Freiberg병의 수술적 치료 (Surgical Treatment of Freiberg's Disease)

  • Freiberg's disease is a pathologic condition of the second or third metatarsal head, rarely the forth or fifth metatarsal head, and it becomes abnormally enlarge due to avascular necrosis of subchondral cancellous bone. From Nov. 1982 to Sep. 1994, we treated surgically 10 cases of the disease who complained the continuous symptoms inspite of proper conservative management. Metatarsal head excision was done in 8 cases and resurfacing of the cartilagenous portion of the metatarsal head in 2 cases. During the average follow up of 55 months, the pain was relieved in all patients who were underwent surgical intervention but the stiffness of the metatarsophalangeal joint was remained in most of cases. It seems to be a logical treatment of choice in younger and active patients that conservative management is the initial treatment but more ablative procedure is needed for the continued symptoms. We can propose the metatarsal head excision or resurfacing of the involved joint is effective treatment method with simple procedure and minimize morbid period than other procedures such as corrective osteotomy.

만성폐쇄성폐질환자의 건강 관련 삶의 질에 영향을 미치는 요인 (Factors Influencing Health-related Quality of Life of Patients with Chronic Obstructive Pulmonary Disease)

  • Purpose: The purpose of this study was to identify the factors influencing health-related quality of life (HRQOL) of patients with chronic obstructive pulmonary disease (COPD). Methods: A sample of 108 Patients diagnosed with COPD was recruited from outpatients, at a university hospital. Data were collected from February to April in 2016 using the St. George's Respiratory Questionnaire, the Hospital Anxiety-Depression Scale, and the COPD Self-efficacy scale. Results: The average score of HRQOL was 47.56 in patients with COPD. Results of the regression analysis showed that dyspnea phase (${\beta}$=.34, p<.001) and presence or absence of oxygen (${\beta}$=.20, p=.009), anxiety (${\beta}$=.29, p<.001), $FEV_1$ % predicted (${\beta}$=-.15, p=.014) were statistically significant in patients' HRQOL. Conclusion: The research findings show that dyspnea phase and anxiety among psychological factors were the most powerful factors that explain the HRQOL of COPD patients when the demographic and disease characteristics were controlled. Identification of quality of life-related factors should be used to inform targeted interventions, in order to improve HRQOL and help patients' cope.

만성관절염 환자의 자가운동에 따른 통증, 피로, 유연성, 건강상태의 비교연구 (A Comparative Study on Pain, Fatigue, Flexibility and Health status between Patients with Self-Exercise and Patient without Self-Exercise)

  • The purpose of this study was examine the effect of self-exercise on patients with chronic arthritis. This study was performed from 1999 to 2001 with 107 subjects. Pain, Fatigue, Flexibility and Health status were measured. Data were analysed by SPSSWIN 10. 0 Package program, frequency, independent t-test and One Way ANOVA. The results were as follows : Of the 107 patients who were sutdied, mean age was 62 years and most of common disease were osteoarthritis. 67.4%was doing self-exercise and self-exercise consisted of 17 types of exercise. Fatigue was statistically different by disease type, duration of disease, education respectively. And fatigue, left shoulder flexibility were significantly different by duration of disease. There weren't significant differences between patient with self-exercise and patient without self-exercise on pain fatigue, flexibility and halth status. These findings confirms that self-exercise do not effective in increasing health improvement for the patients with osteoarthritis. From results of this study indicate that patients with arthritis should have taken systematic exercise such as the self-help education program aquatic exercise program for arthritis patient.

Treatment of Temporomandibular Joint Disorder by Alloplastic Total Temporomandibular Joint Replacement

  • The literature on alloplastic total temporomandibular joint (TMJ) replacement is encouraging, with acceptable improvement of treatment outcomes in terms of both pain level and jaw function. This is a case report on patients who suffered from degenerative joint disease and ankylosis after mandibular condyle fracture or prior TMJ surgery and were treated by TMJ replacement with condyle prosthesis. We obtained good results from the procedures, including total TMJ replacement.

Comprasion of Effectiveness of CT vs C-arm Guided Percutaneous Radiofrequency Lumbar Facet Rhizotomy

  • Background: Facet joint have been implicated as a source of chronic low back pain. Radiofrequency denervation has demonstrated the most solid evidence. To increase safety and efficacy of treatment, computed tomography (CT) guidance injection has been used in several disease. The purpose of this study was to evaluate the efficacy of CT-guided radiofrequency rhizotomy in the treatment of facet joint pain. Methods: A total of 40 patients were randomized to undergo radiofrequency facet joint denervation under CT guidance or C-arm guidance. All patients were examined visual analogue scale (VAS) score before treatment, 1 month, and 3 months after treatment. Results: The VAS in both groups showed significant improvement over the 1-month interval. No significant difference in the VAS score among the group was observed. Conclusions: In this study there was no significant difference between CT guidance lumbar rhizotomy and C-arm guidance lumbar rhizotomy. Therefore CT-guided radiofrequency denervation of the lumbar facet joint was a minimally invasive technique that appears effective.

견관절에 발생한 활액막연골종증의 관절경적 치험례(7년 장기 추시 결과) - 증례 보고 - (Arthroscopic Treatment for Synovial Chondromatosis of the Shoulder Joint (7-Year-Follow-Up) - A Case Report -)

  • Synovial chondromatosis is a rare disease. The knee is the most commonly affected joint but the shoulder joint is very rarely affected. We report typical synovial chondromatosis of the shoulder joint which was managed by arthroscopic removal of multiple loose bodies, synovectomy, good objective and functional outcome was obtained until postoperative 7-year follow-up. Arthroscopy can be used as an effective and useful therapeutic method for synovial chondromatosis of the shoulder joint.

  • Temporomandibular joint (TMJ) prosthesis have been hesitated to use because of bitter memories about Proplast-Teflon interpositional TMJ implants. Many clinicians, however, are trying to use total TMJ prosthesis with getting continuous long-term follow-up results. Alloplastic total TMJ replacement have been applied to much more patients who have failed previous TMJ prostheses or TMJ implants, fibrous/bony ankylosis, severe osteoarthritis, degenerative joint disease, idiopathic condylar resorption, condyle loss by trauma or tumor resection, and other bony destructive diseases. Nowadays three types of total TMJ prosthesis are widely used. In this article, indications, types of prosthesis, safety and stability would be demonstrated with a case report.

다기능 복합관절 연속수동운동 의료기기 설계 (Design of Multifunctional Compound Joint Medical Equipment for Continuous Passive Motion)

  • The number of joint disease patients is increasing every year. Currently, the most CPM(Continuous Passive Motion) equipment uses expensive imported equipment, and one CPM equipment is designed to be used only in one joint, medical personnel or hospitals who are the main users of the medical equipment need to have several types of CPMs for joint rehabilitation. To solve this problem, this paper designed a multifunctional joint medical equipment that enables rehabilitation of knee, shoulder, and elbow joints in one CPM equipment and includes general, intensive, and adaptive exercise functions for effective treatment according to the patient's condition. The patient's condition was diagnosed using a load cell and a current sensor. In this paper, effective rehabilitation methods were presented and high reliability and precision of medical equipment was confirmed through experiments using potentiometer, encoder, and PI controller.

결핵 치료 중 나타나는 관절 증상 (Joint Symptoms During Antituberculous Chemotherapy)

  • 배경 : 항결핵제 투여시 나타나는 관절 증상은 경우에 따라서는 증상의 정도가 심해 일상 생활에 영향을 미치기도 하며 약물 순응도를 저하시키기도 하므로 항결핵제 투여시 발생하는 관절 증상에 대한 이해가 필요한데 이에 대해 체계적으로 기술되어 있는 자료가 부족한 실정이다. 방법 : 6개월이상 항결핵제 33명의 환자를 대상으로 하여 치료 경과 중 특별한 다른 원인이 없이 나타나는 관절 증상에 대해 조사 하였으며 증상을 호소하지 않는 비증상군과 비교함으로써 관절 증상 발생 및 이와 관련한 위험 인자를 확인하였다. 결과 : 관절 증상을 호소하는 환자가 33명중 19명으로 증상 유병률이 58% 에 달하였으며, 증상은 약물 투여 시작 후 1.9$\pm$1.4개월에 나타나서 3.6$\pm$2.5개월 지속되었다. 한 명을 제외하고는 모두 여러 관절을 침범하며 어깨관절과 무릎 관절이 각각 10명(53%), 손가락 관절 6명(32%)으로 흔하였다. 대부분의 환자가 항결핵제의 중단 또는 변경 없이 증상이 자연 소실되었으나, 14명(74%)의 환자는 치료 중 관절 증상에 대해 진통제를 복용하였다. 원안을 확인하기는 쉽지 않았으며 PZA 이외의 약물도 증상 발현에 관여 할 것으로 보인다. 비증상군과의 비교에서 연령, 성별, 기저질환, 치료 전 및 치료 중간의 혈중 요산 농도 등에서 유의한 차이를 확인할 수 없었다. 결론 : 항결핵제 투여 중 관절 증상의 발생은 혈중 요산 농도와 관련이 없으며, 이런 증상은 PZA 이외의 다른 약제에 의해서도 유발될 수 있으므로 증상이 심해 약을 중단하거나 대체해야 할 경우 이를 고려 하여야 한다.

어깨관절 질환에 대한 관절가동술 유형의 연구 동향 : 주제범위 문헌고찰 (Research Trend of Joint Mobilization Type on Shoulder : A scoping review)

  • Purpose : This study sought to investigate research trends regarding joint mobilization type among patients with shoulder joint diseases. Methods : A scoping review was conducted according to the five steps outlined by Arskey and O'Malley and PRISMA-ScR. We searched six domestic databases (ScienceOn, DBpia, Riss, Kmbase, Kiss, KCI) and three international databases (CINAHL, Pubmed, Cochrane central) between 2013 and June 2023. The keyword terms used were 'joint mobilization', 'Kaltenborn', 'Maitland', 'Mulligan', and 'shoulder joint'. Results : There were a total of 44 studies that investigated the topic, and these were divided into quantitative analysis and topic analysis. In terms of publication year, the number of studies within the last five years has increased more than compared to the previous five years, with most of them being randomized clinical trials. In shoulder joint diseases, it was found that the majority of joint movement studies focused on adhesive joint cystitis and shoulder collision syndrome. The Mulligan concept was the most commonly studied type of joint motion. The dependent variables used included pain, joint function (disability), and muscle function. The visual analog scale was the most commonly used for the pain variable, followed by the numeric rating scale. For joint function and disability variables, range of motion was the most commonly used, followed by shoulder pain and disability index, and disabilities of the arm, shoulder, and hand. For muscle function, variables such as muscle tone, strength, and activity were used. Conclusion : We believe that findings of this scoping review can serve as valuable mapping data for joint mobilization research on shoulder joint diseases. Further studies including systematic reviews and meta-analyses based on these results are recommended.
