The survey on life patterns and constitution in obese patients (비만증환자(肥滿症患者)의 생활행태(生活行態) 및 체질(體質)에 관(關)한 조사보고(調査報告))

  • The survey was done on 30 cases of obese patients who were treated by fasting therapy in the Dept. of Oriental Rehabilitation Medicine, oriental medical hospital in Won Kwang University from May 1995 to August 1995. The results are as follows: 1. The ratio of females to males was 1:14. The distribution was 20th decades (63%), 30th decade(13%), 10th decade(6.7%), and 40th decade(6.7%). 2. In the age of obese-prevalence, the middle to high school years was the highest number as 12 persons(40%). The 20th decade(23.3%) and 30th decade(13.3%) were the second and third highest frequency of subjects. 3. In the obesity index, 150% of ideal weight was the highest percentage with persons(40%). 120-129%(30%), 130-139%(23.3%) and 140-149%(6.7%) were in order of frequency. 4. In the family histories of obese patients, it was found that hypertension had a high incidence of 8 persons. Other evidences of family histories were DM (7), cancer (3) and CVD (2). In hereditary tendency of obese parents, it was known that hereditary tendency of obese mothers was high at 14 persons, that of obese fathers was 5 persons, and that of obese parents was 2 persons. 5. In identifying the cause of obesity, it was found that changes of diet patterns was high at 24 persons(80%). Pregnancy, birth and diseases were in order of cause. 6. The review of consumption showed that obese patients ate the same amount as non-obese persons in the case of 21 patients(70%), 8 patients(26.7%) ate more than non-obese persons and 1 person(3.3%) ate less than persons of the same ages. 7. Obese patients consumption of daily snacks was 10 persons(33%). 17 person(57%) of the obese patients ate midnight snacks every 3-4 days. 11 persons(37%) of the obese patients ate out every 3 - 4 days. 8. For the purpose of weight reduction, 15 patients(50%) used exercise. Fasting therapy(36.7%) and food restriction(33.3%) were the second and third methods used by obese patients. 9. In the relation of constitution medicine usage with obese patients, TAE-EUM-IN was 14 persons(46.7%), SO-EUM-IN was 11 persons(36.7%), and SO-YANG-IN was 5 persons(l6.7%).

Clinical Observation on Voice Disorder (음성장애에 대한 임상적고찰)

  • The tests related to air usage are valuable for evaluating phonatory function of clinical cases having glottic incompetence. Measurement of mean air flow rate, maximum phonation time and phonation quotient are important test for voice disorder. Stroboscopy is very useful for clinical evaluation of abnormality in the mode of vocal cord vibration. Author obtained following clinical result from 56 cases of laryngeal disorders in Kurume medical school in Japan. 1) Unilateral laryngeal lesions, are 35 cases (62.5%) and bilateral laryngeal lesions are 21 cases (37.5%). 2) Sex ratio is 39 cases (69.8%) of male and 17 cases (30.2%) of female. 3) In maximum phonation time below 10 seconds are 26 cases (46.4%) and above 10 seconds are 30 cases (53.6%). 4) In phonation quotient below 300 ml/sec are 33cases (58.9%). and above 300ml/sec are 23 cases (41.0%). 5) In mean air flow rate below 300ml/sec are 37 cases (66.1%) and above 300ml/sec are 19 cases (33.9%). 6) Symmetry of vibratory movement of the vocal cord, regularity of vibration, amplitude of vibration, wave on the mucosa and glottic closures are observed by stroboscopic examination. 7) Postoperative voice test and stroboscopic examination revealed good result in compare pre-operation with post-operation.

The Present State of Occupational Injuries and Prevention on East Side of Korea Fishing (동해안 연근해 어업의 산재현황과 예방대책)

  • Fishermen have higher risk of occupational injuries and disease due to frequent machine usage and direct contact to live biological materials. Moreover, growing elderly workers makes the susceptibility to occupational injuries and disease higher. This study was performed to investigate the occupational safety and health status among fishermen. The interview was carried out at Jumunjin and Geojin ports which were representative port at North East side of Korea. The structured questionnaire were used to interview the fishermen from AM 6:00 to PM 11:00 and total respondent were 97 workers. The results were followed; 7 fishermen of all respondents experienced occupational injuries during their work, 5 fishermen were needed to admission longer than 4 days. The injury types were contusion(4 persons), fracture(1 persons) and amputation(2 persons). The cause of injury might be the lack of caution and the unstability of working condition. These results suggested the ergonomical evaluation of working condition and proper management. But there was limited concern and studies on the policy on occupational safety and health on fishermen. So, further study was required to establish the sound policy of fishermen's occupational safety and health.

  • Ginsenf Radix (Panax ginseng C. A. Meyer) has been traditionally used as a herbal medicine for many therapeutic or prophylactic purposes in the oriental countries such as Korea, Japan and China for at least two thousand years and also extensively studied in the modern scientific field of chemistry, biochemistry and pharmacology. The herb is now also indicated for use as tonic or a prophylactic and restorative agent for enhancement of mental and physical capacities, in case of weahess exhaustion tiredness loss of concentration, impotence, cold limbs, during illiness anuor convalescence. Ginseng is commonly used in the form of detections, extract and powderl and ginseng products, in the form of capsules tablets and drinks. And also ginseng radix has been widely traditionally prescribed as an important comuonents of manny Chinese prescriptions or alone in various diseases and for health with its different dosages. Nowadays since rinsenf can be generally classified into food or medicine in many nations, it is very difficult to give any exact desnition on the dosage, which may be of particular importance in clinical applications. In addition, the establishment of the reasonable dosage is currently of great significance to meet the demand for such wide applications. Accordingly in this review paper we summarized the dosage of ginseng on the basis of oriental medical books oriental and western pharmacopeias and modern scientific clinical data. The recent survey demonstrated that the averare dosare of finsenf is considered to be three to four grams per day unless prescribed apart, while one to two grams per day in western countries from the western viewpoint of classification of ginseng as a medicine, surrorted by the dosage of not more than one gram per day in most clinical studies. For that reason, it seems likely that the dosage in western countries is ascribed to the safety of ginseng considering side or unwanted effects. Consequently whether the differences of dosage between oriental and western countries depend on dietary habits and races should be closely investigated. Besides, further studies on the pharmacokinetics and bioavailability of ginseng components in clinical trials need to be done to decide optimum dosage of ginseng.

Floristic study of Mt. Cheongtae area (Pyeongchang-gun, Gangwon-do) (청태산 지역(강원도 평창군)의 식물상)

  • The vascular flora in the Mt. Cheongtae area, located Pyeongchang-gun in Gangwon-do, were surveyed 14 times from April of 2008 to October of 2011. The result of this survey revealed 663 taxa in total, comprising 89 families, 342 genera, 577 species, 6 subspecies, 73 varieties and 7 forms. In the flora of this area, 19 taxa were Korean endemic plants and 24 taxa were rare and endangered plants of Korea. The floristic regional indicator plants found in this area were 136 taxa comprising 4 taxa of grade V, 16 taxa of grade IV, 31 taxa of grade III, 41 taxa of grade II and 44 taxa of grade I. Naturalized plants in Korea numbered37 taxa. In addition, 485 taxa were categorized by usage into 8 groups, as follows: 240 edible, 8 fiber, 177 medical, 60 ornamental, 245 pasturing, 2 industrial, 12 dyeing and 16 timer plants.

Evaluation of interleukin-$1{\beta}$ and 8 in gutka chewers with periodontitis among a rural Indian population

  • Purpose: Smokeless tobacco-based oral-use products like gutka are popular in India. Gutka usage leads to increased periodontal destruction and inflammation; however, the relevant mechanism remains unknown. This study aimed to elucidate the role of gutka in periodontitis by examining its effect on the levels of interleukin (IL) $1{\beta}$ and IL-8 from the gingival crevicular fluid (GCF). Methods: A total of 45 patients were enrolled in this study. Thirty patients with periodontitis (15 gutka chewers [GCP] and 15 nongutka chewers [NGC]) and 15 periodontally healthy controls (HC) were selected. The full-mouth plaque index (PI), gingival index (GI), probing depth (PD), clinical attachment level (CAL), and recession (RC) were recorded. The IL-$1{\beta}$ and IL-8 levels in the GCF of all subjects were assessed through an enzyme-linked immunosorbent assay (Quantikine). Results: The IL-$1{\beta}$ and IL-8 levels were not significantly higher in the GCP group (IL-$1{\beta}$, $369.01{\pm}273.44{\mu}L$; IL-8, $205.97{\pm}196.78{\mu}L$) as compared to those in the NGC group (IL-$1{\beta}$, $195.57{\pm}96.85{\mu}L$; IL-8, $178.61{\pm}149.35{\mu}L$). More gingival RC and loss of attachment was seen among the GCP group (RC: $2.02{\pm}0.31$, P=0.013; CAL: $4.60{\pm}0.56$, P<0.001) than among the NGC group (RC, $1.21{\pm}1.15$; CAL, $3.70{\pm}0.32$); however, PD was deeper among the NGC subjects (P=0.002). PI and GI were significantly higher for the periodontitis group (P<0.001) when compared to the HC, but there was no difference among gutka chewers and non-chewers (P=0.22 and P=0.89). A positive correlation was found between the IL-8 levels and the duration of gutka chewing (r=-0.64, P<0.01). Conclusions: Gutka chewing leads to increased gingival RC and clinical loss of attachment. There was no effect seen in the proinflammatory cytokine levels in the GCF of gutka users.

Evaluation of Pharmacotherapy for the Patients with Depression in Alzheimer's Disease, Parkinson's Disease or Vascular Dementia (알츠하이머병, 파킨슨병 및 혈관성치매 환자들의 우울증에 대한 약물 치료 평가)

  • Background: Prevalence of depression comorbid with neurologic disorders such as Alzheimer' disease (AD), Parkinson's disease (PD) and vascular dementia (VD) is higher than that of primary depression. Antidepressant medications, suggested by many researches for depression comorbid with neurologic disorders such as AD, PD and VD, are mainly selective serotonin reuptake inhibitors (SSRI). Objective: The primary objective of this study is the evaluation of antidepressant drug therapy for AD, PD and VD. Method: This study was a retrospective study based on medical records, carried out for 3 years and 6 months (Jan. 2007~Jul. 2010). Patients, diagnosed as comorbid depression through Beck Depression Inventory (BDI), Cornell Depression Scale (CDS), Geriatric Depression Scale (GDS) among neurologic out-patients of Chungnam National University Hospital because of AD, PD and VD, were selected. The results were evaluated by efficacy and safety of antidepressant drug therapy. Results: In result, the prescribing rates of antidepressants were 30%, 55% and 40% for each AD, PD and VD. Depression cure rates of patients receiving antidepressants vs patients not receiving antidepressants were 40% vs 39%, 33% vs 23% and 38% vs 30% for AD, PD and VD. The frequencies of prescriptoin of SSRI were 21%, 11% and 27% for each AD, PD and VD. The frequencies of prescriptoin of benzodiazepine (BZD) was 61%, 82% and 61% for each AD, PD and VD. The ratio of single BZD prescription was more than that of combination prescription of antidepressants. Tricyclic antidepressants (TCA) were rarely prescribed. The rate of patients with BZD-related side effects was 54%. The most frequent side effects of BZD were dizziness (30%), drowsiness (21%) and headache (16%). Side effects of SSRI were rare. Conclusion: In conclusion, the frequencies of prescription of antidepressants were not common for AD, PD and VD. There was little difference in depression cure rate between patient receiving antidepressants and not receiving. Even though SSRI has to be the highest priority of usage, the frequencies of prescription of SSRI were lower than those of BZD. Additional researches and efforts are required to improve antidepressant drug therapy for neurologic disorders such as AD, PD and VD.

  • As the IT and computer network technology is developed very rapidly, the quantity of digital contents is increased and disseminated more widely. The digital contents is generally expressed in 2 or 3 dimensional multimedia format and the visible human image that is taken from human body is very important because of its variety of usefulness. The KISTI(Korea Institute of Science and Technology Information) is now constructing various Korean human informations such as visible Korean, digital Korean, human bone property and human models. These informations are accessable through the internet. However, these human images are not easily understandable for general users because they are specialized in medical image field and there is no detailed explanation data. In this study, we designed the annotation database and searching interface for KISTI's visible Korean database. This annotation database involved the detailed explanation and special note of visible Korean data and it can connect the image and text data of visible Korean with each other. By studying this database and interface design, the KISTI's visible Korean database is more easily accessable and understandable to general users and it can promote the usage of visible Korean data more widely.

Determinants on Korean Medicine Use among Breast Cancer Patients (유방암환자의 한의약의료서비스이용 결정요인)

  • Objectives : The objective of this study is to determine the present state of patients with breast cancer use of Korean medicine(KM) and predictive factors for the use. Through this, the present study is intended to present reasonable treatment approaches for patients with breast cancer as well as communicating correct information on KM to healthcare providers and presenting objective alternatives for patients with breast cancer management based on the subjects' experience in health benefits obtained from their use of KM. Methods : To collect data for the present study, questionnaire surveys were conducted on outpatients who visited four hospitals located in Seoul, Korea during around three weeks from May 31, 2012. Although the total number of the questionnaire sheet distributed in the form of directly asking questionnaire questions was 300, 12 incomplete questionnaire sheets were excluded. Therefore, the number of questionnaire sheets actually used in analyses was 288 and thus the collect rate was 96%. Results : Major results of this study are as follows. First, the number of subjects who responded to the questionnaire was 288 in total. Forty-six percent of the patients reported KM usage and the most commonly used ginseng and qigong/exercise. KM use was found to be associated with age, experiencing side effects of cancer treamnent. Factors that affect the use of KM were analyzed by Linear Logistic Regression and the results showed that age, experiencing side effects of cancer treatment, effectiveness of cancer treatment, and satisfaction of the treatment were factors that were related with relatively more frequent use of KM. Conclusions : Comparing the previous studies, it could be seen that patients with breast cancer were highly interested in and used KM in which conventional medicine and KM are used simultaneously. Knowledge on the integrative use of KM and conventional therapies is necessary for cancer physicians and traditional Korean medical doctors to help patients make informed choices. KM use may play a role in the positive benefits associated with process of breast center treatment. Healthcare providers should communicate correct information on the KM use that has been scientifically verified and talk with each other openly. The fact that the significant correlation between predictive factors for the use of KM was identified trough the present study is quite meaningful.
