency electrical stimulation increased the serum $17\beta$-estradiol concentration in normal volunteers. Conclusion: These results, in part, suggest that $17\beta$-estradiol has the capacity to supress stressor-induced muscle tension, and electrical stimulation, particularly current of the FM type, has a modulatory effect on the sex steroid hormones, particularly $17\beta$-estradiol, in healthy volunteers.

The Impact of Cervical Stretching Exercise and Cervical Traction on Cervical Pain and Muscle Activity among Patients with Cervical Hypolordosis

  • The purpose of this study is to provide fundamental clinical data for the treatment plan and rehabilitation of patients with cervical hypolordosis by comparing the cervical headache and muscle activity after cervical stretching exercise and cervical traction, which are generally applied to patients with cervical hypolordosis. The research subjects included 20 patients without gender division who were diagnosed with cervical hypolordosis. After applying cervical stretching exercise and cervical traction for six weeks, cervical headache and the activity of the muscles around the cervical vertebra(upper trapezius muscle, sternocleidomastoid muscle, splenius capitis muscle, and anterior temporal muscle) were investigated and the following results were obtained. In a comparison of the within group intervention effects of the two groups, cervical pain statistically significantly decreased in the cervical stretching exercise group. According to the results of analyzing the change of muscle tension in the upper trapezius muscle, both the cervical traction group and showed a statistically significant within group difference in the left and right side. According to the results of analyzing the change in the muscle tension of the splenius capitis muscle, both groups showed a statistically significant within group difference in the left and right side. In a between-group comparison, a statistically significant difference in the right side was observed. These results confirm that cervical vertebra traction and cervical stretching exercise decrease the cervical headache and muscle activity of the upper trapezius muscle and the splenius capitis muscle among patients with cervical hypolordosis.

Length-tension and velocity-force relationships of the torso extensors:Dynamic biomechanical modeling considerations

  • This study investigated the length-tension and velocity-force relations of the torso erectors. A myoelectric based approach was used wherein a dynamic biomechanical model incorporating active and passive tissue charactreistics provided music kinematic estimates during controlled sagittal plan extension motions. A double linear optimization formulation from the literatured provided muscle tension estimates. The data supported a linear length-tension relation toward full flexion for both the erector spinae and latissimus muscles. Velocity trends agreed with that predicted by Hill's exponential relation. The results have implications for muscle tension estimation in biomechanical torso modeling, and suggest a possible low back pain injury mechanism.

장력발생이 근육 분자막에 미치는 영향에 관한 연구 (A Study on the Effects of Muscle Membrane in Tension Development)

  • On contraction of the muscles, marked changes in X-ray reflections are observed, suggesting that conformational changes of contractile molecules and the movement of myosin heads during muscle contraction. The time needed to the peak tension after the onset of stimulation and the amount of peak tension depend on the number of twitch cycle. It was found that the successive twitches decreased not only the time needed to the peak tension after the onset of stimulation but also the time needed to the maximum change of the X-ray intensity. However, the difference of the time between the peak tension and the maximum intensity change($T_{i}-I_{i}$) is nearly the same at any twitch. Based on these results the causes of the decrease of $T_{i}$ and $I_{i}$, and physiological implication of $T_{i}-I_{i}$ are discussed.

  • Spasmodic dysphonia (SD) is a chronic, focal, speech-induced, action-specific dystonia, resulting strained voice. Muscle tension dysphonia (MTD) may also result in a strangled, strained voice quality, usually as a result of compensation for underlying laryngeal disease such as glottal insufficiency. Patients with SD and MTD were suffered from the severely limiting people's communication, especially via telephone and in noisy backgrounds. SD is usually of the adductor type characterized by glottic contractions causing tightness and voice breaks, which is difficult to distinguish from MTD. In this review article, we present the characteritics and management of SD and MTD.

  • Purpose: The purpose of this study is to compare the effects of proprioceptive neuromuscular facilitation (PNF) stretching, based on ballistic stretching and the contract-relax technique, on hip joint flexibility and muscle tone in adults with shortened rectus femoris muscles. Methods: The study involved 40 adults with shortened rectus femoris muscles, identified using the modified Thomas test. Participants were randomly divided into two groups: PNF stretching, employing the contract-relax technique, and ballistic stretching. Measurements included muscle tension, hip joint range of motion, and muscle characteristics. The rectus femoris muscle shortening effect was confirmed by the modified Thomas test, while the flexibility effect was assessed through hip joint motion range. The muscle tension effect was determined using Myoton-PRO. Results: Both stretching methods resulted in significant improvements in modified Thomas test angles and frequency, with the PNF stretching group showing notably greater changes. However, neither stretching method significantly affected decrement or stiffness measurements. These findings suggest that PNF stretching may be more effective for certain outcomes compared to ballistic stretching. Conclusion: In summary, both stretching methods positively influenced flexibility and muscle tension, with PNF stretching showing a greater impact. These findings highlight the importance of selecting the appropriate stretching technique for achieving functional improvements in muscles, which could serve as valuable indicators for preventing and treating muscle injuries in both sports and daily activities.
