Development of Chronic Neutrophilic Leukemia

  • The experimental manipulation of protooncogenes and their gene products is a valuable research tool for the study of human neoplasia. In this study, the recently identified human cervical cancer protooncogene (HccR-2) was expressed in transgenic mice under the control of the tetracycline regulatory system. Mice expressing the HccR-2 transgene showed an altered myeloid development characterized by an increased percentage of mature and band-form neutrophils in the peripheral blood, liver and spleen. This phenotype is similar to human chronic neutrophilic leukemia (CNL) in many ways, which is a rare chronic myeloproliferative disorder (CMD) that presents as a sustained leukocytosis of mature neutrophils with a few or no circulating immature granulocytes, an absence of peripheral blood monocytosis, basophilia, or eosinophilia, and an infiltration of neutrophils into the liver, spleen and kidney. Thus, the HccR-2 transgenic mouse model is imperative not only for investigating the biological properties of the HccR-2 protooncogene in vivo, but also for analyzing the mechanisms involved in the progression of CNL.

  • 1. Vertigo and dizziness is also called as Mok-hyun, Hyun-mo, Pung-hyun, Du-pung-hyun-un, Du-sun. 2. Causes and processes of Vertigo and dizziness are mainly divided into external infection and internal damage. Wind, cold, dampness and summer heat are included in the one and in the other, vital energy and blood deficiency, insufficiency of the liver and kidney, retention of phlegm and fluid, emotional disorder and so on. 3. Symptoms of vertigo and dizziness are disequilibrium, nausea, inability of hearing, vomiting, sweating and syncope. 4. Treating of vertigo and dizziness are recovery of vital energy and blood deficiency, strenthening of insufficiency of the liver and kidney, drying of dampness and removing of phlegm.

개의 간성 뇌증 (Hepatic Encephalopathy in a Dog)

  • Hepatic encephalophthy was diagnosed with serum chemistry, abdominal radiography and ultrasonography in a 2.6kg, 4 year-old maltese dog showing signs of hypersalivation, involutary spasm of facial muscles, ataxia, behavior abnormalities like dullness, sleep disorder, restlessness. In serum chemistry, the level of alanine transferase and aspartate trasferase was mildly elevated, ammonia was severely increased. On abdominal radio-graphs, the size of liver was mildly decreased. In ultrasonographic examination, diffuse lesion with hyperechoic change and decreased vasculature were seen in the hepatic region. But vascular abnormalities of liver were not observed. Drug and dietary therapy were undertaken and severities of clinical sign were alleviated.

  • Gastroesophageal reflux disease (GERD) is a common disorder caused by the reflux of gastric contents into the esophagus. According to the recent classification, GERD can elicit esophageal and extraesophageal syndromes. Laryngopharyngeal reflux (LPR) is defined as laryngeal symptoms with laryngeal inflammation caused by the acid reflux. The prevalence of GERD and LPR is increasing during the past decades in Korea and management of GERD and LPR is a challenging issue in clinical practice. Proton pump inhibitor is the most effective drug in the treatment of GERD. Most patients with LPR are given a 2-month trial of a proton pump inhibitor (PPI), however, there is still little evidence on the diagnosis or the treatment of LPR. During the last years concern have been raised regarding the risk of averse events related to long-term use of PPI. We review the recent update on medical treatment of GERD/LPR.

Venlafaxine에 의한 급성 독성 간염 1예 (Venlafaxine-Induced Acute Toxic Hepatitis)

  • Venlafaxine is among the most widely prescribed antidepressants. It is extensively metabolized to O-desmethylvenlafaxine via cytochrome P450 (CYP) 2D6. We report a case of acute toxic hepatitis resulting from venlafaxine in a 54-year-old woman with pain disorder. During venlafaxine treatment, laboratory tests revealed elevated liver enzymes with a maximum of 169 IU/L for aspartate transaminase (AST) and 166 IU/L for alanine transaminase (ALT). AST and ALT levels returned to normal after 6 days of discontinuation of venlafaxine. The patient was finally diagnosed with acute toxic hepatitis through liver biopsy. This case indicates the importance that clinicians should be aware of the hepatotoxicity of venlafaxine in practice.

  • Ursodeoxycholic acid (UDCA) has long been used as an adjuvant or first choice of therapy for liver disease. Commonly, UDCA has been reported to play a role in improving hyperbilirubinemia and disorder of bromsulphalein. More commonly, UDCA has been used in reducing the rate of cholesterol level in bile juice that can cause cholesterol stone. The effects on the promotion of bile acid release that leads an excretion of toxic materials and wastes produced in liver cells as well as various arrays of liver disease such as hepatitis. Other than already reported in clinical use, immunosuppressive effect has been studied, especially in transplantation. In the study, we hypothesized that UDCA might have a certain role in anti-inflammation through a preventive effect of pro-inflammatory potentials in the brain macrophages, microglia. We found that the treatment of $200\;{\mu}g/ml$ UDCA effectively suppressed the pro-inflammatory mediators (i.e. nitric oxide and interleukin-$1{\beta}$) in rat microglia compared to comparators. Interestingly, RT-PCR analysis suggested that UDCA strongly attenuated the expression of $IL-1{\beta}$ that was comparable with cyclosporine A at 48 h incubation. Conclusively, we found that UDCA may playa cytoprotective role in microglial cells through direct or indirect pathways by scavenging a toxic compound or an anti-inflammatory effect, which are known as major causes of neurodegenerative diseases.

당귀수산가미방(當歸鬚散加味方)을 이용한 간장과 신장의 기능이상을 나타낸 중증(重症) 질박어혈통(跌撲瘀血痛) 환자(患者)에 대한 치험 1례 (A Case Study of Danggwisusan on Severe Contusion with Functional Disorder of Liver and Kidney)

  • The following case study suggests strongly the efficacy of Danggwisusan and acupuncture therapy in the treatment of patients with severe contusion and tissue injury of the liver and kidney. The chief symptoms of this case were severe pain in the right side, purpura hemorrhagica, insomnia and third degree bums at the shoulder and face. Analysis of blood chemistry indicated high AST, high ALT, high BUN, high creatine, high creatine kinase and high LDH. Fallowing medication of the patient with Danggwisusan, the symptoms and indexes for tissue injury decreased rapidily within the first week. This results support a potential role for Danggwisusan in the treatment of severe contusion with tissue injury of the liver and kidney.

Hepatoprotective and antioxidant activity of Leea asiatica leaves against acetaminophen-induced hepatotoxicity in rats

  • Leea asiatica (L.) Ridsdale, a folk medicinal plant is used by the ethnic people of North East India for the treatment of hepatic disorder. In this study, we have investigated the hepatoprotective and antioxidant activity of L. asiatica leaves against acetaminophen induced hepatotoxicity. Methanol extract of L. asiatica (150 and 300 mg/kg/day, p.o.) were administered to rats for three consecutive days followed by single acetaminophen (3000 mg/kg, p.o.) administration on $3^{rd}$ day. After 48 h of acetaminophen administration animals were sacrificed and biochemical estimation of serum, in vivo antioxidant activity using liver tissue were carried out. High levels of serum glutamate oxaloacetate transaminase, serum glutamate pyruvate transaminase, serum alkaline phosphatase, total bilirubin, direct bilirubin, total cholesterol and triglycerides were observed in disease control group, which found near to normal in extract treated groups. Higher dose exhibited significant hepatoprotective activity against acetaminophen induced toxicity. Level of superoxide dismutase, catalase, glutathione peroxidase in liver tissue, and reduced glutathione in liver and blood were also significantly increased in extract (300 mg/kg) treated animals compare to disease control group. In this study we found that leaves of L. asiatica exhibited potent hepatoprotective activity against acetaminophen induced hepatic damage in experimental animals which justify the folklore claim, and the possible mechanism of this activity may be due to strong antioxidant activities of extract.

A Case of Glycogen Storage Disease IV with Rare Homozygous Mutations in the Glycogen Branching Enzyme Gene

  • Glycogen storage disease (GSD) IV is a rare autosomal recessive inherited disorder caused by mutations in the gene coding for glycogen branching enzyme leading to progressive liver disease. GSD IV is associated with mutations in GBE1, which encodes the glycogen branching enzyme. We report a case of GSD IV with rare homozygous mutations in the GBE1 gene (c.791G>A (p.Gly264Glu), which was successfully treated by liver transplantation.
