The Effect of Depression and Self-efficacy on the Quality of Life (요양병원 입원노인의 우울과 자아효능감이 삶의 질에 미치는 영향)

  • Objective : Purpose of this study was to investigate the effect of depression and self - efficacy on the quality of life of the elderly hospitalized in a nursing hospital and to suggest a new direction for improving the quality of life of the elderly. Methods : The target was 212 senior citizens who were hospitalized at a nursing hospital located in K city in North Gyeongsang Province. The questionnaire was structured by adding the characteristics of the subjects, the Korean Senior Depression Index (GDS-K), the Self-effective Index (WHQOL-BRFA), and the World Health Organization's Life-Enhanced scale. coded using SPSS ver.18, the depression, self-efficiency, and quality of life differences according to the general characteristics (t-test, ANOVA), and Self-efficacy, depression, and the correlation coefficient between quality of life and A multI-sentence analysis to see the impact of depression and self-efficacy on the quality of life. Results : Self - efficacy and depression according to subject characteristics were different according to length of stay, and quality of life was different in religion. The correlation between depression, self - efficacy, and quality of life of admitted elderly showed statistically significant negative correlation (p <.01) with depression in both the quality of life and self - efficacy sub - variables. The depression and the self - efficacy of the elderly had the greatest effect on the quality of life. The variables were depression (${\beta}=-.328$), social efficacy (${\beta}=.248$), and physical efficacy (${\beta}=.193$). Conclusion : In order to improve the quality of life of the elderly, it will be necessary to provide and develop medical care services that reduce depression and improve self - efficacy (physical and social).

INFLUENCE OF MINIPLATE SHAPES AS SKELETAL ANCHORAGE FOR APPLICATION OF ORTHOPEDIC FORCE: A THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS (악정형력 적용을 위한 골내 고정원으로서 미니플레이트 형상의 영향: 3차원 유한요소법적 연구)

  • Purpose: This study was performed to evaluate the stress distribution in the bone and the displacement distribution of the miniscrew under orthopedic force with two different types of miniplate design as skeletal anchorage for orthopedic treatment. Materials and methods: Finite element models were made for 6-hole miniplate (0.8mm in thickness), which were designed in two different shapes-one is curvilinear shaped (C plate, Jeil Medical Co., Korea) and another, Y shaped (Y plate), fixed with 3 pieces of miniscrew 2mm-diameter and 6mm-long respectively. A traction force of 4 N was applied in $0^{\circ}$, $30^{\circ}$ and $60^{\circ}$ to imaginary axis connecting two unfixed distalmost holes of the miniplate. Results: The maximum von Mises stress in the bone was much greater in the cortical portion rather than in the cancellous portion. C plate showed greater maximum von Mises stress in the cortical bone than Y plate. The maximum displacement of the miniscrew was greater in C plate than Y plate. The more increased the angle of the applied orthopedic force, the greater maximum von Mises stress in the bone and maximum displacement of the miniscrew. It was observed that in C plate, the von Mises stress in the bone and displacement of the miniscrew were distributed around the distalmost screw-fixed area. Conclusions: The results suggest that Y plate should have the advantage over C plate and in the placement of the miniplate, its imaginary axis should be placed as parallel as possible to the direction of orthopedic force to obtain its primary stability.

EFFECT OF TAPER AND SURFACE AREA OF INNER CROWN ON THE RETENTIVE FORCE OF ELECTROFORMED OUTER CROWN (전기성형술로 제작된 외관의 유지력에 내관의 축면경사도와 표면적이 미치는 영향)

  • Purpose: With gold electroforming system fir the double crown, the secondary crown is electroformed directly onto the primary crown. An even thick layer of high precision can be acquired. It is thought that the retention of electroformed outer crown is primarily acquired by the adhesive force (surface tension) through the saliva which is interposed between precisely fitted inner and outer crown. The purpose of this study was to investigate the effect of taper and surface area of inner crown on the retentive force of electroformed outer crown according to the presence of saliva. Materials and methods: 32 titanium inner crowns with cervical diameter of 8 mm and cone angles of 0, 2, 4, 6 degrees, which had same surface area by regulated height, were machined on a lathe. Another 32 titanium inner crowns with cone angles of 0, 2, 4, 6 degrees, which had doubled surface area by increased cervical diameter. were fabricated. Eight specimens of each group, for a total of 64 titanium inner crowns, were prepared. The electroformed outer crowns were fabricated directly on the inner crowns by using electroforming machine(GAMMAT free, Gramm Technik, Germany). The tertiary frameworks were waxed-up on the electroformed outer crown and cast using nonprecious alloy($Rexillium^(R)III,\;Jeneric^(R)/Pentronh^(R)$ Inc., USA). The cast metal frameworks were sandblasted with alubimium oxides and cemented using resin cement(Superbond C&B, Sun Medical Co., Japan) over the electroformed copings of each specimen. Then, artificial saliva($Taliva^(R)$, Halim Pharm. Co., Korea) was sprayed between the inner and outer crown, and they were connected under 5 kg force. The retentive force was measured by the universal testing machine(Tinius Olsen 1000, Tinius Olsen, USA) with a cross-head speed of 66.67 mm/sec. The direction of cross-head travel was exactly aligned with the path of removal of the respective specimens. This measurement procedures for retentive force of electroformed outer crown with artificial saliva were repeated in the same way without presence of artificial saliva. Results and Conclusion: The following conclusions were drawn: 1. The retentive force of electroformed outer crown was decreased according to increased taper of inner crown(P<.05). 2. The retentive force of electroformed outer crown showed no significant differences according to surface area and the presence of artificial saliva(P>.05).

A Change in the Diagnosis of a Korean Pediatric Outpatients following the COVID-19 Pandemic (코로나19 유행에 따른 한방소아청소년과 외래 환자의 진단명 변화)

  • Objectives This study aims to provide directions for future Korean pediatric medicine treatment and researches by analyzing the characteristics of diseases of Korean pediatric outpatients before and after the COVID-19 epidemic. Methods We analyzed the medical records of 10,868 new patients aged 0-18 who visited the Korean pediatrics at Kyung Hee university hospital from January 1, 2019 to June 30, 2021. All patients were divided into 2 groups; patients who visited from January 1, 2019 to December 31, 2019, before the coronavirus epidemic, and patients who visited from January 1, 2020 to June 30, 2021, after the coronavirus epidemic. Within the two groups, the diagnosis distribution was performed for the entire population, by gender and by age group. Diagnoses were classified as allergy, growth, gastrointestinal, respiratory, general body supplementation, neuropsychiatric, skin, urogenital, musculoskeletal, and other systems. Results According to the comparative analysis results of before and after the COVID epidemic, there was a decreasing trend of diagnoses in relates to allergy, respiratory, urogenital, musculoskeletal system. On the other side, there was an increasing trend in gastrointestinal, general body supplementation, growth, neuropsychiatric, skin and other system. Changes in diagnosis based on gender and age group will be described below. Conclusions This study analyzed the characteristics of diseases of Korean pediatric outpatients before and after the COVID-19 epidemic, and suggested the direction of future Korean medicine treatment and researches for pediatrics. Also, this study aimed to propose proper treatments for children by showing the role of Korean pediatric medicine related to growth and development of children during the epidemic. However, this study has limitations such as failure to track the long-term impact of COVID-19. Thus, long term follow-up studies are needed to show the effect of COVID-19 on Korean pediatric medicine.

A Study on the Evaluation of Radiation Safety in Opened-Ceiling-Facilities for Radiography Testing (천장 개방형 RT 사용시설의 방사선 안전성 평가 연구)

  • Radiography-Testing that verify the quality of welding structures without destruction are overwhelmingly used in industries, but many safety precautions are required as radiation is used. The workers for Radiography-Testing perform the inspection by moving the Iridium-192 radiation source embedded in the transport container of the gamma-ray irradiator within or outside the facility. The general facility is completely blocked about radiation from the outside with thick concrete, but if it is difficult for worker to handle object of inspection, facilities ceiling can be opened. A general facility may be constructed using a theoretical dose evaluation method because all exterior facilities are blocked, but if the ceiling is open, it is not appropriate to evaluate radiation safety with a simple theoretical calculation method due to the skyshine effect. Therefore, in this study, the radiation safety of the facility was evaluated in the actual field through an ion chamber survey-meter and an accumulated dose-meter called as OSLD, and the actual evaluation environment was modeled and evaluated using the Monte Carlo simulation code as FLUKA. According to the direction of the irradiation, the radiation dose at the facility boundary was difficult to meet the standards set by the regulatory authority, and radiation safety could be secured through additional methods. In addition, it was confirmed that the simulation results using the Iridium-192 source were valid evaluation with the actual measured results.

Recent Studies of Breast Cancer in Traditional Chinese Medicine Journals (중의학 논문에 나타난 유방암의 연구 동향에 대한 고찰 - 중의학 논문을 중심으로 -)

  • Purpose: The purpose of this study is to research trends in the study of breast cancer in Traditional Chinese Medicine (TCM) and to establish the further direction for its study. Methods: We reviewed TCM papers published in the last 29 years (1979-2008). Results: 1. We researched 49 papers and the patterns of study were as follows: in vitro studies were 27 papers (55.1%), in vivo studies were 9 papers (18.4%) and clinical studies were 19 papers (38.8%). 2. In vitro studies on breast cancer research in TCM were focused on cytotoxicity (17 papers) and apoptosis (8 papers). Most of in vivo studies (6 papers) were done for the purpose of inducing growth suppression of tumor cell after administration of the test drug. Each drug acted on this effect through various types of mechanism. 3. Unlike in vitro and in vivo studies, clinical studies on growth suppression of tumor cell were rare (4 papers). Most of the studies were focused on reduction of side effect of chemotherapy or synergistic effect with chemotherapy (7 papers), immune regulation (7 papers), and improvement of quality of life (6 papers). 4. Among the treatment method we reviewed, 'Runing Ⅱ(Ⅱ號方)' was the only medication that further studied as clinical trial after experimental study. 5. Since almost all studies have defects like poorly designed model or insufficient data description, it was difficult to make any definite conclusion about these studies. Conclusion: More subsequent clinical studies based on experimental study will be needed afterwards. Strict and high-level study design with detailed description will be needed in further study.

Research Trends in Korean Medicine Treatment for Sarcopenia: Scoping Review (근감소증에 대한 한의 치료 연구 동향: 스코핑 리뷰)

  • Objectives This study examined the trends in domestic and international research on the treatment of sarcopenia and analyzed the effectiveness of the treatment. Based on the results, we suggest directions for future Korean medicine research on sarcopenia. Methods The study was conducted using Arksey and O'Malley's scoping review methodology. It included three international databases (PubMed, EMBASE, Cochrane Library) and five domestic databases (Korean studies Information Service System, Korean Medical database, Oriental Medicine Advanced Search Integrated System, Research Information Sharing Service, ScienceON) were used. Literatures published until April 30, 2023, including the contents of 'sarcopenia' and Korean Medicine treatment', were searched. Results A total of 45 studies were included in the analysis. The number of studies on Korean medicine treatments for sarcopenia is steadily increasing, with the majority of studies coming from Asian countries. Of the 45 studies, 30 were conducted in the field of medicine, and the study design was analyzed into 10 clinical studies and 35 non-clinical studies. Among the 10 clinical studies, the most common treatment interventions were herbs (n=4), followed by Daoist exercises (n=3). The most common diagnostic criteria used in clinical studies were those published by the Asian working group for sarcopenia, and various physical examinations and laboratory tests were used as outcome measures. Conclusions The future direction of Korean medicine research on sarcopenia should be to expand the number of clinical studies applying Korean medicine treatment interventions.

Dissection of Cellular Communication between Human Primary Osteoblasts and Bone Marrow Mesenchymal Stem Cells in Osteoarthritis at Single-Cell Resolution

  • Background and Objectives: Osteoblasts are derived from bone marrow mesenchymal stem cells (BMMSCs) and play important role in bone remodeling. While our previous studies have investigated the cell subtypes and heterogeneity in osteoblasts and BMMSCs separately, cell-to-cell communications between osteoblasts and BMMSCs in vivo in humans have not been characterized. The aim of this study was to investigate the cellular communication between human primary osteoblasts and bone marrow mesenchymal stem cells. Methods and Results: To investigate the cell-to-cell communications between osteoblasts and BMMSCs and identify new cell subtypes, we performed a systematic integration analysis with our single-cell RNA sequencing (scRNA-seq) transcriptomes data from BMMSCs and osteoblasts. We successfully identified a novel preosteoblasts subtype which highly expressed ATF3, CCL2, CXCL2 and IRF1. Biological functional annotations of the transcriptomes suggested that the novel preosteoblasts subtype may inhibit osteoblasts differentiation, maintain cells to a less differentiated status and recruit osteoclasts. Ligand-receptor interaction analysis showed strong interaction between mature osteoblasts and BMMSCs. Meanwhile, we found FZD1 was highly expressed in BMMSCs of osteogenic differentiation direction. WIF1 and SFRP4, which were highly expressed in mature osteoblasts were reported to inhibit osteogenic differentiation. We speculated that WIF1 and sFRP4 expressed in mature osteoblasts inhibited the binding of FZD1 to Wnt ligand in BMMSCs, thereby further inhibiting osteogenic differentiation of BMMSCs. Conclusions: Our study provided a more systematic and comprehensive understanding of the heterogeneity of osteogenic cells. At the single cell level, this study provided insights into the cell-to-cell communications between BMMSCs and osteoblasts and mature osteoblasts may mediate negative feedback regulation of osteogenesis process.

Evaluation of Metal Volume and Proton Dose Distribution Using MVCT for Head and Neck Proton Treatment Plan (두경부 양성자 치료계획 시 MVCT를 이용한 Metal Volume 평가 및 양성자 선량분포 평가)

  • Purpose: The size, shape, and volume of prosthetic appliance depend on the metal artifacts resulting from dental implant during head and neck treatment with radiation. This reduced the accuracy of contouring targets and surrounding normal tissues in radiation treatment plan. Therefore, the purpose of this study is to obtain the images of metal representing the size of tooth through MVCT, SMART-MAR CT and KVCT, evaluate the volumes, apply them into the proton therapy plan, and analyze the difference of dose distribution. Materials and Methods : Metal A ($0.5{\times}0.5{\times}0.5cm$), Metal B ($1{\times}1{\times}1cm$), and Metal C ($1{\times}2{\times}1cm$) similar in size to inlay, crown, and bridge taking the treatments used at the dentist's into account were made with Cerrobend ($9.64g/cm^3$). Metal was placed into the In House Head & Neck Phantom and by using CT Simulator (Discovery CT 590RT, GE, USA) the images of KVCT and SMART-MAR were obtained with slice thickness 1.25 mm. The images of MVCT were obtained in the same way with $RADIXACT^{(R)}$ Series (Accuracy $Precision^{(R)}$, USA). The images of metal obtained through MVCT, SMART-MAR CT, and KVCT were compared in both size of axis X, Y, and Z and volume based on the Autocontour Thresholds Raw Values from the computerized treatment planning equipment Pinnacle (Ver 9.10, Philips, Palo Alto, USA). The proton treatment plan (Ray station 5.1, RaySearch, USA) was set by fusing the contour of metal B ($1{\times}1{\times}1cm$) obtained from the above experiment by each CT into KVCT in order to compare the difference of dose distribution. Result: Referencing the actual sizes, it was appeared: Metal A (MVCT: 1.0 times, SMART-MAR CT: 1.84 times, and KVCT: 1.92 times), Metal B (MVCT: 1.02 times, SMART-MAR CT: 1.47 times, and KVCT: 1.82 times), and Metal C (MVCT: 1.0 times, SMART-MAR CT: 1.46 times, and KVCT: 1.66 times). MVCT was measured most similarly to the actual metal volume. As a result of measurement by applying the volume of metal B into proton treatment plan, the dose of $D_{99%}$ volume was measured as: MVCT: 3094 CcGE, SMART-MAR CT: 2902 CcGE, and KVCT: 2880 CcGE, against the reference 3082 CcGE Conclusion: Overall volume and axes X and Z were most identical to the actual sizes in MVCT and axis Y, which is in the superior-Inferior direction, was regular in length without differences in CT. The best dose distribution was shown in MVCT having similar size, shape, and volume of metal when treating head and neck protons. Thus it is thought that it would be very useful if the contour of prosthetic appliance using MVCT is applied into KVCT for proton treatment plan.

A Study of Dose Stability at Low Monitor Unit Setting for Multiple Irradiated Field (다중 조사면 치료 시 기계적 입력치(MU)에 따른 선량적 안정성에 대한 연구)

  • Purpose : Many authors have been introduced field in field technique and 3-D conformal radiotherapy that increased the tumor dose as well as decreased the dose of abutting critical organ. These technique have multiple beam direction and small beam segments even below 10 MU(monitor unit)for each field. we have confirmed the influence of low MU on dose output and beam stability. Materials and Methods : To study the dose output, the dose for each field was always 90MU, but it divided into different segment size: 1, 2, 3, 5, 10, 15 segments, 90, 45, 30, 18, 9, 6 MU the measurements were carried out for X-ray energy 4 MV, 6 MV, 10 MV of three LINAC(Varian 600C, 2100C, 2100C, 2100C/D), in addition each measurement was randomly repeated three times for each energy. To study the field symmetry and flatness, X-omat V films were irradiated. After being developed, films were scanned and analyzed using densitometer. Results : Influence of low MU on dose is slightly more increase output about $1.2{\sim}2.9%$ in cGy/mu than 90MU, but may not changed beam quality(flatness or symmetry), Output stability depends on dose rate(PRF)rather than beam energy, field size. Conclusion : Presented result are under the limits(out put<3%, flatness<${\pm}3%$, symmetry<2%). The 3 accelerators are safe to use and to perform conformal radiotherapy treatments in small segments, small MU around 10MU. but Even if the result presented here under the limits, continuous adjustments and periodic QA should be done for use of small MU
