  • Evaluation of potentials of chemicals to alter expression of genes that are involved in carcinogenesis may serve useful tools in toxicological research. In this investigation, we developed reporter cell lines that expressed luciferase in response to transactivation of hypoxia inducible factor-1, P53 tumor suppressor and Nur77 of which roles have been well established in cancer development and progression. Whereas these reporter cell lines displayed low constitutive backgrounds, the reporter activities were significantly enhanced in response to $desferriosamine/CoCl_2$, adriamycin or 6-mercaptopurine, which are hypoxia mimicking chemicals, P53 activator or Nur77 inducer, respectively. The activation of the reporter was time- and dose-dependent. Known tumor initiators and promoters, such as phorbol 12-myristate 13-acetate and phorbol 12, 13-dicaprinate induced the reporter activity at as low as 10nM in these stable cell lines. Further, known anti-tumor promoters, such as ascorbic acid and ${\beta}-carotene$ repressed the reporter activities. These results indicate that our stable reporter cell lines could serve as a useful system for rapid assessment of carcinogenicity of toxic chemicals.

Fatal Balamuthia Amebic Encephalitis in a Healthy Child: A Case Report with Review of Survival Cases

  • Balamuthia mandrillaris is one of the 4 amebas in fresh water and soil that cause diseases in humans. Granulomatous amebic encephalitis (GAE), caused by B. mandrillaris, is a rare but life-threatening condition. A 4-year-old, previously healthy, Thai girl presented with progressive headache and ataxia for over a month. Neuroimaging studies showed an infiltrative mass at the right cerebellar hemisphere mimicking a malignant cerebellar tumor. The pathological finding after total mass removal revealed severe necrotizing inflammation, with presence of scattered amebic trophozoites. Cerebrospinal fluid (CSF) obtained from lumbar puncture showed evidence of non-specific inflammation without identifiable organisms. A combination of pentamidine, sulfasalazine, fluconazole, and clarithromycin had been initiated promptly before PCR confirmed the diagnosis of Balamuthia amebic encephalitis (BAE). The patient showed initial improvement after the surgery and combined medical treatment, but gradually deteriorated and died of multiple organ failure within 46 days upon admission despite early diagnosis and treatment. In addition to the case, 10 survivors of BAE reported in the PubMed database were briefly reviewed in an attempt to identify the possible factors leading to survival of the patients diagnosed with this rare disease.

급성 폐동맥 색전증으로 의심된 원발성 폐동맥 골육종 -1예 보고- (Primary Pulmonary Artery Osteosarcoma Mimicking Acute Pulmonary Artery Embolism - A case report-)

  • 원발성 폐육종은 미국의 경우 전체 원발성 폐암 발생률의 0.4% 정도의 비율로 발생하는 드문 질환이며 이중 원발성 폐동맥 골육종은 전 세계적으로 극히 드물게 보고되고 있다. 본 증례는 63세 여자 환자로 흉통과 호흡곤란, 어지러움을 주소로 응급실로 내원하여 검사 중 갑자기 쇼크상태에 빠졌다. 반복적인 심폐소생술을 시행하며 심초음파를 실시한 결과 급성 폐동맥 색전증으로 인한 우심부전증으로 진단하여 인공심폐기 가동하에 응급수술을 시행하였다. 주폐동맥을 절개하였을 때 혈전이 주폐동맥에서 좌우 폐동맥에 걸쳐 존재하였고 종괴가 주폐동맥의 우상부쪽 내막에 붙어 있어서 종괴와 혈전을 제거하였다. 환자는 특별한 문제없이 회복되었으며 술 후 조직검사에서 종괴는 폐동맥 골육종으로 진단되었다. 술 후 시행한 검사에서 폐 이외의 장기에서는 골육종이 발견되지 않았으며 좌하행 폐동맥 내에 잔존하는 종괴와 좌우 폐야에서 다발성 결절들이 관찰되어 혈행성 전이가 의심되어 항암치료와 방사선치료를 시행하였으며 수술 후 16개월에 환자는 잔존하는 종괴와 전이성 결절들의 크기는 줄어든 상태로 특별한 증상 없이 지내고 있다.

  • 나노 크기의 배향성을 갖는 구조물의 제작은 자연에 존재하는 여러 가지 형상의 모방을 가능하게 한다. 고분자는 가격이 매우 저렴하며 합성과 가공 그리고 그 구조가 잘 알려져 있는 장점을 갖고 있어 필름(film)의 표면에 이러한 나노 구조물을 제작하고 나노 구조의 특성을 발현하는데 손쉽게 활용할 수 있는 재료이다. 나노 구조물을 제작하는 방법 중 양극산화를 통하여 제작한 다공성 알루미나 템플레이트(porous alumina template)는 매우 규칙적으로 정렬되어 있고 제어하는 공정이 비교적 쉽고 경제적이기 때문에 이를 이용한 연구가 매우 활발하게 진행되고 있다. 본 총설에서는 양극산화 알루미나 템플레이트의 제작과 이를 이용한 나노 구조 고분자 필름의 제작을 설명하고 이러한 나노 구조 필름의 응용범위 및 응용에 필요한 특성에 대하여 기술하였다.

의상디자인 발상을 위한 구성선의 응용과 전개방법 (The Application and Development Method of Construction Lines for Conceptions of Clothes Design)

  • In today's world where a preference for prestigious brands prevails, it's essential to have creative design conceptions rather than try simple mimicking or following in order to be recognized for creative and objective design works. In that sense, the development of clothes design using construction lines offers a great approach towards the ideas of reinterpreting and creating new designs since it creates new forms by understanding lines as signs of conception and transforming them. That approach allows a designer to improve his or her imagination, creativity and expression in a freer and richer manner, think of many original ideas through diverse changing processes, and develop the skill to visualize the ideas by incorporating geometric forms of abstract lines into clothes. When approaching the conception of clothes design from the standpoint of construction lines, you need to adopt more specific and systematic design methods instead of vague imagination to introduce simple facts or theories to a new design process. That's one of the ways you can approach clothes design easily and enhance your conception skills. The process of clothes design according to the conception of construction lines is composed of the following stages; the preparation stage involves an understanding of the principles and the entire process of clothes design conception based on construction lines; the idea stage requires a designer to make free alterations of the given basic construction lines by moving and replacing the lines and sides in diverse ways; in the creation and expression stage, the designer uses the newly reconstructed construction lines to create clothes design; and in the evaluation stage, the designer makes a presentation of his or her subjective design works and has his or her works assessed and tested in an objective fashion. In today's modern society that demands new and original globalized design, the results of the study will help to invigorate the development of new fashion design in a more systematic and practical manner by suggesting a feasible and gradual educational program for the development of design based on the technique of construction lines.

오존과 자외선을 이용한 유해 환경 개선에 관한 연구 -가정의 유해 세균 및 집먼지진드기 제거를 중심으로- (Ozone and UV Light-mediated Sanitization of Hazardous Environments -Eradication of Pathogens and Ticks -)

  • 본 연구에서는 가정의 여러 유해 환경 중에서도 세균과 바이러스 그리고 집먼지진드기를 효과적으로 제거할 수 있는 방법으로서 오존과 자외선이 그 대안이 될 수 있음을 실험을 통해 검증하고 있다. 실험을 통해 다음과 같은 사실이 확인되었다. 첫째, 가정과 유사한 실험 공간에서 오존과 자외선을 이용한 멸균 실험을 통해 실제 가정의 유해 세균이 수 십분 이내에 $99.0\%$ 제거되는 조건을 확인하였다. 둘째, 오존과 자외선을 이용하여 바이러스가 거되는 조건을 확인하였다. 셋째, 집먼지진드기가 제거되는 조건을 확인하였다 오존만을 적용한 조건과 오존과 자외선을 동시에 적용한 조건으로 나누어 실험한 결과를 통해 집먼지진드기는 오존과 자외선의 동시 노출 조건에서 사멸 율이 더 높게 나타났으며, 오존만을 이용한 조건만으로 집먼지진드기가 제거됨을 확인할 수 있었다.

복강내 종양으로 오인된 외상성 혈종: 증례보고 (Traumatic Organized Hematoma Mimicking Intra-peritoneal Tumor : A Case Report)

  • Blunt abdominal trauma is commonly encountered in the emergency department. The lack of historical data and the presence of distracting injuries or altered mental status, from head injury or intoxication, can make these injuries difficult to diagnose and manage. We experienced a case of traumatic organized hematoma misdiagnosed as intra-peritoneal tumor with intestinal obstruction. A 52-year-old homeless male patient who have chronic alcoholism was admitted via emergency room with infra-umbilical abdominal pain. At admission, he was drunken status and so we could not be aware of blows to the abdomen. He had a unknown large operation scar on mid abdomen. A computed tomography (CT) scan showed the intestinal obstruction of the ileum level with 5.5cm sized mesenteric tumor. We performed adhesiolysis and widely segmental resection of small bowel including tumor with side-to-side anastomosis due to great discrepancy in size. He stated later that he was a victim of the violence before 3 weeks. A final pathologic report revealed well encapsulated, traumatic mesenteric hematoma with organizing thrombi, ischemia and abscess formation with multiple adhesion bands. Finally, the patient was discharged without complications on postoperative day 14.

네거티브 경구 조영제를 이용한 PET/CT 촬영시 나타난 종양성 섭취와 유사한 생리적 장관 섭취 (Colon Cancer Mimicking Physiologic FDG Uptake : with Using of Negative Oral Contrast)

  • A 64-year-old female with glioblastoma multiforme (GBM) was assigned to our department for whole body PET/CT scan. She ingested 1 liter of pure water as negative oral contrast just before PET/CT examination. FDG-PET/CT images showed a very intense hypermetabolic, focal lesion in the abdominal cavity around descending colon. The SUVmax of the lesion was 17.2. But there was no abnormal lesion corresponded to the area of PET scan in the combined contrast enhanced CT scan. We suggested considering a malignant lesion due to very intense glycolytic activity. Conventional abdominal CT scan & colonoscopy were accomplished within one week after PET/CT evaluation. There was no abnormality in both examinations. We executed follow-up PET/CT evaluation after 1 month and couldn't find any abnormality around the corresponding area. So we concluded the hypermetabolism was colonic physiologic uptake. A colonic physiologic uptake is a well known cause of false positive finding. Nuclear physicians should be considered the possibility of malignancy when interpret focal colonic uptake, especially incidental finding. There are a few reports that using of negative oral contrast is able to reduce gastrointestinal physiologic uptakes. But as we can see in this case, although we used negative oral contrast, intense physiologic uptake is detected and maxSUV is able to up to 17.2. So, it is important to keep a fact in mind. Even though there is a colonic physiologic uptake in PET/CT image, it may be able to show very intense hypermetabolism regardless of using negative oral contrast.

재발성 설암으로 오인된 하악골 골수염 1예 (A Case of Mandible Osteomyelitis Mimicking Recurrent Tongue Cancer)

  • Osteomyelitis is an infection of bone or bone marrow, caused by pyogenic bacteria or mycobacterium. Osteomyelitis can be acute or chronic, inflammatory process of the bone and its structures. Chronic osteomyelitis will result in variable sclerosis and deformity of the affected bone. With an infection of the bone, the subsequent inflammatory response will elevate this overlying periosteum, leading to a loss of the nourishing vasculature, vascular thrombosis, and bone necrosis, resulting occasionally in formation of sequestra. These become areas that are more resistant to systemic antibiotic therapy due to lack of the normal Havesian canals that are blocked by scar tissue. At this aspect, not only systemic antibiotic therapy, but also surgical debridement maybe required to remove the affected bone and prevent disease propagation to adjacent areas. We experienced a patient who diagnosed tongue cancer and underwent wide partial glossectomy few years before, with an ulcerative lesion around right retromolar trigon. We diagnosed cancer recurrence because PET indicated hot uptake on mandible which was nearby previous tongue tumor site. The patient received hemiglossectomy via paramedian mandibulotomy, partial mandibulectomy and fibula osteocutaneous free flap reconstruction. But final diagnosis was mandible osteomyelitis on pathology report. Here, we present the case with a review of the related literatures.

비인두암종에 의한 측두하악장애 (Temporomandibular Disorder Caused by Nasopharyngeal Cancer)

  • 측두하악장애 증상으로 나타나는 구강안면부의 통증 및 개구제한 증상은 대부분 관련 구조물 자체의 구조적이거나 기능적인 변화에 의해 유발되지만, 드물게 다른 병적인 상태로 인하여 발생할 수도 있으므로 진단 및 치료에 있어서 주의가 요구된다. 특히 통증 양상 및 임상 소견, 전신적 동반 증상, 치료에 대한 반응 등에 대한 평가에서 두경부 종양과 같이 다른 질환에 의한 측두하악장애 증상이 의심되거나 일반적인 측두하악장애와 다른 소견이 나타나는 경우에는 전산화단층촬영 또는 자기공명영상 등의 부가적인 방사선학적 검사를 적극적으로 시행하여 부가적인 정보를 얻는 것이 필요하다. 본 증례에서는 측두하악장애 증상으로 내원하였던 비인두암종 환자의 진단 및 치료 과정을 통해, 측두하악장애의 진단 및 치료에 있어서 비인두암종 등 두경부 종양의 가능성을 고려해야 할 임상적 소견에 대해 고찰하고자 한다.
