척추 수술 후 발생된 요부 및 좌측 대퇴전부 통증 치료를 위한 대요근 근구 차단술의 효과 -증례 보고- (Psoas Compartment Block for the Relief of Lumbar and Left Thigh Pain after Operation of Second Lumbar Compression Fracture -A case report-)

  • Psoas compartment block has been used to provide anesthesia and analgesia of hip joint. This block is advocated for relief of pain of various origins in the thigh, leg and lumbar area. A-40-year-old women complained of pain in the left thigh and lumbar area after operation of the second lumbar vertebral compression fracture. To relieve pain, caudal block was performed. This block reduced in lumbar pain but left thigh pain persisted. So, we were performed psoas compartment block using mepivacaine and dexamethasone, which relieved the pain in the left thigh and lumbar area. We recommend psoas compartment block as useful and simple method for patients with thigh and lumbar area pain, especially when the epidural block is not feasible.

추나요법에서 복와위 요천관절 신연기법에 대한 제언 (Suggestion of Sacral Lumbar joint distraction technic with prone position in Chuna Manual Therapy)

  • Objectives : This study aimed to introduce a new sacral lumbar joint distraction technique in the prone position in Chuna manual therapy for effective lumbar distraction. Methods : The patient was placed in the prone position. Next, clinicians contacted the sacrum and L1, L2, L3, L4 and L5 spinal processes. Then they were pulled in the direction of the head and leg respectively while pressing down with 30% force to maintain the lumbar lordosis. This is a more effective and safer lumbar distraction technique according to the latest knowledge. Conclusions : This technique can be used for lumbar disc herniation in addition to relaxing the existing lumbar muscles, and is suggested to be used as a safe and effective Chuna manual therapy.

20대 정상성인의 구두굽 높이에 따른 요추전만도의 변화 (Changes of Lumbar Lordosis Caused by Different Heel Heights in Healthy Women)

  • In today's society, many women wear high-heeled shoes, but the effect of heel height on lumbar lordosis has not been clearly defined. The objective of this study was to identify the influence of heel height and general characteristics of subjects on lumbar lordosis. The subjects of this study were 40 healthy women who were students of the Department of Physical Therapy, College of Rehabilitation Science, Taegu University. Flexible ruler measurement was used to measure the lumbar lordosis at barefoot, 3 cm and 7 cm high-heeled standing positions. The results were as follows: 1) Significant statistical decrease in lumbar lordosis was observed as heel heights were increased from barefoot to 7 cm high heel. 2) There were no statistically significant differences between lumbar lordosis according to three different heel heights and weight, body mass index. 3) Lumbar lordosis measured at different heel heights was related to subject's height. With increasing subject's height, lumbar lordosis that measured from each heel height was significantly decreased. As heel heights were increased from barefoot to 7 cm high heel, significant statistical decrease in lumbar lordosis was observed in the subjects whose height were 151~160 cm. 4) Intrarater reliability on lumbar lordosis taken with a flexible ruler was good, with Cronbach ${\alpha}$ values of 0.8971 for barefoot, 0.8107 for 3 cm and 0.9002 for 7 cm high-heeled standing positions.

Effects of Static and Dynamic Stretching on Lumbar Lordotic Angle and Low Back Pain in University Students

  • Background: Lumbar lordosis is a result of muscle shortening and may cause low back pain. Objective: To examine the effects of static and dynamic stretching on lumbar lordosis and low back pain in university students. Stretching is an intervention that can be applied to shortened muscles; however, very few studies have compared the effects of static and dynamic stretching on lumbar lordosis and low back pain. Design: Randomized controlled clinical trial (single-blind) Methods: The 12 selected subjects were randomly assigned static stretching and dynamic stretching groups each containing six students. The subjects in each group performed their respective stretching programs for 17 minutes, 3 times a week for 4 weeks. Lumbar lordotic angle, low back pain, and Oswestry Disability Index (ODI) were measured before and after the intervention. Results: Intragroup comparisons showed significant reductions in lumbar lordotic angle and low back pain in the static stretching group while the dynamic stretching group showed significant decreases in lumbar lordotic angle, low back pain, and ODI. The intergroup comparisons showed significantly greater differences between pre- and post-intervention in lumbar lordotic angle and low back pain in the dynamic stretching group compared to those in the static stretching group while ODI did not show any intergroup difference. Conclusions: The results of this study indicated that, while both static and dynamic stretching helped to reduce the lumbar lordotic angle and low back pain, dynamic stretching was more effective in alleviating lumbar lordotic angle and low back pain compared to static stretching.

Effects of lumbar stabilization exercise according to correct verbal instructions in pain and muscle strengthening of low back pain patient

  • Objective: This study was aimed at investigating the effects of lumbar stabilization exercise according to correct verbal instructions in pain and muscle strengthening of the low back pain patients. Design: A randomized controlled trial. Methods: Twenty subjects with low back pain were selected. They were randomly assigned to one of two groups (10 in each group): namely the lumbar stabilization exercise and lumbar stabilization exercise according to the correct verbal instructions group. The lumbar stabilization exercise group performed lumbar stabilization exercises for 6 weeks (5 times a week). The lumbar stabilization exercise according to correct verbal instructions group performed lumbar stabilization exercise according to correct verbal instructions for 6 weeks (5 times a week). We measured pain, muscle power, proprioception, and body balance before and after exercise by using visual analog scale (VAS), digital handheld dynanometer, Joint repositioning error, time up and go test respectively. Results: We found statistically significant differences in pain, muscle power, proprioception, and body balance in lumbar stabilization exercise and lumbar stabilization exercise according to correct verbal instructions group, before and after (p<0.05). Conclusions: We confirmed the effect of lumbar stabilization exercise according to correct verbal instructions. Thus we thought these results could be used as basic data and reference for low back pain. But we need more study effect of correct verbal instructions on other exercises.

등-허리뼈 분리동작과 슬럼프 동작이 등-허리뼈 척추세움근과 배곧은근 활성도에 미치는 효과 (The Effects of Thoracic-Lumbar Dissociate and Slump Motions on Thoracic-Lumbar Erector Spinae and Rectus Abdominis Activity)

  • Purpose: The purpose of this study was to investigate the effect of thoracic-lumbar dissociation motion and slump motion on thoracic-lumbar erector spinae and rectus abdominis muscle activity. Methods: Seventeen healthy adult volunteers participated in this study. All participants performed two motions (thoracic-lumbar dissociation motion, slump motion). Muscle activation during the two motions was measured using a surface electromyography device. The data from this were collected from the iliocostalis thoracis, iliocostalis lumborum, and rectus abdominis. The activities of these muscles before and after each motion were then compared. Results: The iliocostalis thoracis activation was significantly greater during the thoracic-lumbar dissociation motion than during the slump motion (p <0.05). The iliocostalis lumborum activation was greater during the slump motion than during the thoracic-lumbar dissociation motion (p <0.05). The rectus abdominis activation was lesser during the slump motion than during the thoracic-lumbar dissociation motion (p <0.05). Conclusion: This study confirmed that individual contraction of the erector spinae muscles is possible during thoracic-lumbar dissociation motion, which increases the stability of the thoracic spine. In addition, this motion could improve control of the rectus abdominis. Therefore, thoracic-lumbar dissociation motion should be considered for rehabilitation programs for patients with kyphosis and back pain.

  • Low back pain (LBP) is a significant in today's society, with lifetime include factors associated with LBP ar reporter. Among the causes, aberration of posture may play a role in the development of LBP. Many investigators have assessed the curvature of spine in standing posture. But LBP is associated with Lumber Hyperlordosis of Hyperlordosis is controversial Subjects: In conservative treatment(acupuncture, herb med, manipulation & TENS. exercise, potural correction) for a 40 years old woman who had low back pain(V AS) be caused by decrease lumbar lordotic angie. Objectives: The object is change of lumbar lordotic angle of a 40 years old woman who had low back pain with Lumbar hyperlordosis, In conservative treatment. Method: In conservative treatment, We added taping therapy(mechanical correction taping of Kinesio Taping) about Lumbar Lordosis. Conclusion: We experienced a 40 years old woman who had love pack pain with Lumbar hyperlordosis. In conservative treatment, Her pain was Improved by additional taping therapy In company with decrease of Lumbar Lordosis. 1. abnormal spinal curvature, specially lumbar hyperlordosis act on induction & perpetuation agent for low back pain 2. In a patient had low back pain with lumbar hyperlordosis, change of lumbar lordotic angle is of utility value for the effect of treatment and assessment of prognosis. 3. pain control is more relative with change of lumbosacral angle than lumbar lordotic angle, in patient had low back pain with lumbar hyperlordosis. 4. mechanical taping therapy with elastic adhesive tape is effective for patient had low back pain with lumbar hyperlordosis

안정화운동과 신장운동이 요통환자의 근력과 통증에 미치는 영향 (The Effects of Stabilization Exercise and Stretching Exercise on Muscle Strength and Pain of Patients with Lower Back Pain)

  • Purpose : The purpose of this study was to compare the effect of three types of therapeutic exercises by applying them to lower back pain patients. Methods : This program was conducted for 30 patients 30 to 55 years old, who visited a rehabilitation center due to lower back pain. We separated participants into three groups with different therapeutic exercises: one with lumbar stabilization exercises, another with stretching exercises, and the other with both exercises. Each exercise was held once a day, 3 days a week, for 6 weeks. We analyzed the effect of these exercises by checking the change of lumbar muscle strength and pain relief. Lumbar muscle strength was measured by AS-Med and pain strength was estimated by VAS. Results : The result of the programs was established according to the following list: 1) Lumbar stabilization exercises and stretching exercises lead to higher lumbar muscle strength and pain relief (p<0.05). 2) Lumbar muscle strength in the lumbar stabilization exercise group was significantly higher than the stretching exercise group (p<0.05). 3) Pain relief in the stretching exercise group was significantly higher than the lumbar stabilization exercise group (p<0.05). Conclusion : This study shows all of the groups experienced higher lumbar muscle strength and pain relief. Specifically, there was higher lumbar muscle strength in the lumbar stabilization exercise group and higher pain relief in the stretching exercise group. Therefore the ideal intervention to improve lumbar muscle strength and pain relief for patients is to implement both lumbar stabilization exercises and stretching exercises.

The effects of motion taping on young males' lumbar stabilization exercise

  • PURPOSE: This study was conducted to examine the effects of motion taping on the effects of lumbar stabilization exercise. METHODS: The subjects of this study were 24 normal males with no problem in their nervous systems or musculoskeletal systems. The subjects were randomly assigned to a control group of 12 subjects and an experimental group of 12 subjects. The control group was applied with lumbar stabilization exercise while the experimental group was applied with lumbar stabilization exercise after being attached with tapes. The intervention period was four weeks in total and the subject underwent the exercises three times per week. The exercise time per session was set to one hour in principle consisting of warm-up exercise and cool-down exercise for 10 minutes each and main exercise for 40 minutes. Lumbar muscle strength was measured as a factor to examine the effects of lumbar stabilization exercises. RESULTS: The control group showed an increase in lumbar muscle strength from $111{\pm}6.30kg$ to $113{\pm}6.23kg$ and the experimental group showed an increase in lumbar muscle strength from $108{\pm}6.46kg$ to $116{\pm}5.21kg$. The increases shown by both groups were statistically significant and the experimental group that was applied with motion taping showed a larger increase in lumbar muscle strength compared to the control group. CONCLUSION: Lumbar stabilization exercise is considered to be good for lumbar stability and muscle strengthening and lumbar stabilization exercise applied with motion taping is considered to show larger effects for muscle strengthening and the improvement of lumbar functions.

요부 안정화 운동 프로그램이 축구선수의 체간 및 하지 근활성도와 균형에 미치는 영향 (Effects of Lumbar Stability Exercise Program on Trunk, Lower Extremity of Muscle Activity and Balance in Soccer Player)

  • Purpose: The purpose of this study is to provide an efficient and scientific basis for muscle activity (%MVIC) of RA, EO, VL, HS muscles and balance in soccer players through dynamic lumbar stability exercise and static lumbar stability exercise. Methods: This study included 23 soccer players belonging to D University of J province who attended the program for 30 minutes at a time and three times a week for 4 weeks. Of these 13 attended the dynamic lumbar stability exercise (DLSE) program and 10 the static lumbar stability exercise (SLSE) program. The differences between the effects of the dynamic lumbar stability exercise program and static lumbar stability exercise program were analyzed. Results: To increase muscle activity (%MVIC) and balance (WPL), the dynamic lumbar stability exercise program was more effective than was the static lumbar stability exercise program. 1) The %MVIC of trunk muscle (RA &EO) and lower extremitys muscle (VL & HS) increased from before training to after training in the case of the participants who performed the dynamic lumbar stability exercise. 2) The whole path length (WPL) decreased from before the training to after the training. The 2 groups significantly differed in this regard. Conclusion: Dynamic lumbar stability exercise program helps to improve the balancing ability and muscle activity in a soccer players who requires both muscle activity and balance than does any other players.
