A Fatal Intracerebral Hemorrhage Complicated by Compartment Syndrome of the Upper Arm (상지 구획 증후군 이후 발생한 치명적인 뇌출혈)

  • Compartment syndrome has a wide spectrum from muscle pain to a life- threatening condition, such as acute renal failure and disseminated intravascular coagulation (DIC). Intracerebral hemorrhage (ICH) due to compartment syndrome has not been reported. We report a patient who presented with ICH leading to death. A 25-year-old female with no significant past history developed extensive compartment syndrome followed by rhabdomyolysis, acute renal failure, DIC, and ICH. Although the patient underwent a fasciotomy and hemodialysis and received aggressive resuscitation with massive transfusions of blood and intravenous fluids, she died. This case stresses the importance of early diagnosis and prompt treatment of compartment syndrome to prevent devastating complications.

Transient Visual Acuity Decrease after Inferior Alveolar Nerve Block Anesthesia (하악공 전달마취 후 발생한 일시적인 시력 저하)

  • One of the most common procedures in dentistry is the inferior alveolar nerve block anesthesia but visual problem can occur during this procedure. Transient visual acuity decrease after inferior alveolar nerve block anesthesia can result from unintended intravascular injection. In this case report, we present ocular complication that has rarely been reported. The understanding of anatomy related to this case is discussed with suggestions for proper management of the patients. The dental management for ocular complications includes a correct diagnosis, management by understanding of patient's anatomic variations. This will prevent intravascular injection.

A Retrieved Sparganum of Spirometra erinaceieuropaei from a Korean Man during Mechanical Thrombectomy

  • Human sparganosis is a zoonotic disease caused by infection and migration of the plerocercoid of Spirometra spp. Although sparganosis were reported from most parts of the body, the sparganum parasitizing inside cerebral artery is remarkably uncommon. We report a case of cerebral intravascular sparganosis in an elderly patient with acute ischemic stroke who was diagnosed by retrieving sparganum during mechanical thrombectomy. Finally, the parasites were identified as Spirometra erinaceieuropaei using multiplex PCR and cox1 gene sequencing.

  • Objectives : The purpose of study was to investigate the effect of ILlB(intravascular laser irradiation of blood) on serum lipoprotein(a) Methods : The 12 case is diagnosed as cerebrovascular diseases on computed tomograghy. The 18 case is not observed abnormality on computed tomograghy but, they have neurological sign. Blood test was enforced on the first medical examination, The 15 case with plasma lipoprotein(a) greater than 30 mg/dl have been classified into abnormal group and the 15 case of less than 30 mg/dl have been classified into control group. after ten times ILlB, It was observed that the change of plasma lipoprotein(a) and a fibrinogen of blood clotting factor. Conclusions : 1. After ten times ILlB, plasma lipoprotein(a) numerical value was decreased on 13 case of abnormal group. 2. Plasma lipoprotein(a) numerical value was decreased on 5 case of control group. 3. The twenty in fifteen patient's with hyperlipoproteinemias have been fibrinogenemias and after ILIB treatment, in the all of 16 case with hyperfibrinogenemia in the first medical examination to both abnormal and control group, plasma fibrinogen numerical value was decreased.

  • A method for observing intra blood vessel ducts which are threadlike bundle of tubules which form a part of the BongHan duct system. By injecting 10% dextrose solution at a vena femoralis one makes the intravascular BongHan duct thicker and stronger to be easily detectable after incision of vessels. The duct is semi-transparent, soft and elastic, and composed of smaller tubules whose diameters are of $10{\mu}m$ order, which is in agreement with BongHan theory.

Destructive Radiologic Development of Intravascular Papillary Endothelial Hyperplasia on Skull Bone

  • Intravascular papillary endothelial hyperplasia (IPEH) is a rare vascular benign lesion that rarely involves the central nervous system with or without skull invasion. We report a rare case of IPEH on the skull bone, which displayed destructive radiologic development associated with hemorrhage. A 14-year-old male presented with an incidentally detected a small enhancing, left frontal osteolytic lesion. Previously, he underwent operation and received adjuvant chemoradiation therapy for cerebellar medulloblastoma. Follow-up magnetic resonance imaging revealed a left frontal bone lesion, which expanded to an approximately 2 cm-sized well-circumscribed osteolytic lesion associated with hemorrhage for 20 months. Frontal craniectomy and cranioplasty were performed. Destructive change was detected on the inner table and diploic space of the skull. The mass had a cystic feature with hemorrhagic content without dural attachment. Pathologic examination showed the capsule consisted of parallel collagen lamellae representing a vascular wall, vascular lumen, which was pathognomonic for IPEH. Immunohistochemical staining revealed that the capsule was positive for CD34 and factor VIII, which favor the final diagnosis of IPEH. This was the first case of intracalvarial IPEH.

  • Lipomas are common soft tissue tumors that are located in the body tissues containing adipose tissues. However, lipomas arising from the walls of a vein are very rare. Intravascular lipomas have been described most commonly in association with the inferior vena cava. Intravascualar lipomas involving the subclavian vein are rare. We are reporting a case of an asymptomatic lipoma of the right subclavian vein, growing into the right brachiocephalic vein.
