Inverted Maxillary Incisor의 치료 (TREATMENT OF INVERTED MAXILLARY INCISOR)

  • Inverted maxillary incisor is that maxillary incisor rotates to the counterclockwise direction. The incisal margin and root apex of the impacted incisor is palpated at the mucobuccal fold near the labial frenum and on the palate among the rugae, respectively. Orthodontists confront ectopically erupting teeth in various locations. In the past, extraction of impacted teeth that deviated from their normal course of eruption had been performed indiscriminately. But, if it has not any clearcut contraindications, effeort should be made to achieve optimal esthetic results by conservative means, combining the skills of oral surgeon and orthodontist. The present report provides an illustration of satisfactory correction of a inverted maxillary incisor with surgical intervention and many springs that correct the tooth axis. This technique provides the clinician with an additional means to avoid unnecessary extraction of inverted teeth in certain cases.

한국인의 상악중절치 위치 및 길이에 관한 연구 (A Study on the Position and Length of the Maxillary Central Incisor in Koreans)

  • The arrangement of anterior artificial teeth is an important factor on the esthetics in the construction of complete dentures and prostheses. During construction of the complete denture the position of maxillary central incisor plays an important role in the arrangements of artificial teeth. The proper position of maxillary central incisor provides natural appearance of anterior teeth and the anterior reference point of occlusal plane. Many methods have been utilized to eatablish the position of the maxillary central incisor in complete denture prosthodontics. However, there has not been provided for the reference for Korean yet. This study was aimed to extablish a guide for the position of the maxillary central inciosr in Koreans. The horizontal and vertical distance between the maxillary central incisor and incisive papilla in Koreans were measured. 1. The mean value of horizontal distance from the maxillary central incisor to the incisive papilla had significant sexual difference, The average distance was 12.648mm in male and 11.385mm in female. 2. The horizontal distance of the incisive papilla had sexual difference, the average distance was 6.182mm in male and 5.622mm in female. 3. The vertical distance from the maxillary central incisor to the incisive papilla had not sexual difference, the average distance was 7.21mm. 4. The crown length of the maxillary central inciosrs from the distal interdental papilla had not sexual difference, the right central incisor was 6.40mm(mean) and the left central inciosr was 6.43mm(mean). 5. The clinical crown length of the maxillary central incisors had not sexual difference, the right central incisor 10.35mm(mean) and the left central incisor was 10.43mm(mean). 6. The shapes of the incisive papilla were pear(68.4%), oval(16%), triangular(6%), irregular(5%), rectangular(4%), and inverted pear(1%) shape.

  • The author has sought to determine the time and the sequence of permanent teeth eruption in Korean children. The study group consisted of 15,671 healthy children (male 8,015 ; female 7,656) aged 5-13 years old who lived in Seoul or Cheongju. The results were as follows : 1. The eruption times of permanent teeth were 0.45 years earlier in females than in males. 2. The ages corresponding to $ER_{50}$ of permanent teeth were as follows : In Maxilla 1) central incisor was 7.37 yrs 2) lateral incisor was 8.50 yrs 3) canine was 10.83 yrs 4) 1st premolar was 10.30 yrs 5) 2nd premolar was 11.09 yrs 6) 1st molar was 6.49 yrs 7) 2nd molar was 12.79 yrs In Mandible 1) central incisor was 6.40 yrs 2) lateral incisor was 7.41 yrs 3) canine was 10.18 yrs 4) 1st premolar was 10.26 yrs 5) 2nd premolar was 11.15 yrs 6) 1st molar was 6.32 yrs 7) 2nd molar was 12.05 yrs 3. The eruption sequence of permanent teeth by Z-test was as follow: In Male 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Maxillary central incisor 4th : Mandibular lateral incisor 5th : Maxillary lateral incisor 6th : Mandibular canine, Maxillary and Mandibular 1st premolar 7th : Maxillary canine 8th : Maxillary and Mandibular 2nd premolar 9th : Mandibular 2nd molar 10th : Maxillary 2nd molar In Female 1st : Mandibular 1st molar, and Mandibular central incisor 2nd : Maxillary 1st molar 3rd : Mandibular lateral incisor, Maxillary central incisor 4th : Maxillary lateral incisor 5th : Mandibular canine, Maxillary and Mandibular 1st premolar 6th : Maxillary canine 7th : Maxillary and Mandibular 2nd premolar 8th : Mandibular 2nd molar 9th : Maxillary 2nd molar 4. The corresponding permanent teeth in the mandible generally erupted earlier than the corresponding permanent teeth in the maxilla by an average of 0.73 years, but the mean eruption time of mandibular 1st premolars was almost the same as those of maxillary 1st premolars, and the mean eruption time of mandibular 2nd premolars was 0.06 years later than those of maxillary 2nd premolars. 5. There is no significant difference between left and right arch in the eruption time and sequence. 6. Generally, the ages of permanent teeth eruption tended to be earlier than those of Dr. Cha's data from 1963.

  • Inverted maxillary incisor is that maxillary incisor rotates counterclockwise direction. The cause of this 'Inverted incisor' is the injury of the deciduous predecessor transmitted to the developing permanent tooth germ or displacement of permanent tooth crown portion from unknown origin. Dilaceration, defined as a distorted root from, may result from mechanical injury during eruption period or ectopic development of tooth germ. This article presents a case of an inverted and dilacerated maxillary right central incisor. Through orthodontic traction, the dilacerated and inverted incisor was successfully moved into the proper position.

Esthetic improvement in the patient with one missing maxillary central incisor restored with porcelain laminate veneers

  • This article describes esthetic improvement in a patient with a missing maxillary left central incisor. Space analysis of the anterior dentition showed that minor tooth rearrangement was needed. Optimal space distribution for restorations was attained by orthodontic treatment. Through transforming tooth shape with porcelain laminate veneers, the maxillary left lateral incisor was transformed into central incisor and the maxillary left canine into a lateral incisor. The maxillary right central incisor was also restored for esthetic improvement. In a case of changing a tooth shape with porcelain laminate veneers, pre-treatment evaluation, space analysis and diagnostic wax-up are important factors.

20대 대학생들의 상악중절치 색조와 구강보건 및 식습관 관련 특성 (Oral health and eating habits characteristics relating to maxillary central incisor color of the university students in their twenties)

  • Objectives : This study aims to prepare the basic data of the teeth color by measuring tooth mode using the color of the maxillary central incisor of the college students in their twenties as the dental colorimeter, and oral health and eating habit characteristics were examined. Methods : The maxillary central incisor(1 teeth) of the subjects of study as 467 students(male 89, female 378) was measured during the period from November 14, December 2 2011, so the color of total 467 teeth were examined, and oral health and eating habit characteristics were researched. Results : 1. As the results of measurement of the shade, value, hue of maxillary central incisor by the general characteristics of the subject, the major(shipbuilding information, special rehabilitation for young children) was higher at 3.3 of the shade, and the grade(1), age($$\geq_-20$$), major(beauty culture & cosmetic, occupational therapy) showed higher at +2 of the value, and the grade(1) and all major showed higher at R3 of the hue. 2. As the results of measurement of the shade, value, hue of maxillary central incisor by the oral health characteristics, the subjective teeth color(yellow) and the education of oral health(no) showed higher at 2.5 of the shade. 3. As the results of measurement of the shade, value, hue of maxillary central incisor by the eating habit characteristics of the subjects, the chocolate showed higher at 3.0 and 2.0 of the shade, and the red pepper paste showed higher at +2 and STD of the value, and the green tea showed higher at $R_3$ of the hue. Conclusions : This study demonstrates that the color of the maxillary central incisor showed differences various tendency according to the oral health and eating habit characteristics.

Analysis of the width ratio and wear rate of maxillary anterior teeth in the Korean population

  • PURPOSE. The purpose of this study was to compare the width ratio of maxillary anterior teeth according to age in the Korean population and to evaluate the maxillary central incisor width-to-length (W/L) ratio, given differences in age and gender. MATERIALS AND METHODS. Ninety-three Korean adults were divided into 3 groups (n = 31) by age. Group I was 20 - 39 years old, Group II was 40 - 59 years old, and Group III was over 60 years of age. After taking an impression and a cast model of the maxillary arch, the anterior teeth width ratio and central incisor W/L ratio were calculated from standard digital images of the cast models using a graph paper with a digital single lens reflex (DSLR) camera. The calculated ratios were compared among all groups and central incisor W/L ratio were analyzed according to age and gender. All comparative data were statistically analyzed with one-sample t-tests, one-way ANOVAs with Tukey tests, and independent t-tests. RESULTS. No significant differences in maxillary anterior teeth ratios were found among the age groups. The maxillary central incisor W/L ratios in Group III were the greatest and were significantly higher than those in the other groups. The central incisor W/L ratio of men was higher than that of women in Group II. CONCLUSION. Maxillary anterior teeth width ratios were similar in all age groups in the Korean population. The maxillary central incisor was observed as worn teeth in the group over 60 years of age, and a significant difference between genders was found in 40 to 50 year olds.

The maxillary incisor labial face tangent: clinical evaluation of maxillary incisor inclination in profile smiling view and idealized aesthetics

  • Background: To test the hypothesis that in profile smiling view, for ideal aesthetics, a tangent to the labial face of the maxillary central incisor crowns should be approximately parallel to the true vertical line and thereby perpendicular to the true horizontal line. Methods: An idealized female image was created with computer software and manipulated using the same software to construct an "ideal" female profile image with proportions, and linear and angular soft tissue measurements, based on currently accepted criteria for idealized Caucasian profiles. The maxillary incisor labial face tangent was altered in 5° increments from 70 to 120°, creating a range of images, shown in random order to 70 observers (56 lay people and 14 clinicians), who ranked the images from the most to the least attractive. The main outcome was the preference ranks of image attractiveness given by the observers. Results: The most attractive inclination of a tangent to the labial face of the maxillary incisor crowns in profile view in relation to the true horizontal line was 85°, i.e. 5° retroclined from a perpendicular 90° inclination. The most attractive range appears to be between 80 and 90°. Excessive proclination appeared to be less desirable than retroclination. Beyond 105° most observers recommend treatment. Conclusion: In natural head position, the ideal inclination of the maxillary incisor crown labial face tangent in profile view will be approximately parallel to the true vertical line and thereby approximately perpendicular to the true horizontal line.

A retrospective institutional study of human age determination by evaluating the pulp length and width ratio of the maxillary lateral incisor on panoramic radiographs in Indonesian subjects

  • Purpose: The pulp length to width (PL/W) ratio of the maxillary lateral incisor can be used as an age determination method. This study aimed to investigate the correlation between the PL/W ratio of the maxillary lateral incisor on panoramic radiographs and human chronological age in Indonesian subjects. Materials and Methods: This study analyzed with 134 maxillary lateral incisors on 113 panoramic radiographs from patients who visited the Oral and Maxillofacial Radiology Unit of Dental Hospital Universitas Padjadjaran, Bandung, Jawa Barat, Indonesia, from 2013 to 2018 (age range: between 11 to 60 years). The pulp length was measured from the pulp chamber roof to the apical foramen, and the pulp width was measured on the cervical area of the cementoenamel junction in millimeters using Fiji ImageJ open-source software. Simple linear regression (in SPSS) was used to analyze the results. The reliability of the observers was evaluated. Results: The PL/W ratio of the maxillary lateral incisor was significantly correlated with chronological age (P<0.01). No statistically significant difference was found in the PL/W ratio between the left and right maxillary lateral incisors(P=0.333). There was a very strong correlation (r=0.939) between the PL/W ratio of the maxillary lateral incisor and human chronological age, with the following formula: age= -3.057+1.875×PL/W ratio (R2=0.882, standard error of estimate: 4.659). Conclusion: The PL/W ratio of the maxillary lateral incisor on panoramic radiograph can be used for age determination in Indonesian subjects.
