  • A rapid decrease of total fertility rate to 1.08 in 2005 prompted the Korean government to plan and implement a '5-year plan for ageing society and population policy' starting from 2006. The 1st and 2nd 5-year plans had not shown any discernible impact on the fertility and the 3rd 5-year plan was launched in 2016. However, the fertility rate is going down further. The author reviewed the contents and assessment reports of the fertility promotion plan to suggest ideas for complementing the shortcomings of it. Author defined the major determinants of marriage and child birth as philosophy, politics, sense of value, social norm, culture, healthcare, and education. The plan was examined in view of these determinants. Transformation of Korea from an agricultural society to an industrialized society in a short period of time had brought about changes in most of the determinants of marriage and child birth; in particular philosophy and sense of value. These aspects were not put into consideration in the plan. Author suggested to launch a social education program for the general public to establish a sound philosophy of life, reform the sense of value on family, child birth and education, and cultivate the skill to draw a consensus through discussions on the social issues. A special program to promote marriage of women at the optimum age for child birth was proposed. The government should implement well balanced policy for economic development and labor. Multidisciplinary approach was recommended for these tasks.

For the evaluation of the program, 70 North Korean young adults who were attending two alternative schools for North Korean defectors were recruited. The program had taken place once a week for 13 or 19 weeks. Effectiveness of the health promotion program was evaluated using anthropometric measurement, 3-day food records and a questionnaires that comprised the Hospital Anxiety and Depression Scale (HADS), the Short Form with 36 questions (SF-36) and health behaviors. The surveys were proceeded at the beginning and after the program. Results: After health promotion program, participants' height was significantly increased (p=0.004) and body fat mass (0=0.004) and percentage of body fat mass (p=0.003) were significantly decreased. The number of subjects who ate breakfast alone was decreased whereas the number of subjects who ate breakfast with friends was increased (p<0.001). There were no significant changes in dietary intakes, mental health status and quality of life. North Korean young adult defectors' willingness to participate and interests in the health promotion program were high, however the practice rate was low. Conclusion: The health promotion program could induce interests and willingness to participate, but bring about limited effects on the health behaviors. These results imply that a health promotion program for North Korean young adult defectors should have a long-term strategy as well as short-term plan. Furthermore, it should be based on their health problems, health related behaviors, academic performance and daily life matters.

  • Recently, South and North Korea summits agreed to implement the cooperation and promotion of medical healthcare at the third inter-Korean summit in Pyeongyang on September 18, 2018. Therefore, the South Korean government and its affiliated organizations have been looking for ways to establish a specific plan for support and exchanges in the field of healthcare and medical technology. In the background of the above new policy, radiation medicine technology can also contribute to the exchange in the field of healthcare and science & technology for the peaceful coexistence of one Korea. In this review, we analyzed and showed the status of healthcare and radiation medicine infrastructure in North Korea in order to further develop a plan to share the benefits of radiation medicine with North Korea in the exchange of healthcare and science & technology.

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level. • Objectives: This article reviewed researches on intersectoral partnership of school health programs to suggest importance, current status and development plan of those. Methods: Domestic research papers were screened though Research Information Sharing Service (RISS), Koreanstudies Information Service System (KISS), Medical Library Information System (MEDLIS), Korean Medical Database (KMbase), and digital national assembly library. International papers were searched mainly via Pubmed. Results: Since intersectoral partnership is effective, comprehensive and universal approach for school health programs, many collaborative programs are universally progressing in various domain including smoking, alcohol, mental health and sexual behavior. On the other hand, intersectoral partnership of school health programs in Korea is still in the beginning stage, and there are few intersectoral partnerships. Moreover, there are several limitations including top-down approach, passive participation and lack of studies on the effect of intersectoral partnership. However, intersectoral partnerships in school health program including tuberculosis control system in school or WEE project are in progress, so it is worth looking forward in the future. Conclusions: To achieve successful intersectoral partnership in school health programs, appropriate partnership guideline for Korean society, policy support, active participation and improvement of consciousness in community stakeholders are needed.

  • Although the New National Health Promotion Plan 2010 target to reduce health inequalities, whether the program will be effective for reducing the health inequalities in Korea remains quite unclear. More and more developed countries have been started to concentrate on comprehensive policies for reducing health inequalities. The health policies of the UK, Netherlands, and Sweden are the most wellknown. I propose that a comprehensive blueprint for tackling health inequalities in Korea should be made and that it must contain five domains: a target, structure and process, life-course approach, area-based approach, and reorganization of health care resources. The target should be based on determinants of health and more attention should be paid to socioeconmic factors. The structure and process require changes from the national health care policy based on medical services to the national health policy that involves the establishment of a Social Deputy?Prime Minister and the strengthening multidisciplinary action. A life-course approach especially focused on the early childhood years. Area-based approach such as the establishment of healthy communities, healthy schools, or healthy work-places which are focused on deprived areas or places is also required. Finally, health care resources should be a greater investment on public resources and strengthening primary care to reduce health inequalities. The policy or intervention studies for tackling health inequalities should be implemented much more in Korea. In addition, it is essential to have political will to encoruage policy action.

In Liverpool evaluation was done by a stakeholder group called Liverpool Local Involvement Network(LINK), while in Wonju by Yonsei Healthy City Research Center. The evaluation tool was two types; quantitative and qualitative analysis. Liverpool mostly used qualitative and added quantitative, vice versa in Wonju. Conclusions: Evaluation plan for Healthy city projects need to be made in the first phase of the projects, instead of in the end. Moreover, it is important to include stakeholder in conducting qualitative analysis for unquantifiable evidence of effectiveness, as well as quantitative analysis.
