중.고등학교 여학생의 월경 특성과 월경곤란증에 관한 연구 (A Study on Middle and High School Girl Students' Menstruation Characteristics and their Menstruation Dysmenorrhea)

  • Middle and high school girl students' menstruation characteristics, appearance of menstruation dysmenorrhea, and coping with the menstruation dysmenorrhea were examined in order to give information in instructing girl students about menstruation dysmenorrhea, in delivering health education in the school, and establishing health policy. 970 girl students in middle and high schools in Cheju-do were selected and surveyed using the questionnaire the author developed based on the previous studies. The findings are as follows. First, the age of first menstruation was average $12.9{\pm}1.22$ years. The younger they were, the earlier their first menstruation came. The most common menstrual cycle was such one that continuously irregular from the beginning or that regular and irregular cycle run together. The first irregular menstrual cycle changed regular one within 1 or 2 years. Most of the respondents had normal menstrual cycle of $21{\sim}35$ days and menstruation period of $3{\sim}8$ days. Second, symptom of menstruation dysmenorrhea was orderly; hurt in the underbelly, tired, hurt in the waist, and headache. It was higher with high school students than middle school students. For the most part, time of heavy menstruation dysmenorrhea was the 2nd day after menstruation starts and period of menstruation dysmenorrhea was 2 days. More than the half of the respondents felt that the present menstruation dysmenorrhea was similar with the first one and they felt not comport in daily life due to menstruation dysmenorrhea, and 'hard to live a normal life' was higher with the high school girl students than with the middle school students. Third, how to cope with menstruation dysmenorrhea was enough sleep, relax and rest, taking a hot water shower, listening to music or watching a movie, taking medicine, and doing excercise. Few of the respondents have ever visited a doctor due to menstruation dysmenorrhea, and the reason was mostly irregular menstruation and heavy dysmenorrhea. Forth, when they took medicine due to menstruation dysmenorrhea, they gathered information on the medicine from family members and friends, while extremely low from a pharmacist or a doctor. The most of them have ever took medicine during menstruation once and took standard dose. While excess dosage was higher with high school students than with middle school students. From these findings, a good many girls are experiencing menstruation dysmenorrhea since their first menstruation. To discover early the severely abnormal findings and to treat, more interest from home and school, counseling and sexual education on time, and expert's diagnosis are necessary. Rather than unconditionally taking pain-relievers which can plainly solve the pain, continuous health education at home and in the school should be carried on for the girls to practice proper measures one think effective or an expert recommends.

여고1학년 학생들의 월경양상에 대한 조사연구(I) (The Research for First Grade High School Girl Students' Menstruation(I))

  • Purpose : Because girls at puberty· are lack in sex ability, temporary menstruation disorder can be occured. This disorder is considered that will be disappeared as growing, so people used to leave the disease untreated and just watched. But clinically I frequently experience not to disappear. So I have carried out this study to investigate the actual condition of young girls's menstrual disorders. Methods : I researched 440 high school girls in Pusan by Menstruation Diary which I made about menstrual cycle, duration, amount and pain. The results were managed by the ststistics. Results :1. Menstrual cycle 1) In disorder of menstrual cycle, persons who have Bate menstruation are more than persons who have premature menstruation. 2) Persons who have normal menstrual cycle are in 124 persons(28.51%), the others who have severe premature menstruation or late menstruation more than one time for 4-7 months are in 311 Persons.(71.49%) 2. Menstrual duration and amount 1) Persons who have normal menstrual amount we in 66-89%, hypermenorrhea is in 1-11%, hypomenorrhea is in 5-21%. 2) In the study of menstrual duration, persons more than one thirds are irregular in thier menstrual amount every menstruation. 3) In the study of MMQ, persons who are irregular in thier menstrual amount every menstruation are in 125persons.(29.76%) 3. Menstrual pain 1) Persons who have slight menstrual pain are in 289 persons(65.98%), the middle is in 86 persons(19.63%) the severe is in 34 persons(7.76%) by MMP. 2) Persons who are irregular in thier menstrual pain every menstruation are in 145 persons.(33.11%) Conclusion : In menstrual cycle, there are more persons who have irregular menstrual cycle than normal.(71.49%) In menstrual duration and amount, more persons have normal menstrual amount.(66-89%) In menstrual pain, persons who have slight menstrual pain are the most.(65.98%)

월경과 수면 (Menstruation and Sleep)

  • There are several factors which are more likely to have sleep disorders in fertile women with menstruation than adult men. Menstrual cycle plays an important role in them. We describe herein the overview about the association of menstrual cycle and sleep disorders by viewing the interactions of menstrual cycle and circadian rhythm. We review how menstrual cycle affects sleep-wake cycle by reviewing menstrual cycle and estrous cycle to understand these interactions. Menstrual cycle and estrous cycle are mainly affected by hormonal cycle and light-dark cycle, respectively and they are generally determined in monthly rhythm and annual rhythm, respectively. The determination of estrous cycle is also affected by cyclic changes of hormones besides light-dark cycle. Although sleep-wake cycle almost alternates according to estrous cycle in non-primate mammals, it is hardly affected by menstrual cycle in primate mammals as compared with estrous cycle. But menstrual cycle affects sleep-wake cycle via desynchronization of sleep-wake cycle and temperature rhythm. The decrease of amplitude and phasic change during luteal phase in the daily fluctuation of body core temperature can partially contribute to the induction of sleep disorders in fertile women. In addition to this, premenstrual syndrome which nearly happens during luteal phase commonly have sleep problems. Therefore, we suggest that menstrual cycle and PMS can partially contribute the increase of sleep disorders in fertile women.

한국과 미국 여대생의 월경전후기 불편감, 월경에 대한 태도, 성역할 수용도의 횡문화적 차이에 관한 연구 (Transcultural Differences on Perimenstrual Discomforts, Menstrual Attitudes and Sex Role Acceptance between Korean and American College Students)

  • This study was performed to compare the transcultural differences on perimenstrual discomforts, menstrual attitudes and sex role acceptance between Korean and American college students. The subjects, 2557 nursing students were selected from 13 universities all around Korea, and 4 universities in the eastern, western, and middle areas of the United States. The data were collected using the MDQ (Menstrual Distress Questionnaire by Moos), the MAQ(Menstrual Attitude Questionnaire), and the Sex Role Acceptance Scale, the subjects answered the MDQ for three points (premenstrual, menstrual, intermenstrual based on their recollections) between July, 1996 and April, 1997. The findings are as follows : 1. There were significant differences in the age of menarche, duration of menstruation, and menstrual cycle between the Koreans and the Americans. 2. As for sex role acceptance, Americans more frequently denied the traditional female role than did Koreans. 3. There were significant differences between the two groups on five categories of menstrual attitude (menstruation is a phenomena that weakens women physically and psychologically, menstruation is a bothersome phenomena, menstruation is a natural phenomena, menstruation onset is a predictable phenomena, and menstruation is a phenomena that does not influence womens' behaviors non is expected to). 4. For the menstrual discomfort scores, there were significant differences between the two groups on all six categories of the MDQ(pain, attention deficit, behavioral changes, AMS response, water retention, and negative emotion) for the three points (premenstrual, menstrual, intermenstrual period). 5. The most common complaints for the Korean women were pain during the perimenstrual period and fatigue during the intermenstrual period. In contrast, for the Americans the most common complaint was negative emotions during the perimenstrual period, but less pain than the Koreans, and a similarity to Koreans during the intermenstrual period. 6. Regularity of the menstrual cycle was positively correlated with age of menarche, menstrual cycle, and duration of menstruation. 7 For the interrelationship between of Perimenstrual discomforts, menstrual attitudes, and sex role acceptance, there were significant correlations for both group as follows : First, the MA category "menstruation is a bothering phenomena" was negatively correlated with sex role acceptance, second, the MA category "menstruation is a phenomena that weakens the women physically and psychologically" was positively correlated with the MA category "menstruation onset is a predictable phenomena." For the further research, it is recommended that efficient nursing intervention programs for perimenstrual discomforts, be developed and qualitative research to demonstrate the cultural differences as the feminism view point be done.

Recurrent hemoptysis in a 26-year-old woman with a ground-glass opacity lesion of the lung

  • Hemoptysis is a major reason for emergency department (ED) visits. Catamenial hemoptysis (CH), a rare condition of thoracic endometriosis, can cause recurrent hemoptysis but is difficult to diagnose in the ED due to the scarcity of cases and nonspecific clinical findings. We report a case of a 26-year-old woman who presented to the ED with recurrent hemoptysis since 2 years without a definite cause. Her vital signs and blood test findings were unremarkable. Chest computed tomography (CT) did not show any specific lesions other than a non-specific ground-glass opacity pattern in her right lung. She was on day 4 of her menstrual cycle and her hemoptysis frequently occurred during menstruation. Although there was no histological confirmation, based on her history of hemoptysis during menstruation and no other cause of the hemoptysis, the patient was tentatively diagnosed with CH and was administered gonadotropin-releasing hormone. She had no recurrence of hemoptysis for 3 months. While CH is difficult to diagnose in the ED, the patient's recurrent hemoptysis related to menstruation was a clue to the presence of CH. Therefore, physicians should determine the relationship between hemoptysis and menstruation for women of childbearing age presenting with repeated hemoptysis without a definite cause.

다낭성난소증후군으로 인한 무월경, 희발월경 환자 20명의 증례 보고 (Case Report of 20 Amenorrhea or Oligomenorrhea Patients due to Polycystic Ovarian Syndrome)

  • Objectives: We evaluated the effect of Korean medical treatment in case of irregular menstruation (amenorrhea, oligomenorrhea) due to Polycystic ovarian syndrome (PCOS).Methods: 20 Amenorrhea or oligomenorrhea Patients with PCOS were treated with Korean medical treatment at least 3 months. We analyzed clinical data of 20 patients and checked effectiveness of treatment through the change of menstrual pattern.Results: After Korean medical treatment, Patients who complained menstrual irregularity due to PCOS were improved in menstrual condition. Among 14 amenorrhea patients, 12 patients (85.7%) started menstruation within 3 months. Among 6 Oligomenorrhea patients, 4 patients (66.7%) got better menstruation pattern than before.Conclusions: PCOS Patients who got Korean medical treatment showed better condition in menstruation pattern. Amenorrhea patients started menstruation without contraceptive pill and oligomenorrhea patients got better menstruation cycle.

경폐(經閉)(무월경(無月經))에 관(關)한 임상적(臨床的) 고찰(考察) (A Clinical Study on Amenorrhoea(經閉) Patients)

  • An amenorrhoea means the pathologic condition that menstruation stops before the post menopausal period. Clinically, this is classified to both primary amenorrhoea and secondary amenorrhoea. Primary amenorrhoea indicate what has no first menstruation until 14 without secondary sexual sign or what has no first menstruation until 16 with secondary sexual sign before the time. Secondary amenorrhoea is diagnosised when a women with normal menstrual cycle before the onset doesn't have a menstruation over 6 months or appeals amenorrhoea for three times of her normal menstrual cycle. This clinical study was done on the patients who had come to gynecology department of oriental Hospital of Kyung Hee Medical Center from August 1, 1994 to July 31, 1995, complaining of amenorrhoea. The results obtained were as follows: 1. The amenorrhoea patient rate among outpatients who came to the deptment of gynecology was 3.4%. 2. The ratio between primary amenorrhoea and secondary amenorrhoea was 1 : 6.5. 33. The patient rate via other hospitals was 75% and Unremarkable finding(47.6%) was most numerous according to other hospital's diagnosis and hyperprolactinemia(11.9%), premature menopause(11.9%) were the second numerous diagnosis. 4. Unremarkable history(56.7%) was most numerous and among history, the fast(weight loss; 30%) was most numerous. 5. The most general symptom of amenorrhoea patient was indigestion(51.7%). 6. The most frequently used prescribtion for non-insurance was Onpojongoktang(溫胞種玉湯 ; 55%), for insurance was Gamisoyosan(加味逍遙散 ; 16.7%). 7. 25% patient show menstruation in their therapy and among this, 80% patient show menstruation within 40 days. 8. 26.9% secondary amenorrhoea patient show menstruation in their therapy and only one primary amenorrhoea patent(12.5%) shows the same result. 9. Among the effective used prescribtion, Onpojongoktang(溫胞種玉湯 ; 46.7%) is most numerous.

일부 도시지역에 있어서 중, 고교생의 월경에 관한 조사연구 (Survey on Menstruation of Middle & High School Girls in an Urban Area)

  • The conclusions were obtained from the detailed survey of 1,285 students experiencing menstruation, who were chosen among 1,717 students in middle & high school girls in Seoul. The survey was conducted during July 20-July 24, 1971. 1. Age of Menarche An average age of menarche was 13.3$\pm$1.07. The earliest age of menarche was 9 and the latest age 18. Ages of menarche were between 12 and 14 in 84.3 percent of the students surveyed. a. By present age distribution, the aged students were lower, than younger students in the average age of menarche. b. By father′s educational levels, among the students whose fathers were graduated from high schools, college & ever the earliest average age of menarche was found with 13.2, and among the students whose fathers were graduated from primary schools that wag latest with 13.6. c. By father′s occupations, among the students whose fathom engaged in "workers not classifiable"the earliest age of menarche was found with 12.5$\pm$0.27, among the students whose fathers were in "service business"the second was 12.9$\pm$1.07, and among the students whose fathers were in "miners, quarrymen and related workers"that was latest with 13.8$\pm$1.14. d. By economic status, among the students of "wealthy"families the age of menarche was 13.1$\pm$0.25, the among the students of "ordinary"families the lags of menarche 13.3$\pm$1.06. and the among the students of "poor" families that was 13.8$\pm$0.31. e. By home discipline, among the students being treated "rigid" the age of menarche was 13.5$\pm$1.13, among the students being treated "moderate"the age of menarche was 13.3$\pm$0.22, and those being treated "indifferent" that was 13.0$\pm$0.26. f. By students physical condition, among the students of "good" condition the average of menarche was 13.3$\pm$0.16, and among the students "poor" that was 13.5$\pm$0.31. 2. Menstruation a. For the six months after the average of menarche 39.0 percent of the students had normal menstruations, and 61.3 percent of them had abnormal ones. Of the students with abnormal menstruation 21.7 percent had abnormal menstruation from time to time, 25.4 percent had no menstruation for one month to three months, 7.2 percent had menstruation for four to six months and 6.7 per cent had no menstruation for more than sin months. Most students became to have normal menstruations a few months later the age of menarche. b. At the time interviewed, the percentile of cycle of menstruation as following: 23 days types: 46.8 percent 30 days types: 40.6 percent others : 12.6 percent The average cycle of menstruation was every 28.9 days. c. The average duration of menstruation is 4.69 days. d. The subjective symptoms during menstruation period: Out of the total 89.7 per cent had some pains, while 10.3 percent had no symptom. Among the symptoms, abdominal pain occupied 29.9 percent, neurotic symptoms 19.0 percent and lumbago 15.1 percent. e. By attitude or Action at first physical change, "Treated it by own experience" : 30.0 percent "Don′t know what to do because of ignorance" : 20.1 percent "Asked others about it" : 43.0 percent

월경중 염미감각의 변화에 관한 연구 (Salt Taste Acuity and Menstruation)

  • It was the purpose of this study to determine if the changes in the sense of taste occur with the periods of the menstrual cycle in 14 women aged 20 to 22 years. Results showed that the perceived intensity responses to different suprathreshold salt concentrations and the ad libitum salting level in soybean sprout soup did not differ significantly according to the different periods of the menstrual cycle. But women in the three or five days period previous to menstruation were sensi\ulcornertive at the lower salt concentration of 0.25%, but, on the contrary, were insensitive at the higher concentration of 1.25%, with increasing ad libitum salt preference in soybean sprout soup. Also, they were more or less high in the intensity slopes of perceived saltiness on the linear regression. The data suggests that a physiological mechanism for increasing salt intake may develop during the three or five days right before menstruation.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 마황행인감초석고탕(麻黃杏仁甘草石膏湯) 투여 후 호전된 증례 2례 고찰 (Two Case Reports treated by Mahwang-Haeangin-Gamcho-Seokgo-tang based on Shanghanlun Provisions)

  • Objectives: In this paper, two cases which showed the meaningful results on the patients' chief complaints were analyzed. The patients were treated with the Mahwang-Haengin-Gamcho-seokgo-tang herbal medication based on Shanghanlun disease pattern identification diagnostic system. Methods: The patients were diagnosed based on Shanghanlun, disease pattern identification diagnostic system. In case 1, the change of menstruation cycle was noted and pre-menstrual discomforts were measured with Menstrual Distress Questionnaire(MDQ). In case 2, Quality of life questionnaire for adult Korean asthmatics (QLQAKA) was used to estimate the quality of the patient's life. Results: All the symptoms were improved after the Mahwang-Haengin-Gamcho-seokgotang treatment. In case 1, the menstruation cycle decreased to 30 days average. MDQ score decreased 143 to 103. In case 2, the change of the QLQAKA score as 1.647 average point is considered as a meaningful improvement. Conclusion: With great difference to a 'Symptom-Medicine' diagnostic system, the disease pattern identification diagnostic system seeks the pathologic pattern through the patient's whole life. More studies and multiple cases based on the diagnostic system are needed to prove this possibility later.
