Alveolar cleft bone grafting: factors affecting case prognosis

  • Objectives: The goal of this retrospective study was to determine the significance and impact of several factors on the alveolar cleft bone grafting procedure. Materials and Methods: The medical records were reviewed. In addition, x-rays were checked. The size of every cleft was measured in this retrospective study. The analyzed factors included sex, age, type of cleft, size of the cleft, and the type of flap used in surgery. The patients were characterized into group A (no complications, Bergland scale 1 or 2), group B (complications or Bergland scale 3), or group C (failure cases). Statistical analysis was performed with a P-value set at 0.05. Results: There were 32 cases in group A, 26 in group B, and 9 in group C. Multinomial logistic regression showed an association between the type of the cleft and the size of the cleft, with the presence of complications, or achieving type 3 on the Bergland scale, with odds ratios of 5.118 and 6.000, respectively. The type of cleft was related to failure with an odds ratio of 4.833. Given a small sample, statistical analysis could not be performed to evaluate the relationship between the size of the cleft and group C. Age, sex, and the type of the flap were not significant factors. Conclusion: The cleft size of more than 10 mm and bilateral clefts were listed regarding their effect on the procedure. Clinicians should not overlook these factors. In addition, patients must be informed of any risks that are present.

Postoperative Complications and Their Risk Factors of Completion Total Gastrectomy for Remnant Gastric Cancer Following an Initial Gastrectomy for Cancer

  • Purpose: Completion total gastrectomy (CTG) for remnant gastric cancer (RGC) is a technically demanding procedure and associated with increased morbidity. The present study aimed to evaluate postoperative complications and their risk factors following surgery for RGC after initial partial gastrectomy due to gastric cancer excluding peptic ulcer. Materials and Methods: We retrospectively reviewed the data of 107 patients who had previously undergone an initial gastric cancer surgery and subsequently underwent CTG for RGC between March 2002 and December 2020. The postoperative complications were graded using the Clavien-Dindo classification. Logistic regression analyses were used to determine the risk factors for complications. Results: Postoperative complications occurred in 34.6% (37/107) of the patients. Intra-abdominal abscess was the most common complication. The significant risk factors for overall complications were multi-visceral resections, longer operation time, and high estimated blood loss in the univariate analysis. The independent risk factors were multi-visceral resection (odds ratio [OR], 2.832; 95% confidence interval [CI], 1.094-7.333; P=0.032) and longer operation time (OR, 1.005; 95% CI, 1.001-1.011; P=0.036) in the multivariate analysis. Previous reconstruction type, minimally invasive approach, and current stage were not associated with the overall complications. Conclusions: Multi-visceral resection and long operation time were significant risk factors for the occurrence of complications following CTG rather than the RGC stage or surgical approach. When multi-visceral resection is required, a more meticulous surgical procedure is warranted to improve the postoperative complications during CTG for RGC after an initial gastric cancer surgery.

Puncture needle with a hard plastic sheath and plastic wings minimizes repuncture attempts in ultrasound-guided paracentesis: a retrospective case-control study

  • Background: This study was performed to evaluate periprocedural factors, complications, and repuncture rate of the newly developed puncture needle and compare it with the routinely used puncture needle for ultrasound (US)-guided paracentesis. Methods: We retrospectively identified 137 patients who underwent US-guided paracentesis between July 2018 and March 2019. Among them, 82 patients underwent US-guided paracentesis with a newly developed puncture needle. The other 55 patients underwent US-guided paracentesis with a routinely used puncture needle. The periprocedural factors, complications, and repuncture rate were compared between the two groups using the Mann-Whitney U test and Fisher exact test. The repuncture-associated factors were assessed using logistic regression analysis. Results: There were no major or minor complications in either group. The rate of repuncture was significantly lower in the group using the newly developed puncture needle compared with the group using the routinely used puncture needle (p=0.01). The duration of the procedure was significantly shorter with the newly developed puncture needle compared with the routinely used puncture needle (p=0.01). In univariate analysis, the thickness of the abdominal wall (p=0.04) and the use of the newly developed puncture needle (p=0.01) were significantly associated with the rate of repuncture. In multivariate analysis, only the use of the newly developed puncture needle was significantly associated with the rate of repuncture. Conclusion: Using this novel puncture needle with a hard plastic sheath and plastic wings, the rate of repuncture and the duration of the procedure were decreased without complications of US-guided paracentesis.

너스수술의 합병증에 대한 고찰: 위험인자 분석과 예방책의 제시 (Analysis of Complications Associated with the Nuss Procedure: Risk Factors and Preventive Measures)

  • 배경: 너스수술법은 최신 수술법으로서 훌륭한 교정효과를 인정받고 있다. 그러나 이 수술법은 아직 시행 초기로서 해결되어야 할 문제점들이 적지 않다. 저자들은 3년여의 경험을 통해 그간 발생한 합병증들을 분석하고 이에 대한 해결책들을 제시하고자 한다. 대상 및 방법 : 1999년 8월부터 2003년 10월까지 순천향대학교 천안병원에서 너스수술을 받은 335명의 의무기록을 후향적으로 분석하였다. 나이는 중앙값이 8세(범위: 16개월∼46세)였다. 소아환자 (15세 이하)가 264명(78.8%)이었고 성인(16세 이상)은 71명(21.2%)이었다. 형태 분류상 대칭형이 193명(57.6%), 비대칭형이 142명(42.4%)이었다. 다 변수분석(Multivariate Logistic Regression)을 통하여 합병증 발생의 위험인자를 구하였다. 결과: 수술의 합병증은 총 61예 (18.9%)에서 발생하였다. 조기 합병증(수술 후 30일 이내)은 49예 (15.2%)로 기흉(n=23, 6.9%), 상처 seroma (n=12, 3.6%), 막대편위 (n=8, 2.4%) 등이었고, 만기 합병증(수술 후 31일 이후)은 총 12예(3.7%)로 심낭염(n=5, 1.5%), 막대편위 (n=4, 1.2%), 혈흉 (n=3, 0.9%) 등이 빈번하였다. 합병증의 예방기법의 개발로 본 경험의 후기에 합병증이 감소하였다(99년 이전: 15/51 (29.4%) vs 2000년 이후: 34/284 (12.2%), p=0.04). 대협곡형 (Grand Canyon type)은 다른 형태에 비해 합병증 발생률이 높았다(대협곡형: 12/30 (40.0%) vs 다른 형태: 37/305 (12.3%), p<0.001). 다변수 분석에서 합병증의 주요 위험인자는 누두흉의 심도 (OR=2.88, p=0.038), 대협곡형(OR=2.82, p=0.044)과 수술시기1 (OR=4.05, p=0.001)이었다. 결론 너스수술의 합병증은 주로 심한 비대칭 협곡형과 수술 경험의 부족(수술시기 1)과 관련이 있었다. 경험의 축적과 변형 기법의 개발로 합병증을 줄일 수 있었으므로, 현재의 변형 너스수술법은 합병증의 발생률이 낮은 안전한 수술방법으로 사료된다.

Prescription of antibiotics after tooth extraction in adults: a nationwide study in Korea

  • Objectives: This study aimed to understand the nationwide patterns of antibiotic prescription after tooth extraction in adult patients. Materials and Methods: This study analyzed dental records from the National Health Insurance Service-National Sample Cohort (NHIS-NSC) database on 503,725 tooth extractions performed in adults (≥19 years) during 2011-2015. Patient sex, age, household income, systemic disease (diabetes mellitus and hypertension), type of dental institution, region of dental institution, year of prescription, and type of tooth extraction procedure were considered. The antibiotic prescription rate and broad-spectrum antibiotic prescription frequency were analyzed using chi-squared tests. Factors affecting the prescription of broad-spectrum antibiotics were evaluated using multivariate logistic regression analysis. Results: The rate of antibiotic prescription after tooth extraction was 81.85%. Penicillin was most commonly prescribed (45.25%), followed by penicillin with beta-lactamase inhibitors (18.76%), metronidazole (12.29%), and second- to fourth-generation cephalosporins (11.52%). The proportion of broad-spectrum antibiotics used among all prescribed antibiotics was 45.88%. Conclusion: The findings of this study demonstrate that the rate of antibiotic prescription after tooth extraction is higher in Korea than in other countries. Furthermore, broad-spectrum antibiotics are used more frequently, which may indicate unnecessary drug prescription, an important contributor to antibiotic resistance.

모의비행 훈련을 통한 비행적성 판단모형 연구 (A Study on the Model of the Pilot Aptitude through the Simulated Flight using the Pilot Aptitude Research Equipment)

  • The Pilot Aptitude Research Equipment (PARE) at the Republic of Korea Air Force Academy had been installed to study the pilot aptitude of the cadets and the student pilots(Navy officers and Air Force officers from the pilot scholarship programs and the ROTC). The T-37 simulated flight program and procedure, and the automatic evaluated program for simulated flight have been orderly developed to use the PARE effectively. The cadets who entered started to get simulated flight training by using those developed programs. Their flight situation has been recorded by the automatic evaluated program whenever they got the training. And then the cadets who took part in the simulated flight started the elementary combat flight training in 1,999 after getting appointed to an office and finished the advanced combat flight training in 2,001. The study of the relationship between the simulated flight and the combat flight training has begun after finding their combat flight training results. The Logistic Discriminal Analysis, technique of the SAS statistical analysis package was used to study the pilot aptitude model through the simulated flight training. This study showed that it is possible to pre-estimate the result of the combat flight training using the PARE machine.

Factors associated with Intermittent and Light Smoking among Korean High School Students: Intermittent and Light Smoking among Korean Adolescents

  • Purpose: The purpose of this study was to identify factors associated with intermittent and light smoking among Korean high school students. Methods: In this cross-sectional study, we employed secondary data from the 2015 Korea Youth Risk Behavior Web-Based Survey, and used the biopsychosocial model as a framework. The analysis was performed using the data of 2,851 high school students who smoked. We defined intermittent and light smoking as smoking on 1 to 29 days in a 30-day period and no more than 10 cigarettes per day. A logistic regression analysis using the complex samples procedure was conducted. Results: Among all the participants, 1,231 (43.2%) were intermittent and light smokers. Factors significantly predicting intermittent and light smoking were gender and grade (biological factors); subjective stress (psychological factor); and mother's smoking, sibling's smoking and academic achievement (sociocultural factors). Conclusion: In smoking cessation programs, health care providers both at school and in the community should consider the unique biological, psychological, and sociocultural characteristics of intermittent and light smoking behavior among high school students.

수술장갑의 천공에 영향을 미치는 특성 분석 (Character Analysis that influences to Surgical Gloves Punctures)

  • Purpose: The purpose of this study was to examine the puncture incidence of surgical gloves in an operational setting. Methods: The 277 surgical gloves were used collecting at one general hospital located in Seoul from February 1, 2011 to April 3. The data were analyzed by $x^2$ test, Fisher's exact test, and multiple logistic regression. Results: In the dental and orthopedic department, the puncture incidence were significantly higher than others. The time of wearing gloves demonstrated significant difference: The group 181-240 minutes had a significantly higher than those with under 90, 91-180, or over 240 minutes. The influencing factors were as follows. Compared to that of the 2nd assistants, the puncture incidence rates of the surgeons were 9.91 times, scrub nurses were 8.39 times higher respectively. The participants in work experience under 1 year showed a 4.58 times higher than those with over 7 years. In addition, compared to the puncture incidence rate of neurosurgery department, the 17.41 times in cardio-thoracic surgery, 13.89 times in dental surgery, 4.93 times in gynecology, and 4.97 times in orthopedics higher respectively. Conclusion: There is a need for training operational room personnels to occasionally exchange the gloves even during the procedure and to use double surgical gloves.

주요 상병 별 환자의 의료기관 선택성향 분석 (An Analysis of the Diseases Specific Medical Service Organization Selection Factors of Patients)

  • The relaxation of the regulation in selection of medical institution allows patients to use their own judgement in choosing proper institution for their diseases. Since the change of the regulation, there should have been many changes in medical institution selection behavior. The analysis of the change in disease specific selection pattern is critical because there be an optimal selection criteria that ensure the efficient and effective utilization of medical resources. This study analysis the institution selection factors by comparing the choice among the cases of acute diseases, the cases of chronic diseases, inpatient services, outpatient services, and emergency medical service. The comparisons performed in terms of size, class and other characteristics of medical institutions. For the study the nationally surveyed database was used and the data were analyzed using logistic regression procedure. The results indicates that the primary care facilities were not properly utilized. This study speculates that the reason for the undesirable pattern of utilization is that the roles of primary care facilities in the healthcare delivery system was not clearly defined. Based on the results, the medical policy implications are discussed.

Prevalence of Tobacco and Alcohol Consumption among Fishermen in Udupi Taluk, Karnataka, India: a Cross-Sectional Study

  • Background: Stress associated with fishing is known to trigger consumption of alcohol and tobacco among fishermen. This study aimed to assess the prevalence of tobacco and alcohol consumption among fishermen in Udupi Taluk in the state of Karnataka, India, and to study associations with health status and job stress. Materials and Methods: The study was conducted among 825 fishermen in Udupi Taluk of Karnataka between January-June 2015, using a two stage cluster sampling procedure. Associations between variables of interest were assessed using multivariable analysis and logistic regression models. Results: The prevalences of consumption of tobacco, alcohol and either of these substances were 64.2%, 45.6% and 86.9% respectively. There was a positive association between alcohol and any form of tobacco consumption with income but none with respondent's job stress and health status. Conclusions: Our study concluded that fishermen with poor health status are seen more among tobacco and alcohol users.
