Reconstruction of defect in the anterior part of the maxilla to enable implant placement or prothesis is a complicated treatment due to the anatomical position and lack of soft tissues. Two cases are presented in which autogenous iliac PMCB(particulate marrow and cancellous bone) with titanium mesh were used for premaxilla reconstruction and alveolar bone repair of the anterior maxillas prior to denture and implants fixation respectively. Cancellous bone from the anterior iliac crest was compressed and placed against a titanium mesh fixed to the bone of palate in a patient with severe defect of the anterior maxilla. There were no problem in the healing, and the anterior maxillas of two patients had increased height and width during the initial healing and remodeling. The clinical reports describe the use of titanium mesh for reconstruction of premaxilla. Autogenous bone grafts were harvested from the iliac crest and were loaded on a titanium mesh that were left in the patient's maxilla for 6 months before they were removed respectively. The radiographic analysis demonstrated that a 10mm vertical ridge augmentation had been achieved. In guided bone regeneration, the quantity of bone regenerated under the barrier has been demonstrated to be directly related to the amount of the space under the membrane. This space can diminish as a result of membrane collapse. To avoid this problem which involved the use of a titanium mesh barrier to protect the regenerating tissues and to achieve a rigid fixation of the bone segments, were used in association with autologous bone in 2 cases. The aim of this study was to evaluate the capability of a configured titanium mesh to serve as a mechanical and biologic device for restoring a vertically defected premaxilla.

The combined use of smart materials, complementing each others' characteristics and resulting in devices with optimised features, is providing new solutions in many industries. The use of ingenious combinations of smart materials has led to improvements in actuation speed and force, signal-to-noise ratio, sensor precision and unique capabilities such as self-sensing self-healing systems and energy autonomy. This may all give rise to a revival for numerous families of smart materials, for which application proposals had already reached a stationary situation. It may also provide the boost needed for the definitive industrial success of many others. This study focuses on reviewing the proposals, preliminary studies and success cases related to combining smart materials to obtain multifunctional, improved systems. It also examines the most outstanding applications and fields for the combined use of these smart materials. We will also discuss related study areas which warrant further research for the development of novel approaches for demanding applications.

Purpose: Human lipoaspirate cells are relatively easy to obtain in large quantities without cell culture. The aim of this in vitro pilot study was to determine the effect of cell therapy using uncultured lipoaspirate cells on cell proliferation and collagen synthesis of diabetic fibroblasts, which are the major contributing factors in wound healing. Methods: In order to get diabetic fibroblasts, dermis tissues were obtained from foot skin of diabetic patients who underwent debridements or toe amputations(n = 4). In order to isolate lipoaspirate cells, the same diabetic patients' abdominal adipose tissues were obtained by liposuction. The diabetic fibroblasts were co - cultured with or without autogenous lipoaspirate cells using porous culture plate insert. Initial numbers of the lipoaspirate cells and diabetic fibroblasts seeded were 15,000 cells/well, respectively. For cell proliferation assay, two treatment groups were included. In group I, diabetic fibroblasts were cultured with the insert having no cells, which serves as a control. In group II, the lipoaspirate cells were added in the culture plate insert. For collagen synthesis assay, one additional group(group III), in which diabetic fibroblasts were not seeded in the well and only lipoaspirate cells inside the insert were incubated without diabetic fibroblasts, was included for a reference. Results: One hundred to one hundred sixty thousand lipoaspirate cells were isolated per ml of aspirated adipose tissue. After 3 - day incubation, the mean cell numbers in group I and II were 17,294/well and 22,163/well. The mean collagen level in group I, II, and III were 29, 41, and 2 ng/ml, respectively. These results imply that both cell proliferation and collagen synthesis in the lipoaspirate cell treatment group were 28 and 44 percents higher than in the control group, respectively(p < 0.05). Conclusion: Uncultured lipoaspirate cell autografts may stimulate the wound healing activity of diabetic fibroblasts.
The purpose of this study was to compare effects of the bioceramics on healing processes of the alveolar bone defects in dogs. Five adult dogs aged 1 to 2 years were used in this study. Experimental alveolar bone defects were created surgically with a #1/2 round bur at the furcation area of the buccal surface of the mandibular 3rd, 4th premolars and 1st molar. Fifteen experimental alveolar bone defects were devided into three groups according to the type of graft materials. The groups were as follows : 1) flap operation with dense hydroxyapatite( DHA group ) 2) flap operation with porous hydroxyapatite( PHA group ) 3) flap operation with natural coral ( NC group ) At 1, 2, 4, 6, and 12 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Mallory stain for light microscopic evaluation. The results of this study were as follows : 1. In every group, inflammatory cell infiltrations were seen at 1st weeks due to surgical trauma, however inflammatory response owing to graft materials were not seen. 2. In every group, the appearance of connective tissue around graft materials was loosely formed at the initial stages, however the connective tissue was densely formed at 2 weeks. 3. The presence of osteocytes were observed at 2 weeks in the natural coral group, however the osteocytes were appeared at 6weeks in the dense hydroxyapatite group. 4. A new bone was formed from the base and walls of the defect and gradually expanded toward the graft materials. 5. A resorption of the natural coral occurred irregularly at the periphery of the material, therefore the size and shape of the natural coral were reduced at 6 weeks. 6. At 12 weeks, the porous hydroxyapatite and natural coral were surrounded by newly formed bone most completely, however dense hydroxyapatite was surrounded by newly formed bone in part.
A hospital is the most important infra-facility of the places which take care of people's body in social environment. There exist several environmental factors in the ways to heal the human body in hospital ward, but this study tried to look into the improvable pleasant sickroom environment with focus on light environment among the factors. In other words, this study aims at the research on proper daylight inflow into sickroom space as basic data for understanding the link between healing environment and natural lighting. In the simulation analysis through this research, this study completed the initial simulation using Autodesk Revit 2011 with focus on two types of individual multi-bed room units of the two general hospitals located in Gwangju City. This study made a simulation analysis of The two multi-bed rooms looking to the west using the weather data on Gwangju district, which is the strong point of ECOTECT2011. Conclusively, looking into the analysis of the simulation model in time of attaching the length of in & outside light shelf, the angle controlling of light shelf, the daylight factor and DA were found to show the tendency to decrease in the numerical value due to the decrease in sunlight inflow as the simulation model moved more toward the room from the window in comparison with the existing analysis of multi-bed rooms. Particularly, this study was able to read that the daylight factor and DA were more decreasing to improve at the light shelf than the existing bedrooms; conclusively, this study judges that the natural lighting simulation analysis could be helpful in improving the healing environment as basic data.
The objective of this study was to investigate the effects of implanted chitosan applied to surgically created wound in pigs. Six healthy $2{\sim}3$ months old Landrace and Yorkshire mixed breeds of both genders were used. A 2 cm straight skin incision was made and undermined skin ($2{\times}2cm$) over on the each pig's both sides of dorsal midline at 0, 7. 14 and 18 days. One wound (left side) was implanted 0.4 mg of cotton type chitosan and other wound was treated saline (3 ml). Each wound was closed with two interrupted suture of 2-0 sutures. The wounds created at 18, 14.7 and 0 days were named post-wounding day (PWD) 3, 7, 14 and 21, respectively. At 21 days after initial wounding, each wound was taken for histological observations. Reepithelialization tended to be greater in the chitosan group than in the control group at PWD 3 and 14. Granulation tissue formation did not show especial differences in two groups. Number of inflammatory cells was lesser statistically in level in the chitosan group than those in the control group at 21 days after wounding (p<0.05). Fibroblasts and neovasculature tended to be greater in the chitosan group than in the control group at PWD 3 and 7, and tended to be lesser in the chitosan group than in the control group at PWD 14 and 21. Collagen and fibrin were observed to be evenly distributed around the wound in the chitosan group. But collagen and fibrin were observed to be converged along the wound in the control group.
Primary fixation is one of the most important factor in establishing adequate osseointegration between implant and bone. To evaluate the initial healing response of bone around implants without primary bone contact, this study was designed to create considerable space between implant and bone in 5 mongrel dogs, about 1-year old. After 3 holes of 6.0mm in diameter were prepared at the femur neck of the dogs, commercially pure titanium thread type implants(STERI-$OSS^{(R)}$), 8mm in length and 3.8mm, 5.0mm and 6.0mm in diameter, were inserted. Implants were supported by only nonresorbable membrane($Teflon^{(R)}$), and the penetration of upper soft tissue into the gap was inhibited by it. The each implant was positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 were inserted in 2 dogs for mobility test and removal torque test.Fluorescent dyes were injected in order of Doxycycline, Alizarin Red S, and Calcein at intervals of 2 weeks. At 4-, 8-, and 12-week after placement, 3 dogs were sacrificed for histologic observation, and at 8- and 12-week after placement, 2 dogs were sacrificed for mobility test using $Periotest^{(R)}$ (Simens AG, Bensheim, Germany) and torque test using Autograph AGS-1000D $series^{(R)}$(Japan). The result were as follows: 1. The wider the gap between bone and implant was, the less bone maturity was, and the later osseointegration was occurred. Trabecular direction of new bone around implant was changed from parallel to perpendicular to the implant, and the gap was filled with new bone, over time. 2. There was a decreasing tendency over time in the mobility of all implants, but the wider gap between bone and implant was, the smaller decrease of the mobility was. 3. There was a increasing tendency over time in the removal torque gauge of all implants, and the wider gap was, the smaller increase of the removal torque gauge was. The results suggest that osseointegration in case of implant without primary bone contact may be obtained by guided bone regeneration technique with prolonged healing period, but the time of second surgery should be considered carefully.
Recently, frequently using hyperbaric oxygen therapy is known for its effectiveness on the healing of problem wounds such as osteomyelitis, osteoradionecrosis and gas gangrenous wound etc. The main objectives of this study was to determine the best protocol of its application of hyperbaric oxygen therapy. Author used 238 rats (Sprague-Dawley strain) deviding into 2 Groups, experimental I group for skin and experimental II group for palatal mucosal ischemic wounds, and observed its effects by microscopically. The obtained results summarized as follows; 1. Severe infiltration of inflammatory cells was observed in initial stages of both control and experimental I group. The infiltration showed decreasing tendency at 5th day of experimental D, E, F group while at 8th day in control group In d, f of experimental II group showed decreasing tendency at 8th experimental day while at 12th day in control group. 2. Macrophages appeared at 2nd day in D of experimental I group while at 6th day in control group. In d, f of experimental II group appeared at 6th day while at 10th day in control group. 3. As to the proliferation of capillary blood vessels showed at 3-4 day in most of experimental I group severely while at 8th day in control group. In experimental II group, it was at 8th day and 12th day respectively. 4. The proliferation of fibroblasts showed rather rapider in experimental I group, at 4-6th day, while at 8th day in control group. In experimental II group, it was at 8th day and 12th day respectively. 5. As to the collagen formations, it was at 4th day in experimental I group while at 8th day in control group. In experimental II group, it was at 6th day and 10th day respectively. 6. 5 rats (2.0 %) in E group and 8 rats (3.4 %) in f group showed oxygen toxic reaction, such as unstable attitude and tremor, during the experiments. This hyperbaric oxygen animal experiments disclosed excellent effects on the ischemic wound healing and it is thought to be the best protocol of its application was on D group (2.5 ATM. and 2 hrs, exposure).
A total of 29 eyes (25 dogs: one eye, 2 dogs: both eyes) with indolent corneal ulcer were treated with grid keratotomy from January 2008 to March 2010. The corneal lesions were reevaluated at 7-14 day intervals. The treatments had been repeated until fluorescein dye was not retained on the cornea and the epithelium did not appear to be loosely attached to the stromal layer. The healing rate of the corneal ulcers was 86.2%. The mean healing time ($mean{\pm}SD$) was $15.92{\pm}9.19$ days, ranged from 7 to 39 days. The lesions of remaining 4 eyes had deteriorated or not improved for more than 6 weeks. In those cases, $3^{rd}$ eyelid flap following grid keratotomy was applied. After 2 weeks, all of the eyes healed by the treatment. The results in this study suggest that grid keratotomy could be an excellent choice as an initial treatment for superficial corneal ulcers in dogs. In the cases of recurrence or to promote healing of the lesions, however, $3^{rd}$ eyelid flap following grid keratotomy is recommended.
The dental implants for edentulous Patients have been used for more than 20 years. After the introduction of osseointegration by $Br{\aa}nemark$, the commercially pure titanium implants were accepted by most practitioners. Recently dental implants are used for orthodontic anchorages as well as prosthetic abutment. Many researchers have reported implants as a good orthodontic anchorage through basic research and clinical evaluation. But previous researches were done after the healing time for osseointegration of inserted implants. If dental implants are to be used for prosthetic abutment the healing time for osseointegration is necessary, but orthodontic forces to implants are different from bite force regarding its amount of force, duration and direction. The authors evaluated the effect of orthopedic force to implants on bone tissue before osseointegration. 48 implants were placed at 12 rabbits. 2 implants into left side and 2 implants into right side were inserted along the long axis of femur respectively 2 weeks (2 weeks group), 4 weeks (4 weeks group) and 6 weeks (6 weeks group) after implants placement, 300g force had been applied to the implants at left side femur by Ni-Ti close coil spring for 4 weeks (experimental group) and no force applied to implants at right side femur (control group). After the force application for 4weeks, rabbits were sacrificed and microscopic evaluation was done by hematoxylin-eosin stain and Masson trichrome stain. The result3 were followed. 1. All implants in experimental group remained rigid after the force application for 4 weeks. 2. More fibrous tissue between bone and implants were noticed at 2 weeks experimental group than 2 weeks control group 3. More bone remodeling was noticed at 4weeks group than 2 weeks group and it was difficult to find out fibrous tissue between bone and implants at both experimental and control group of 4 weeks group. 4. It was hard to distinguish experimental group from control group at 6 weeks group. Therefore if initial stability can be obtained on implant insertion, it can be possible to use implants as a orthodontic anchorage before the healing time for osseointegration.
