  • Pregancy is a part of women's life and is common process experienced by most women. On the other hand, a few cannot be pregnant. There are many reasons of infertility and myoma uteri of pelvic mass is a rare reason of infertility. Complication of pregnancy accompanied with myoma uteri is largely affected by the size or location of mass and the incidence by myoma uteri in pregnancy rarely happened, but mass with long pedunculus causes torsion as uterus is getting bigger. In labor myoma uteri causes the abnormal condition of fetal presentation and results in the abnormal childbirth. In case of myoma uteri, maternal mortality is low and there by medical hazards also decrease, but as the life insurance medical is the extensive prediction science, we believe that the prediction of perinatal or the condition of afterbirth by knowing the location, size of myoma uteri contributes to the medical examination. We experienced one case of Elderly primigravida with myoma uteri in Med Dept of Dae Han Kyouk life Insurance Co. Ltd. which the insured with medical examination believe herself as menopause. So we report the observations with studies.

An Analysis of the Local Medical Examination Paper from the Ming Period on 'He who does not know Yi(易) cannot speak of medicine' in the Yixuetongzong(醫学統宗) (『의학통종(醫學統宗)』 「부지(不知)『역(易)』부족이언태의(不足以言太醫)」의 분석)

  • Objectives : To provide foundation for research on local medical examination of the Ming period, the contents on 'He who does not know Yi(易) cannot speak of medicine' in the Yixuetongzong(醫學統宗) written by He Jian was analyzed. Methods : Proofreading of the original texts, followed by interpretation of words and phrase and translation into Korean were done. The resulting contents were then analyzed from the perspective of the theory of Medical Yi(Yi-Yi). Results : In this medical exam paper for physicians, He answers himself on topics such as the Four Symbols[四象], Eight Symbols[八象], the Sixty-four hexagrams[Liushisigua], the combinations of the Organs and Bowels[Zangfu], Nine Orifices[Jiuqiao], and Five Body Parts[Wuti], the origin of the pulse, the reasoning behind calling the pulse 'the station for blood[xuezhifu]', what the 'Tianzhen Weihezhi Qi' stands for and the meaning of the Twelve Organs[shierguan]. He also supports the perspective of the 'Medicine and Yi having the same roots[Yi-yi tongyuan]', arguing that due to this, the body's physiology could be explained through the theories of Yi[易]. This shows that He understood the Yi-yi theory as a larger category. Conclusions :He's medical exam paper reflects his uniqueness in the field of the Yi-yi theory that precedes that of Zhao Xianke.

  • Background: Low back pain is a frequent condition that results in substantial disability and causes admission of patients to neurosurgery clinics. To evaluate and present the therapeutic outcomes in lumbar disc hernia (LDH) patients treated by means of a conservative approach, consisting of bed rest and medical therapy. Methods: This retrospective cohort was carried out in the neurosurgery departments of hospitals in KahramanmaraŞ city and 23 patients diagnosed with LDH at the levels of L3-L4, L4-L5 or L5-S1 were enrolled. Results: The average age was $38.4{\pm}8.0$ and the chief complaint was low back pain and sciatica radiating to one or both lower extremities. Conservative treatment was administered. Neurological examination findings, durations of treatment and intervals until symptomatic recovery were recorded. $Las{\grave{e}}gue$ tests and neurosensory examination revealed that mild neurological deficits existed in 16 of our patients. Previously, 5 patients had received physiotherapy and 7 patients had been on medical treatment. The number of patients with LDH at the level of L3-L4, L4-L5, and L5-S1 were 1, 13, and 9, respectively. All patients reported that they had benefit from medical treatment and bed rest, and radiologic improvement was observed simultaneously on MRI scans. The average duration until symptomatic recovery and/or regression of LDH symptoms was $13.6{\pm}5.4$ months (range: 5-22). Conclusions: It should be kept in mind that lumbar disc hernias could regress with medical treatment and rest without surgery, and there should be an awareness that these patients could recover radiologically. This condition must be taken into account during decision making for surgical intervention in LDH patients devoid of indications for emergent surgery.

Multilobular Osteochondrosarcoma of the Cranium in a Miniature Pinscher Dog

  • An eight-year old neutered male miniature pinscher dog was referred with a mass on the right frontal region. Radiography, computed tomography (CT) and magnetic resonance imaging (MRI) examination were performed. Imaging results showed mineralized mass invaded frontal bone without brain parenchymal change and distant metastasis. The mass was removed mostly by surgical resection, and the dog was discharged without any complications. On the histopathological examination, the mass was diagnosed as grade I multilobular osteochondrosarcoma. Though the mass recurred after the surgical resection, the dog is still alive to date without metastasis.

A Study on the Refusal of National Examination for Medical Students: Focusing on the Signaling Game Theory (의과대학생 국가시험 거부 사태에 대한 고찰: 신호게임 이론을 중심으로)

  • Background: Conflict in the medical world in 2020 led to the rejection of the national examination for doctors. This study explained the process until the end of the national test refusal situation triggered in 2020 through the signaling game theory. The government has succeeded in requiring medical students to take the national exam. Methods: To explain the rejection of the national examination, we first compose and show an example of two small non-signaling games where medical students know which type the government is of, then combine them to play a signaling game. Results: The behavior of the government and medical students was examined through the signaling game model. In the context of the coronavirus disease 2019 (COVID-19) pandemic, the government makes an ultimatum, whatever the type. And the medical students accept it. They judged that the government could not be expected to abolish the policy. If COVID-19 had not occurred, medical students would have been able to continue the confrontation. Conclusion: The government instilled in the other party the perception that the government would not bend its policies because it was the surly type and would not be afraid of a strong confrontation. Through the image created in this way, the government was forced to accept the ultimatum by medical students. Academically, this study is to deal with the policy-making process through the signaling game theory. In the area of health care policy, this study suggests that various situations such as the type of government or the spread of COVID-19 can become important in addition to the rationality of the policy itself.

The Effect of Recorded Video Monitoring on Students' Self Reflection of Patient-Physician Interaction (녹화영상 활용 학습법이 학생들의 '환자-의사관계'에서의 자기성찰에 미치는 영향)

  • The aim of this study is to examine the effect of recorded video monitoring on students' self-reflection after completing their clinical performance examination. Taking into account the particular cases involved in the examination, the present study utilized history-taking, physical examination, and patient education as bases for evaluating information-establishment ability, and asking, listening, understanding, explaining, and connectedness as the bases for evaluating patient-physician interaction ability. Student self-monitoring through recorded video feedback was carried out three days after completion of their clinical performance examination. Students self-evaluated their performance with a 10-point scale before and after self-monitoring. The results of this study show that students have a general tendency to lower their own self-evaluation scores after self-monitoring. Although there was not a statistically significant change of interrelationship in the information-establishment ability evaluation, there was a meaningful change of interrelationship in the patient-physician interaction ability evaluation after self-monitoring; specifically, in the case of acute lower abdominal pain, a high correlation was found (r=0.31, p=0.02) between the evaluation scores of standardized patients and students related to patient-physician interaction ability. This implies that self-monitoring enables the students to acquire a reflective viewpoint from which to evaluate their own performance. Therefore, it can be said that self-monitoring through recorded video feedback is a valuable method for students to use in reviewing their performance in patient-physician interactions.

Study for the Effect of CWS-A on the Cerebral Hemodynamic Changes and the Safety Examination (CWS-A 방의 뇌혈류역학적 변화 및 안전성에 관한 연구)

  • This study was designed to investigate the effects of a new herbal medicine named CWS-A on the changes in regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats, and further to determine the mechanism of action. Also, this study was designed to investigate the effects of CWS-A on the changes in rCBF in cerebral ischemic rats and the safety examination of CWS-A in rats regared to body weight change, renal and liver function test, CBC, $LD_{50}$ and histological observation in rats. The rCBF was significantly and stably increased and MABP was significantly decreased by CWS-A during the period of cerebral reperfusion in normal rats. However, there were no here were no significant changes of rCBF in ischemic rats. In additional, there were no significant changes on the safety examination. In conclusion, these results suggest that medication of CWS-A is effective for the improve the cerebral blood flow and CWS-A can be prescribed as vertigo(眩暈) symptoms treatment.

MR Imaging-Histopathologic Correlation of Radiofrequency Thermal Ablation Lesion in a Rabbit Liver Model: Observation during Acute and Chronic Stages

  • Objective: To determine the ability of MR imaging to detect the pathological changes occurring in radiofrequency (RF) thermal lesions and to assess its accuracy in revealing the extent of tissue necrosis. Materials and Methods: Using an RF electrode, thermal lesions were created in the livers of 18 rabbits. The procedure involved three phases. In the acute phase, six animals were killed the day after performing thermal ablation with RF energy, and two on day 3. In the subacute and chronic phases, eight rabbits underwent percutaneous hepatic RF ablation. After performing MR imaging, two animals were sacrificed at 1, 2, 4, and 7 weeks after the procedure, and MRpathologic correlation was performed. Results: In the acute phase, the thermal ablation lesions appeared at gross examination as well-circumscribed, necrotic areas, representing early change in the coagulative necrosis seen at microscopic examination. They were hypointense on T2-weighted images, and hyperintense on T1-weighted images. Gadolinium-enhanced MR imaging showed that a thin hyperemic rim surrounded the central coagulative necrosis. In the subacute phase, ablated lesions also showed extensive coagulative necrosis and marked inflammation at microscopic examination. Beyond two weeks, the lesions showed gradual resorption of the necrotic area, with a peripheral fibrovascular rim. The size of lesions measured by MR imaging correlated well with the findings at gross pathologic examination. Conclusion: MR imaging effectively demonstrates the histopathological tissue change occurring after thermal ablation, and accurately determines the extent of the target area.

Clinical Study of Acupucture Effect on Microtraumatic Injuries of the Knee Joint (슬관절 미세손상환자의 침치료에 관한 임상적 고찰)

  • Objectives : This study was performed to evaluate the clinical effect of acupuncture on microtraumatic injuries of the knee joint. Methods : Among the outpatients with knee joint pain who visited to Department of Acupuncture & Moxibustion, National Medical Center from February 2000 to September 2000, we selected 15 cases that showed normal finding in both X-ray and neurological examination, and that were categorized into microtraumatic injuries by the physical examination based on the Cyriax's orthopedic medicine. During acupuncture therapy, the patients were ordered to move involved side actively. The effect was assessed through questionnaires of CNRS(Cincinnati Knee Rating System). Results : We investigated 11 female and 4 male patients. The most common distribution of age was 30's(40%). The most commom duration of symtoms was from 3 to 5 months(40%). The most common microtraumatic injury was pes anserinus bursitis(33.3%), and followed by iliotibial band friction syndrome(20%), and patellar tendinitis(20%). In the CNRS, the mean number of before treatment was 60.6 and after was 66.5. 10 cases among the 15 patients were improved(66.6%). Conclusions : These results suggest that the acupuncture therapy combined with active movements of involved side was effective treatment modality on microtraumatic injuries of the knee joint.

The Pricing Strategy for the Performance of Medical Service -­ Based on the Segmentation for the N­block tariff Pricing of Medical Examination­ - (의료서비스의 성과 제고를 위한 가격전략 -­건강검진료 다단계가격책정을 위한 시장세분화를 중심으로­-)

  • This research objective is to determine the optimal price break points for n­block tariff, because comparing non­linear pricing with uniform pricing on the basis of profit, n­block tariff outperforms two­part tariff, all unit discount price schedule, and uniform pricing. Although the merits of non­linear pricing are well documented, the attempt to practice the non-linear pricing in medical service sector has been relatively rare. The determination of the parameters under n­block tariff is the interesting decision making agenda for marketers. Under n­block tariff, the marketers should decide the optimal price break points and the optimal marginal price for each price zone. The results can be summarized as follows: The researchers found that mixture model can be the feasible methodology for determining the optimal number of n­block tariff and identifying the optimal segmentation criteria. We demonstrate the feasibility and the superiority of the mixture model by applying it to the database of medical examination. The results appear that the number of patients per month can be the optimal segmentation variable. And 6­block tariff is the optimal price break for this medical service.
