Background: Factors associated with the prognosis of patients with small cell lung cancer (SCLC) is relatively unknown, than of those with non-small cell lung cancer. This study was undertaken to identify the prognostic factors of SCLC. Methods: The medical records of 333 patients diagnosed with SCLC at tertiary hospital from January 1, 2008, to December 31, 2012 were retrospectively reviewed. Patients were categorized by age (${\leq}65$ years vs. >65 years) and by extent of disease (limited disease [LD] vs extensive disease [ED]). Overall survival and progression free survival rates were determined. Factors associated with prognosis were calculated using Cox's proportional hazard regression model. Results: Most baseline characteristics were similar in the LD and ED groups. Eastern Cooperative Oncology Group (ECOG) performance status (PS), first chemotherapy regimen, and prophylactic cranial irradiation (PCI) differed significantly in patients with LD and ED. Mean ECOG PS was significantly lower (p<0.001), first-line chemotherapy with etoposide-cisplatin was more frequent than with etoposide-carboplatin (p<0.001), and PCI was performed more frequently (p=0.019) in LD-SCLC than in ED-SCLC. Prognosis in the LD group was better in younger (${\leq}65$ years) than in older (>65 years) patients, but prognosis in the ED group was unrelated to age. Conclusion: This study showed that overall survival (OS) was significantly improved in younger than in older patients with LD-SCLC. Univariate and multivariate analyses showed that age, PCI and the sum of cycles were significant predictors of OS in patients with LD-SCLC. However, prognosis in the ED group was unrelated to age.
Background: Colorectal cancer is one of the major health problems worldwide. However, limited studies have been reported from ASEAN countries. This study was conducted to evaluate clinical characteristics and survival of colorectal cancer cases aged <65 years and ${\geq}65$ years in the central region of Thailand. Materials and Methods: Clinical information, histological features, endoscopic findings and treatment outcome were collected and reviewed from Thammasat University Hospital, Pathumthani, Thailand between November 2011 and October 2015. Results: A total of 121 colorectal cancer patients, comprising 69 men and 52 women with a mean age of 65.8 years, were included. There were 57 aged <65 years and 64 aged ${\geq}65$ years. Common presenting symptoms were abdominal pain (37%), weight loss (34%) and anemia (32%). Mean duration of symptoms prior to diagnosis was 173 days. However, longer diagnosis time was demonstrated in patients aged <65 years than age more than ${\geq}65$ years (119.4 vs 58.4 days, P-value=0.30). Colonic fungating mass was the most common endoscopic finding (64.4%) and the location was significantly more commonly left than right side of the colon, both in younger and elderly groups (87.7% vs 12.3%, P=0.02 and 70.3% vs 29.7%, P=0.02, respectively). Adenocarcinoma with moderated differentiated was the most common histology (67.3%). More than half of the patients presented with advanced stage (28.9% with TNM stage 3 and 38.8% TNM stage 4, respectively). Overall 1-year and 5-year survival rates were 76.9% and 5%. Conclusions: Most colorectal cancer patients in Thailand have adenocarcinomas and present at advanced stage with poor prognosis. Screening of high risk patients and early detection might be essential factors to improve the treatment outcome and overall survival rate of colon cancer patients in Thailand and other ASEAN countries.
Objectives: The aim of this study was to explore nursing students' attitudes towards sexually active elderly, and to compare their attitudes with those of non-nursing students. Methods: Study subjects consisted of 118 nursing students, and 134 non-nursing students, recruited from two universities. The data were collected from September to November 2001. Vignettes, which presented hypothetical situations of sexual activities of the elderly (65 years of age, 80 years of age/sexual activity mentioned. non-mentioned), were provided as a research method, as well as the use of a structured questionnaire. Vignettes showed the hypothetical situations by male elder's sexual activity and their age. Respondents were asked to address their thoughts in the questionnaire. Results: 1. Nursing students showed more favorable attitudes toward the elderly than non-nursing students, regardless of the presence of sexual activity of the elderly. 2. No significant difference was found in nursing students' attitudes toward the elderly aged 65 and 80 years, between those who were sexually active and those who were not. These results were the same in non-nursing students. 3. Nursing students showed more favorable attitudes toward the sexually active elders aged 65 years (5.6 1.43), than toward sexually active elders aged 80 years (4.89 1.55). This result was also the same in non-nursing students. 4. The variables significantly affecting attitudes toward the sexually active elderly in nursing students were familiarity with old persons (p=. 02), presence of living grandparents (p=. 05), and the experience of caring for old people (p=. 01). However, in non-nursing students, familiarity with old people was the only significant variable that affected their attitudes toward the sexually active elderly. 5. In nursing students, one variable to predict attitudes toward the sexually active elderly was the experience of caring for old people (p=. 03), accounting for 10% of the total variance. In non-nursing students, familiarity with old people was the only variable to attitudes toward the sexually active elderly (p=. 03), accounting for 3% of the variance for attitude. In both student groups, the variables that predicted attitudes toward the sexually active elderly included college major (p=. 03), school year (p=. 01), familiarity with old people (p=. 02), accounting for 12% of the variance for attitude by these variables. conclusion: Nursing students showed more favorable attitudes toward the sexual activity of elders 65 years of age or over, than non-nursing students. However, both the nursing and non-nursing students showed negative attitudes toward the elderly who were 80 years of age when compared with those 65 years of age. There should be a consideration in nursing education curriculums that university students may have negative attitudes towards sexual activity of the elderly over 80 years of age.
This study was done to research the general information, causes of stroke, risk factors, complications during admission of geriatric CVA inpatients. We retrospectively studied 208 above 65 years old geriatric CVA inpatients treated in Ajou university hospital from 1994 to 2003 by medical record. These were divided into two groups by following years. We were gathered information about sex, onset age, cause of stroke, number of stroke, complications, housing, family, insurance, smoking, alcohol. Most of incidence of the stroke was noted in the group of young old age (65~74 years old) and old age (75~84 years old). The occurrence rate of male stroke (38.5%) was less than of female stroke (61.5%) and the ratio of male to female was 1:1.6. The occurrence rate of ischemic stroke (72.6%) was higher than of hemorrhagic stroke. The occurrence rate of ischemic stroke increased more and more at the late stage. The most common risk factors for stroke was hypertension and complications during hospitalization were neurogenic bowl and bladder. These results of epidemiologic study may help above 65 years old geriatric CVA early treatment and prevention, rehabilitation and use basic data for multiple prospective study using stroke registry.
Objective: Older persons with diabetes mellitus (DM) are particularly more likely to have fallen in the previous year than those without DM. The purpose of this study was to investigate the relationship between the risk of falls and type 2 DM in older adults who are 65 years of age or above. Design: A systematic review. Methods: PubMed and other two databases were searched up to August 2, 2018. Observational and cohort studies evaluating fall risk in people who are 65 years of age or above with DM were included. This review extracted the following information from each study selected: first author's surname, year of publication, country, average follow-up period, sex, age at enrollment, study population, measurement variables, relative risk, 95% confidence intervals and controlled variables. Results: This review involved nine cohort studies with 3,765 older adults with DM and 12,989 older adults without DM. Six studies compared with or without DM and two studies compared fallers with non-fallers with DM. Risk factors for falls included impaired cognitive function, diabetes-related complications (peripheral nerve dysfunction, visual impairment), and physical function (balance, gait velocity, muscle strength, and severity of physical activities). Conclusions: People who are 65 years of age or above with DM have increased risk of falling caused by impaired cognitive function, peripheral nerve dysfunction, visual impairment, and physical function in community-dwellers. For adults who are 65 years of age or older with DM, research fields and clinical settings should consider therapeutic approaches to improve these risk factors for falls.
South Korea will soon be a super-aged society, as more than 20.6% of its population will be 65 years and older by 2025. As of 2022, 17.5% of the total population in South Korea is 65 years and older, which exceeds the set threshold for an aged society, where more than 14% of the population is 65 years and older. The proportion of older subway construction workers has increased. Aging workers and their work stress negatively impact their workability. A previous study demonstrated that the stress index measured using the uBioMacpa measurement device (Macpa stress index) had a significant correlation with work stress in South Korea. The device tests vascular health and measures stress levels via Macpa signal analysis. In this study, the pulse waves of subway construction workers were measured using uBioMacpa to identify their stress levels. The stress levels were analyzed by age, years of service, job position, employment type, and work type. Herein, these statistics could not be easily represented by a normal distribution; therefore, the Kruskal-Wallis test, a nonparametric statistical method, was used for the analysis of data. The results showed that age, job position, employment type, and working type affected the Macpa stress index and the stress levels of workers increased with age. In terms of job position, technical engineers were more stressed than other workers because of their poor working environment. In terms of employment type, daily-wage workers were more stressed than other workers. In terms of working type, tunneling, waterproofing, and construction scored the highest Macpa stress indexes without any significant difference, whereas earthworks scored the lowest. Based on the analysis of Macpa stress index, safety and health management plans were proposed to reduce the stress levels of workers. Moreover, a manual for efficient stress management must be developed for subway construction workers.
The purpose of the study is to construct a life table for population. It is based on the fraction of last year of life, $a_X$. The data necessary for this purpose have been obtained from the 1975 Population Census Report of Korea and the Vital Statistics of Shindong-myon, Chunseong-gun, Gangwon Province which is collected for the Seoul National University public health program. Age specific death rate is adapted by the Model Life Table, West, Level 21. For the age groups of less than 5 years of age, when the record of vital events can be obtained, the fractions are calculated from the community vital statistics. And for the age groups older than 5 years of age, Greville's Method is used. The findings are summarized as follows: 1) The fraction of last year of life in infantile group is 0.3684 for males, 0.3711 for females, and in 1-4 years of age group 1.2164 for males, 1.2274 for females. Both are more than those of Japan and U.S. in 1963. 2) Infant mortality rate is 42.37 for males, 31.77 for females per 1,000 live births. 3) The mortality curves show that a higher rate is observed under 1 year of age. It drops to the lowest point at around 10 years of age and rises again as the age increases. 4) The age estimated half-survival rate is during the age group of 70-74 for both sexes. 5) Life expectancy at the age of 0 shows 65.73 years for males and 69.22 years for females.

Objective : To evaluate the clinical and radiological outcomes of lumbar interbody fusion and its correlation with various factors (e.g., age, comorbidities, fusion level, bone quality) in patients over and under 65 years of age who underwent lumbar fusion surgery for degenerative lumbar disease. Methods : One-hundred-thirty-three patients with lumbar degenerative disease underwent lumbar fusion surgery between June 2006 and June 2007 and were followed for more than one year. Forty-eight (361%) were older than 65 years of age (group A) and 85 (63.9%) were under 65 years of age (group B). Diagnosis, comorbidities, length of hospital stay, and perioperative complications were recorded. The analysis of clinical outcomes was based on the visual analogue scale (VAS). Radiological results were evaluated using plain radiographs. Clinical outcomes, radiological outcomes, length of hospital stay, and complication rates were analyzed in relation to lumbar fusion level, the number of comorbidities, bone mineral density (BMD), and age. Results : The mean age of the patients was 61.2 years (range, 33-86 years) and the mean BMD was -2.2 (range, -4.8 to -2.8). The mean length of hospital stay was 15.0 days (range, 5-60 days) and the mean follow-up was 23.0 months (range, 18-30 months). Eighty-five (64.0%) patients had more than one preoperative comorbidities. Perioperative complications occurred in 27 of 133 patients (20.3%). The incidence of overall complication was 22.9% in group A, and 18.8% in group B but there was no statistical difference between the two groups. The mean VAS scores for the back and leg were significantly decreased in both groups (p < 0.05), and bony fusion was achieved in 125 of 133 patients (94.0%). There was no significant difference in bony union rates between groups A and B (91.7% in group A vs. 95.3% in group B, p = 0.398). In group A. perioperative complications were more common with the increase in fusion level (p = 0.027). Perioperative complications in both groups A (p = 0.035) and B (p = 0.044) increased with an increasing number of comorbidities. Conclusion : Elderly patients with comorbidities are at a high risk for complications and adverse outcomes after lumbar spine surgery. In our study, clinical outcomes, fusion rates, and perioperative complication rates in older patients were comparable with those in younger populations. The number of comorbidities and the extent of fusion level were significant factors in predicting the occurrence of postoperative complications. However, proper perioperative general supportive care with a thorough fusion strategy during the operation could improve the overall postoperative outcomes in lumbar fusion surgery for elderly patients.
연구배경: 폐암의 발생은 접점 증가추세로 65세 이상환자가 전체 폐암의 50% 정도를 차지하지만, 잦은 만성 질환의 동반, 각종 장기의 기능저하로 수술, 방사선치료나 항암화학요법과 같은 적극적 치료를 감당하지 못할 것을 우려하여 적절한 치료를 받지 못하고 있어 노령 폐암환자의 임상적 특성의 분석을 통하여 향후 이들에 대한 대책수립하고자 본 연구를 시행하였다. 방 법: 1994년 9월부터 1998년 8월사이에 서울특별시립 보라매 병원에 입원하여 폐암으로 진단받은 환자 207명을 대상으로, 65세이상의 I군과 65세미만의 II군으로 분류하여 후향적 분석을 하였다. 결 과: 60대에서 36.2%로 가장 높은 발병률을 보였고, 65세 이상의 남자환자가 전체환자의 42%였다. I군에서 호흡곤란을 주소로 하는 환자가 유의하게 많았으나, 이외의 증상이나 병기, 조직학적 분류상두군간에 유의한 차이는 없었다. ECOG기준 활동도 3, 4인 환자가 II군의 12.9%에 비해 I군에서는 35.2%로 많았다. 소세포폐암의 치료는 두 군간에 차이가 없었으나, 비소세포폐암 환자는 II군의 35%에 비해 I군 환자의 74%가 보존적 치료만을 받았고, 수술이나 근치적 방사선치료를 받은 환자는 I군에서 14%에 불과하였다. 두 군간에 생존을 비교하면 I군의 중앙생존기간이 10개월로 II군의 17.4개월에 비해 떨어지지만, 보존적 치료외에 어떠한 치료라도 받았던 환자에서는 두군간에 차이를 보이지 않았다. 활동도, 병기, 조직학적 아형만이 다변량분석시 생존에 영향을 미치는 유의한 인자였고 연령은 생존과 무관한 인자였다. 결 론: 폐암은 노령환자의 질환이라고 할만큼 65세 이상에서 호발하며, 환자의 연령에 의해서 예후가 결정되는 것이 아니므로 노령환자라 하더라도 폐암에 대한 적극적 진단 및 치료가 필하다고 하겠다.
The present study was aimed to investigated whether there are age-and sex-related differences in serum levels of lipid peroxides, retinol , and $\alpha$-tocopherol in Korean adults. The subjects were 441 persons , including 268 men and 173 women. Those of each sex were divided into four age group : 20-29, 30-39, 40-49 and 50-65 years, and their lifestyles and serum levels were compared . Men smoked and drank less as age increased , but 99.9% of women of did not smoke and only 11% drank more than once a month. Lipid peroxides of males increased gradually with age, while those of females showed greater levels in the 50-65 years group thon younger groups. Lipid peroxides, retinol and $\alpha$-tocopherol concentrations, which were adjusted for age, were significantly higher in males than in females. Lipid peroxide levels adjusted for total lipid were positively correlated with age in males than in females. Serum levels of $\alpha$-tocopherol adjusted for total lipid were positively correlated with age both in males and in females , while retinol was neither. The results indicate that serum levels of lipid peroxides, retinol , and $\alpha$-tocopherol are affected by age and that the response could be different between males and females.
