A Case Report of a Multiple System Atrophy-C Patient Treated by Korean Medicine (소뇌성 다계통위축증(MSA-C) 환자 한방 치험 1례)

  • Objectives: This study evaluates the effect of Korean medicine in a patient with multiple system atrophy-C (MSA-C). Methods: A patient diagnosed with MSA-C was treated with herbal medicine (Boyangwhano-tang hap Yeokgan-san), acupuncture, and moxibustion for 8 weeks. Clinical improvements were evaluated by the unified MSA rating scale (UMSARS) and the scale for the assessment and rating of ataxia (SARA). Results: Improvements in the total scores of UMSARS Part I, II, and IV and SARA were observed after Korean medicine treatments. However, there were no improvements to the urinary and autonomic dysfunctions. Conclusions: Korean medicine treatment may be an effective treatment for a degenerative, progressive disease like MSA-C.

  • Purpose: To report on a patient with subarachnoid hemorrhage and deep vein thrombosis treated with a combination of Korean and Western medicine. Methods: A patient diagnosed with subarachnoid hemorrhage and deep vein thrombosis was treated with herbal medicine (加減淸熱瀉濕湯 (Gagam-cheongyeolsaseub-tang)), acupuncture (GV2, GB20, LI11, LI4, TE5, GB34, ST36, SP9, ST38, GB39, SP6, LR3), moxibustion (CV6, CV9), and Western medicine for deep vein thrombosis and subarachnoid hemorrhage. To evaluate edema, one of the classic symptoms of deep vein thrombosis, we measured leg circumference at a specified time. Results: After taking Gagam-cheongyeolsaseub-tang and being treated with acupuncture, moxibustion, and Western medicine, the patient showed improvement in leg edema. The left calf circumference decreased from 37.5 cm to 34.5 cm. Conclusions: Korean medicine, acupuncture, moxibustion, and Western medicine may provide effective treatment for deep vein thrombosis in patients with subarachnoid hemorrhage, but further study is needed.

  • Purpose: The purpose of this paper is to report the improvement of a patient with amyotrophic lateral sclerosis after long-term combined Korean medical treatment. Methods: A patient diagnosed with amyotrophic lateral sclerosis was treated with herbal medicine, acupuncture, pharmacopuncture, moxibustion, and rehabilitation for four separate hospital stays. To evaluate their respiratory discomfort and limb weakness, we used Manual Muscle Testing, the Pulmonary Function Test, and the Korean Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised. Results: The weakness of the muscles of the lower extremities and respiratory function was improved. Conclusion: We consider that combined Korean medicine treatments might be an effective treatment for muscle weakness and respiratory discomfort of amyotrophic lateral sclerosis. To verify the effectiveness of these treatments, further research is required.

  • Objectives: This study presented the case of a 49-year-old Korean female with phantom limb pain after below right knee amputation and aimed to assess the effectiveness of Korean medicine treatment. Methods: The patient was treated with scalp acupuncture, electroacupuncture and herbal medicine. We executed a numerical rating scale (NRS), conducted a global assessment (G/A), administrated a Short-Form McGill Pain Questionnaire 2 (SF-MPQ-2) and measured total daily sleep time to evaluate symptom improvement. Results: The patient's G/A scores decreased from 10 to 2 and SF-MPQ-2 points decreased from 20 to 6 after treatment. The total daily sleep time did not changed due to anxiety. Conclusions: This study suggests that Korean medicine treatment could be effective in treating phantom limb pain after amputation. Further studies are needed.

  • Objectives: We examined the effects of Sinbi-tang on a patient with pontine hemorrhage and respiratory disturbance. Methods: A patient diagnosed with pontine hemorrhage and respiratory disturbance was treated with herbal medicine and acupuncture. We checked the peripheral oxygen saturation ($SpO^2$) by oximetry three times a day, and obtained an average value. We evaluated the improvement of symptoms by changes in the average applied $O^2$ and average $SpO^2$. Results: After taking Sinbi-tang and treatment with acupuncture therapy, the patient showed improvement in respiratory disturbance. Applied $O^2$ was gradually decreased from 4 L/min to 0 L/min and $SpO^2$ was stabilized. Conclusions: Korean medicine may provide effective treatment for respiratory disturbance in patients with pontine hemorrhage, but further study is needed.

Traditional Korean Medicine for Skin Toxic Side Effects from Afatinib in a Non-Small Cell Lung Cancer Patient: A Case Report (Afatinib 복약 후 발생한 비소세포성 폐암환자의 피부독성 부작용에 대한 한방치료 1례)

  • Objectives: We report a case of traditional Korean medicine (TKM) treatment for skin side effects after taking afatinib (Giotrif$^{(R)}$). Methods: A 62-year-old female who was diagnosed with non-small cell lung cancer stage 4 (T4N2M1b) and was on treatment with afatinib (29.56 mg/day for 4 months) complained of skin toxicity as a side effect. For 16 admission days, the patient was treated with acupuncture, moxibustion, and herbal medicine (oral decoction and external ointment). Results: Improvement of skin toxicity was measured by a numeric rating scale. In addition, Quality of life (QOL) was measured using EORTC Quality of Life Questionnaire, Core 30 (EORTC QLQ-C30) and EORTC Quality of Life Questionnaire, 13-item lung cancer-specific module (EORTC QLQ-LC13) Developed by the European Organization for Research and Treatment of Cancer (EORTC). Tumor size and carcinoembryonic antigen (CEA) were also examined during follow up. Conclusions: After a combined TKM treatment, skin toxicity symptoms and quality of life scales were significantly improved with no side effects. The tumor size was not changed on computed tomography during follow-up period. CEA levels were decreased.

  • Objectives: We treated a stroke patient with vancomycin-resistant enterococci (VRE) colonization using Gami-sipjeondaebo-tang. Methods: A patient diagnosed with intracranial hemorrhage with VRE colonization was treated with herbal medicine and acupuncture. We checked general conditions every day and performed stool VRE culture once a week. We evaluated the improvement of symptoms by change in VAS grade, general weakness, and stool VRE culture results. Results: After a treatment of Gami-sipjeondaebo-tang combined with acupuncture therapy, improvement of the general condition was observed. Also, VRE colonization was no longer detected in the stool culture. Conclusions: This case report proved the effect of Korean medicine for a stroke patient with VRE colonization, but further study is needed.

  • Purpose: The aim of this study was to report the improvement of Guillain-Barre syndrome after long-term combination treatment with Korean medicine. Methods: A patient was diagnosed with Guillain-Barre syndrome and treated with herbal medicine, acupuncture, pharmacopuncture, moxibustion, and exercise, including quadruped walking after three hospital admissions. To evaluate muscle strength and weakness, we measured manual muscle function, gait pattern, and the speed of quadruped walking. Results: The patient's muscle weakness in the extremities and gait stance were improved. The speed of quadruped walking was increased. Conclusion: We consider that combined treatment with Korean medicine might be effective for the muscle weakness of Guillain-Barre syndrome with a poor prognostic factor. To verify the effectiveness of this treatment, further research is needed.
