Acute Urinary Retention in a 47-month-old Girl Caused by the Giant Fecaloma

  • We present a case of a 47-month-old female suffering from acute urinary bladder neck obstruction and bilateral hydronephrosis secondary to a fecaloma. Fecaloma is defined as an accumulation of inspissated feces in the colon or rectum giving the appearance of an abdominal mass. A fecaloma can be developed by diverse causes and the causes of the fecaloma in this case were septum reformation after the Duhamel procedure and long-term constipation. Chronic constipation is very common at outpatient clinic. However, acute urinary retention and voiding difficulty caused by fecaloma in the giant Duhamel pouch has never been reported in Korea. We would like to present our case with acute urinary retention due to a fecaloma and suggest that fecaloma might be considered as one of the causes for acute urinary retention, especially in cases with previous Duhamel operation for repair of Hischsprung disease.

Experimental study on the retention of aerosol particles through concrete cracks under high Reynolds number flow

  • In the event of severe accidents in pressurized water reactor (PWR) nuclear power plants, the potential leakage of radioactive aerosols through containment cracks poses a considerable radioactive hazard to the public. Understanding aerosol transport and retention in cracks helps reduce the conservatism and uncertainty of radioactive hazard assessment. Concrete cracks are recognized as a pivotal pathway for the leakage of radioactive aerosols, and several researchers have undertaken experimental investigations concerning the aerosol transport and retention in concrete cracks. However, the majority of these studies have rather low gas flow Reynolds numbers. In this work, an experimental setup is built to study aerosol transport and retention in concrete cracks under high Reynolds number flow. The TiO2 aerosol with a mass median diameter of 1 ㎛ and two concrete crack specimens are used in experiments. The results of gas flow experiments indicate that the Reynolds number is capable of reaching 10547. Combining the flow experimental data and Suzuki's formula, the equivalent heights of these two crack specimens are approximated as 303.67 ㎛ and 231.48 ㎛. The experimental results indicate a notably high retention rate of aerosols, exceeding 0.8. Furthermore, under high Reynolds number flow, the retention rate varies over a relatively narrow range, with the larger the equivalent height of the crack resulting in a lower retention rate. The experimental results match well with the mechanistic analysis based on inertial deposition theory, demonstrating the rationality of the inertial deposition theory.

A Study on Diagnostic Pattern Questionnaire Associated with Body Mass Index in 20-40's Women (20-40대 여성의 체질량지수에 따른 한방변증지표의 특성 연구)

  • Objectives The purpose of this study was to examine the characteristics of diagnostic pattern questionnaire associated with Body Mass Index in premenopausal women. Methods We studied 41 patients visiting Gangdong Kyung Hee University Hospital from 1st April 2011 to 25th May 2011. The subjects were categorized in two groups, overweight & obesity group (n=12) and low weight & normal group (n=29). We studied the difference of diagnostic pattern questionnaire scores between two groups by Independent samples T-test and correlation between diagnostic pattern questionnaire scores of overweight & obesity group by Pearson's correlation coefficient test using SPSS for windows (version 13.0). Results Blood stasis, retention of undigested food, cold pattern scores of overweight & obesity group were non-significantly higher than low weight & normal group. Heat pattern score of overweight & obesity group was significantly higher than low weight & normal group. Cold-heat, phlegm-cold, blood stasis-cold, phlegm-blood stasis, phlegm-retention of undigested food significantly showed positive correlation coefficient in overweight & obesity group. Conclusions The results suggest that obese women tend to show heat pattern. It seems to be that multiple factors such as phlegm, blood stasis, retention of undigested food are causative of obesity.

Numerical Simulation of Turbulence-Induced Flocculation and Sedimentation in a Flocculant-Aided Sediment Retention Pond

  • A model combining multi-dimensional discretized population balance equations with a computational fluid dynamics simulation (CFD-DPBE model) was developed and applied to simulate turbulent flocculation and sedimentation processes in sediment retention basins. Computation fluid dynamics and the discretized population balance equations were solved to generate steady state flow field data and simulate flocculation and sedimentation processes in a sequential manner. Up-to-date numerical algorithms, such as operator splitting and LeVeque flux-corrected upwind schemes, were applied to cope with the computational demands caused by complexity and nonlinearity of the population balance equations and the instability caused by advection-dominated transport. In a modeling and simulation study with a two-dimensional simplified pond system, applicability of the CFD-DPBE model was demonstrated by tracking mass balances and floc size evolutions and by examining particle/floc size and solid concentration distributions. Thus, the CFD-DPBE model may be used as a valuable simulation tool for natural and engineered flocculation and sedimentation systems as well as for flocculant-aided sediment retention ponds.

A Study on the Retention of Title under UCC (UCC상 소유권유보에 관한 연구)

  • This study describes the retention of title under UCC. UCC ${\S}$ 2-401 prescribes that any retention or reservation by the seller of the title (property) in goods shipped or delivered to the buyer is limited in effect to a reservation of a security interest. It means that the retention of title generates a security interest in the same way like a charge. To be effective, a security interest must be attached or perfected. Once attachment occurs, a security interest is enforceable against the buyer. Once a security interest is perfected, a security interest is enforceable against third parties. Under UCC, a simple retention of title clause is valid as long as it is incorporated into the contract of sale. If the seller acquires a perfected security interest, he as a secured party can enjoy a priority against other creditors. But if a security interest is not perfected but PMSI is afforded, the seller has a special priority. Secondly, under UCC ${\S}$ 9-335 and 9-336, if a security interest under an enlarged retention of title clause is perfected, the seller as the secured party has a valid security interest in the product or mass into which his goods have been mixed or commingled. But in terms of assession of goods, as the seller has a security interest only in his goods, if the seller wants to enlarge his security interest in the whole, a security agreement should cover its intention. Thirdly, under UCC ${\S}$ 9-315, a security interest in proceeds of subsale of goods supplied by the seller is a perfected security interest if the security interest in the original collateral was perfected. But if the proceeds are cash proceeds, it should be identifiable from other funds of the buyer.

Estimation of Kinetic Coefficient and Assimilated Nutrients Mass in SBR Process (연속회분식 반응 공정에서 동역학적 계수 및 미생물합성에 사용된 영양물질 산정)

  • In this study, we investigated the variations of the kinetic coefficients and Chemical Oxygen Demand (COD), N and P mass used for assimilation of a sequencing batch reactor (SBR) system with the variation of SRTs; SRTs of 7.5, 10.0, 12.5, 15.0 and 20.0 days were tested in one cycle of SBR operation to determine the optimum conditions for the operation of the SBR and estimate its COD, nitrogen and phosphorus removal efficiencies. The SBR system was operated under the conditions as follows: an operation time of 6 hours per cycle, a hydraulic retention time (HRT) of 12 hours, an influent COD loading of $0.4kg/m^3/day$, and an influent nitrogen loading of $0.068kgT-N/m^3/day$. The yield coefficient (Y) and decay rate coefficient ($k_d$) were estimated to be 0.4198 kgMLVSS/kgCOD and $0.0107day^{-1}$ by calculating the removal rate of substrate according to the variation of SRT. Considering total nitrogen amount removed by sludge waste process, eliminated by denitrification, and in clarified water effluent with reference to 150 mg/cycle of influent nitrogen amount, the percentage of nitrogen mass balance from the ratio of the nitrogen amount in effluent (N output) to that in influent (N input) for Runs 1~5 were 95.5, 97.0, 95.5, 99.5, and 95.5%, respectively, which is well accounted for, with mass balances close to 100%.

Ca and P Balance in Korean Female Adolescents (청소년기 여자의 칼슘과 인 평형 연구)

  • Intakes and excretions of calcium and phosphorus were determined for 8 female adolescents(aged 16.3$\pm$0.5y; body mass index 20.4$\pm$1.3kg/$m^2$; body fat 33.3$\pm$2.5%; bone mineral density of lumbar spine in L2-L4; 0.96$\pm$0.08g/$\textrm{cm}^2$) when they consumed diets basal and high in calcium for 6 days each. All subjects consumed a basal Ca diet containing 800mg, Korean RDA level of the subjects, and a high Ca diet containing 1200mg, RDA plus 2 SDs of calcium intake. The diets provided 58% of energy intake as carbohydrate, 25% as fat, and 17% as protein. Food, urine, and fecal composites were collected during the last 3 days of each feeding period. Fecal excretion of calcium was significantly greater on the high Ca than on the basal Ca diet. Hence, apparent absorption rate was significantly lowered from 40.9% on the basal Ca diet to 33.1% on the high Ca diet. There was no significant difference in calcium retention between the two diet periods but it tended to be greater on the high Ca diet. However, excretions and retention of phosphorus did not show any significant difference even though intake was significantly increased from 1,253mg on the basal Ca diet to 1,583mg on the high Ca diet. The results indicate that higher calcium intake than the Korean RDA level is recommended for adolescents to meet peak bone mineral accretion and attain a desirable level of calcium retention. Dynamic MRM Measurements of Multi-Biomarker Proteins by Triple-Quadrupole Mass Spectrometry with Nanoflow HPLC-Microfluidics Chip

  • The development of clinical biomarkers involves discovery, verification, and validation. Recently, multiple reaction monitoring (MRM) coupled with stable isotope dilution mass spectrometry (IDMS) has shown considerable promise for the direct quantification of proteins in clinical samples. In particular, multiple biomarkers have been tracked in a single experiment using MRM-based MS approaches combined with liquid chromatography. We report here a highly reproducible, quantitative, and dynamic MRM system for validating multi-biomarker proteins using Nanoflow HPLC-Microfluidics Chip/Triple-Quadrupole MS. In this system, transitions were acquired only during the retention window of each eluting peptide. Transitions with the highest MRM-MS intensities for the five target peptides from colon cancer biomarker candidates were automatically selected using Optimizer software. Relative to the corresponding non-dynamic system, the dynamic MRM provided significantly improved coefficients of variation in experiments with large numbers of transitions. Linear responses were obtained with concentrations ranging from fmol to pmol for five target peptides.

Identification of Iranian Vectors of Malaria by Analysis of Cuticular Hydrocarbons

  • Twenty-eight Anopheles species has been so-far identified in Iran, while only 8 species was proved as malaria vector. In this study, we principally examined the cuticular hydrocarbon (CHC) potency in identification of Iranian vectors of malaria and then differentiation of vector and non-vector species of Anopheles. Seven species of malaria vectors and the non-vector species, Anopheles claviger were collected throughout Iran. Female extracts were made out of every five conspecific specimens by surface immersion in pure n-hexane. Each sample was injected into a FID-GC instrument along with the known concentrations of standards. CHC profiles of the eight Anopheles species indicated no qualitative difference. The average mass of each eluted CHC were compared using Repeated ANOVA and Mann-Whitney tests. Results confirmed a significant difference in mass of each single CHC at a specific retention time (RT). Statistical comparison of CHC mass in An. sacharovi, An. stephensi, An. culicifacies and An. fluviatilis at RT 39.6 indicated significant differences (P<0.05) among these species. Analysis of CHC mass of An. dthali, An. superpictus & An. sacharovi at RT 28.5, An. stephensi & An. sacharovi at RT 30.7 and An. sacharovi & An. claviger at RT 30.6 similarly indicated significant differences (P<0.05). An. sacharovi could be distinguished from other species, which showed only trace, by integratable peaks at retention times of 29.7, 31 and 32.6. Similarly, An. claviger could be distinguished from the other species with a trace peak at RT 30.6. In order to separate An. stephensi from the five other species, the integratable peak at RT 30.7 was used. An. dthali could be identified at RT 26.2 by an integratable peak v.s. the trace peaks of other species. An. superpictus had indicator peaks at RTs 27.4 & 28.5 v.s. trace peaks of other species. An. maculipennis with its trace peak at RT 39.6 could be easily differentiated from An. fluviatilis & An. culicifacies. This study proved that all of the examined species of Anopheles could be well identified based on their quantitative differences in CHCs, except for An. fluviatilis & An. culicifacies for which no CHC indicator peak was detected.

Identification of Pitfalls Related to the Analysis of Liquid Chromatography-Tandem Mass Spectrometry and Liquid Chromatography-Time of Flight Mass Spectrometry (액체크로마토그래프-질량분석기를 이용한 정성 및 정량 오류의 확인)

  • BACKGROUND: To identify the sources of inaccuracy in LC/MS/MS methods used in the routine quantitation of small molecules are described and discussed. METHODS AND RESULTS: Various UPLC coupled to triple quadrupole mass spectrometer and time of flight (TOF) were used to identify the potential sources of inaccuracy and inducing the pitfalls of qualification and quntitation during the veterinary drug residue analysis. Some of stable isotope labelled veterinary drugs, which were used as internal standards, presented "cross-talk", regardless of manufactures of mass spectrometer and types of spectrometer. Group of sulfonamides also presented inaccuracy qualification and quantitation due to the multi-residue analytical method with the same fragment ions at the close retention times. CONCLUSION: The phenomena of "cross-talk" occurring between subsequently monitored transition from stable isotope labelled and isotope non-labelled authentic chemical were identified. To prevent errors and achieve more accurate data during the analysis of small molecules by LC/MS/MS SRM method, Followings should be taken care of and kept checking; purity and concentration of stable isotope as an internal standard, prevention of carry-over during the separation in column, minimizing the ion suppression by matrix effect, identification of retention time, precursor ion and product ion, and full knowledge of data processing including smoothing and peak integration.
