Fracture of the crown in a permanent incisor is relatively common. When it occurs with pulp exposure, it presents both restorative and endodontic problems. In the restoration of a fractured incisor, reattachment of the original fragment or restoration with a composite resin is preferred over a temporary crown. If fractured fragment is intact, the tooth can be restored with reattachment of the fragment. An exposed pulp in a young crown-fractured incisor is usually treated with either pulp capping or pulpotomy depending on the size of an exposure and time elapsed since injury. However, in teeth showing vital and/or hyperplastic pulp tissue at the exposure, only superficial layers of the pulp and surrounding dentin should be removed : i.e. partial pulpotomy can be performed in immature as well as mature teeth. This paper reports 2 cases of crown-fractured permanent incisors with pulp exposure that had been treated by reattachment of original fragment followed by partial pulpotomy or partial pulpectomy. The following results are obtained. ; 1. Fragment reattachment is an acceptable semi-permanent restoration of crown fractured young permanent incisor. 2. Partial pulpotomy is recommended as the treatment of choice in crown-fractured permanent teeth with pulp exposure.
The purpose of this study was to evaluate shade selection using conventional visual assessment in vivo natural teeth and porcelain-fused-to-metal (PFM) crown. Maxillary central incisors, lateral incisors and canines of one hundred twenty four college women were used as vivo natural teeth. Fifty one PFM crown for maxillary central incisor fabricated by dental laboratory were used as experimental materials. Using Vitapan Classical Shade Guides, shade selection of natural teeth was measured by each college woman and shade selection of PFM crown was measured by three ceramists with more than ten years career. Both natural teeth and PFM crown shade selection were measured through Shade Eye-Ex. From the shade selection comparing, following results were obtained. The results were as follows: 1. The shade matching identification of natural teeth between the shade selection using Vitapan Classical Shade Guides and the shade selection using Shade Eye-Ex was 27.4% in maxillary central incisor, 13.7% in lateral incisor and 18.5% in canine. 2. Among the shade selection of PFM crown by three ceramists, the shade evaluation of three ceramists were same only in ten cases. In twenty case, those of two ceramists were same. 3. The shade matching identification of PFM crown between the shade selection using Vitapan Classical Shade Guides and the shade selection using Shade Eye-Ex was 38.6% in average. These results suggest that the shade selection using conventional visual assessment should be dealt with care in clinic and need a credible method for shade matching color.
Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar. A radicular cyst is a slowly-growing epithelial sac lining a pathologic cavity in the bone at the apex of a tooth. Large radicular cyst revealed by radiographic examination almost always appear to involved several teeth. The author observed a 31 year old woman with a labial swelling and palatal mucosal expansion of the maxillary anterior region. Radiographic examination indicated that the lesion extended from the left maxillary lateral incisor to the right lateral incisor. Cyst consisted of tan to gray yellow slightly firm to partly hard plaque like tissue measuring 3.4 x 2.2 x 0.3㎝. The author has treated the cyst by means of enucleation with apicoecomies of the involved teeth except the right central incisor. Recovery proceed satisfactorily and there were no clinical or radiologic signs of recurrence 8 months later.
Fusion of teeth is a developmental anomaly. It occurs at the stage of tooth formation, which determines the shape and size of the tooth crown, when one or more teeth fuse at the dentin level during the morphodifferentiation of the dental germs. Such teeth show macrodontia and may cause crowding, as well as esthetic and endodontic problems. In this article, we report a rare case of a maxillary central incisor fused to a supernumerary tooth showing labial and palatal talon cusps, which was orthodontically moved across the midpalatal suture. A 13-year-old Caucasian boy sought treatment for the unesthetic appearance of his maxillary central incisor and anterior crowding. He was rehabilitated successfully via a multidisciplinary approach involving orthodontic, nonsurgical endodontic, periodontal, and prosthodontic treatments. After a 26-month treatment period, the patient's macroesthetics and microesthetics were improved. The overall improvement of this macrodontic tooth and its surrounding tissues through multidisciplinary treatment was documented using cone-beam computed tomography.

The purpose of this study is to determine the developmental relationship between the maxillary primary central incisors and their permanent successor. The auther took 315 cases of lateral cephalogram of the children (males were 171, females were 141) Angular change of the teeth and horizontal and vertical linear change were observed. The obtained results were as follows: 1. The inclination of the long axes of both incisor teeth was relatively stable and labio-version of both incisore was significant at 7 years of age. 2. The distance between the incisal edge of the permanent central insisor and the resorbing apex of the primary maxillary central incisor remained within 2mm of each other, 3. Vertical growth of the maxillary anterior portion was greater than horizontal growth from 6 to 7 years of age. 4. There was not a significant sexual difference.
Purpose: Pathologic tooth migration (PTM) commonly occurs in the anterior region and is associated with periodontal disease. The treatment of PTM of anterior teeth can be complex and time consuming, and a multidisciplinary approach is often required. Materials and Methods: The patient was a 38-year-old woman with a chief complaint of saving and realigning her elongated maxillary left central incisor. This paper describes the successful combined periodontal regenerative (guided tissue regeneration) and prosthetic treatment and a 2-year follow-up of maxillary central incisor with pathologic tooth migration, deep intrabony defect, and poor prognosis. Results: The right maxillary central incisor was restored by laminate veneer and the left by all-ceramic crown. The patient had no pain and discomfort and was satisfied with the outcomes of her treatment for 2 years. She has maintained her recall program at the Department of Periodontology at 3 months interval. Conclusion: The key step in the successful treatment of PTM in anterior region is to obtain a high level of cooperation from the patient. Maintenance of the treatment result of PTM is dependent on the continuous preservation of periodontal health.
Traumatic injuries to an immature permanent tooth may result in cessation of dentin deposition and root maturation. Endodontic treatment is often complicated in premature tooth with an uncertain prognosis. This article describes successful treatment of two traumatized maxillary central incisors with complicated crown fracture three months after trauma. The radiographic examination showed immature roots in maxillary central incisors of a 9-year-old boy with a radiolucent lesion adjacent to the right central incisor. Apexogenesis was performed for the left central incisor and revascularization treatment was considered for the right one. In 18-month clinical and radiographic follow-up both teeth were asymptomatic, roots continued to develop, and periapical radiolucency of the right central incisor healed. Considering the root development of these contralateral teeth it can be concluded that revascularization is an appropriate treatment method in immature necrotic teeth.
The mesiodistal crown diameters and crown length of the permanent anterior teeth have been studied and analyzed about the mean size, S.D, S.E etcs from stone models of 100 Korean male and 74 female. From the study, the following conclusions were made : 1. Statistical differences of left and right teeth were not found in the mesio distal crown diameters and crown length of the permanent anterior teeth. 2. The mean values of mesiodistal crown diameters of permanent anterior teeth were slightly larger in male than in females and statistical differences of males and females were found in maxillary right central incisor, mandibular left canine, mandibular right lateral incisor and mandibular right canine. 3. The mean values of crown length of permanent anterior teeth were slightly larger in male than in female and statistical differences of males and females were found in mandibular left canine, mandibular right canine. 4. Mesiodistal crown diameters and crown length of maxillary left lateral incisors and right lateral incisors of female are larger than that of male.
A study on the impacted teeth of the anterior segment was carried out. The data for this study were compiled form 1739 outpatients of the Department of Orthodontics, College of Dentistry, Seoul National University. The following conclusions were obtained; 1. The frequency of the impacted teeth of the anterior segment was 2.8% and the frequency of male was 4.0%, that of female was 2.1%., respectively. 2. The order of occurrence of the impacted teeth in the anterior segment was maxillary canine mesiodens, maxillary central incisor, maxillary lateral incisor, mandibular canine. 3. The etiologic factors of the impaction were the space deficiency for eruption, cleft palate, the prolonged retention of deciduous teeth, dentigerous cyst. 4. The favorable results of treatment and prognosis were obtained from the impacted teeth of the normal shape after surgical exposure and adhered the plastic attachment and inducted them into the dental arch.
