Image findings of hepatic lymphoma have been reported as variable, ranging from single or multiple small nodules to diffuse infiltrative patterns. On MRI, most hepatic lymphomas show T1 low signal intensity, T2 high signal intensity. Dynamic imaging reveals a hypointense appearance in the arterial phase, followed by delayed enhancement in the portal venous and transitional phase. In the hepatobiliary phase using a hepatocyte-specific contrast agent (which have recently aided in increasing the access to the focal liver lesions), hepatic lymphoma is known to exhibit low signal intensity. We report a case of hepatic lymphoma, which shows iso-signal intensity on hepatobiliary phase, using gadoxetic acid (Gd-EOB-DTPA).
The purpose of this study was to characterize focal hepatic lesions through pre and post ferucarbotran-enhanced T2 and T2*-weighted imaging and to help differentiate benign and malignant lesions 대상 및 방법: Consecutive 34 patients with 52 hepatic lesions underwent MRI before and after intravenous bolus injection of ferucarbotran (Resovist Sobering, Berlin, Germany) for evaluation of focal hepatic lesions. Lesional signal-intensity and lesion-to-liver contrast changes were measured by contrast-to-noise ratio (CNR) from region of interest.
Sarcomatous Intrahepatic cholangiocarcinoma is a rare but an aggressive variant of cholangiocarcinoma with a very poor prognosis. A 79-year-old man was admitted to our hospital because of incidentally found liver mass. Magnetic resonance imaging (MRI) revealed well-defined hypointense mass on T1WI and heterogeneous hyperintense mass on T2WI. Gd-EOB-DTPA enhanced study shows peripheral rim-like enhancement in arterial phase and progressive concentric filling of contrast in delayed phase. And mass shows significant enhancement in hepatobiliary phase. The pathologic diagnosis was intrahepatic cholangiocarcinoma with sarcomatous change.
Intraperitoneal metastatic hepatocellular carcinoma (HCC) is uncommon. Although rare, it can spontaneously rupture and cause hemoperitoneum similar to primary HCC in the liver. We present a case of intraperitoneal metastatic HCC that had spontaneously ruptured and appeared as an irregularly margined hemorrhagic mass with T1 high and T2 dark signal intensities on magnetic resonance imaging. Ruptured HCC is a life-threatening emergency with high mortality rate. Spontaneously ruptured intraperitoneal metastatic HCC should be considered if a patient with a history of HCC presents with acute abdomen, although rare.
Two dogs presented with vomiting and head pressing. In both dogs, a large vessel was revealed in computed tomography (CT) angiography, which was found to leave the portal vein (PV) cranial to the splenomesenteric confluence and enter the pre-hepatic caudal vena cava cranial to the right renal vein. The flow of portal blood to the liver was not identified. Based on CT angiography, the dogs were suspected to have congenital PV aplasia with portocaval shunting. Diagnostic imaging of potential malformations for PV continuation should be conducted before attempting shunt closure.
A 1-year-old male Persian cat was presented for castration. Liver incarcerated in a peritoneopericardial diaphragmatic hernia (PPDH) was diagnosed through pre-anesthetic tests. Multiple homogeneous hyperechoic nodules in the hepatic parenchyma were identified using ultrasound. The nodules showed decreased attenuation compared with normal hepatic parenchyma, and the herniated hepatic parenchyma showed increased arterial and decreased portal enhancement on computed tomography. From the histopathology, we diagnosed hydropic degeneration with portal fibrosis and myelolipoma. This report presents diagnostic imaging features of hepatic myelolipoma incarcerated in a PPDH in a cat. When perfusion of the hepatic parenchyma is altered, surgical treatment should be considered.

Accurate liver segment segmentation based on radiological images is indispensable for the preoperative analysis of liver tumor resection surgery. However, most of the existing segmentation methods are not feasible to be used directly for this task due to the challenge of exact edge prediction with some tiny and slender vessels as its clinical segmentation criterion. To address this problem, we propose a novel deep learning based segmentation model, called Boundary-Aware Dual Attention Liver Segment Segmentation Model (BADA). This model can improve the segmentation accuracy of liver segments with enhancing the edges including the vessels serving as segment boundaries. In our model, the dual gated attention is proposed, which composes of a spatial attention module and a semantic attention module. The spatial attention module enhances the weights of key edge regions by concerning about the salient intensity changes, while the semantic attention amplifies the contribution of filters that can extract more discriminative feature information by weighting the significant convolution channels. Simultaneously, we build a dataset of liver segments including 59 clinic cases with dynamically contrast enhanced MRI(Magnetic Resonance Imaging) of portal vein stage, which annotated by several professional radiologists. Comparing with several state-of-the-art methods and baseline segmentation methods, we achieve the best results on this clinic liver segment segmentation dataset, where Mean Dice, Mean Sensitivity and Mean Positive Predicted Value reach 89.01%, 87.71% and 90.67%, respectively.
Hepatocellular carcinoma is the most common primary tumor in the liver. FDG PET has been applied for staging and treatment planning of hepatocellular carcinoma. It could reflect tumor prognosis because glucose metabolism assessed by FDG PET is known to have correlations with the differentiation and aggressiveness of the tumor. Although the ability of FDG PET to detect well-differentiated or low grade tumors and intra-hepatic lesions is not good, it is expected to playa major role in pre-surgical assessments for liver transplantation because it is useful in detecting extra-hepatic lesions and unexpected distant metastases with a better diagnostic performance than other conventional imaging modalities. Additionally, FDG PET has an advantage to screen other cancers through whole body scanning. As a new tracer for PET, Acetate demonstrates higher sensitivity and specificity to FDG in evaluating hepatocellular carcinoma. It thus seems that simultaneous use of Acetate PET with FDG PET could be helpful in diagnosis, especially detecting extra-hepatic metastases.
