Recruitment Patterns of Lumbar Extensor and Hip Extensors in Trunk Flexion and Extension (체간 굴곡과 신전 시에 요부 신근과 고관절 신근의 동원패턴)

  • Purpose: We determined the recruitment pattern of lumbar elector spinalis, gluteus maxims, inner and outer hamstring muscle during trunk flexion and extension. Methods: Thirty healthy subjects(male; 15, female; 15) without low back pain and other problems in lower extremities participated in this study. To measure the recruitment pattern, the onset times of electromyographic activity of the muscles were recorded during trunk flexion and return(extension) to standing position. Results: The medial and lateral hamstring muscle was activated first, next elector spinalis, the last, gluteus maximus in trunk flexion. In trunk extension to standing position, the order of recruitment was similar to trunk flexion although the frequency is different. There were different between male and female in flexion and extension movement. Conclusion: The recruitment order of lumbar extensor and hip extensors in trunk flexion and extension will provide database in evaluation and intervention of lower back pain and lumbo.pelvic rhythm disorder.

Parachordoma of the Chest Wall -1 case report- (흉벽에 발생한 유척삭종(Parachordoma) -1예 보고-)

  • Parachordoma is a very rare, slow-growing, and low-grade malignant tumor that occurs in the extremities and trunk. The differential diagnosis includes extraskeletal myxoid chondrosarcoma and chordoma in the histologic finding. Thus, histologic findings with immunohistochemistry may be helpful in distinguishing parachordoma from extraskeletal myxoid chondrosarcoma and chordoma. I report with a brief review of literatures one case of parachordoma of the chest wall which was successfully treated by en-bloc resection and chest wall reconstruction using 2 mm Gore-Tex$^{\circledR}$ soft tissue patch and free from recurrence for 16 months.

  • Malignant fibrous histiocytoma(MFH) is relatively rare in the oro-maxillofacial region, particularly in the oral cavity. MFH usually arise in the extremities, the thigh being the most common site. The incidence of malignant fibrous histiocytoma in bones is rather low compared with that in soft tissues. MFH is predominant in the 40s and 50s. Histologically, the lesion are said to show high cellularity with fibrous stroma, cellular and nuclear pleomorphism, an admixture of fibroblast-like spindle cells which tend to be arranged in whorls or cartwheel or storiform patterns, rounded mononuclear cells and multinucleated giant cells. The cells frequently have abundant eosinophilic cytoplasm which has a foamy or vesicular appearance. Treatment consists of varying combinations of radiation therapy, chemotherapy, and surgery. We have observed a case of malignant fibrous histiocytoma occured in the right maxilla of 32-year-old woman.

Lumbar Sympathetic Block for Spinal Cord Infarction Patient -A case report- (요부교감신경차단으로 척수경색 환자의 하지통증 치료 -증례보고-)

  • This report describes a case of spinal cord infarction after acupuncture. The patient was treated with lumbar sympathetic block with using C-arm fluoroscopy. A 66-year-old patient with chronic low back pain and radiating pain in the lower limb was treated with acupuncture and he suddenly had a loss of motor and sensory of both lower extremities. His clinical presentaion and neuroimaging studies were consistent with spinal cord infarction. He was treated with steroid megatherapy and he showed improved in motor function, but there was no pain relief despite the phamacological treatments that were combined with caudal blockade. He visited to our hospital and had lumbar sympathetic blockade performed. The pain was relieved without any related complication after 1 month (VAS $9/10{\rightarrow}2/10$), and he has been content with the results of treatment.

  • Erythema nodosum is a nodular erythematous eruption usually limited to the low extremities. Erythema nodosum seems to be Gwadungjun(瓜藤纏) and Damp-heat flow(濕毒流注) in oriental medicine. The symptom of erythema nodosum is similar to that of Gwadungjun, so we investigated the literatures of oriental medicine and recet medicine. The results were summarized as follows; 1. In Oriental medicine, erythema nodosum regarded as Damp-heat flow and Gwadungjun. 2. Inflammatory nodus of lower extremity, ie, similar to Gwadungjun, Damp-heat flow, Bichonbal(비천發), Samnibal(三里發), Ududok(魚두毒), etc. 3. Erythema nodosum results from inner damp-heat(內有濕熱), affection due to pathogenic wind(外感風邪). 4. Treatment is to clear away heat(淸熱), remove dampness(利濕), heat from blood(凉血), cold-damp(祛寒), and promote blood flood(活血通絡). 5. We can use Bangpungtongsungsan(防風通聖散), Sopunghwa1hyultang(疏風活血湯), and Danguijumtongtang(當歸拈通湯), etc.

A Change of Balance Ability by Leg strength exercise of Female University Students (여대생의 하지근력운동에 따른 균형능력의 변화)

  • Purpose. This study was conducted to find out about the differences in the women's ability to balance women in the movement to strengthen their lower extremities. Methods. The study was a functional reach test and Y Balance test, to examine the static and dynamic balance of women in their 20s who do not have a surgical history at H University. For the strengthening of the lower strength, the lower leg muscles were reinforced by half squats and the thera band movement. The SPSS ver.18.0 Wilcoxon test was used to see the difference in balance as an analysis. Results. The results of the study showed a statistically significant difference in balance ability before and after the intervention. Conclusions. Steady low-strength campaigns can improve muscle strength and balance, and further improve quality of life.

The Effect of One Leg Deadlift Exercise on the Muscle Activity of Lower Extremities according to the Weight Characteristics

  • Purpose: This study examined the effects of a kettlebell and aqua bag on the gluteus maximus, gluteus medius, and vastus medialis lower extremity muscle strength during a one-leg deadlift, a top-down exercise. Methods: Twenty healthy adults were enrolled in the one-leg deadlift exercise under two conditions. The muscle activity of the gluteus maximus, gluteus medius, and vastus medialis was measured by surface electromyography (S-EMG). A paired t-test was used for statistical analysis, and the statistical significance criteria were set to p<0.05. Results: A comparison of the muscle activity according to the degree of contraction of the gluteus maximus revealed low muscle activity in AD exercise compared to KD exercise (p<0.05), and there was no significant difference in the gluteus medius and vastus medialis (p<0.05). Conclusion: These results suggest that when the one-leg deadlift exercise is performed according to the weight characteristics, the gluteus maximus is suitable for intervention using a kettlebell, and the gluteus medius and vastus medialis are more suitable for intervention by providing instability using an aqua bag.

Primary Fibrosarcoma of Right Atrium: A Case Report (우심방에 발생한 원발성 섬유성 육종: 1례 치험 보고)

  • A 51 year old man was admitted to the Thoracic and Cardiovascular Department of Kyungpook University Hospital on April 7, 1976, with chief complaints of orthopnea and the chest pain for about 3 months. Physical examination showed narrow pulse pressure, puffy face, engorged neck veins at sitting position, distant heart sound, enlarged liver and edematous upper extremities. The chest roentgenogram demonstrated markedly enlarged cardiac silhouette. Low voltage and the low to diphagic T`s were noted on the electrocardiogram. Paroxysmal ventricular tachycardia was developed intermittently and was subsided spontaneously. Repeated pericardiocentesis were performed each of which yielded from 100 to 300ml. but intractable cardiac failure was progressed. The bacteriology and cytology of the pericardial fluid were not revealed any specific findings. The pericardiectomy was performed to release the intractable cardiac tamponade. Pericardium was found to be thickened and cardiac constriction was noted. The thickened pericardium was easily removed. A large hen`s egg sized dark blue tumor mass occupied the anterior wall of the right atrium and two thumb tip sized pearl gray tumors were placed at the just below portion of the main pulmonary artery. The biopsy report revealed primary fibrosarcoma of the heart. The patient was improved from the symptoms of the cardiac failure during the postoperative course.

Melanoma Incidence Mortality Rates and Clinico-Pathological Types in the Siberian Area of the Russian Federation

  • Russian rates for melanoma incidence and mortality are relatively low as compared to some other white populations but the tumor is of increasing importance. In this paper, data are based on a retrospective descriptive analysis of melanoma epidemiology and clinicopathological characteristics in Krasnoyarsk Territory belonging to the Siberian Federal District of the Russian Federation. The age-adjusted incidence and mortality rates for the period 1996-2009 were determined with subsequent retrospective analysis of clinicopathological data of 103 primary melanoma cases. Our results showed that incidence and mortality rates in the region under consideration match the Russian national trends and correspond to epidemiological data of the countries of Eastern Europe. Stratification of melanoma cases by age, sex, clinicopathological state and localization revealed a prevalence of lesions on the trunk and lower extremities. Most melanomas diagnosed were of superficial spreading type and the third Clark's level of tumor invasion and stage II according to AJCC. In spite of comparatively low rates of incidence and mortality the trend to increase of melanoma cases in the region under consideration obviously calls for more attention and further investigation.

Non-Contact Injury Risk in Lower Extremity depending on Global Positioning System Variables among Female Field Hockey Players (여자필드하키선수의 Global Positioning System 변인에 따른 비접촉성 하지부상 발생위험도)

  • This study aimed to qualify the amount of movement during game-based training and competition by using a GPS and to identify the non-contact injury risk in lower extremities for female field hockey enrolled in Korean national team. A total of 52 players were participated in this study and their GPS data collected during training and competition were averaged for 1 week and 4 weeks. And then, an injury risk in lower extremities was calculated for each category of the amount of movement in GPS variables that were related to non-contact injury. In forwards, the injury risk was the lowest in the moderate-low category of total distance covered and repeated high-intensity effort bout and the high category of high intensity distance for 1 week, but the risk decreased as the amount of high intensity distance increased for 4 weeks. In midfielders, the injury risk was the lowest in the low category of total distance covered, high intensity distance, repeated high-intensity effort bout and deceleration bout for 1 week.
