The Effect of Handwashing Improving Program and MRSA Carrier Screening Program on the MRSA Infection Rates in an Intensive Care Unit (손씻기 향상프로그램과 MRSA 보균자 색출프로그램이 MRSA감염 발생률에 미치는 영향)

  • Purpose: To assess the effect of handwashing improving program and MRSA carrier detection program on MRSA(methicillin resistant Staphylococcus aureus) infection rate in a intensive care unit. Method: The intervention was Nosocomial Infection(NI) control program consisted of hand washing improving program and identification and treatment of MRSA carrier. Data on the NI and MRSA infections were collected by an infection control nurse based on the definition of CDC. MRSA infection rates were calculated by the number of MRSA infection per 100 admissions or 1,000 patients-days. The difference of MRSA infection rates between pre and post intervention was tested by Chi-square at =.05. Result: MRSA infection rates 3.0% or 3.2 per 1,000 patient-days at the pre, 4.6% or 3.7 per 1,000 patient-days at the post, and the differences were not statistically significant (p=.411, p=.769 respectively). Conclusion: The handwashing improving program and MRSA carrier detection program was not effective in reducing the Nosocomial Infection(NI) or MRSA infection rates. It is recommended further studies with a longer intervention and follow-up period.

Effect of Infection Control Education on Knowledge, Attitude and Self-Confidence of Student Nurses About Nosocomial Infection Control (감염관리 교육이 간호학생의 병원감염관리에 대한 지식, 태도 및 자신감에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of infection control education, knowledge, attitude and self-confidence of student nurses, regarding nosocomial infection control. Method: This was a nonequivalent control group nonsychronized design. The participants were 83 student nurses who were conveniently assigned to the experimental or a control group. The data were collected from March 5 to June 11, 2008 and analyzed based on Fisher's exact test and t-test. Result: The 1st hypothesis, "Posttest nosocomial infection control knowledge scores for the experimental group will be higher than scores for the control group" was supported (t=2.057, p=.043). The 2nd hypothesis, "Posttest nosocomial infection control attitude scores for the experimental group will be higher than those for the control group" was not statistically significant (t=.466, p=.643), The 3rd hypothesis, "Posttest nosocomial infection control self-confidence scores for the experimental group will be higher than those for the control group" was not statistically significant (t=2.290 p=.025). Conclusion: This infection control education was effective in increasing the levels of knowledge and self-confidence for student nurses regarding nosocomial infection control.

A Study on the Perception of Infection Control in Ultrasound Diagnostic Room (초음파검사실 감염관리 인식에 대한 연구)

  • The purpose of this study is to understand the infection prevention system and infection management perception of Ultrasound diagnostic room in medical institutions by sonographer's in general practitioner, semi and general hospitals. As a research method, a statistical analysis was conducted by surveying a total of 20 items in four category to determine the infection perception of sonographer's according to gender, age, working institution, and working period. As a result of infection awareness, males were in the ultrasound gel management category, women were in the transducer management category, all age groups were in the transducer management system category, and the General Practitioner, semi and general hospitals. were in the transducer management system category, and general hospitals showed a high degree of infection awareness in the category of disinfection and sterilization, and in the category of transducer management in the entire working period. In the results of the analysis of the correlation between the sub-factors of the perception of infection control in the Ultrasound diagnostic room the degree of correlation between each category showed a middle degree correlation(r>0.5) and a significant difference by category(p<.01). In conclusion, the prevention of infection in the ultrasound diagnostic room, which sonographer's firms think, was found to be the most efficient ultrasound diagnostic room infection control to implement transducer and gel management in accordance with the principles of disinfection and sterilization.

Detection of HBV Resistance to Lamivudine in Patients with Chronic Hepatitis B Using Zip Nucleic Acid Probes in Kerman, Southeast of Iran

  • HBV infection is contagious and may be transmitted vertically or horizontally by blood products and body secretions. Over 50% of Iranian carriers have contracted the infection prenatally, making this the most likely route of transmission of HBV in Iran. This study assesses the resistance to Lamivudine in patients with chronic hepatitis B infection using a new ZNA probe Real Time PCR method. To evaluate the effectiveness of Lamivudine therapy for chronic hepatitis B infection, a study was conducted on 70 patients (63 men and 7women), who had received the drug first line. All patients were tested for the presence of HBsAg and HBeAg, the serum ALT level and the HBV DNA load before and after treatment. In all samples resistance to Lamivudine was tested with the ZNA Probe. Our results showed that ZNA Probe Real Time PCR method could detect wild type,YMDD, and its mutants, tyrosine-isoleucine-aspartate-aspartate and tyrosine-valine-aspartate-Aspartate. Among an estimated seventy patients with chronic hepatitis B infection, 18 (25.7%) were resistant to lamivudine. Only one patient was negative for presence of HBS-Ag (5.6%) and two patients were negative for HBe-Ag (11.1%). Real-time PCR with Zip nucleic acid probes is a sensitive, specific and rapid detection method for mutations in the YMDD motif, which will be essential for monitoring patients undergoing Lamivudine antiviral therapy.

  • Studies were made on the relation of mastitis infection rate to the management and sanitations of dairy farms by examining 46 dairy herds. The results obtained were as follows: 1. The higher infection rate was observed in farms managed by employees than by owners, and in impoprted cattle than cattle horn in Korea. 2. Type of education and the period of experience of management were not correlated to the infection rate. 3. The infection rate was the highest in Summer and in cattle of 5~8 lactating age group. 4. It was observed that the amount of water supply, time of cleaning stall, frequency of disinfection, the method of using cleaning towel and the method of milking were correlated to the infection rate.

  • Purpose: The purpose of this study was to explore the content related to nosocomial infection control in the course on Fundamentals of Nursing. Method: Participants were 49 faculty who were teaching courses in Fundamentals of Nursing in universities and colleges in Korea. The questionnaire was composed of 55 items related to nosocomial infection control. Results: Eighteen items out of 55 items were taught in more than 80% of the universities and colleges. These included principles of infection control, principles and effect of hand washing, method of hand washing, hand scrubs, and donning sterile gown and gloves. Conclusion: The most effective interventions for infection control: including asepsis, hand washing, infection control for urinary catheterization, and infection control for IV sites were taught in most universities and colleges. However, the time assigned for teaching these items and the importance placed on practice were not considered sufficient.

  • Purpose:The purpose of this study was to apply the disinfection method using chlorhexidine in practice on disinfection of vascular access for hemodialysis. Methods: This study was designed as a randomized controlled trial for examining effectiveness on infection of the vascular access device for hemodialysis when using chlorhexidine and betadine/alcohol. One-hundred-thirty study participants were separated into two groups randomly. Infection signs of the vascular access device for hemodialysis were observed and recorded before disinfection on vascular access device. Result: Before the study, there was no difference between the experimental group (chlorhexidine group) and the control group (betadine/alcohol group) in general characteristics and hematological index. Incidence of infection rate of chlorhexidine group was 0 percent and the betadine/alcohol group was 1.5 percent. There was no significant difference between the two groups. Conclusion: This study examined the effectiveness of prevention of infection with the disinfection method using chlorhexidine and betadine/alcohol. The disinfection method using chlorhexidine is considered an effective and alternative method of betadine/alcohol.

THE CLINICAL STUDY OF MANDIBULAR FRACTURE

  • This is a retrospective study on 219 patients with mandibular fracture. The patients were treated in the Dept. of Oral Maxillofacial Surgery of WON KWANG UNIV. HOSPITAL from Aug. 1, 1984 to Sept. 30. 1988. The results were as follows. 1. The mandibular fractures occured most frequently in the twenties(35%) and male were predominant (74.7%) than females. 2. The most frequent etiologic factor was traffic accident(34.3%). 3. The most common location of fracture was symphysis(37.1%). And angle(27.6%), condyle(25.7%), ramus(1.6%) were next in order of frequency. 4. In mandible fracture, they have an average 1.8 fracture line. 5. The use of plate & screw system were more increased in the comparison of each year. 6. Intermaxillary fixation period was more reduced from the concept of 6 weeks fixation, due to the use of Plate & screw system. 7. Postoperative acute wound infection was developed 9.6% in 219 mandibular fracture patients. The compression osteosynthesis was most common cause of acute wound infection than any other treatment method. 8. Postoperative malocclusion was developed 4% in 219 mandibular fracture. And the compression osteosynthesis was most common cause of malocclusion. 9. Acute wound infection was detailed by the approach method. The Intraoral & extraoral combination method was most common cause on acute infection and intraoral, extraoral approach method was next in order of frequency. 10. Normal mouth opening process was proportioned to IMF period. The short IMF period have a fast normal mouth opening process.

INTERPRETING A SINGLE ANTISTREPTOLYSIN O TEST: A COMPARISON OF THE 'UPPER LIMIT OF NORMAL' AND LIKELIHOOD RATIO METHODS

  • Single serologic tests may occasionally influence clinicians in making diagnoses. The antistreptolysin O (ASO) test is a frequently used tool for detecting recent Streptococcus pyogenes infection and is helpful in the diagnosis of diseases like rheumatic fever. Using data from a 1989 prospective study of 600 healthy male military recruits, in which 43% experienced S. pyogenes upper respiratory tract infection (2-dilution rise in ASO), this report compared two methods of interpreting a single ASO titer. Using the 'upper limit of normal' (80 percentile) method, recruits with an ASO titer of greater than 400 showed evidence of recent S. pyogenes infection. This method had a sensitivity and specificity of only 65.9 and 81.9% respectively. In contrast to the 'yes-no'. dichotomy of the 'upper limit of normal' method. the likelihood ratio method statistics were ASO value specific, more consistent with clinical judgment, and better emphasized the caution clinicians must use in interpreting a single ASO test.

Effects of Infection Control Education for Families of VRE Patients (VRE 환자의 보호자를 위한 감염관리 교육의 효과)

  • Purpose: The purpose of this study was to examine the effects of infection control education for families of patients infected with vancomycin resistant enterococcus (VRE). Method: Forty family members of VRE patients were chosen from a university hospital and assigned to the experimental or control group. The experimental group was provided infection control education that consisted of one-on-one instruction using an information booklet, hand-washing video, and demonstration of hand washing practice. Dependent variables were self-reported knowledge and performance of VRE infection control measures, and the number of hand washings when entering and leaving patients' rooms. Results: Knowledge and performance scores were significantly higher for the experimental group compared to the control group. The experimental group washed their hands significantly more often when entering and leaving patients' rooms than the control group. Conclusion: Infection control education for family members of VRE patients was effective in improving knowledge and performance of infection control measures as well as improving the practice of hand washing. Further investigation is needed on the effects of infection control education for families on the actual VRE colonization and/or infection rate.
