Purpose: This study aimed to evaluate the adherence to the clinical guideline for endotracheal suction in nurses working at intensive care units (ICU) and to identify the characteristics of nurses with good adherence. Methods: This study was a descriptive study to evaluate the pattern of adherence and its related factors to endotracheal suction. One hundred fifty five nurses working at ICU participated in this study. We used a questionnaire developed based on American Association for Respiratory Care (AARC) guidelines and other associated factors from previous studies. Results: Around half of the participants reported that their adherence to the clinical guideline was poor. Items deviated from the recommended guideline were reasons for initiating a suctioning, applied suction pressure ranged from 20 to 200mmHg, and applied catheter size from 6 to 17 french. Other factors deviated were the depth of inserted catheter, and inappropriate use of normal saline instillation. The most significant factor was related to hospital; the misused or misled clinical protocol. Conclusions: The adherence to the clinical guidelines of the endotracheal suction in ICU nurses was not appropriate, which might contribute to the patient health outcomes. More enhanced continuing education as well as hospital regulation is warranted.
In this study, we isolated ginseng crude saponin (GCS) from Korean red ginseng (KRG) and determined the ginsenoside content in it to investigate the physiological and pathological effects of GCS on acute pulmonary inflammation induced by intratracheal instillation of cigarette smoke condensates (CSC) and lipopolysaccharide (LPS) solution in BALB/c mice. GCS was orally administered at doses of 10 mg/kg and 25 mg/kg for 3 weeks. The recovery rate of GCS from KRG was 6.5 % and total ginsenosides from GCS was 1.13 %, and the content of Rb1 was the highest among them. Total inflammatory cells in the lung homogenates and bronchoalveolar lavage fluid (BALF) increased following intratracheal administration of CSC and LPS. However, GCS administration impaired this increase. Furthermore, it inhibited the increase in leukocytes in the blood, considerably decreased neutrophils in BALF, and declined infiltration of inflammatory cells and deposition of collagen in the tracheal and alveolar tissue. In this study, GCS was found to have a protective effect against acute pulmonary inflammation and it may be beneficial in preventing various respiratory diseases.
Objectives: To evaluate the biodurability of Korean Chrysotile(KC), the changes in fibers numbers and changes in the element composition of fibers from the lung of Sprague-Dawley rats instilled KC(average size $4.74{\mu}m$, $59,043{\times}10^6$ fibers/mg) was estimated. Methods: Rats were administered 1 mg KC(low group) or 2 mg KC(high group) by a single intratracheal instillation. At each time point(5 days, 5 weeks, 10 weeks), the numbers of KC fibers and the changes of element composition(atomic %) of KC fibers from the lung of the rats were analyzed with transmission electron microscope equipped with energy dispersive X-ray spectrometer. Results: Over time, the number of fibers within the lungs of animals were reduced. The average length of the low and high group is significantly reduced from 5 days after administration. Over time, the fiber ratio of at least $5{\mu}m$ remaining in the lung tissue of the low concentration group was up but the high group was reduced. From day 5 after administration, the composition ratio(Mg) was significantly decreased in all groups. Conclusions: Size and composition of Korean Chrysotile in the lung tissue of rats was changed from 5 days.
This study was performed employing the two stage aerated submerged biofilter of media pore size 1.5cm and 2cm, and infiuent substrate concentrations were 30.25mg COD/l, 50.1mg COD/l respectively. The purpose was to determine the treatment efficiency at the low concentration infiuent, reaction order and substrate flux with application of variable-order model that was presented by Rittmann and McCarty. . The results are as follows. 1. Treatment efficiency of 1st reactor was about BOD 82% and COD 76%, when effluent concentration was BOD 3.9 ~ 6.8, COD 7.1 ~ 12.5 mg/l, and this effluent concentration didn't satisfy the water quality grade I, II of river and lake. But as treated effluent of 1st reactor with 2nd reactor, we could achieve appropriate water quality, since instillation of 2nd reactor was needed. 2. Difference of media pore size between 1.5cm and 2cm didn't effect significantly to treatment efficiency and since this of 2nd reactor was about BOD 60%, COD 50%, an consideration of economic point of view should be carried out in field application. 3. Reaction order and substrate flux was varied 0.9851~0.9956 and 0.0028~0.0405 mg/$cm^{2} \cdot day$, and the substrate flux was increased as infiuent substrate concentration increased.
Two kinds of sepiolite (sepiolite 1, sepiolite 2), a $500^{\circ}C$ heat treated sepiolite (sepiolite 500), and a $700^{\circ}C$ heat treated sepiolite (sepiolite 700) were analyzed for their physicochemical properties. After these sepiolites were instilled into rat lungs, the effects of these substances on lung function and biochemical changes were evaluated. In addition, the fibers in the lungs were counted and characterized after the lungs were treated for electron microscopical analysis. The lungs instilled with sepiolites increased their weight and tidal volume statistically significantly compared with the unexposed control. The numbers of lymphocytes and polymorphonuclear cells (PMN) in the bronchoalveolar lavage (BAL) fluid also increased compared with the control, indicating the sepiolite induced inflammation. The heat treated sepiolites, however, did not show any toxicological differences from the untreated sepiolites. Although sepiolite showed less change in fiber atomic % compositions ( sepiolite 500, Si 0.9%, p <0.01 ; sepiolite 700, Si 3.7%, p<0.05) than chrysotile (Si 9.7%, p<0.01), the durability of the fibers in the lungs could not be determined in this subchronic experiment.
Wikyung-Tang(WKT) is herbal medication used in abcess-causing respiratory disease. Previous in vitro study demonstrates that WKY presents anti-proliferative effects in A549 cells. Here we show that WKY protects mice against lipopolysaccharide(LPS)-induced acute lung injury (ALI). We pretreated mice orally with WKY(2.34 and 5.85 g/kg body weight) 1, 24 and 48 hours before intratracheal administration of LPS. For same condition, control group was pretaken orally distilled water before LPS administration. 24 hours after LPS intratracheal instillation, bronchoalveolar lavege fluids(BALF) was obtained to measure protein and proinflammatory cytokines(TNF-${\alpha}$, IL-$1{\beta}$, IL-6). Protein and proinflammatory cytokines in BALF of WKT treated groups were totally decreased. Statistically, Protein, TNF-${\alpha}$ and IL-$1{\beta}$ of high concentrate WKT treated group decreased significantly compared with control group. In conclusion, WKY had some anti-inflammatory effect in a clinically relevant model of ALI. these results indicated that WKY was effective in inhibiting ALI and might act as a potential therapeutic reagent for treating ALI in the future.
Broussonetia papyrifera and Lonicera japonica have long been used in the treatment of inflammatory disorders, especially respiratory inflammation, in Chinese medicine. Previously, phytoformula (BL) containing B. papyrifera and L. japonica was found to exert strong anti-inflammatory activity in vitro and in vivo. In this study, the effects of BL on lung inflammation including bronchitis were examined in vitro and in vivo. BL (10-100 ${\mu}g$/ml) inhibited nitric oxide (NO) production of lipopolysaccharide (LPS)-treated alveolar macrophages, MH-S cells, primarily by down-regulating inducible NO synthase. BL also inhibited production of the proinflammatory cytokines, TNF-${\alpha}$ and IL-6. Against an animal model of pleural cavity inflammation, BL (200-400 mg/kg) significantly inhibited 5 h and 24 h carrageenan-induced pleurisy in rats when administered orally. Additionally, BL inhibited experimental bronchitis induced by intratracheal instillation of LPS to rats. Taken together, these results indicate that BL may be effective for the treatment of human lung inflammation as well as bronchitis.
The objective of this study was to determine the influence of drug lipophilicity on the extent of ocular and systemic absorption following topical solution instillation in the pigmented rabbit. ${\beta}-Blockers$ of various lipophilicity were chosen as model drugs, $25\;{\mu}l$ of a 15 mM drug solution in isotonic pH 7.4 buffer was instilled, and ocular tissue and plasma drug concentrations were monitored. Ocular absorption was apparently increased in all eye tissues, but non-corneal absorption ratio was decreased by increasing of drug lipophilicity. Systemic bioavailability was ranged from 61% for atenolol to 100% for timolol, and at least 50% of the systemically absorbed drug reached the blood stream from the nasal mucosa. Occluding the nasolacrimal duct for 5 min reduced the extent of systemic absorption of timolol and levobunolol, but did not do so for atenolol and betaxolol. Taken together, the ocular absorption of topically applied ophthalmic drugs would be modest for lipophilic drugs. By contrast, the systemic bioavailability would be modest for drugs at the extremes of lipophilicity, and the nasal contribution to systemically absorbed drug diminished with increasing of drug lipophilicity.
Background: Based on the assertion that apocynin diminishes acute lung injury (ALI) by inhibition of NADPH oxidase, the effect of apocynin was tested in interleukin-$1{\alpha}$ (IL-1)-induced ALI in rats. Methods: IL-1 was insufflated into the trachea of Sprague-Dawley rats to induce ALI, and apocynin (8 mg/kg) was given intravenously for inhibition of NADPH oxidase. In addition, we determined whether apocynin inhibited generation of superoxide anions from isolated human neutrophils. Five hours after IL-1 instillation, lung injury parameters, expression of cytosolic phospholipase A2 (cPLA2) by cells from bronchoalveolar lavage (BAL), an index of oxidative stress in lung tissues (${\gamma}$-glutamyltranspeptidase, activity), and ultrastructure of alveolar type II (AT II) cells were evaluated. Results: Apocynin decreased the generation of free radicals from phorbol myristate (PMA)-activated neutrophils in vitro, but did not ameliorate ALI. IL-1 induced enhancement of the expression of cPLA2 on neutrophils was not altered by apocynin. Conclusion: Apocynin induced suppression of the generation of superoxide anions from neutrophils by inhibition of NADPH oxidase does not attenuate IL-1-induced ALI in rats.
The purposes of this study were to develop the empowerment education program, to describe the experiences of diabetes patient's empowerment process and to develop a theoretical model of the diabetes patient's empowerment process. Method 1. : The development of the empowerment program for the diabetes patients: The strategies of the empowerment education program were enhancement of problem - solving, decision making, self-efficacy, self-control. participation and mutual support. Method 2. : According to the grounded theory methodology of Strauss and Corbin, the qualitative data was collected with in depth interviews and participants observations until its saturation when the 25 consented subjects were participating and interacting with the other subjects in the empowerment education program. Results: With the analysis of the data, 29 categories were generated. The core category generated, which was a central phenomenon of the empowerment process, was named powerlessness. The intervening conditions facilitating or impeding the empowerment process were discovered as supportive systems through the participation of group meeting, problem solving dialogue, and the knowledge deficit of self-care. The action/interaction strategies were developed as the paricipating, dialoguing, questioning, supporting system, self-controlling, self efficacy, enhancing self-esteem. stress relaxing and instillation of hope.
