  • A 32-year-old female patient and her sister show high levels of high density lipoprotein (HDL) cholesterol in regular health checkups, since female patient was 11 years old. The patient's serum total cholesterol was 285 mg/dL and HDL cholesterol was 113 mg/dL. Her sister's total cholesterol was 240 mg/dL and the HDL cholesterol measured to be 90 mg/dL. Lipoprotein pattern and cholesteryl ester transfer activity gene analysis were examined in these patients. We found c.1321+1G>A (IVS14+1G/A) hetero mutation in cholesteryl ester transfer protein (CETP) genes. Generally, CETP mediates transfer and exchange of triglycerides and cholesteryl ester between plasma lipoproteins. Also we investigated a key role of HDL-CE and Apo A-1 metabolism. Patients with low levels of CETP have increased serum HDL levels. We hereby report two Korean cases of CETP deficiency in a family. Brief literature review ensues with the cases.

  • Soybean, rice and nuts are the staple foods in South East Asia. In order to clarify the function of these foods in relation to coronary heart diseases, we have done a series of experiments using animals and humans. Soybean protein preparations (SPI) in comparison with an animal protein, casein (CAS), resulted in reducing atherosclerotic lesion development in apolipoprotein (apo) E deficient mice that develop an advanced lesion similar to that in humans. This action was attributed to the protein, since the ethanol-extracted SPI (EE-SPI), from which isoflavones and saponins removed, lowered the lesion development in apo E-deficient mice. However, The EE-SPI, compared to the SPI, resulted in a decrease of mRNA for key proteins of cholesterol metabolism (low density lipoprotein receptor and cholesterol 7$\alpha$-hydroxylase) in hypercholesterolemic rats, followed by an elevation of the serum cholesterol level, indicating a contribution of isoflavones to the serum cholesterol level. Rice protein (RPI) was also effective to lower the lesion development in apo E deficient mice. Both the SPI and RPI led to an increased level of the serum NOiN03, metabolites of NO. This effect appeared to be attributed to their high content of arginine. Besides, C57BL/6J mice fed a diet containing whole grain rice had an elevation of the serum high-density lipoprotein cholesterol. Furthermore, rice bran oil and the unsaponifiable fraction that is rich in plant sterols were effective to lower serum cholesterol levels in hypercholeserolemic rats. Finally, we tested if walnuts consumption would be hypocholesterolemic for Japanese. A diet containing walnuts effectively lowered the serum total and low density lipoprotein cholesterol in Japanese men and women. These animal and human experiments would be relevant to advocate publics to keep consuming diets including soybean, rice and nuts for their healthful life.

  • Lifestyle and dietary behavior intervention as the primary prevention of lipid disorder seems safe and compatible with other treatments of cardiovascular diseases. Cross-sectional associations between lifestyle factors and dietary behavioral factors with plasma lipid and lipoprotein levels were analyzed in 189 middle-aged men in Suwon, Korea. Overnight fasting plasma levels of total cholesterol, high-density lipoprotein(HDL)-cholesterol, triacylglycerol and glucose were analyzed. Blood pressure and anthropometric data were also measured. Lifestyle factors such as smoking status, alcohol consumption and frequency of physical exercise were evaluated by a self-administered questionnaire. Questions regarding dietary behavior were also asked. The subjects were 43.8%${\pm}$7.9 years old, and 23.8%${\pm}$2.6kg/m$^2$. From stepwise regression analyses, significant correlates with total cholesterol level were body mass index(BMI), alcohol intake(negative), age and coffee drinking(model R$^2$=14.3%). BMI, breakfast-skipping, age, and sleeping hours were significant for triacylglycerol level(model R$^2$=15.8%). BMI, alcohol drinking(negative), age, and coffee drinking were significant for low-density lipoprotein(LDL)(model R$^2$=11.7%). Age(negative), BMI(negative), alcohol drinking, stress level(negative), physical exercise, and cigarette smoking(negative) were significant for high-density lipoprotein(HDL)(model R$^2$=12.1%). From stepwise regression analyses, excluding BMI and age as factors in the model, alcohol intake(negative) and coffee drinking were significantly correlated with total cholesterol level(model R$^2$=4.4%) : breakfast-skipping with triacylglycerol(model R$^2$=3.2%) : alcohol intake (negative) with LDL level(model R$^2$=3.4%) : alcohol intake, physical exercise and stress level(negative) with HDL level(model R$^2$=6.3%). The findings suggest that a healthy daily lifestyle and dietary behavior may have an anti-atherogenic effect by altering plasma lipid and lipoprotein levels in middle-aged Korean men. • The mixture of Ginseng Radix and Crataegi Fructus (Gen-CF) was developed to increase the pharmacological effect of ginseng in the treatment of hypercholesterolemia and prevention of cardiovascular disease. This study evaluated the effects of Gen-CF on serum lipids of hypercholesterolemic rats in vivo, as well as its antioxidant activities in vitro, and explored its clinical effects on patients with hypercholesterolemia. In vitro, Gen-CF displayed 1,1-diphenyl-2-picrylhydrasyl and superoxide radical scavenging activities, and inhibited hemolysis induced by 2,2'-azobis-2-amidinopropane dihydrochloride in a dose-dependent manner. In vivo, Gen-CF significantly inhibited the increases of total cholesterol, low-density lipoprotein cholesterol and triglyceride in high cholesterol-diet and Triton WR-1339 models. It also significantly inhibited the decrease of high-density lipoprotein cholesterol in these models. In the clinical trial, Gen-CF significantly lowered total cholesterol, low-density lipoprotein cholesterol, triglyceride, total lipid and phospholipid, with no adverse events, including hepatic or renal toxicity. The data suggest that Gen-CF has the potential to treat hypercholesterolemia and prevent cardiovascular disease.

  • An aqueous extract of oriental herbal composition named Onchengyeum and curcumin, an antioxidant isolated from turmeric (Curcuma Zonga L.) reduced hepatotoxicity induced by carbon tetrachloride ($CCI_4$). Improved liver function was observed by measuring the activities of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), blood urea nitrogen (BUN), creatinine (CRE), total cholesterol (T-CHO), triglyceride (TG), low density lipoprotein cholesterol (LDL-CHO), high density lipoprotein cholesterol (HDL-CHO), total protein (TP), albumin (ALB) and total bilirubin (BIL) in serum. Hepatic parameters monitored were levels of cholesterol (CHO), triglyceride (TG), and malondialdehyde (MDA) and activities of cytochrome P450 (CYP), NADPH-CYP reductase, superoxide dismutase (SOD), catalase (CAT), glutathione (GSH), glutathione S-transferase (GST), glutathione reductase (GR), and glutathione peroxidase (GPx). The histopathological examination showed that the treatment of Onchengyeum and curcumin relieved the ballooning degeneration of hepatocytes which had been generated by $CCI_4$. The results suggested that hepatoprotective effects of Onchengyeum and curcumin possibly are due to their promising antioxidative activity.

  • The lowering effect of cholesterol in Sprague Dawley rats was investigated with panaxydol which was purified from the petroleum ether soluble fraction(PESF) of Korean red ginseng. The level of total cholesterol(TC), Triglyceride(TG) and low density lipoprotein(LOL)-cholesterol in serum was reduced by $48\%,\;47\%\;and\;41\%,$ respectively while high density lipoprotein(HDL)-cholesterol was in creased up to $29\%$ as compared with their control values when the panaxydol(20 umoles. 5 mq/kq/day) was adminstered by intraperitoneal rout for 3 consecutive days along with a $1\%$ cholesterol diet. The hepatic ester cholesterol content which was increased in proportion to the cholesterol content of the diet in the control, clearly decreased with panaxydol administration to about $40\%$ regardless of the two diet cholesterol content. $1\%\;or\;2\%.$ A threshold of supression on the serum lipid levels in both administration routes was observed: the maximium suppression in i.p. and p.o. administration was observed to be at 5mq/kq b.w. and in the range of 50 - 100 mg/kg b.w., respectively. Panaxydol may reduce serum lipid and cholesterol levels by inhibiting cholesterol absorption and/or by modulating the cholesterol metabolism. at least in part.

  • BACKGROUND/OBJECTIVES: High levels of plasma low-density lipoprotein (LDL) cholesterol are an important determinant of atherosclerotic lesion formation. The disruption of cholesterol efflux or reverse cholesterol transport (RCT) in peripheral tissues and macrophages may promote atherogenesis. The aim of the current study was to examine whether bioactive ellagic acid, a functional food component, improved RCT functionality and high-density lipoprotein (HDL) function in diet-induced atherogenesis of apolipoproteins E (apoE) knockout (KO) mice. MATERIALS/METHODS: Wild type mice and apoE KO mice were fed a high-cholesterol Paigen diet for 10 weeks to induce hypercholesterolemia and atherosclerosis, and concomitantly received 10 mg/kg ellagic acid via gavage. RESULTS: Supplying ellagic acid enhanced induction of apoE and ATP-binding cassette (ABC) transporter G1 in oxidized LDL-exposed macrophages, facilitating cholesterol efflux associated with RCT. Oral administration of ellagic acid to apoE KO mice fed on Paigen diet improved hypercholesterolemia with reduced atherogenic index. This compound enhanced the expression of ABC transporters in peritoneal macrophages isolated from apoE KO mice fed on Paigen diet, indicating increased cholesterol efflux. Plasma levels of cholesterol ester transport protein and phospholipid transport protein involved in RCT were elevated in mice lack of apoE gene, which was substantially reduced by supplementing ellagic acid to Paigen diet-fed mice. In addition, ellagic acid attenuated hepatic lipid accumulation in apoE KO mice, evidenced by staining of hematoxylin and eosin and oil red O. Furthermore, the supplementation of 10 mg/kg ellagic acid favorably influenced the transcriptional levels of hepatic LDL receptor and scavenger receptor-B1 in Paigen diet-fed apoE KO mice. CONCLUSION: Ellagic acid may be an athero-protective dietary compound encumbering diet-induced atherogenesis though improving the RCT functionality.

  • This study investigated the effects of Opuntia ficus-indica and other natural resources (OF) in db/db and C57 mice. Plasma triglycerides, cholesterol, alanine aminotransferase (ALT) activity, aspartate aminotransferase (AST) activity, fecal bile acid excretion, the histopathological appearance of the liver, and cholesterol-related mRNA expression were determined. Mice (12 db/db mice and 12 C57 mice) were assigned to diabetic-control (db-C), diabetic-OF treatment (db-OF), normal-control (C57-C), and normal-OF treatment (C57-OF) groups. Animals in the control group were fed an AIN-76 recommended diet and animals in the OF group were fed an experimental diet containing 5% of OF for 4 weeks. Concentrations of total plasma cholesterol, triglyceride, low density lipoprotein (LDL)-cholesterol, and very low density lipoprotein (VLDL)-cholesterol decreased with the administration of OF. In contrast, high density lipoprotein (HDL)-cholesterol levels were minimally affected by the experimental diet. Plasma AST and ALT showed lower activities in the db-OF group, and the fecal excretion of bile acid was reduced in the db-OF group. Histopathological analysis of the liver showed that fatty liver conditions in the db-OF group were more improved than db-C. Low-density lipoprotein receptor (LDL-R) and cholesterol 7${\alpha}$-hydroxylase (CYP7A1) mRNA expression were increased in the db-OF group as well. However, 3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMG-CoA-R) mRNA expression was lower in the db-OF group. These results provide experimental evidence about improved lipid metabolism of the OF feeding in the db/db mice.

  • The purpose of this study is to promote understanding of physical activity and metabolic syndrome in middle-aged adults and to provide basic data of interventions development for the prevention and management of metabolic syndrome. Using the 2020 data for Korea National Health and Nutrition Examination Survey, 1,786 middle-aged adults between the ages of 40 and 64 with no missing data were analyzed. As a result of the study, 56.5(±2.1)% of men and 52.9(±1.81)% of women were sufficient activity group among physical activity, and the proportion of men was higher than that of women, but it was not statistically significant(t=1.27, p=.207). The prevalence of metabolic syndrome was 38.9(±2.1%) of men and 25.4(±1.5)% of women, the prevalence of men was significantly higher than that of women(t=5.12, p<.001). Compared to the insufficient activity group, the sufficient activity group had a 0.71(95% CI: 0.57~0.88) times the risk of developing low HDL(high density lipoprotein)- cholesterol(p=.002), and this pattern was maintained even after adjusting for age, education level, body mass index, smoking status, and drinking status(p=.002). Based on the results of this study, a physical activity and metabolic syndrome risk group in middle-aged adults should be selected, and physical activity promotion program to improve high density lipoprotein-cholesterol among metabolic syndrome indicators should be developed.
