Is Job Insecurity Worse for Mental Health Than Having a Part-time Job in Canada?

  • Objectives: A growing number of people depend on flexible employment, characterized by outsider employment status and perceived job insecurity. This study investigated whether there was a synergistic effect of employment status (full-time vs. part-time) and perceived job insecurity on major depressive disorder. Methods: Data were derived from the 2012 Canadian Community Health Survey-Mental Health of 12 640 of Canada's labor force population, aged 20 to 74. By combining employment status with perceived job insecurity, we formed four employment categories: full-time secure, full-time insecure, part-time secure, and part-time insecure. Results: Results showed no synergistic health effect between employment status and perceived job insecurity. Regardless of employment status (full-time vs. part-time), insecure employment was significantly associated with a high risk of major depressive disorder. Analysis of the interaction between gender and four flexible employment status showed a gender-contingent effect on this link in only full-time insecure category. Men workers with full-time insecure jobs were more likely to experience major depressive disorders than their women counterparts. Conclusions: This study's findings imply that perceived job insecurity may be a critical factor for developing major depressive disorder, in both men and women workers.

Associations of Spiritual Well-being, Attitude toward Death and Quality of Life among Alcoholics Anonymous (익명의 알코올 중독자의 영적 안녕, 죽음에 대한 태도와 삶의 질의 관련성)

  • Purpose: The aim of the study was to investigate associations of spiritual well-being, attitude toward death, and quality of life among Alcoholics Anonymous (AA). Methods: This study was cross-sectional and descriptive in design. The data was collected from August to September 2018 with 133 AA members drawn from two provinces of South Korea. The data were analyzed using descriptive statistics, independent t-tests, one-way ANOVA with Turkey tests, Pearson's correlation coefficients, and hierarchical multiple linear regression analyses using SPSS/WIN 20.0 program. Results: The existential spiritual well-being (β=.52, p<.001), attitude toward death (β=.24, p<.001), dual diagnosis (β=-.17, p=.003), occupation (β=.12, p=.035) of the participants were significant factors, which explained 63.7% of the variance of quality of life. Conclusion: The study findings highlight the need to develop psychological nursing strategies to enhance the spiritual well-being and improve a positive attitude toward death based on the job and dual diagnosis among AA members to improve their better quality of life.

Prevalence of Dementia in the Elderly of an Urban Community in Korea (도시지역 노인들의 치매유병률에 관한 단면조사연구)

  • Objectives: In Korea, as in most countries, there will be a sharp increase in the number of dementia patients in the near future. However basic data on dementia prevalence, which is important in defining epidemiologic characteristics and in implementing preventive strategy, are limited. This study was conducted to estimate the prevalence rate of dementia in the urban elderly aged 65 or older in Kwangmyung, Korea. Methods: A two phase design was used for case finding and case identification. In phase I, a representative sample aged 65 or older was selected and interviewed by door-to-door survey with a Korean version of the Mini-Mental State Examination (K-MMSE). In phase II, Of the 946 subjects interviewed in phase 1,356 elderly were randomly selected disproportionately according to K-MMSE score. Of these elderly, 223 (61.5%) underwent standardized clinical evaluations, including psychiatric interview, neurological examination, and neuropsychological assessment. Dementia was diagnosed by the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria. The diagnosis of Alzheimer's disease (AD) was made by National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer's Disease and feinted Disorders Association(NINCDS-ADRDA) criteria and vascular dementia (VD) by DSM-IV. Results: The overall weighted prevalence rate of all dementia among Kwangmyung residents aged 65 or older was 12,8%(age-adjusted rate: 13,0%, 95% Confidence Interval[CI]: 10.6-15.3%). Women had much higher prevalence rate than men even when age was controlled(15.9%[95% CI 12.6-19.2%] vs 7.5%[95% CI 4.0-10.4%]), The rates of dementia were 5.2%, 12.2%, 17.0%, and 34.3% for the age groups of 65-69, 70-74, 75-79 and 80 and over, respectively. The rate of AD appeared to be slightly higher than that of VD(5.2% vs 4.8%), though not statistically significant. Most of the cases(69%) were mild dementia according to CDR(<1) in these subjects. Conclusions : These results showed that the prevalence rate of dementia among urban elderly in Korea appears to be higher than those of other Asian countries.

THE TREATMENT RESULTS OF A DAY TREATMENT PROGRAM FOR CHILDREN WITH DEVELOPMENTAL DISORDERS (발달 장애 아동에게 시행된 주간 치료실 프로그램의 치료 결과에 대한 고찰)

  • Objective:This was conduced to evaluate the effect of early intervention to promote the development of children with developmental disorders. Methods:49 children(31 in PDD group and 18 in DLD group) participated in a one year day treatment program conduced from 1996 to 1999. They were performed PEP, CARS, and SMS. They were grouped by diagnosis, comorbidity, chronological age and CARS score at the beginning of the program and the treatment effect was compared. Results:The children who participated in the day treatment program showed significant increase in their PEP, SMS score, and decrease trend in their CARS score. When children were grouped by diagnosis, comorbidity, chronological age, and severity in CARS score we did not find out significant difference between groups. Conclusion:Our data suggest that the day treatment program which emphasis on development is effective in treating children with developmental disorders.

A Phenomenological Study of Tension Relaxation in Middle-Aged Women with Sleep Disorders in Singing Bowl (싱잉볼의 수면장애 중년여성의 긴장이완에 관한 현상학 연구 -소리치료, 싱잉볼(singing bowl)적용-)

  • Middle-aged women experience diverse psychological and physical stresses and tensions through the menopause, aging, and taking up too many social roles, which often leads to sleep disorder. But there are not many scientific researches on the issue. Therefore, the research of efficacy of singing bowl that helps relax middle-aged women who have sleep disorder is needed to improve their health. This study was conducted with Giorgi's phenomenological method with 4 middle-aged women with sleep disorder, whose Pittsburgh sleeping quality scale marked over 5 points, who took part in singing bowl experience for 40 minutes and did the interview. The study resulted in following conclusions that through participants' singing bowl experience, 518 constructive meanings, 15 meaning units, 7 sub components, and 2 group subjects were drawn up. Firstly, when participants took part in the singing bowl experience, they underwent physical, mental, imagery experiences which are physical pain and muscle relaxation, mental relaxation, and positive mindset and imaginary images and colors. Secondly, physical changes that the participants felt after singing bowl experience was increased physical relaxation, improved physical symptoms and more physical activities, and psychologically participants also felt reduced anxiety, increased happiness and positive mentality. And behavioral changes are relaxed movements, increased activities, and improved sleep is huge benefit through changes of sleep pattern. In this study, singing bowl is confirmed to be effective both physically and psychologically on relaxing the middle-aged women's tensions that are caused by sleep disorders and singing bowl experience is meaningful in that the middle-aged women as well as modern people with the sleep disorders can be helped to have better lives.

ASSOCIATION BETWEEN THE PERCEIVED PARENTING AND PROBLEM BEHAVIORS IN KOREAN MALE ADOLESCENTS (청소년에서 자녀가 지각한 부모 양육태도와 문제 행동의 연관성 - 서울시내 남자 고등학생을 대상으로 -)

  • Objectives:Dysfunctional parent-child relationships have been associated with various mental and conduct disorders in adolescence and mental problems in adulthood as well. Most studies have done in clinical settings and little is known about the relationship between parenting and adolescent problem behaviors in community settings. This study is done to compare the perceived parenting and problem behaviors between problem behavior group and those of non-problem behavior group in male adolescents. Methods:The subjects of this study were 147 male high school students in Seoul, who do not have any known mental or conduct disorders and live with their real parents. Parental Bonding Instrument and Youth Self Report were administered to evaluate the perceived parenting and problem behaviors of adolescents. Results:There were significantly higher scores of mother overprotection in problem behavior group when comparing to non-problem behavior group. There was a significant association between mother overprotection and problem behavior when adjusting the number of family members, mother's job, parents' education level, grades, and religion. Conclusion:The parenting such as mother overprotection can be helpful to predict the problem behaviors in male adolescents, and appropriate parenting may prevent various problem behaviors in male adolescents.

Results of Public Awareness Survey of Panic Disorder at the 2014 Mental Health Exposition in Seoul (2014년 정신건강박람회 공황장애 인식도 조사)

  • Objective : Despite high prevalence and effective treatments of panic disorder, lots of patients are not properly treated due to lack of awareness of the disorder. This study summarizes and presents the results from Public Awareness Survey of Panic Disorder during the Mental Health Exposition held in Seoul in April, 2014. Methods : A total of 401 participants who visited the booth of the Korean Academy of Anxiety Disorder agreed and completed the survey. The questionnaires comprised of three sections; first, after given a case of patient with panic disorder, participants were asked to choose a diagnosis and treatment options. Second, participants were asked to differentiate the symptoms of panic disorder from those of other mental disorders and answer where they acquired the knowledge of the disorder. Third, visual analogue scales were used to get more detailed information for several issues about panic disorder. The incidence and ratio for each question were provided and compared. Results : Among the participants, 78% reported a patient within the case need treatment, and 30% accurately recognized it was panic disorder. As for treatment needed, 40% selected psychotherapy by psychiatrists, 28% chose counseling by psychologist, 23% said that they can overcome it by self-care. Only 2% of participants selected the pharmacotherapy as treatment needed. Approximately 40% of participants have encountered information about the disorder from gossips of celebrities, 32% from mass-media, and merely 6% from medical professionals. About 80% of participants could discriminate the symptom of panic disorder from those of depression, schizophrenia, or generalized anxiety disorder. Conclusion : Our results suggest that substantial proportions of participants have the awareness of panic disorder, while as for treatment they were strongly biased against pharmacologic treatments. Most of their source of the awareness was not relied upon professional information. Efforts for giving correct information and increasing public awareness of panic disorder are needed to bridge a gap between professionals and general public.

Cardiac Complications Associated with Eating Disorders in Children: A Multicenter Retrospective Study

  • Purpose: Eating disorders often result in somatic complications, including cardiac abnormalities. Cardiac abnormalities may involve any part of the heart, including the cardiac conduction system, and can lead to sudden cardiac death. The current study aimed to evaluate the incidence of cardiac complications in pediatric patients with eating disorders and their associated factors. Methods: We retrospectively analyzed patients aged 10-18 years who were diagnosed with DSM-V (Diagnostic and Statistical Manual of Mental Disorder-V) eating disorders and underwent electrocardiography (ECG) and/or echocardiography between January 2015 and May 2020. Results: In total, 127 patients were included, of whom 113 (89.0%) were female. The median body mass index (BMI) was 15.05±3.69 kg/m2. Overall, 74 patients (58.3%) had ECG abnormalities, with sinus bradycardia being the most common abnormality (91.9%). Patients with ECG abnormalities had significantly lower BMI (14.35±2.78 kg/m2 vs. 16.06± 4.55 kg/m2, p<0.001) than patients without ECG abnormalities, as well as lower phosphorus and higher cholesterol levels. Among the 46 patients who underwent echocardiographic evaluation, 23 (50.0%) had echocardiographic abnormalities, with pericardial effusion being the most common (60.9%). The median left ventricular mass (LVM) and ejection fraction were 67.97±21.25 g and 66.91±28.76%, respectively. LVM and BMI showed a positive correlation (r=0.604, p<0.001). After weight gain, the amount of pericardial effusion was reduced in 3 patients, and 30 patients presented with normal ECG findings. Conclusion: Cardiac abnormalities are relatively frequent in patients with eating disorders. Physicians should focus on this somatic complication and careful monitoring is required.

The Impact of M&L Psychotherapy on the Growth and Development of a Socially Disadvantaged Adolescent with Anxiety Disorders - Quantitative and Qualitative Analysis of a Single Case (M&L 심리치료가 불안장애를 가진 취약계층 청소년의 건강한 성장에 미치는 영향 - 단일증례의 양적, 질적 분석)

  • Objectives: This study investigates the impact of M&L psychotherapy on an adolescent with anxiety disorders transitioning into adulthood, using a retrospective mixed-method approach. Methods: An adolescent with Diagnostic Statistical Manual of Mental Disorders (DSM)-5-diagnosed anxiety disorders underwent combined treatment with M&L psychotherapy and complex Korean medicine therapy over a period of approximately two years. Patient records tracked the diagnosis, treatments, and progress. In-depth interviews were also conducted. The study used a convergent parallel mixed-method approach, integrating quantitative analysis from psychological assessments, including the State-Trait Anxiety Inventory (STAI-X), the Core Seven Emotions Inventory-short form (CSEI-s), and the Minnesota Multiphasic Personality Inventory-2 (MMPI-2), with qualitative analyses of traditional Korean medicine counseling session notes, interviews, and other qualitative psychological assessments. Results: State anxiety decreased from 40 to 36, and trait anxiety decreased from 42 to 34. MMPI-2 L scale scores decreased slightly to 64T, whereas S scale scores increased to 62T, and FRS scores decreased to 70T from an initial score of 78T. Qualitative analysis revealed an increase in the MMPI-2 Es scale, identified as "subjectivity." Joy (喜) emerged as the highest emotional profile score in the CSEI-S. Scores for anger (怒), thought (思), fear (恐), and fright (驚) decreased, indicating improved negative emotions. Qualitatively, expressions of determination to "move forward" and "strengthen the mind" were noted, suggesting enhanced "lower danjeon (下丹田)." Conclusions: Integrating traditional Korean medicine treatment with M&L psychotherapy for a vulnerable adolescent with anxiety disorders demonstrated efficacy. Further research is warranted to substantiate the effectiveness of M&L psychotherapy and advocate for its wider adoption in mainstream practice.

Effects of Mental Health Levels and Oral habits on Temporomandibular Joint Symptom in Some Adolescents (일부 청년기 성인의 정신건강과 구강습관이 턱관절 증상에 미치는 영향)

  • This study investigates the effects of mental health and daily oral habits on temporomandibular joint symptoms. The study was conducted from September 1 to 30, 2019, enrolling a total of 402 young adults. We determined that mental health levels of anxiety, depression, and social maladjustment are significantly different, depending on life satisfaction (p<0.001), subjective health status (p<0.001), and trauma (p<0.01). Treating jaw joints exerted a significant difference in social maladjustment (p<0.05). Temporomandibular joint symptoms are significantly higher among women (p<0.05), drinkers (p<0.01), trauma (p<0.01) and jaw joint treatment patients (p<0.001). Mental health levels (p<0.001), oral habits (p<0.001), and temporomandibular joint symptoms (p<0.001) are all positively correlated. In addition, oral hygiene has the greatest influence on temporomandibular joint symptoms of trauma. Furthermore, factors experienced during treatment of the jaw joints also affect the temporomandibular joint symptoms. We conclude that it is imperative to improve oral health and malpractice in treating jaw joint disorders, and also develop a program that effectively improves jaw joint health, by combining mental health counseling and daily lifestyle improvements.
