Background: We developed a novel interlocking three-dimensional (3D) miniplate design with an adjustable configuration. As this device is new, surgeons must become familiar with its application. This study evaluated the usability and learning curves associated with the novel interlocking 3D miniplate for mandibular fracture fixation. Methods: The study participants, nine plastic surgeons, were asked to apply an interlocking 3D miniplate and a standard miniplate to polyurethane mandible models. The participants had completed the Basic Craniomaxillofacial Osteosynthesis course during residency and had operated on craniomaxillofacial fractures within the past 5 years. They were instructed to place the interlocking 3D miniplate three times and the standard miniplate once. We assessed the time required for implant placement, the comfort level of the surgeons, and the biomechanical stability of the plates. Biomechanical testing was conducted by subjecting the mandible to forces ranging from 10 to 90 N and the displacement was measured. Results: The results indicate increasing comfort with each attempt at placing the interlocking 3D miniplate, with a significant difference between the first and third attempts. Additionally, a reduction in application time was noted with repeated attempts, suggesting improved efficiency. Biomechanical tests showed comparable stability between the tested plates. Conclusion: Multiple attempts at applying the interlocking 3D miniplate resulted in increased comfort and reduced application time. These findings indicate that, despite its novelty, the interlocking 3D miniplate is relatively straightforward to apply and has a short learning curve. However, surgeons must have specific qualifications to ensure proper training and minimize errors during placement.
전극 접촉저항은 생리학적 측정에 중대한 인자이며, 전기적 임피던스 측정을 수행할 때 정확성에 제한적 요인이 될 수 있다. 생체전기임피던스 값들은 인간피부에 삽입되는 전극을 이용하여 하부 조직의 도전율과 유전율에 의해서 계산할 수 있다. 본 연구에서는 피지, 각질층, 표피층, 진피, 피하조직 및 근육층의 인체 피부의 생리적 변화를 검출하는데 주안점을 두고 있으며, 피하조직에 삽입되는 전극의 최적설계를 위해 유한요소법을 사용하였다. 이를 위해 전극의 길이(50 mm, 70 mm), 재질(금), 모양(직사각형, 둥근사각, 육각기둥) 및 깊이(22.325 mm)에 따른 전극설계의 차이를 유한요소법을 통해 피하조직 층으로부터 얻어지는 정보를 바탕으로 기하학적으로 평가하였다. 생체임피던스 실험에서 전극모양과 인가전압에 따라서 피하조직에서 생체임피던스 차이가 가장 크게 나타남을 확인하였다. 본 연구의 모의실험은 피부의 전기적 임피던스 측정과 해석에 관한 물리적 현상뿐만 아니라 다른 형태의 전극 설계에 관한 특성들을 설명할 수 있을 것이다.
3개월 동안 사용한 마모된 칫솔의 마모 정도와 양상을 관찰하고, 새 칫솔과 마모된 칫솔의 잇솔질 전 ${\cdot}$ 후 치태제거효율을 single-use design으로 비교 ${\cdot}$ 평가하여 3개월 주기의 칫솔 교체 주기의 근거를 임상적으로 확인 해보고자 하였다. 치주적으로 건강한 치과 대학생 42명을 대상으로 설문지를 통해 잇솔질 습관을 조사하고, 3개월간 동일한 칫솔과 치약을 사용하게 하였다. 3개월 후 피시험자를 무작위로 두 군(I, II)으로 나누고, 치석제거술을 시행한 뒤 2주후에 내원하도록 하였으며 내원 전 48시간동안은 잇솔질을 하지 않도록 지시하였다. 2주후 I군은 새 칫솔을, II군은 마모된 칫솔을 사용하도록 하였으며 잇솔질 전 ${\cdot}$ 후에 각각 구강 내를 erythrosin으로 염색한 후 6개의 Ramfjord 치아의 plaque score를 Patient Hygiene Performance (PHP) index로 측정하였다. 2주간의 washout period 후에 다시 치석제거술을 시행한 뒤, I군이 마모된 칫솔을, II군은 새 칫솔을 사용하게 하여 동일한 방법으로 PHP index를 각각 측정하였다. 마모된 칫솔은 수거하여 brushing surface area의 면적으로 마모도를 평가하였다. 결과는 paired t-test와 Pearson's correlation analysis로 통계처리 하였다. 2명이 탈락하였고 잇솔질 전 ${\cdot}$ 후에 대한 전체 부위, 치간 부위, 변연치은 부위의 plaque score는 두 칫솔 모두 통계학적으로 유의성 있게 감소하였으며 (p<0.0001), 두 칫솔을 비교한 경우에는 새 칫솔이 마모된 칫솔보다 치태 감소량이 통계학적으로 유의성 있게 많았다 (p<0.0001). 칫솔의 마모도는 평균 50.6% 증가하였으며, 마모도 증가에 따른 치태 감소량에는 직선적인 상관관계가 있었으나 통계학적인 유의성은 없었다. (전체 부위 r=-0.58, p=0.72 / 변연치은 부위 r=-0.50, p=0.76). Single-use design에서 3개월 동안 마모된 칫솔은 치태제거 능력에 있어서 새 칫솔보다 덜 효율적이였다. 칫솔의 마모도는 구강 위생 관리에 영향을 미치는 중요한 요인이며, 마모된 칫솔은 정기적인 교체가 요구된다. 또한, 치간 부위를 포함한 변연치은 부위의 치태를 정확하게 평가할 수 있는 치태지수에 대한 연구가 필요하겠다.
최근 다양한 증례 보고에서 소개되고 있는 jaw motion tracking은 환자의 안궁 이전 및 개별화된 하악 운동 경로를 기록한 후, 이를 computer-aided-design/computer-aided-manufacturing (CAD-CAM) 소프트웨어의 가상공간상에 재현하는 방법이다. 본 증례의 환자는 오랜 기간 구치부의 상실로 인해 교합평면의 붕괴가 관찰되었기에, 수직 교합 고경의 증가를 동반한 완전 구강 회복술을 계획하였다. 우선 jaw motion tracking을 진행하여 새로운 중심위 상에서 환자의 하악 운동을 기록한 후, 이 정보를 환자의 초진 구내 데이터 및 3차원 안면 스캔 데이터와 조합하여 가상 환자를 생성하였다. 가상 환자 상에서 진행한 디지털 왁스업과 임플란트 식립 계획을 바탕으로 임시 보철물을 제작하였다. 새롭게 설정된 수직 교합 고경 상에서 적절한 견치 유도를 보이는 임시 보철물의 검증을 통해, 최종 보철물로 이행하였다. 이처럼 디지털 치의학 기술의 장점을 활용하여 환자는 저작능과 심미성의 개선에 만족하였기에 본 증례를 보고하고자 한다.
최근 디지털 치의학이 발달함에 따라 진료실 기반의 구내스캔과 computer-aided design/computer-aided manufacturing (CAD-CAM)을 이용한 완전 디지털 방식의 의치 제작이 증가하는 추세이다. 디지털 방식의 제작은 진료실과 기공실에서의 과정을 간소화하고 디지털 데이터를 보완할 수 있는 장점이 있다. 본 증례는 임플란트 식립이 불가하거나 지연되는 전치부 결손 환자에서 완전 디지털 제작 방식을 이용한 임시 가철성 의치의 수복 증례이다. 인공치와 의치상을 각각 디자인하고 프린팅하여 접착하는 방법과 인공치와 의치상을 일체형으로 디자인하고 프린팅하여 제작하는 두 가지 방법으로 임시 가철성 의치를 제작하고 구강 내 시적하였다. 발치 된 상태의 구내스캔, 임시 가철성 보철물을 구강 내에 장착 후의 구내 스캔과 보철물 자체의 스캔을 이용하는 삼중 스캔 기법으로 의치 적합도를 비교하였고, 두 가지 방법 모두에서 양호한 적합도를 보였다. 진료실 기반의 완전 디지털 방식을 이용하여 간소화된 과정으로 환자와 술자 모두 만족하는 결과를 얻었기에 본 증례를 보고하는 바이다.
교정용 미니임플랜트는 작은 크기로 삽입 부위에 제한이 적어 고정원 조절에 널리 이용되고 있다. 그러나 잦은 탈락의 단점이 있어 초기 안정성을 향상시키기 위한 다양한 형태와 길이에 관한 연구가 시도되어왔다. 이 연구의 목적은 교정용 미니임플랜트의 형태와 길이에 따른 초기 안정성에 대한 기계역학적 성질을 비교 분석하는 것이다. 미니임플랜트의 직경은 1.6 mm였으며, 형태는 원통형과 원추형, 길이는 6 mm와 8 mm로 구성되어 원통형 6 mm, 원통형 8 mm, 원추형 6 mm, 원추형 8 mm군과, 중앙 1/2에서부터 상방으로 나사산이 점차적으로 작아지는 변형된 원추형 8 mm군을 포함하여 총 5군으로 구성되었다. 각 군은 10개의 미니임플랜트로 구성되었고, 균일한 밀도의 polyurethane foam에 삽입 후 제거하였다. 시간에 따른 삽입 및 제거 토크의 변화와 각각의 최대 토크 등을 측정하여 기계역학적 성질을 비교 분석하였다. 동일한 길이에서 원추형은 삽입과 제거 시 원통형보다 유의성 있게 더 큰 토크를 보여주었다 (p<0.001). 동일한 형태에서 삽입과 제거 시 긴 미니임플랜트가 짧은 미니임플랜트보다 유의성 있게 더 큰 토크를 보여주었다 (p<0.001). 원추형 6 mm는 원통형 8 mm에 비해 삽입 시 유의성 있게 낮은 토크를(p<0.001) 보여주었지만, 제거 시 비슷한 토크를 보여주었다. 변형된 원추형 8 mm에서는 최대 제거 토크 후 지속적으로 높은 제거 토크를 보여주었다. 미니임플랜트의 초기 안정성은 원추형에 의해 향상될 수 있으며, 상부 나사산의 작은 크기와 형태는 미니임플랜트의 초기 안정성에 긍정적인 영향을 미치는 것으로 보인다.$76^{\circ}$였다. 본 연구는 스크류의 역학적인 특성을 알아보고자 세 종류의 self-drilling형의 교정용 미니 스크류 (Type A; 순수한 원통형, Type B; 내경이 증가하는 구간이 있는 부분적인 원통형, Type C; 원통형과 원추형이 함께 있는 복합형)를 피질골의 두께를 다르게 설정한 인공골 시편에 삽입하였다. 일정한 회전 속도와 수직력을 부여할 수 있는 구동식 토오크 시험기(Biomaterials Korea, Seoul, Korea)를 이용하여 삽입 토오크를 측정하고 만능시험기(Instron 3366, Instron, Norwood, MA, USA)를 이용하여 Pull-out 강도를 측정하여 다음과 같은 결과를 얻었다. 피질골 두께의 증가에 따른 최대 삽입 토오크(maximum insertion torque) 값은 Type A는 차이가 없는 반면(p > 0.05), Type B와 Type C에서는 유의한 증가가 관찰되었다 (p < 0.05). 모든 피질골 두께에서 Type C > Type A > Type B의 순으로 최대 삽입 토오크 값이 크게 나타났다 (p < 0.05). Pull-out 강도는 모든 경우에 Type A가 Type B와 Type C보다 낮게 측정되었다 (p < 0.05). Type별로 피질골 두께와 최대 삽입 토오크, Pull-out 강도 간에 통계적으로 유의한 상관관계가 있었다 (p < 0.05). 이상의 결과는 미니 스크류 식립을 위한 디자인 선택 시 식립부위의 피질골 두께를 고려해야 함을 시사한다.
The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. Various periodontal procedures have been used throughout the years in an attempt to reestablish attachment of periodontal tissues to root surfaces affected by periodontitis. Flap debridement surgery has been demonstrated to be a successful procedure in gaining the probing attachment level and reducing probing depth. A tendency towards impaired wound healing following periodontal procedures in smokers has been clinically documented. But, previous clinical studies on healing response in smokers are based on a retrospective design. The purpose of this study was to evaluate the treatment outcome following flap debridement surgery in smokers compared to nonsmokers. 25 patients with moderate to advanced periodontitis were included for study. Among these patients, 13 patients were smokers, and 12 patients were nonsmokers. Mucoperiosteal flap was raised with the sulcular incision. No antibiotic treatment was administered postsurgery. The patients was recalled at monthly intervals during a period of 6 months following the surgery. The patients were received supragingival scaling and oral hygiene reinforcement. All the recordings, including modified O' Leary plaque control record, bleeding on probing, probing pocket depth, probing attachment level,were recorded, presurgery and 6 months postsurgery. The changes of all the recordings at 6 months after flap debridement surgery revealed the following results: 1. PI on all the dentitions and surgical sites showed no statistical significance between smokers and nonsmokers at presurgery. But, smokers demonstrated a significantly lower % of PI than nonsmokers at 6 months postsurgery. 2. Smokers demonstrated a greater % of BOP sites than nonsmokers on the surgical sites and all the dentitions, presurgery and 6 months postsurgery. But, there was no statistical significance between two groups. 3. Smokers exhibited significantly less reduction of probing depth in the 3 mm or less probing pocket depth(PPD) group, 6mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery. 4. Smokers exhibited significantly less gain of probing attachment level(PAL) in the 3mm or less PPD group, 6 mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery.
The present study was performed to assess and compare the clinical and microbiological effects following local application of 2% minocycline gel or 0.1% chlorhexidine subgingival irrigation to augment scaling and root planing in patients with moderate to advanced chronic adult periodontitis. 32 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. In each patient, the quadrants that had 2 or more teeth with $5{\sim}8mm$ probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the split-mouth design. All patients received standardized oral hygiene instructions at the beginning of the study and all remaining teeth received scaling and root planing until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3 week in the test side. The 0.1% chlorhexidine solution and the normal saline were irrigated subgingivally for about 30 seconds in the positive control side and negative control side respectively. The clinical and microbiological analysis carried out at 0, 4, 8, and 12 weeks . The results of this study were as follows; 1. In saline irrigation group, there was no adjunctive effects in probing pocket depth reduction, sulcular bleeding index and no significant changes in relative proportions of subgingival bacteria. 2. The chlorhexidine irrigation as an adjunct to scaling and root planing results in reduction in the plaque index and sulcular bleeding index, but there was not statistically significant. The relative proportion of spirochetes was significantly reduced, but the proportion of motile rods was no significant reduction. 3. The minocycline gel delivered subgingivally as an adjunct to scaling and root planing provide significant benefit in reducing probing depths and sulcular bleeding index compared to saline and chlorhexidine irrigation groups. 4. The relative proportions of spirochetes and motile rods were significantly reduced and the proportions of cocci and non-motile bacteria were correspondingly increased in the minocycline gel group. In conclusion, minocycline gel delivered subgingivally as an adjunct to scaling and root planing induces clinical and microbial responses more favorable for periodontal health than saline and chlorhexidine subgingival irrigation.

The clinical and microbiological analysis was carried out at 0, 4, 8, and 12weeks. The results of this st udy were as follows; 1.2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

제 13 조 (홈페이지 저작권)
① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
