Exercise Barriers in Korean Colorectal Cancer Patients

  • Background: It has been proved that participating in exercise improves colorectal cancer patients' prognosis. This study is to identify barriers to exercise in Korean colorectal cancer patients and survivors. Materials and Methods: A total of 427 colorectal cancer patients and survivors from different stages and medical status completed a self-administered questionnaire that surveyed their barriers to exercise and exercise participation. Results: The greatest perceived exercise barriers for the sampled population as a whole were fatigue, low level of physical fitness, and poor health. Those under 60-years old reported lack of time (p=0.008), whereas those over 60 reported low level of physical fitness (p=0.014) as greater exercise barriers than their counterparts. Women reported fatigue as a greater barrier than men (p<0.001). Those who were receiving treatment rated poor health (p=0.0005) and cancer-related factors as greater exercise barriers compared to those who were not receiving treatment. A multivariate model found that other demographic and medical status were not potential factors that may affect exercise participation. Further, for those who were not participating in physical activity, tendency to be physically inactive (p<0.001) and lack of exercise skill (p<0.001) were highly significant barriers, compared to those who were participating in physical activity. Also, for those who were not meeting ACSM guidelines, cancer-related exercise barriers were additionally reported (p<0.001), compared to those who were. Conclusions: Our study suggests that fatigue, low level of physical fitness, and poor health are most reported exercise barriers for Korean colorectal cancer survivors and there are differences in exercise barriers by age, sex, treatment status, and physical activity level. Therefore, support for cancer patients should be provided considering these variables to increase exercise participation.

  • Objective : The purpose of this study is to investigate about comparison of Eastern-Western medicine on the acne. Methods : We searched Eastern and Western medicine books for acne. We analyzed these books and examined category, definition, etiology, classification, internal and external methods of treatment of acne. Results : The results were as follows. 1. In Eastern medicine, Acne belongs to the category of the Bunja(粉刺), Jwachang(痤瘡), Pyepungbunja(肺風粉刺). In Western medicine, the other name of Acne is acne vulgaris. 2. In Eastern medicine, the definition of Acne includes manual extraction of comedones and skin appearance. In Western medicine, Acne is a common skin disease during adolescence and a chronic inflammatory disease of pilosebaceous unit of self localization. It is characterized by noninflammatory, open or closed comedones and by inflammatory papules, pustules, and nodules and it affects the areas of skin with the densest population of sebaceous follicles, these areas include the face, neck, back, and the upper part of the chest. 3. In Eastern medicine, the cause and mechanism of Acne arose from the state of internal dampness-heat and spleen-stomach internal qi deficiency due to dietary irregularities and then invaded external pathogen such as wind-dampness-heat-cold-fire in lung meridian lead to qi and blood heat depression stagnation. So it appears in skin. In Western medicine, the etiology and pathogenesis of Acne is clearly not identified, but there are most significant pathogenic factors of blood heat depression stagnation. In Western medicine, the etiology and pathogenesis of Acne is clearly not identified, but there are most significant pathogenic factors of Acne; Androgen-stimulated production of sebum, hyperkeratinization and obstruction of sebaceous follicles, proliferation of Propionibacterium acnes and inflammation, abnormaility of skin barrier function, genetic aspects, environmental factors etc. 4. In Eastern medicine, differentiation of syndromes classifies clinical aspects, and cause and mechanism of disease; the former is papular, pustular, cystic, nodular, atrophic, comprehensive type; the latter is lung blood heat, intestine-stomach dampness-heat, phlegm-stasis depression, thoroughfare-conception disharmony, heat toxin type. In Western medicine, it divides into an etiology and invasion period, and clinical aspects; Acne neonatorum, Acne infantum, Acne in puberty and adulthood, Acne venenata; Acne vulgaris, Acne conglobata, Acne fulminans, Acne keloidalis. 5. In Eastern medicine, Internal methods of treatment of Acne are divided into five treatments; general treatments, the treatments of single-medicine and experiential description, the treatments depending on the cause and mechanism of disease, and clinical differentiation of syndromes, dietary treatments. In Western medicine, it is a basic principles that regulation on production of sebum, correction on hyperkeratinization of sebaceous follicles, decrease of Propionibacterium acnes colony and control of inflammation reaction. Internal methods of treatment of Acne are antibiotics, retinoids, hormone preparations etc. 6. In Eastern medicine, external methods of treatment of Acne are wet compress method, paste preparation method, powder preparation method, pill preparation method, acupuncture and moxibustion therapy, ear acupuncture therapy, prevention and notice, and so on. In Western medicine, external method of treatments of Acne are divided into topical therapy and other surgical therapies. Topical therapy is used such as antibiotics, sebum regulators, topical vitamin A medicines etc and other surgical therapies are used such as surgical treatments, intralesional injection of corticosteroids, skin dermabrasion, phototherapy, photodynamic therapy, and so on. Conclusions : Until now, there is no perfect, effective single treatment. We think that Eastern medicine approach and treatment can be helpful to overcome the limitations of acne cure.

Colorectal Cancer Screening Practices of Primary Care Providers: Results of a National Survey in Malaysia

  • The incidence of colorectal cancer has been increasing in many Asian countries including Malaysia during the past few decades. A physician recommendation has been shown to be a major factor that motivates patients to undergo screening. The present study objectives were to describe the practice of colorectal cancer screening by primary care providers in Malaysia and to determine the barriers for not following recommendations. In this cross sectional study involving 132 primary care providers from 44 Primary Care clinics in West Malaysia, self-administered questionnaires which consisted of demographic data, qualification, background on the primary care clinic, practices on colorectal cancer screening and barriers to colorectal cancer screening were distributed. A total of 116 primary care providers responded making a response rate of 87.9%. About 21% recommended faecal occult blood test (FOBT) in more than 50% of their patients who were eligible. The most common barrier was "unavailability of the test". The two most common patient factors are "patient in a hurry" and "poor patient awareness". This study indicates that colorectal cancer preventive activities among primary care providers are still poor in Malaysia. This may be related to the low availability of the test in the primary care setting and poor awareness and understanding of the importance of colorectal cancer screening among patients. More awareness programmes are required for the public. In addition, primary care providers should be kept abreast with the latest recommendations and policy makers need to improve colorectal cancer screening services in health clinics.

고성능 탄화수소계 고분자 전해질막의 합성 전략 (Synthetic Strategies for High Performance Hydrocarbon Polymer Electrolyte Membranes (PEMs) for Fuel Cells)

  • 연료전지는 화석연료, 특히 내연기관을 대체할 수 있는 가장 대표전인 에너지 기술이다. 가장 중요한 핵심 재료 중 하나로서 연료기체의 장벽 역할을 함과 동시에 수소이온전달 역할을 하는 고분자 전해질 막(PEM)이 있다. PEM 내부에서 수화 채널은 수소이온의 전달통로 역할을 하기 때문에, 많은 연구자들은 높은 함수율을 저가습 상태에서도 유지하여 우수한 수소이온 전달 능력을 보유할 수 있는 상분리현상을 통한 친수성 채널 형성에 대하여 초점을 맞추어 왔다. 본 총설에서는 이러한 낮은 가습조건에서도 높은 수소이온전도도를 갖는 술폰화 PEM들의 합성 전략에 대하여 논의 하여보고, 다른 연구자들의 고성능 탄화수소계 PEM의 설계에 도움을 주고자 하였다.

연안여객선관광사업의 활성화 방안에 관한 연구 - 통영지역 주요항로를 중심으로 - (A Study on the reinvigoration strategy for Coastal Passenger Service: focused on the main service line at Tongyoung)

  • 우리나라 연안여객선사업은 목포, 여수, 마산/통영, 부산 지역을 중심으로 하여 항로 및 사업장이 구성되어 있다. 특히 통영지역의 경우 섬관광자원을 바탕으로 한 천혜의 육 해상자원 및 사적지가 많아 지역경제를 포함한 연안여객선사업 활성화의 기회 요인으로 작용하고 있다. 하지만 섬자원의 관광자원화를 통한 지역경제의 활성화 도모는 지역 연안여객선업체의 체계적인 관리와 운영에 달려 있음에도 불구하고 이에 대한 체계적인 연구가 이루어지지 않고 있다. 따라서 본 연구에서는 통영지역의 관광자원 및 연안여객선사업의 현황을 진단하였으며, 그 결과 이에 대한 체계적인 관리 방안 및 전략이 수립되고 운영되어 진다면, 그 성장가능성은 충분한 것으로 나타났다. 이를 위해 우선적으로 관광자원과 연안여객선사업의 연계전략이 수립되어야 하며, 다음으로 지역경제는 물론 연안여객선업체의 수익창출에 있어서 관광산업의 중요성을 인식할 수 있는 방안이 마련되어야 할 것이다.

휠체어 사용자의 대변기 이용을 위한 화장실 유효바닥면적에 대한 연구 (A Study on Effective Floor Area for Wheelchair Users to Use Toilet)

  • Purpose: The purpose of this paper is to provide logical basis on enlargement of toilet floor space for the disabled by "ACT ON GUARANTEE OF PROMOTION OF CONVENIENCE OF PERSONS WITH DISABILITIES, THE AGED, PREGNANT WOMEN. ETC" Revised Enforcement regulation [Asterisk 1] (2018.2.10.) and to help with making plans, designs and establishing policies by conducting experimental study with the enhanced floor area regulations. Methods: We conduct Experimental study, field work and interview for this paper. In experimental study, verification of the effective floor area was proceeded by measuring activity space for people in manual wheelchair, electric wheel chair and electric scooter and also reviewed useable area with the space expanded 0.2m both width and depth. In filed work, conducted an observation what other things (ex. sanitary equipment, hand rail and so on) can also affect the active space as well as effective floor space. In the interview, other problems and requirements that were not revealed were experimental study and observation of field work. Results : it's expected to provide better access to toilets for the disabled with the law revision of effective floor area. But, with the usage increasement of enlarged wheel chair and electric wheel chair, expanding side areas of toilet is also highly likely to be required going forward. Implications : Additional research is required on locations of exists and internal arrangement that effect the usage of toilet. It should be led to Universal design by making additional investigation and verification of the users patterns.

Determination of the Breast Cancer Risk Levels and Health Beliefs of Women With and Without Previous Mammography in the Eastern Part of Turkey

  • Objective: This research was conducted to determine the breast cancer risk levels of women with and without previous mammography and their beliefs on breast cancer and mammography. Methods: The sample for this descriptive research consisted of women aged 50 years or older who were registered at the Family Health Center in the city center of Erzurum. The research was conducted with a total of 420 women with at least one mammography (210) and without mammography (210) who presented to the center on Wednesdays and Thursdays for any reason between 1 January 2010 and 1 January 2011. Research data were collected using the personal information "Breast Cancer Risk Assessment Form" accepted and recommended by the Turkish Ministry of Health, and the Champion's Health Belief Model Scale for Breast Cancer and Screening (CHBMS). Data were evaluated using percentages and means with the t-test. Results: According to the research data, 89.8% of the women were found to be in the low risk group, 87.6% with and 91.9% without mammography. When the health beliefs of women with and without mammography were compared, it was found that susceptibility, seriouness, motivation, mammography benefit scores were higher among those with mammography (p<0.01). The mammography barrier score average was higher in the group without mammography (p<0.01). Conclusion: Knowing women's health beliefs, which have positive and negative effects on participating in mammography screening, may increase the rate of mammography uptake among women. Moreover, women with high breast cancer risk may be determined by increasing society's level of knowledge on breast cancer and risk factors.

  • Idiopathic nephrotic syndrome (INS) in children is characterized by massive proteinuria and hypoalbuminemia. Minimal change nephrotic syndrome (MCNS) is the most common form of INS in children. The pathogenesis of MCNS still remains unclear, however, several hypotheses have been recently proposed. For several decades, MCNS has been considered a T-cell disorder, which causes the impairment of the glomerular filtration barrier with the release of different circulating factors. Increased levels of several cytokines are also suggested. Recently, a "two-hit" theory was proposed that included the induction of CD80 (B7-1) and regulatory T-cell (Treg) dysfunction, with or without impaired autoregulatory functions of the podocyte. In contrast to the well-established involvement of T cells, the role of B cells has not been clearly identified. However, B-cell biology has recently gained more attention, because rituximab (a monoclonal antibody directed against CD20-bearing cells) demonstrated a very good therapeutic response in the treatment of childhood and adult MCNS. Here, we discuss recent insights into the pathogenesis of MCNS in children.

열 및 기계적 반복하중 하의 내열금속 표면 홀 주변 산화막의 변형 및 응력해석 (Cracking Near a Hole on a Heat- Resistant Alloy Subjected to Thermo-Mechanical Cycling)

  • 가스터빈엔진 내의 블레이드에서는 표면에 외부의 찬 공기를 흘려주는 작은 냉각 홀들을 가공하고 열 차단 코팅시스템을 코팅하는 방법으로 기지금속을 고온에서 보호한다. 열 차단 코팅은 열피로 과정에서 산화막의 성장 및 접합층과 산화막의 열팽창계수의 불일치로 산화막내부에 잔류응력이 발생하며 궁극적으로 코팅층의 분리를 유발한다. 본 연구에서는 내열합금 시편 표면에 작은 홀을 가공하여 여러 가지 고온 유지 조건에서 열 및 기계적 피로 시험을 수행하여 홀 주위의 산화막의 변형을 관찰하였다. 실험결과 기계적 피로가 홀 주위의 산화막의 변형에 중요한 영향을 미치며, 동일한 산화막 두께에서 고온 유지 시간이 짧을수록 변형이 쉽게 발생 하였다. 또한 본 연구에서는 홀 주위 산화막의 응력해석을 위한 이론적인 연구도 시도되었다.

무균성수막염과 뇌염환자 사이의 인구학과 검사소견의 비교: 나이와 C-반응단백질의 중요성 (Comparison of the Demographic and Laboratory Profiles of Patients with Aseptic Meningitis and Encephalitis: Significance of Age and C-reactive Protein)

  • Background: Viruses can cause either meningitis or encephalitis. It is unclear why some people suffer from aseptic meningitis, and others acquire aseptic encephalitis when infected with the same viral pathogens. The aim of this study was to compare demographic and laboratory factors between patients with aseptic meningitis and encephalitis. Methods: The demographic and laboratory differences were analyzed according to age, sex, diabetes, hypertension, C-reactive protein in the blood, white blood cell and protein in the cerebrospinal fluid, and glucose ratio (cerebrospinal fluid/blood). Additionally, we analyzed the nation-wide differencesin age between the patients with aseptic meningitis and those with encephalitis in Korea. Results: The patients with aseptic encephalitis were older, more likely to have hypertension, and had higher levels of C-reactive protein than did the patients with aseptic meningitis. However, the numbers of white blood cells in the cerebrospinal fluid were significantly higher in the patients with meningitis than in the patients with encephalitis. Multivariable analysis revealed that age >49 years, hypertension and a C-reactive protein level >5.81 mg/dL were independent and significant variables in the prediction of aseptic encephalitis. Additionally, the patients with aseptic encephalitis were older than those with aseptic meningitis in the nation-wide Korean database. Conclusions: Older age, hypertension, and higher levels of C-reactive protein are useful factors for the prediction of aseptic encephalitis.
