Convergence Factors Affecting Critical Thinking Disposition, Clinical Decision Making Ability, Patient Safety Knowledge and Attitude to Perform Safety Nursing Activities in Clinical Practice on Nursing Students (간호대학생의 비판적 사고성향과 임상의사결정능력 및 환자안전 지식, 태도가 임상실습 시 환자안전 수행능력에 미치는 융복합적 영향요인)

  • The purpose of this study was to investigate the effect of critical thinking, clinical decision making ability, patient safety knowledge and attitude ability to perform patient safety activities during clinical practice on nursing student's. Data were analyzed using t-tests and one-way ANOVA using the SPSS 21.0 program. As a result, the subjects' safety nursing activities was positively correlated critical thinking disposition(r=.278, p=.001), clinical decision making ability(r=.202, p=.014), patient safety attitude(r=.421, p<.001).The significant predictors of perform safety nursing activities were gender(${\beta}=.175$, p=.031), patient safety education(${\beta}=-2.266$, p=.025), critical thinking disposition(${\beta}=3.354$, p=.001), patient safety attitude(${\beta}=.368$, p<.001). These factors explained 31.5% of the variance(R2=.561, F=4.56 p<.001). It is necessary to establish a curriculum for critical thinking disposition, clinical decision making ability, and patient safety education in order to improve patient safety performance necessary for clinical practice education of nursing college students.

Effect of marriage consciousness, occupational consciousness, gender equality consciousness on the childbirth will of nursing students (간호대학생의 결혼의식, 직업의식, 양성평등이 출산의지에 미치는 영향)

  • The purpose of this study was to investigate the effect of marriage consciousness, occupational consciousness, and gender equality consciousness on childbirth will in nursing students and to provide the basic data of to control childbirth will. Data were collected from 242 nursing students in B city and analyzed by t-test, ANOVA, Pearson correlation coefficient, and multiple regression using SPSS/WIN 21.0. The average of marriage consciousness was 3.43±.36, the average of occupational consciousness was 3.30±.47, the average of gender equality consciousness was 3.85±.37, the average of childbirth will consciousness was 3.48±.42. The childbirth will according to the general characteristics was not significantly different. The study subject's childbirth will was correlated with occupational consciousness(r=.279, p<.001), occupational consciousness(r=.143, p=.026), gender equality consciousness(r=.263, p<.001), and marriage conscious was correlated occupational consciousness(r=.279, p<.001). The factors influencing childbirth will of the study subjects were marriage consciousness(𝛽=.215,p=.001), gender equality consciousness(𝛽=.239, p<.001), and the explanatory power of these variables was 11.9%. According to this study, it is necessary to confirm the effects of developing an intervention program and applying this program to improve the decisions of nursing students concerning marriage and having children.

Antihypertensive Effects of Novel Isoflavone-Free Black Soy Peptide Mixture as HO-1 Inducer (Heme 산화효소 발현 유도체로서 Isoflavone-Free 검은콩 펩타이드의 항고혈압 활성)

  • We previously reported that the novel isoflavone-free peptide mixture (black soybean peptide, BSP) had several beneficial effects like antiobesity and hypotriglyceridemic effect. However, there are no reports for BSP on anti-hypertensive activity. BSP induced heme oxygenase-1 (HO-1) in HUVECs, thus investigated the HO-1-induced activity in HUVECs and the anti-hypertensive effects in SHR animal model. BSP significantly induced HO-1 expression both at transcriptional and protein levels in a time- and dose-dependent manner as measured by RT-PCR and Western blot analysis, respectively. These inductions were abolished by pretreatment of N-acetyl-cystein (NAC, 1~10 mM), but not by employing Tempol, a superoxide dismutase (SOD) mimetic (1~5 mM). As expected, enzymatic activity (~2 fold) determined by bilirubin formation assay and cGMP concentration (~6 fold) were significantly increased in BSP-treated cells. Based on the numerous evidences on the beneficial effects of HO-1 and our results, we investigated in vivo effects of BSP on the antihypertensive activity. The administration of BSP (1% in drinking water) significantly decreased mean blood pressure (BP) (from $218.6{\pm}6.99$ to $190.0{\pm}3.42$ mm Hg, p<0.01). This result indicates that BSP is strong inducer of HO-1 expression, which may be triggered by oxidative stress, and has anti-hypertensive activity.

  • In spite of the R&D level of Korea, the efforts to protect the R&D results from outflowing has not been raised up. We investigated the current status of security education and the level of researcher's awareness for research security in the government-financed institutes. Also, we attempted to find out the needs for institutionalization of the security education. We conducted a survey and in-depth interviews of all the security officers in the thirty-seven government-financed institutes. The results show that the awareness level of the researchers for R&D security is below adequate level, and that security education is necessary in order to increase the security awareness. Also, it is necessary to institutionalize the security education.

Factors Influencing General Hospital Nurses' Confidence about Advance Directives (종합병원 간호사의 사전의료의향서에 대한 자신감 영향요인)

  • Purpose : The purpose of this study was to identify the factors influencing the general hospital nurses' confidence about the advance directives (ADs). Methods : This is a descriptive cross-sectional study. A total of 105 nurses were asked questions about characteristics, knowledge, attitude, learning needs, and confidence the about ADs, using structured questionnaires. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression. Results : The mean scores of knowledge were 7.26±1.47; attitude, 2.80±0.31; learning needs, 4.22±0.62; and confidence, 2.57±0.74. It was observed that knowledge had significant correlations with attitude (r=.43, p<.001), learning needs (r=.29, p=.003), and confidence (r=.40, p<.001), and attitude had significant correlations with learning needs (r=.31, p=.001) and confidence (r=.45, p<.001). Using multiple regression analysis, it was found that confidence was significantly influenced by knowledge (β=.27, p=.007), attitude (β=.21, p=.043), and experience of nursing or counseling a patient who wrote ADs (β=-.19, p=.046). The explanatory power of this model was 37.9% (F=5.16, p<.001, R2=.379). Conclusions : These findings suggest the need to develop a program to enhance nurses' knowledge, positive attitude, and experience of nursing or counseling a patient who wrote ADs.

Transcervical or Laparoscopic Insemination of Frozen-thawed Semen in Estrus-synchronized Himalayan Tahrs (Hemitragus jemlahicus)

  • Four estrus-induced Himalayan tahrs (Hemitragus jemlahicus) were inseminated with frozen-thawed semen by laparoscopic or transcervical insemination techniques with no regard to the site of ovulation in non-breeding season. In June and July, 2009, estrus was synchronized by Eazi-Breed $CIDR^{(R)}$ (Controlled internal drug release; Pfizer Animal Health, New Zealand) insertion for 16 days and PG 600 (PMSG 400IU, hCG 200 IU; Intervet, Netherlands) injection (IM) a day before removing $CIDR^{(R)}$. Forty eight hours later, laparoscopic or transcervical insemination was done to each of two tahrs under anesthetic condition inducted by ketamine (1.5 mg/kg) and medetomidine (0.09 mg/kg). For examination of estradiol and progesterone, blood was collected right before $CIDR^{(R)}$ insertion, PG 600 injection, $CIDR^{(R)}$ removal and insemination. Estradiol levels of four tahrs (No. 1, 2, 3, 4) before $CIDR^{(R)}$ insertion and insemination were 13.3, 8.8, 14.3, 12 pg/ml and 23.5, 25.5, 21.1, 11.5 pg/ml, respectively. Progesterone levels of four tahrs (No. 1, 2, 3, 4) before $CIDR^{(R)}$ insertion and insemination were 1.8, 0.05, 0.63, 0.61 ng/ml and 1.03, 0.37, 1.48, 2.12 ng/ml. Except for No. 4 tahr, cervices showed cervical mucus and opened enough to penetrate with embryo transfer gun sheet usually used for cows. Therefore, No.4 was laparoscopically inseminated together with No. 1. In conclusion, none of four Himalayan tahrs was pregnant. However, we proved that estrus could be induced by CIDR and PG 600 injection in non-breeding season, and laparoscopic or transcervical insemination with frozen-thawed semen could be one of assisted reproductive techniques in Himalayan Tahr.

A Study on Oral Health Knowledge, Recognition, Practice and Satisfaction of Patients by Applying a Targeted Program within a Dental Hygiene Process (치위생과정에서의 일부 프로그램 적용에 따른 환자의 구강건강지식, 인식, 실천과 만족도 조사)

  • The purpose of this study was to evaluate a clinical preventative care program, based on a dental hygiene process (accessment, diagnosis, planning, implementation, evaluation; ADPIE) in a dental clinic, by analyzing patient recognition, knowledge, practice, and satisfaction with respect to oral health. The collected data (in percentages) were analyzed Fisher's exact test and paired t-test using IBM SPSS ver. 21.0 (IBM Co., USA). This study demonstrated a significant difference in oral health knowledge, recognition, and practice before and after the clinical preventive care program (p<0.05). The results were significant in the individual preventive plan within the planning stage, and in the professional teeth cleaning implementation stage (p<0.05). This result can be attributed to the sympathy of the dental hygienist (p<0.05). There was a positive correlation between recommending dental checkups and regular checking of the (r=0.552, p<0.05), undergoing radiography (r=0.434, p<0.01), following an individual preventive plan (r=0.568, p<0.01), undergoing proximal machine teeth cleaning (r=0.437, p<0.05), following tooth brushing instructions (r=0.552, p<0.05), and the evaluation results (r=1.000, p<0.05). Our results demonstrate, that the clinical preventive care program, based on dental hygiene, is an effective program. Given the positive effect of dental revisits and patient recommendations promoting dental hygienists, it is hoped that this preventative program will be widely used.

The Effect of Maladaptive Perfectionism, Self-leadership, and Social Support on Nursing students' Clinical Practice Stress

  • The purpose of this study was to identify factors influencing maladaptive perfectionism, self-leadership, and social support on nursing students' clinical practice stress. Participants were 150 nursing students who were enrolled at four universities in Chungcheongbukdo. Data were collected using questionnaire by from 03 to 30 November, 2022. The measurement tools consisted of the Almost Perfect Scale-Revised, Self Leadership Questionnaire-Revidsed, Scale of Social Support, and Clinical Practice Stress Scale-Revised. Results revealed that higher levels of maladaptive perfectionism were associated with increased clinical practice stress. Decreased self-leadership, satisfaction with nursing major, number of clinical practice in other regions, and social support was also associated with increased clinical practice stress. Findings indicate that specific characteristics such as maladaptive perfectionism and decrease of social support can lead to increase clinical practice stress. In this paper, we propose that active considerations for these psychological characteristics are important when reducing clinical practice stress for nursing students.

Comparative Study of Acticoat® & Allevyn® on Infected Full-thickness Wound of the Rat Skin (흰쥐의 감염된 전층피부결손에 미치는 은 함유 드레싱제재와 폴리우레탄 폼 드레싱제재의 비교관찰)

  • Purpose: $Allevyn^{(R)}$(Smith & Nephew, England) is a type of polyurethane foam material with good wound discharge absorption. $Acticoat^{(R)}$(Smith & Nephew, England) is a silver coated dressing material which is effective in infected wound. The purpose of this study is to compare the effects of dry gauze, $Acticoat^{(R)}$ and $Allevyn^{(R)}$ on infected full-thickness wound healing in rat. Methods: One hundred and twenty rats were divided into 3 groups: group I(dressing with dry gauze, n=40), group II(dressing with $Allevyn^{(R)}$, n=40), group III(dressing with $Acticoat^{(R)}$, n=40). A $15{\times}15mm$ square full-thickness wound was made on the dorsum and left open for 24 hours. The size of wound defects were measured each dressing changes. The histological evaluation was performed on the 3rd day, 7th day, 14th day, 21th day. Results: After the wound was left open for 24 hours, typical findings of bacterial infection was observed. After the 7th day, group III showed larger area of epithelialization, smaller defect size compared to those of two other groups. Complete replacement by fibrotic scar tissue was observed in group III with no signs of inflammation on the 14th day. By day 21, the average defect size in group III was decreased from initial 100% to 3.63%. while in group I and II, it was decreased to 62.66% and 53.62%, respectively. Conclusion: $Acticoat^{(R)}$ is an effective tool in the treatment of infected wound.

Histological Validation of Cardiovascular Magnetic Resonance T1 Mapping for Assessing the Evolution of Myocardial Injury in Myocardial Infarction: An Experimental Study

  • Objective: To determine whether T1 mapping could monitor the dynamic changes of injury in myocardial infarction (MI) and be histologically validated. Materials and Methods: In 22 pigs, MI was induced by ligating the left anterior descending artery and they underwent serial cardiovascular magnetic resonance examinations with modified Look-Locker inversion T1 mapping and extracellular volume (ECV) computation in acute (within 24 hours, n = 22), subacute (7 days, n = 13), and chronic (3 months, n = 7) phases of MI. Masson's trichrome staining was performed for histological ECV calculation. Myocardial native T1 and ECV were obtained by region of interest measurement in infarcted, peri-infarct, and remote myocardium. Results: Native T1 and ECV in peri-infarct myocardium differed from remote myocardium in acute (1181 ± 62 ms vs. 1113 ± 64 ms, p = 0.002; 24 ± 4% vs. 19 ± 4%, p = 0.031) and subacute phases (1264 ± 41 ms vs. 1171 ± 56 ms, p < 0.001; 27 ± 4% vs. 22 ± 2%, p = 0.009) but not in chronic phase (1157 ± 57 ms vs. 1120 ± 54 ms, p = 0.934; 23 ± 2% vs. 20 ± 1%, p = 0.109). From acute to chronic MI, infarcted native T1 peaked in subacute phase (1275 ± 63 ms vs. 1637 ± 123 ms vs. 1471 ± 98 ms, p < 0.001), while ECV progressively increased with time (35 ± 7% vs. 46 ± 6% vs. 52 ± 4%, p < 0.001). Native T1 correlated well with histological findings (R2 = 0.65 to 0.89, all p < 0.001) so did ECV (R2 = 0.73 to 0.94, all p < 0.001). Conclusion: T1 mapping allows the quantitative assessment of injury in MI and the noninvasive monitoring of tissue injury evolution, which correlates well with histological findings.
