Utility of Serum Procalcitonin for Diagnosis of Sepsis and Evaluation of Severity (혈청 프로칼시토닌(serum procalcitonin) 측정을 통한 패혈증 진단 및 중등도 평가의 유용성)

  • Background: Early recognition and treatment of sepsis would improve patients' outcome. But it is difficult to distinguish between sepsis and non-infectious conditions in the acute phase of clinical deterioration. We studied serum level of procalcitonin (PCT) as a method to diagnose and to evaluate sepsis. Methods: Between 1 March 2009 and 30 September 2009, 178 patients had their serum PCT tested during their clinical deterioration in the medical intensive care unit. These laboratories were evaluated, on a retrospective basis. We classified their clinical status as non-infection, local infection, sepsis, severe sepsis, and septic shock. Then, we compared their clinical status with level of PCT. Results: The number of clinical status is as follows: 18 non-infection, 33 local infection, 39 sepsis, 26 severe sepsis, and 62 septic shock patients. PCT level of non-septic group (non-infection and local infection) and septic group (sepsis, severe sepsis, septic shock) was $0.36{\pm}0.57$ ng/mL and $18.09{\pm}36.53$ ng/mL (p<0.001), respectively. Area under the curve for diagnosis of sepsis using cut-off value of PCT >0.5 ng/mL was 0.841 (p<0.001). Level of PCT as clinical status was statistically different between severe sepsis and septic shock ($^*severe$ sepsis; $4.53{\pm}6.15$ ng/mL, $^*septic$ shock $34.26{\pm}47.10$ ng/mL, $^*p$ <0.001). Conclusion: Level of PCT at clinical deterioration showed diagnostic power for septic condition. The level of PCT was statistically different between severe sepsis and septic shock.

De novo Assembly and Analysis of Amur Sturgeon (Acipenser schrenckii) Transcriptome in Response to Mycobacterium Marinum Infection to Identify Putative Genes Involved in Immunity

  • Fish mycobacteriosis is a common bacterial disease in many species of freshwater and marine fish and has caused severe loss of fish production. Mycobacterium marinum has been the most prevalent pathogen observed in several outbreaks of mycobacteriosis of farmed sturgeons in China. However, the immune responses and pathology of sturgeons in mycobacterial infection are rarely studied. Therefore, we used the Illumina RNA-seq method to analyze the transcriptome profile of Acipenser schrenckii challenged with Mycobacterium marinum. To begin, 168,220 non-redundant contigs were acquired from the infection and control groups, and among these, 33,225 contigs have acquired annotations. A total of 4,043 differently expressed (DE) contigs between the two groups were identified, and among these, 2479 were up-regulated and 1564 were down-regulated in the infected fish. A total of 1,340 DE contigs with acquired annotations in KEGG were enriched for 124 pathways including the TNF signaling pathway, and the Toll-like receptor signaling pathway. The roles of DE genes involved in significant pathways and other processes were discussed. The 2,209 DE contigs that have yet to acquire proper annotation may represent candidate genes associated with infection in sturgeons and are expected to serve as immunogenetic resources for further study. To our best knowledge, this is the first transcriptome study on sturgeons under bacterial infection.

Analysis and Evaluation of Pathogen Contamination Status in Interventional Angiography Room (인터벤션실의 오염실태 분석 및 평가)

  • In the radiology department, where radiation is used in medical institutions to perform examinations with various equipment, the field of surgical treatment is the intervention angiography room. Accordingly, strict infection control is required. The purpose of this study was to determine the contamination status by detecting pathogens before and after disinfection in the intervention angiography room, and to determine the degree of death by using a disinfectant, sodium dichloride isocyanurate, which is mainly used in the intervention angiography room. The subjects were 10 medical institutions of general hospital level or higher with an intervention angiography room in the P city, and 12 places with high contact frequency during examinations and procedures were sampled and requested to an analysis institution. As for the sample collection method, up/down, left/right directions were used to increase precision. Before disinfection, all procedures were completed, and after disinfection, exposure was performed using a disinfectant for at least 10 minutes, and detection was performed using a transport medium. As a result, in the pathogen analysis, most pathogens were detected in a humid environment or in a place with high contact frequency for microorganisms to thrive. The detected pathogens were found in the general environment or were human flora. It is a pathogen that does not cause disease under normal healthy host conditions. However, it was found to be an opportunistic infection that causes opportunistic infection depending on the case or situation in which the body's resistance is weakened. In addition, as a result of using the disinfectant mainly used in the intervention angiography room, it was found that more than 93.3% of them died. Therefore, the data of this study will be used as good basic data for the evaluation of pathogens in the intervention angiography room and will be of great help in infection control.

  • Deep sternal wound infection (with or without acute mediastinitis) is a serious complication of open heart surgery, with high rates of associated morbidity and mortality, and prolonged hospitallization. The result of treatment largely depends on timely diagnosis and appropriate surgical management. Postoperative deep sternal wound infections in 4 cases were successfully treated with extensive debridement, pressurized jet irrigation with antibiotic solution, sternal refixation by Robicsek method, and dilute antibiotic irrigation via irrigation-suction system. We report 4 cases with review of articles.

Rapid determination of baculovirus titers an antibody-based assay

  • A novel method is developed to yield virus titers in 10 h, is easy to .perform using 96-well plates, and applicable to both any Autographa californica nucleopolyhyderovirus (AcNPV) and Bombyx mori nucleopolyhedrovirus (BmNPV)-based recombinant baculovirus. This assay uses an antibody to a DNA-binding protein to detect the infected cells via immune-staining. The titer is determined by counting foci produced due to infection of virus under a fluorescent microscopy. The required incubation period was shortened considerably because infected cells expressed viral antigens at the post infection time of 4 h. Therefore, 10 hours were enough to estimate the virus titer including virus infection time, insect cell culture, and estimation of virus titer.

The Application of RNA Transcript Conformation Polymorphism in Resolving Mixed Infection of PVY Isolates

  • A method based on RNA-transcript conformation polymorphism (TCP) was tested for detection of two PVY isolates in a mixed infection. Differences in electrophoretic mobility of RNA transcripts copied from PCR products of each virus isolate enabled the distinction between the two virus isolates in a mixed infection. The identities of the RNA transcripts and hence of the infecting virus isolates were determined by annealing to reference oligonucleotides containing unique strain-specific sequences visualized by retardation of transcript mobility in gel. The ratio at which both virus isolates could be detected was as low as 1:10. The suitability of this procedure for the study of mixed virus infections is discussed.

A Study on the Diffuser Location for the Reduction of Airborne Infection in Operation Room (수술실내 공기감염억제를 위한 공조용 급배기구 위치 선정에 관한 연구)

  • The air ventilation system of operation rooms has been studied to prevent the cross infection during the operation. Operation rooms and air ventilation systems of three University hospitals were investigated. The distribution of microbe was measured by cultivating air samples in the operation room. A two-dimensional model for the cross-section of an operation room was developed for the CFD(Computational Fluid Dynamics) analysis. The characteristics of air flow in the empty operation room and in occupied operation room were calculated by using a CFD program. The current diffuser location of an old hospital did not deliver the clean air to the operation part efficiently. A new method to locate diffusers that improve air venting with little increase of the cost of equipment was suggested.

Prevalence of Cryptosporidium baileyi Infection of Chickens In South Kyeonggi Area (경기남부지역의 닭에 있어서 Cryptosporidium baileyi의 감염상황)

  • A total of 1050 layer and broiler chickens from 63 flocks of 21 poultry farms in South-Kyeonggi area, aged from 1 to 18 weeks, were investigated for the prevalence of Cryptosporidium baileyi infection from May 1992 to March 1993. The results have shown that 44 chickens(4.19%) were infected with C.(baileyi during the period of investigation) Fecal samples were treated by using Sheather's flotating technique and were examined under the light and phase contrast microscopy and then stained by Kinyoun method. Cryptosporidium oocysts were found in 35 out of 650 broilers(5.4%) and 9 out of 400 layer chickens(2.3%) aged mainly from 2 to 12 weeks. The regional infection rate were 4.7% in Pyeong-taek, 5.1% in An-sung and 2.3% in Yong-in, respectively. The average size of isolated oocyst was about $5.23{\times}4.92{\mu}m$ and the oocysts were orally inoculated into 2-day-old SPF chickens for the histological examination of oocyst in bursa of Fabricius. The study has concluded that C. baileyi infects chickens and oocysts were isolated in South-Kyeonggi area.

Prevalence of Parasite Infection of Poultry(turkey, helmeted guineafowl, pheasant, duck) in Chonbuk Province (가금(칠면조, 오리, 호로새, 꿩)의 장내 기생충 감염상황)

  • In order to monitor the parasites, fecal samples were taken from turkey (n=157), helmeted guineafowl(n=149), pheasant(n=190) and duck(n=190) in Chonbuk province. The identification of the parasites were determined by the fecal examination using the fluatation method and microscopical examination. The results were obtained as follows;1. The detection rate oi the parasites from 4 species of poutry was 47.2%(n=324 heads) out of 686 heads. 2. The identification rate was 85.9% in helmeted guineafowl, 63.2% in pheasant, 44.6% in turkey and 3.2% in duck, in order. 3. The mixed infection rate such as single, double, triple and quadrupl was 25.4%(174 heads), 14.1%(97 heads), 7.3%(50 heads) and 0.4%(3 heads), respectively. 4. The parasites isolated were identified as Capillaria spp. in 225 heads, Eimeria spp in 169 heads, Heterakis gallinarum in 116 heads, Ascaridia galli in 16 heads, Hymenolepis spp. in 3 heads and Strongyloides avium in 1 head, in order.

  • In this paper, we study the effect of Cytotoxic T Lymphocyte (CTL) and antibody immune responses on the virus dynamics with both virus-to-cell and cell-to-cell transmissions. The infection rate is given by Holling type-II. We first show that the model is biologically acceptable by showing that the solutions of the model are nonnegative and bounded. We find the equilibria of the model and investigate their global stability analysis. We derive five threshold parameters which fully determine the existence and stability of the five equilibria of the model. The global stability of all equilibria of the model is proven using Lyapunov method and applying LaSalle's invariance principle. To support our theoretical results we have performed some numerical simulations for the model. The results show the CTL and antibody immune response can control the disease progression.
