The Effect of Eicosanoid Analogues on the Change to Blood Pressure in Rat (Eicosanoid 유도체가 흰쥐 혈압 변화에 미치는 영향)

  • Arachidonic acid (AA, C20 : 4, $\omega$-6) and eicosapentanoic acid (EPA,C20 : 5, $\omega$-3), which are polyunsaturated fatty acids forming eicosanoids, were tested for their effects on blood pressure in Wistar rats and SHR. AA is the most important precursor for the biosynthesis of eicosanoids which include the prostaglandins, prostacyclin (PGI$_2$), thromboxane $A_2$ (TXA$_2$) and the leukotriens. TXA$_2$is a potent vasoconstrictor and a powerful inducer of platelet aggregation causing myocardial infarction and hypertention. In contrast, PGI$_2$ induces vasodilation and inhibits platelet aggregation. In this study, AA markedly increased blood pressure, but its effect was antagonized by both EPA, a structural analog of AA, and dazmegrel, a TX synthetase inhibitor. Also, AA enhanced the antihypertensive effects of hydralazine and captopril, and EPA reduced TXA$_2$ production. These results indicate that the hypotensive effects of EPA might be closely related to the decrease in TXA$_2$ biosynthesis due to competitive inhibition by structural similarity of the EPA to the AA, the precursor of TXA$_2$.

In vivo Pharmacological Evaluation of Newly Synthesized Nonpeptidic $AT_1$ Receptor Antagonists in Rats

  • This study was conducted to characterize the in vivo pharmacology of KR-30988, KR-30992 and losartan, new AT antagonists, given as i.v. cumulative doses, in two antimal models of high renin, conscious renal artery-ligated hypertensinve rats (RHRs) and nomotensive rats anesthetized with urethane (90 mg/kg, i.p.) and .alpha.-chloralose (90 mg/kg, i.p.), with a special emphasis on the phamacological characterization of the latter model. In conscious RHRs, KR-30988, KR-30992, losartan and captopril caused a dose-dependent decrease in blood pressure, their relative potencise ($ED_{20}$) being 0.057, 0.028, 0164 and 0.018 mg/kg, i.v., repectively. In anesthetized rats, 2 hours after anesthesia, plasma renin activity was increased from 7.31 tp 34.07 ng/ml/h, the level approximately 1.5 times greater than the highest level in RHRs. In anesthtized rats, the $ED_{20}$s for all four compounds were 0.004 mg/kg i.v., respectively. By comparison, $ED_{20}$sfrom anesthetized rats were 4 to 5 times smaller than those from conscious RHRs, with a good correlation (.gamma. = 0.999) noted between thetized rats to the hypotensive activity of the compounds and the same order of potencies intwo models. These results suggest that, in addition to PHRs, the normotensive rats anesthetized as above can serve as a suitable model for the rapid phamacological evaluation of $AT_1$ receptor antagonists.

  • These studies were conducted in an attempt to investigate effects of 'Daeshiho-Tang' on the blood vessels, activities of liver enzyme and excretory action of bile juice in serum of $CCl_4-intoxicated$ rabbits, glucose and total cholesterol levels in serum of alloxan-induced diabetic rabbits. The result of these studies were summarized as follows; 1. Hypotensive ind vaso-dilatating actions due to vascular smooth muscle relaxation were noted in frogs, rabbits and dogs. 2. GOT, GPT and total cholesterol in serum of $CCl_4-intoxicated$ rabbits showed significant depressive effects. 3. Significantly increased biliary secretary effect was noted in $CCl_4-intoxicated$ rabbits. 4. Glucose and total cholesterol levels showed markedly decraese in alloxan-induced diabetic rabbits. Considering the experimental studies results, it is suggested that 'Daeshiho-Tang' has therapeutic efficacy to treat febrile diseses and metabolic diseases.

Traumatic Coronary Artery Dissection in a Young Woman after a Kick to Her Back

  • We present the case of a 38-year-old woman admitted to our outpatient clinic with accelerating back pain and fatigue following a kick to her back by her husband. Upon arrival, we detected ST segment elevation in the D1, aVL, and V2 leads and accelerated idioventricular rhythm. She had pallor and hypotension consistent with cardiogenic shock. We immediately performed coronary angiography and found a long dissection starting from the mid-left main coronary artery and progressing into the mid-left anterior descending (LAD) and circumflex arteries. She was then transferred to the operating room for surgery. A saphenous vein was grafted to the distal LAD. Since the patient was hypotensive under noradrenaline and dopamine infusions, she was transferred to the cardiovascular surgery intensive care unit on an extracorporeal membrane oxygenator and intra-aortic balloon pump. During follow-up, her blood pressure remained low, at approximately 60/40 mmHg, despite aggressive inotropic and mechanical support. On the second postoperative day, asystole and cardiovascular arrest quickly developed, and despite aggressive cardiopulmonary resuscitation, she died.

  • Traditional system of medicine mentioned the use of plants in treatment of various human ailments. Mochrus (Bombax ceiba Linn.) is one of the medicinal plants used in Unani medicine since time immemorial. It is a very tall tree with approximately 150 feet height. It is widely found in temperate Asia, tropical Asia, Africa and Australia. In India, it can be found at altitudes up to 1500 m. In peninsular India, the tree is very common in the dry as well as moist deciduous forests and near rivers. It is reported to possess qabiz (constipative); mujaffif (siccative); muqawwi rehm (uterine tonic); nafi sailan rehm (beneficial in leucorrhea); mumsik wa mughalliz mani (increase consistency of semen); muallide mani (production of semen); habise tams (amenorrhoic); dafi fasaad khoon wa safra (purifies blood and bile); taskeen hiddat aza (hotness of organs); muqawi asnan wa lissa (tonic to teeth and gums) etc. Ethnobotanical studies prove that it is used in diarrhoea, asthma, impotency, boils, wounds, leprosy, pimples and many other skin diseases. It is an anthelmintic remedy since ancient time. Pharmacological studies like hypotensive, antioxidant, analgesic, anti-inflammatory, antipyretic, antiangiogenic, anti- bacterial, cytotoxic, hepatoprotective, diuretic, anthelmintic, anticancer, spermatogenic and anti-helicobacter pylori activities have been evaluated for various parts of this plant that confirms to its use in classical medicine.

  • Aucubin, the natural product, which is isolated from Aucuba japonica, has a variety of pharmacological effects such as liver-protective function, inhibition of liver RNA and protein biosynthesis, hypotensive activity and antimicrobial effect, etc. This study was performed to investigate the effects of iridoid compounds on wound healing. The author prepared 0.1% aucubin solution and 0.1% aucubin ointment as an active form, aucubigenin to which aucubin was converted by ${\beta}$-glucosidase. Artificial surgical wound was made on either 1cm lateral side of the dorsal midline along the axis of spine of Sprague-Dawley rats under sterile technique. Application of 0.1% aucubin solution or 0.1% aucubin ointment to surgical wound was done daily. Light microscopic examination was performed on the postsurgical 3 days, 5 days, and 9 days. The 0.1% aucubin solution group epithelialized earlier than the control group and the fibrosis of granulation tissue of both aucubin groups were more prominent than the control group. Collectively, this study suggests the possibility of aucubin as a topical agent. Further research should be performed on the mechanism of aucubin on wound healing and proper formulation for effective topical agents.

  • 1. In the rabbit and the dog, the blood pressure response to water extract and methanol extract obtained from Atractylodes rhizoma alb'a was investigated. 2. Water extract and methanol extract, when administered into the rabbit and the dog by the route of vein, produced fall of the blood pressure. 3. The depressor response of the rabbit to water extract and methanol extract was not affected by $Avicel{\circledR}$, propranolol and atropine. 4. The depressor response by water extract and methanol extract in the rabbit was not affected by guanethidine, but water extract and methanol extract produced elevation of blood pressure in this rabbit. 5. Pretreatment of rabbit with chlorisendamine or phenoxybenzamine weakened the depressor response to water extract and methanol extract, and the both extracts produced secondary elevation of blood pressure in this rabbit. 6. The pressor response of the chlorisondamine-treated rabbit to water extract and methanol extract was not affected by atropine. 7. Water extract decreased the pressor action of tyramine and depressor action of pilocarpine and isoproterenol, but did not affect the blood pressure response of nor einephrine, angiotensin and dimethylpehnyl piperazinium iodide(DMPP).

A Case of Giant Papillary Thyroid Carcinoma Resection with Massive Intraoperative Bleeding (술 중 대량 출혈을 동반한 거대 갑상선유두상암종 절제술 1례)

  • A 62-year-old female patient had goiter for twenty years. She visited out-patient clinic with a hoarse voice and intermittent breathing difficulties. About protruding 15cm sized mass located the anterior neck and right vocal cord paralysis was observed. Preoperative CT scan was strongly suspected of thyroid gland cancer and cervical lymph node metastasis. Therefore, fine needle aspiration test was performed and surgical treatment was planned with the histopathologic results (papillary thyroid carcinoma). Surgery was performed with total thyroidectomy, bilateral cervical lymph node dissection, and right selective nodal lymph node dissection (level II-V). During operation right thyroid seemed to be adherent to surrounding tissue and the blood vessels were extremely engorged. There was hypotensive crisis because of intraoperative excessive bleeding. However it was managed by repetitive transfusion. The operation was completed without abnormalities. She underwent 4 times of bleeding control operation due to postoperative bleeding. After complications were improved, we are currently undergoing out-patient follow up without morbidity.

Induction of Cardiovascular Anaphylaxis and Basic Pharmacological Analysis of Involved Mediators in Pithed Rats

  • Active cardiovascular anaphylactic response was induced in ovalbumin-sensitized, pithed Sprague-Dawley and Wistar rats. On intravenous administration of the antigen, ovalbumin, marked tachycardia and pressor responses were immediately elicited. Thereafter, a delayed long-lasting severe hypotensive response was observed. These anaphylactic cardiovascular responses were maximal 2-3 weeks after the sensitization, and the response was slightly diminished 6 weeks after sensitization. The immediate pressor response was blocked by a non-selective serotonin antagonist methysergide at a dose-dependent manner, but not by histamine receptor antagonists mepyramine (pyrilamine) or cimetidine. The delayed hypotension was reduced either by histamine $H_1$ receptor antagonist mepyramine or $H_2$ receptor antagonist cimetidine, both in a dose-dependent manner. The tachycardic response was not influenced by serotonin or histamine receptor antagonists examined in this study. Differently from the cardiovascular responses, there was no observable bronchial contraction in Sprague-Dawley rat trachea in contrast to Wistar rat where the trachea contracted to in vitro antigen challenge. The cardiovascular anaphylactic model seems to be useful for studying cardiovascular events that occur exclusively in peripheral heart-blood vessel systems. The involvement of two major anaphylactic mediators, serotonin and histamine, is partially demonstrated.

Characterization of γ-Aminobutyric acid(GABA) produced by a lactic acid bacterium from button mushroom bed

  • ${\gamma}$-Aminobutyric acid(GABA) is a four carbon non-protein amino acid that has several well-known physiological functions, such as a postsynaptic inhibitory neurotransmitter in the brain and induction of hypotensive and tranquilizer effects. A lactic acid bacterium was isolated from button mushroom bed, which is showing high GABA productivity by TLC or HPLC analysis. The strain was identified as Lactobacillus hilgardii by analysis of 16S rDNA gene sequence. When the maximum production of GABA by L. hilgardii was investigated with various concentration of monosodium glutamate, the yield of GABA reached to be 53.65 mM at 1% mono sodium glutamate (MSG) in flask cultivation. A Glutamate decarboxylase (GAD) enzyme, which was known to convert MSG to GABA, was purified from a cell-free extract of L. hilgardii and the molecular weights of purified GAD was estimated to 60,000 by SDS-PAGE. The optimum pH and temperature of GAD were at pH4.6 and at $37^{\circ}C$, respectively. The GAD activity was increased by the addition of sulfate ions such as ammonium sulfate, sodium sulfate and magnesium sulfate, indicating that the increase of hydrophobic interaction causes the increase of GAD activity.
