Study of Normative Gingival Proportion in Anterior Maxilla (상악 전치부 치은선의 평균치에 관한 연구)

  • Tooth is the most important element in esthetic consideration on facial area. Tooth alignment which is in harmony with gingiva, lips, and face is also key element. The purpose of this study was to give a clinical discipline for restoration of gingival contour, which contains a ratio of maxillary 6 anterior teeth, research for gingival contour etc., in case of rehabilitation of maxillary anterior teeth. 300 Dankook university dental school students ,who is their twenties and free from periodontitis, participate in this study. The result was from each 70 males and females who had normal occlusion and tooth alignment. Length, width and length/width ratio of maxillary 6 anterior teeth were 0.74-0.81 in male, 0.81-0.84 in female. There was significant difference between male and female. The most deepest position of gingiva in maxillary central incisor and canine was located in distal part of teeth and maxillary lateral incisor was middle portion. In maxillary central incisors, gingival line of the most deepest point appeared significant difference between male and female (p<0.01) whereas there was no difference in lateral incisors and canines. Distance between interdental papilla apex and the most deepest portion appeared significant difference between male and female. Distance of the deepest position of gingiva is statistically significant except maxillary central incisors and lateral incisors. Standard deviation and mean of the width of labio-lingual were measured in cervical area and there was a significant difference both male and female.(p<0.01) From this result, we could get the mean of maxillary anterior gingival line and these results have great value in clinical guidance in studying maxillary anterior teeth.

  • Maxillary lateral incisors usually exhibit a single root with a single canal. However, maxillary lateral incisor teeth with unusual morphology of root canal system are frequently reported. These cases of variable root canal anatomy can be treated well by nonsurgical endodontic methods. A detailed description of root canal morphology is fundamental for successful endodontic treatment. Treatment using an operating microscope, radiographs from different angles, and cone-beam computerized tomography (CBCT) can produce more predictable endodontic outcomes.

A LONGITUDINAL STUDY ON THE WIDTH CHANGE OF ANTERIOR KERATINIZED GINGIVA DURING TRANSITIONAL PERIOD (치아 교환시기중 전치부 각화치은의 폭경변화에 관한 연구)

  • Keratinized ginigva has clinical singificance in periodontal health because it plays important roles in resistance to mechanical trauma, to penetration of bacteria, and to tensional stress by muscle attachment. In order to investigate the width change of anterior keratinized gingvial during transitional period, the width of kerainized gingiva on anterior teeth was measured annually in elementary school children with deciduous dentition until the successive anterior teeth were completely erupted. The distance from the gingival margin on mid-portion of crown to mucogingival junction was measured by Boley gauge(Hu-Friedy, U.S.A.) and was recorded as the width of keratinized gingiva. The difference of the width of keratinized gingiva according to sex and dentition was analyzed statistically by Student t-test. Following results were obtained : 1. In deciduous dentition, it was shown that the width of keratinized gingiva on maxillary central incisors($3.28{\pm}0.83mm$) was the widest(p<0.01), and that on maxillary lateral incisors, mandibular central and lateral incisors was $2.69{\pm}0.86mm$, $2.51{\pm}0.71mm$, $2.43{\pm}0.68mm$, respectively. 2. In mixed dentition, it was shown that the width of keratinized gingiva on maxillary central incisors ($5.10{\pm}0.86$) was the widest(p<0.01), and that on maxillary lateral incisors, mandibular central and lateral incisors was $4.35{\pm}0.83mm$, $3.51{\pm}0.60mm$, $3.57{\pm}0.66mm$, respectively. 3. The width of anterior keratinized gingiva was significantly increased after the successive anterior teeth were completely erupted(p<0.001). The width of keratinized gingiva on maxillary central and lateral incisors, and mandibular central and lateral incisors was increased by $1.82{\pm}0.83mm$, $1.65{\pm}0.69mm$, $0.99{\pm}0.39mm$, and $1.14{\pm}0.98mm$, respectively. 4. There was no statistical significance in the difference of the width of anterior keratinized gingiva between male and female(p>0.05).

Color Comparison of Maxillary Primary Anterior Teeth and Various Composite Resins using a Spectrophotometer (분광광도계를 이용한 상악 유전치부와 다양한 복합레진 색조의 비교)

  • The purpose of this study was to compare the color of maxillary primary central incisors, lateral incisors, canines with the color of various composite resins using a spectrophotometer VITA Easyshade®V. One researcher measured the color of sound maxillary primary anterior teeth of 100 children aged 1 to 6 and specimens which were made by composite resins with a total of 10 types and 31 shades, and CIE L*, a*, b* values were obtained. There was no clinically recognizable color difference between the maxillary primary central and lateral incisors. However, the maxillary primary canines showed difference in color above the clinical acceptability threshold with the primary central and lateral incisors. These findings showed no significant color differences between men and women, and no significant color changes with age. A1 shade is the most common in all maxillary primary anterior teeth. Composite resins, which are thought to be similar to the color of maxillary primary anterior teeth were selected from 9 types of maxillary primary central incisors, 6 types of primary lateral incisors, 6 types of primary canines.

A retrospective institutional study of human age determination by evaluating the pulp length and width ratio of the maxillary lateral incisor on panoramic radiographs in Indonesian subjects

  • Purpose: The pulp length to width (PL/W) ratio of the maxillary lateral incisor can be used as an age determination method. This study aimed to investigate the correlation between the PL/W ratio of the maxillary lateral incisor on panoramic radiographs and human chronological age in Indonesian subjects. Materials and Methods: This study analyzed with 134 maxillary lateral incisors on 113 panoramic radiographs from patients who visited the Oral and Maxillofacial Radiology Unit of Dental Hospital Universitas Padjadjaran, Bandung, Jawa Barat, Indonesia, from 2013 to 2018 (age range: between 11 to 60 years). The pulp length was measured from the pulp chamber roof to the apical foramen, and the pulp width was measured on the cervical area of the cementoenamel junction in millimeters using Fiji ImageJ open-source software. Simple linear regression (in SPSS) was used to analyze the results. The reliability of the observers was evaluated. Results: The PL/W ratio of the maxillary lateral incisor was significantly correlated with chronological age (P<0.01). No statistically significant difference was found in the PL/W ratio between the left and right maxillary lateral incisors(P=0.333). There was a very strong correlation (r=0.939) between the PL/W ratio of the maxillary lateral incisor and human chronological age, with the following formula: age= -3.057+1.875×PL/W ratio (R2=0.882, standard error of estimate: 4.659). Conclusion: The PL/W ratio of the maxillary lateral incisor on panoramic radiograph can be used for age determination in Indonesian subjects.

The Apico-Coronal and Mesio-Distal Positions of the Gingival Zenith in the Maxillary Anterior Dentition (상악 전치부에서 치은 정점의 근원심적, 수직적 위치)

  • The gingival zenith is the most apical point of the gingival outline. This study aimed to quantify the mesio-distal and apico-coronal displacements of gingival zenith in maxillary anterior dentition. 50 healthy patients, who ranged in age from 25 to 36 years were recruited for the study. Alginate impressions of the study group were made and poured with the type III stone. The gingival zenith positions of maxillary anterior teeth were measured in a mesial-lateral direction from the vertical bisected midline. The apico-coronal displacements of the gingival zenith of lateral incisors were measured from a line joining the gingival zeniths of the central incisor and canine. The gingival zeniths were distally displaced in 90%, 53% and 23% of the maxillary cetral incisors, lateral incisors and canines respectively. The mean distal displacements of the gingival zeniths to the vertical bisected midline of central incisors, lateral incisors and canines were 0.90 mm, 0.29 mm and 0.06 mm respectively. The gingival zeniths of lateral incisors were positioned $0.73{\pm}0.43$ mm coronally to the line joining the gingival zenith of central incisors and canines. Consideration of these findings may improve restorative procedures in the anterior maxillary dentition.

  • The original sample in this investigation included 36 children around the age of eight (mean age:8 year-lmonth) at the beginning. Study casts were obtained and measured every 6 months in two years of longitudinal study period in order to observe the changes of maxillary dental arch as well as the eruptional status of the maxillary lateral incisors. The results were as follows.: 1) The length of upper dental arch was increased gradually during the examination period. 2) The width between maxillary first molars was increased gradually during the examination period. 3) Intercanine distance in upper dental arch was increased gradually and the increment was conspicuous immediately after the eruption of maxillary lateral incisors.

  • Objectives: Management of a horizontal root fracture of an anterior teeth is challenging and often requires multiple approaches for improving the functional and esthetic outcomes. This case report describes the treatment and 2-yr follow up of 3 maxillary incisors with horizontal root fracture. Two maxillary central incisors were treated with Mineral Trioxide Aggregate (ProRoot MTA, Dentsply, Tulsa, OK, USA). Left maxillary lateral incisors were treated with endodontic treatment and submerged. During 2-yr of follow-up evaluation, the root-fractured teeth of the present patients were well retained in the arch, showing periodontal healing even after endodontic treatment.

A Study of the shade of between maxillary and mandibular anterior teeth in the Korean (한국인의 상하악 전치부 색조에 관한 연구)

  • Purpose: The purpose of this study was to spectrophotometrically evaluate the shade difference between of maxillary and mandibular anterior teeth in the Korean by the standard of vita classical shade guide using $SpectroShade^{TM}$. Material and methods: In this study, the shades of healthy anterior teeth were examined and analyzed using the digital shade analysis of $SpectroShade^{TM}$. This study examined 80 individuals in their twenties, thirties, fourties, fifities ages and 40 males and 40 females, presenting 12 healthy, unrestored maxillary and mandibular anterior teeth. Tooth brushing and oral prophylaxis were performed prior to evaluation. The $SpectroShade^{TM}$ was used to acquire images of the 12 maxillary and mandibular anterior teeth. These images were analyzed using $SpectroShade^{TM}$ Software, and shade maps of each tooth were acquired. The shade difference of upper and lower, and gender differences and ages difference were investigated and analyzed with CIE $L^{*}a^{*}b^{*}$ color order system. One-Way ANOVA test was used to find out if there were significant differences between groups tested and Sheffe multiple comparison was used to identify where the differences were. Results: 1. Shade differences were significant (P < .05) between maxillary and mandibular central incisor, lateral incisor, canine. 2. No significant differences in shade distribution were seen between lateral incisors and central incisors. 3. Canine's shade difference were more significant than central incisor's and lateral incisors's. 4. No significant differences in shade distribution were seen between genders in maxillary and mandibulr central incisor, lateral incisor, canine. 5. No significant differences in shade distribution were seen in order of years in maxillary and mandibulr central incisor, lateral incisor, canine. Conclusions: The results of this study show that 1. Shade difference was founded in maxillary and mandibular anterior teeth and ${\Delta}E^{*}$ value was more than 2.0. 2. Canine's shade difference were more significant than central incisor's and lateral incisors's and between central incisors and lateral incisors shade differences were no significant. 3. No significant differences in shade distribution were seen between genders in maxillary and mandibular anterior teeth. 4. No significant differences in shade distribution were seen in order of years grade in maxillary and mandibular anterior teeth.

Correction of palatally displaced maxillary lateral incisors without brackets

  • This article describes the orthodontic treatment of a 25-year-old Korean female patient with anterior crowding, including palatally displaced lateral incisors. Her facial profile was satisfactory, but 3.5 mm of maxillary anterior crowding was observed. To correct this crowding, we decided to minimize the use of the conventional fixed orthodontic appliances and employed a less bulky and more aesthetic appliance for applying light continuous force. We determined the final positions of the maxillary teeth via a working model for diagnostic set up and achieved space gaining and alignment with simple Ni-Ti spring and stainless steel round tubes. Tooth alignment was achieved efficiently and aesthetically without the conventional brackets.
