Delaminated Rotator Cuff Tear: Concurrent Concept and Treatment

  • Delaminated rotator cuff tear pertains to the horizontal split of the tendon substance. As reported previously, the presence of a delaminated tear and incidence of delaminated rotator cuff tear ranges from 38% to 92%. The different strain intensities applied across the rotator cuff tendon, and the shear stress between the bursal and articular layers seem to play a role in its pathogenesis. In a delaminated rotator cuff tear, the degree and direction of retraction between two layers differ, with accompanying intrasubstance cleavage. A surgeon therefore needs to consider and carefully evaluate the tear characteristics when repairing delaminated rotator cuff tear. Delaminated rotator cuff tear is considered to be a poor prognostic factor after rotator cuff repair, but numerous surgical repair techniques have been introduced and applied to resolve this problem. Recent literature has reported good clinical outcomes after delaminated rotator cuff repair.

History of Esophagogastric Junction Cancer Treatment and Current Surgical Management in Western Countries

  • The incidence of esophagogastric junction (EGJ) cancer has been significantly increasing in Western countries. Appropriate planning for surgical therapy requires a reliable classification of EGJ cancers with respect to their exact location. Clinically, the most accepted classification of EGJ cancers is "adenocarcinoma of the EGJ" (AEG or "Siewert"), which divides tumor center localization into AEG type I (distal esophagus), AEG type II ("true junction"), and AEG type III (subcardial stomach). Treatment strategies in western countries routinely employ perioperative chemotherapy or neoadjuvant chemoradiation for cases of locally advanced cancers. The standard surgical treatment strategies are esophagectomy for AEG type I and gastrectomy for AEG type III cancers. For "true junctional cancers," i.e., AEG type II, whether the extension of resection in the oral or aboral direction represents the most effective surgical therapy remains debatable. This article reviews the history of surgical EGJ cancer treatment and current surgical strategies from a Western perspective.

  • For large power applications, multi-stacked tape should be used because single tape is limited in flowing demanded current capacity. Besides insulation between layers is needed for safe operation because high voltages are generated in those applications. In this study, considering those situations which mentioned above, we measure the magnetization loss in several multi-stacked tape samples having the different insulation thicknesses and various packing numbers of tape by external magnetic field having various incidence angles.

Perspectives : Understanding the Pathophysiology of Intraventricular Hemorrhage in Preterm Infants and Considering of the Future Direction for Treatment

  • Remarkable advances in neonatal care have significantly improved the survival of extremely low birth weight infants in recent years. However, intraventricular hemorrhage (IVH) continues to be a major complication in preterm infants, leading to a high incidence of cerebral palsy and cognitive impairment. IVH is primarily caused by disruption of the fragile vascular network of the subependymal germinal matrix, and subsequent ventricular dilatation adversely affects the developing infant brain. Based on recent research, periventricular white matter injury is caused not only by ischemia and morphological distortion due to ventricular dilatation but also by free iron and inflammatory cytokines derived from hematoma and its lysates. The current guidelines for the treatment of posthemorrhagic hydrocephalus (PHH) in preterm infants do not provide strong recommendations, but initiating treatment intervention based on ultrasound measurement values before the appearance of clinical symptoms of PHH has been proposed. Moreover, in the past decade, therapeutic interventions that actively remove hematomas and lysates have been introduced. The era is moving beyond cerebrospinal fluid shunt toward therapeutic goals aimed at improving neurodevelopmental outcomes.

Effect of internal angles between limbs of cross plan shaped tall building under wind load

  • The present study revealed comparison the pressure distribution on the surfaces of regular cross plan shaped building with angular cross plan shaped building which is being transformed from basic cross plan shaped building through the variation of internal angles between limbs by $15^{\circ}$ for various wind incidence angle from $0^{\circ}$ to $180^{\circ}$ at an interval of $30^{\circ}$. In order to maintain the area same the limbs sizes are slightly increased accordingly. Numerical analysis has been carried out to generate similar nature of flow condition as per IS: 875 (Part -III):1987 (a mean wind velocity of 10 m/s) by using computational fluid dynamics (CFD) with help of ANSYS CFX ($k-{\varepsilon}$ model). The variation of mean pressure coefficients, pressure distribution over the surface, flow pattern and force coefficient are evaluated for each cases and represented graphically to understand extent of nonconformities due to such angular modifications in plan. Finally regular cross shaped building results are compared with wind tunnel results obtained from similar '+' shaped building study with similar flow condition. Reduction in along wind force coefficients for angular crossed shaped building, observed for various skew angles leads to develop lesser along wind force on building compared to regular crossed shaped building and square plan shaped building. Interference effect within the internal faces are observed in particular faces of building for both cases, considerably. Significant deviation is noticed in wind induced responses for angular cross building compared to regular cross shaped building for different direction wind flow.

  • Multi-crystalline silicon solar cells is not exist a specific crystal direction different from single crystalline silicon solar cells. In functional materials, therefore, isotropic wet etching of mc-Si solar cell is easy the acid solution rather than the alkaline solution. The reflectance of wet texturing process is about 25% and the reflectance of RIE texturing process is achieved less than 10%. In addition, wet texturing has many disadvantages as well as reflectance. So wet texturing process has been replaced by a RIE texturing process. In order to apply BIPV, RIE and wet textured multi-crystalline silicon solar cell modules was manufactured by different kind of EVA sheet. Moreover, in case of BIPV, the short circuit current characteristics according to the angle of incidence is more important, because the installation of BIPV is fixed location. In this study, we has measured SEM image and I-V curve of RIE and wet textured silicon solar cell and PV module. Also we has analyzed quantum efficiency characteristics of RIE and wet textured silicon solar cell for PV modules depending on incidence angle.

Investigation of earthquake angle effect on the seismic performance of steel bridges

  • In this paper, it is aimed to evaluate the earthquake angle influence on the seismic performance of steel highway bridges. Upper-deck steel highway bridge, which has arch type load bearing system with a total length of 216 m, has been selected as an application and analyzed using finite element methods. The bridge is subjected to 1992 Erzincan earthquake ground motion components in nineteen directions whose values range between 0 to 90 degrees, with an increment of 5 degrees. The seismic weight is calculated using full dead load plus 30% of live load. The variation of maximum displacements in each directions and internal forces such as axial forces, shear forces and bending moments for bridge arch and deck are attained to determine the earthquake angle influence on the seismic performance. The results show that angle of seismic input motion considerably influences the response of the bridge. It is seen that maximum arch displacements are obtained at X, Y and Z direction for $0^{\circ}$, $65^{\circ}$ and $5^{\circ}$, respectively. The results are changed considerably with the different earthquake angle. The maximum differences are calculated as 57.06%, 114.4% and 55.71% for X, Y and Z directions, respectively. The maximum axial forces, shear forces and bending moments are obtained for bridge arch at $90^{\circ}$, $5^{\circ}$ and $0^{\circ}$, respectively. The maximum differences are calculated as 49.12%, 37.37% and 51.50%, respectively. The maximum shear forces and bending moments are obtained for bridge deck at $0^{\circ}$. The maximum differences are calculated as 49.67%, and 49.15%, respectively. It is seen from the study that the variation of earthquake angle effect the structural performance of highway bridges considerably. But, there is not any specific earthquake angle of incidence for each structures or members which increases the value of internal forces of all structural members together. Each member gets its maximum value of in a specific angle of incidence.

스키에 의한 전방십자인대 손상에 대한 고찰 (The study of anterior cruciate ligament injury after a ski accident)

  • The ACL(Anterior cruciate ligament) is the smallest of the four main ligaments of the knee, but it is the primary stabilizer. Injuries have a tendency to occur when the foot is firmly planted and the knee is twisted. Any sports that requires acceleration, change of direction and deceleration can increase the chances of suffering an ACL tear. The incidence of severe knee sprains that involve the ACL are at an all-time high. Since 1980, the number of these injuries have increased at least three-fold. Although the rate of increase has been much less dramatic since the middle 1980s, even the ultra-modem releasable ski binding has not been able to start reducing the incidence of ACL injuries. An ACL injury prevention program developed for downhill skiers by the Vermont Safety Research group emphasized increasing awareness of situations that can potentially result in an ACL injury and pre-planning strategies if events, leading to these situations, begin to fall in place. As part of the above study by Ettlinger et al., an educational prevent program was developed to teach these principles and thus reduce the rate of serious knee injuries. Four thousand instructors and patrol at 20 ski resorts who received the training had a 62% decline in serious knee injuries compared to a similar group that did not receive this training. Whenever you fall, try not to fully straighten your legs. Don't try to get up until you've stopped sliding(unless you are try to avoid an obstacle or other skier). When you're down, stay down. And don't land on your hand. So, if you feel yourself falling: arms forward, ski plates together, hands over knee. Then you will be able to save your ACL.

연속 대절편 제작을 이용한 후두암의 병리조직학적 연구 (Histopathologic study of laryngeal cancer with serial section)

  • 후두암의 치료계획에 있어 어려운 점들로는 종양의 3차원적 위치, 점막하를 통한 전파, 후두연골의 침윤 여부 등이다. 특히 후두연골 침윤은 국소재발과 경부전이의 빈도가 높아 예후가 좋지 않다. 저자들은 후두암의 전파와 후두연골에의 침윤 양상을 이해하고 술전 임상적 진단의 정확도를 높여 치료방향을 설정하는데 도움을 얻고자 1991년 4월부터 1992년 11월까지 후두암으로 혹은 전적출술을 시행했던 예중 18례의 후두표본을 대상으로 연속 대절편을 제작하여 병리조직학적 관찰을 하고 다음과 같은 결과를 얻었다. 1. 후두연골 침윤의 양상은 주로 연골의 골화된 부위를 침범하였고 골화되지 않은 연골의 침윤은 매우 드물었다. 2. 침윤된 골화연골에서는 연골막이 건전하더라도 골수를 통해 전파하였다. 3. 연골막은 종양전이의 아주 강한 방어벽이었다. 4. 후두연골 침윤의 빈도는 갑상연골, 피열연골, 윤상연골, 후두개연골 순이었고 횡성문암에서는 후두연골 침윤이 88.9 % 로 상당히 높았다. 5. 후두연골 침윤여부에 대한 술전 CT의 민감도는 100%, 특이도 62.5%, 정확도 82.3%이다.

몬테카를로 방식에 의한 스퍼터율 계산에 관한 연구 (Calculation of Sputter Yield using Monte Carlo Techniques)

  • 본 논문에서는 몬테카를로 방식을 사용하여 이온의 에너지에 대한 타겟 원자의 스퍼터율(Sputter Yield), 이온의 주입 각도에 대한 스퍼터율, 이온의 주입에 따른 타겟 원자의 발산 분포를 3차원으로 시뮬레이션 하였다. 중(중)이온으로 (Ar/sup +/)을 사용하였고, 경(輕)이온으로 (H/sup +/)을 사용하여 10 eV에서 100 KeV 영역의 에너지에 따른 스퍼터율을 계산하였다. 또한, 스퍼터 타겟 물질로서 Cu, Al을 사용하여 계산하였고, 실험치와 일치함을 확인하였다. 스퍼터율은 입사 이온의 에너지가 증가함에 따라 증가하는 경향을 보이지만, 임계점 이후에는 점차적으로 감소하는 경향을 보였다. 중이온에 의한 스퍼터에서는 임계점이 10 KeV 영역이었고, 경이온에 의한 스퍼터에서는 1 KeV 이하 영역이었다. 또한, 이온의 주입 각도에 따라서 타겟의 스퍼터율은 점차적으로 증가하였고, 68° 부근에서 최대 스퍼터율을 기록하였다. 이온의 주입 각도에 따른 타겟 원자의 분포도에서는 각도가 커짐에 따라서 타겟 표면 법선 방향으로 방출되는 원자의 수가 많아짐을 확인하였다. 본 연구에서는, CRAY T3E 슈퍼컴퓨터에서 시뮬레이션을 수행하였으며, 구현된 몬테카를로 스퍼터 시뮬레이터의 GUI(Graphic User Interface) 환경을 구축하였다.
