In the presence of severe non-calcific mitral regurgitation, a dilated mitral annulus, absence of severe subvalvular chordal thickening, and no major loss of leaflet substance, reconstructive surgery for mitral regurgitation is a reasonable approach. Between March, 1980 to November, 1980, 5 patients underwent mitral annuloplasty using Carpentier Ring for severe mitral regurgitation. All 5 cases were of rheumatic in origin. There were no ruptured chorda tendinae or ruptured papillary muscles except slight thickening of the leaflet. There was no operative or late death. As is Carpentier`s report, in patient with severe mitral regurgitation who meet above criteria, mitral annuloplasty using Carpentier Ring is preferable to valve replacement.

This paper investigates the structural analysis and design of mechanical heart valve through the numerical analysis methodology. In a numerical analysis methodology application to the thickness minimization structural design of mechanical heart valve, fluid analysis is performed for the blood flow through a bileaflet mechanical heart valve. Simultaneously the kinetodynamic analysis is carried out to obtain the appropriate structural condition for the structural analysis. Thereafter the structural static analysis is also carried out to confirm the thickness minimization structural condition(minimum thickness shape of leaflet).

Surgical treatment is possible for the obstructive form of hypertrophic cardiomyopathy and transaortic left ventricular septal myectomy and myotomy has been the procedure of choice. If coexisting intrinsic mitral valve disease exists, mitral valve replacement has been performed. But abnormal systolic anterior motion of anterior mitral leaflet[SAM] with intrinsic normal mitral valve disease is the typical feature of IHSS and we prefer not to replace mitral valve. 3 patients underwent transaortic myotomy and myectomy for IHSS with mitral regurgitation. 2 patients of them have coexisting intrinsic mitral valve diseases such as mitral valve vegetation and chorda rupture. Concomittent mitral valve replacement were performed. 1 patient shows SAM of mitral anterior leaflet but has intrinsic normal mitral valve morphologically and transepicardial echocardiogram and direct monitoring of pressure gradient during the operative procedure gives better information for subsided mitral regurgitation. Post operative course during the 12 months follow-up was uneventful.

A four month-old intact female Maltese (weighing 1.8 kg) was present with major complaints of severe coughing and dyspnea. Diagnostic imaging studies revealed abnormally elongated chordae tendineae attached to anterior leaflet and shortened chordae tendineae attached to posterior leaflet of mitral valve causing mitral insufficiency and regurgitant blood flow into left atrium, indicating congenital mitral valve dysplasia. Therapy was directed to control existing clinical signs and to prevent further deterioration of cardiac enlargement using conventional cardiac medications. The dog is currently survived and being monitored at regular interval. This is the first case report describing mitral valve dysplasia causing mitral insufficiency of dog in Korea.

We experienced a case of ruptured aneurysm of the sinus of Valsalva, and this resulted in simultaneous aortic and tricuspid valve endocarditis through a shunt. The echocardiography showed a ruptured sinus of Valsalva aneurysm to the right atrium with a shunt. The aortic non-coronary cusp was fibro-thickened with vegetation. Vegetations of the septal leaflet and the anterior leaflet of the tricuspid valve were also found. The blood culture grew Enterococcus garllinarum. We replaced both tricuspid and aortic valve with successful surgical result.

From July 1984 to September 1990, 316 patients of congenital heart diseases were operated and 15 patients died. Hospital mortality was 4.75%. Five patients of 73 PDA had residual shunt after operation: 4 were ligated under support of Dacron patch, 1 was closed through the pulmonary arteriotomy under CPB. 3 patients were reoperated. No patient had residual shunt or reopening among the patients of simple ligation or division and suture. During the ligation of PDA, Dacron patch for protection from tearing may disturb the complete interruption of shunt. If the tissue around the ductus arteriosus looks weak or fragile, division and suture may be more reliable other than ligation with supporting patch. If the septal leaflet of tricuspid valve is adherent around the VSD, remained opening of VSD may be closed with simple suture directly. In these cases, the incidence of postoperative residual shunt is as high as the incidence of more large VSD closed with patch [10.9%: 9.6%]. During the direct closure of remained opening of VSD, another leaking route should be looked for carefully beneath the septal leaflet of tricuspid valve.

Potassium phosphonate inhibited less mycelial growth of Phytophthora cactorum in vitro than metalaxyl-M. But the net protection rate in leaflet test and whole plant pot test was greater in potassium phosphonate than metalaxyl-M. P. ginseng appeared to have an induced resistance against P. cactorum with phosphonate around 50~100 ppm.

Till June 1988, We experienced 3 cases of single atrium at Dept. of Thoracic k Cardiovascular Surgery of National Medical Center. Case 1 was 26 years old female, and had single atrium associated with partial cleft in mitral anterior leaflet and PAPVC. Atrial septation with Dacron patch and MVR [I-S, 31 mm] were done, followed by TAP, De Vega. Postoperative course was good, OPD follow-up for about 4 years with normal physical activity and ordinary working. Case 2 was 4 years old female with single atrium alone. Atrial septation with pericardial patch. Good postoperative course and OPD follow-up for about 2 years with appropriate physical growth. Case 3 was 22 years old female, single atrium with complete cleft in the anterior and posterior mitral leaflet, and septal tricuspid cleft were identified. Atrial septation with polystan bovine pericardial patch and repair of mitral anterior cleft. Postoperatively, residual regurgitation of mitral and tricuspid valve, but tolerable in some limitation of physical activity.

기계식 인공심장판막을 통한 혈액의 유동과 이 유동에 관련된 판첨의 거동특성을 수치해석기법을 이용하여 연구하였다. 혈액은 맥동류, 층류, 비압축성 유동으로 가정하였으며 유체-고체의 상호작용을 고려하기 위하여 혈액의 유동방정식과 고체의 운동방정식이 동시에 계산되었다. 심실과 대동맥에서의 압력파형을 경계조건으로 사용하였다. 연구의 결과로서 혈액유동과 판첨의 거동이 예측되었으며, 판막을 통한 3개의 제트가 발견되었으며 vortex가 판첨의 끝단에서 발생하여 하부로 흘러가는 것이 관찰되었다. 판첨의 닫힘 거동은 열림 거동에 비하여 2배정도 빠르게 진행되었으며 sinus에서 2개의 큰 vortex가 관찰되었다. 유체-고체 상호작용을 고려하는 본 연구방법은 향후 판막의 연구와 개발에 매우 유용할 것으로 판단된다.

This report describes the echocardiographic features of an indirect Gerbode defect in a young cat. Echocardiography revealed high-velocity, turbulent systolic flow directed from left ventricle to right ventricle through a ventricular septal defect. The flow immediately entered the right atrium through a tricuspid septal leaflet. The indirect-type Gerbode defect was confirmed through necropsy. When a high-velocity turbulent flow in the RA without pulmonary hypertension is observed on echocardiography, Gerbode defect should be considered.
