Airway Remodelling in Asthma (기관지 천식에서의 기도 개형)

  • Asthma is characterized by a chronic inflammatory disorder of the airways that leads to tissue injury and subsequent structural changes collectively called airway remodelling. Characteristic changes of airway remodelling in asthma include goblet cell hyperplasia, deposition of collagens in the basement membrane, increased number and size of microvessels, hypertrophy and hyperplasia of airway smooth muscle, and hypertrophy of submucosal glands. Apart from inflammatory cells, such as eosinophils, activated T cells, mast cells and macrophages, structural tissue cells such as epithelial cells, fibroblasts and smooth muscle cells can also play an important effector role through the release of a variety of mediators, cytokines, chemokines, and growth factors. Through a variety of inflammatory mediators, epithelial and mesenchymal cells cause persistence of the inflammatory infiltrate and induce airway structural remodelling. The end result of chronic airway inflammation and remodelling is an increased thickness of the airway wall, leading to a increased the bronchial hyperresponsiveness and fixed declined lung function.

  • Lymphocytic hypophysitis is a clinically rare disease, and it has been known to be an autoimmune disease which mainly affects pregnant women at the end of gestation or right after delivery. The authors experienced a case of lymphocytic hypophysitis in a 29-year-old pregnant woman with rapid progressing visual disturbance. Sella MRI showed a mass-like lesion of hypophysis and hypertrophy of pituitary stalk with evidences of hypopituitarism. Cesarean section was done and then TSA was performed. The pathologic diagnosis was lymphocytic hypophysitis. After TSA, visual acuity was improved and visual field defect was recovered. She was given thyroid hormone replacement therapy because of transient partial hypopituitarism for 6 months after surgery. One must consider the probability of lymphocytic hypophysitis, if there are alteration of visual acuity and visual field defect which aggravate rapidly during pregnancy due to mass effect, decreased serum hormonal levels shown in hypopituitarism and sella MRI findings of hypertrophy of pituitary stalk and enlargement of pituitary gland.

  • An enlarged peroneal tubercle causes lateral ankle and foot pain, and which is a cause for stenosing peroneal tenosynovitis. In this report, we present a case of stenosing tenosynovitis of the peroneus longus tendon associated with hypertrophy of the peroneal tubercle without involvement of the peroneus brevis tendon. Surgical excision of the enlarged peroneal tubercle along with exploration of the peroneal tendons was successful.

  • RNA polymerase II carboxyl-terminal domain (pol II CTD) phosphatases are a newly emerging family of phosphatases that are members of DXDX (T/V). The subfamily includes Small CTD phosphatases, like SCP1, SCP2, SCP3, TIMM50, HSPC129 and UBLCP. Extensive study of SCP1 has elicited the diversified roles of the small C terminal domain phosphatase. The SCP1 plays a vital role in various biological activities, like neuronal gene silencing and preferential Ser5 dephosphorylation, acts as a cardiac hypertrophy inducer with the help of its intronic miRNAs, and has shown a key role in cell cycle regulation. This short review offers an explanation of the mechanism of action of small CTD phosphatases, in different biological activities and metabolic processes.

A Report of Left Heart Failure by Aortic Valvular Stenosis in a Mongrel Dog (개에서 대동맥판막협착에 의한 좌심부전 증례)

  • A 4 year old mixed female dog was presented dyspnea, anorexia and syncope. In blood smear test, a lot of heartworm larvae were observed. In thoracic radiography, "reverse D" shape was observed. So, this case was considered with heartworm disease. In echocardiography, marked left heart hypertrophy was presented. In M-mode view, aortic valvular stenosis was found. After mongrel dog died, aortic valvular stenosis was reconfirmed in necropsy. After all, the dog died of pulmonary edema because of aortic valvular stenosis caused left heart failure.t failure.

  • The prevalence of canine heartworm(Dirofilaria immitis) infection in 150 mixed-breed dogs(male : 54, female : 96) from February to December 1997 was investigated by using antigen test kit(ICT $GOLD^{TM}$ HW, Synbiotics, USA) based on immunochromatographic assay in Inchon city. Also, gross and histopathological findings of an antigen positive dog were carried out. The results were summarized as follows ; 1. Four dogs were positive from 150 tested dogs(2.7%). They were all more than 2 years old and infection rates in male and outdoor dogs was higher than those in female and indoor, respectively. Species of infected dogs were Pug(2) , German Sheperd(1) and Great-dane (1). 2. Regional infection rates were closely related with housing system in the city. 3. Pathological findings of antigen-positive dog was excessive enlargement, congestion and hemorrhage of lung and D immitis in heart and histologically hemosiderin, hypertrophy of pulmonary alveoli wall and irregular hypertrophy of pulmonary artery inner wall. Microfilaria was observed in pulmonary artery and arteriole, ventricle and splenic artery.

Studies on the Effects of Evodiae Fructus on the Cardiovascular System in N-nitro-L-arginine Methyl Ester-induced Hypertensive Wistar Rats (오수유가 N-nitro-L-arginine methyl ester로 유발한 고혈압흰쥐의 심혈관계에 미치는 영향)

  • The present study examined the effect of a methanol extract of Evodiae Fructus on the cardiovascular function in N-nitro-L-arginine methyl ester (NAME)-induced hypertensive Wistar rats after treatment over 6 weeks. In rats treated with NAME, blood pressure, weight of heart, aorta media thickness and media/lumen ratio significantly (p<0.05) increased, whereas coronary flow and heart rate of isolated heart significantly (p<0.05) decreased compared with control group at 6 weeks. In rats treated with NAME and Evodiae Fructus, blood pressure, aorta media/lumen ratio significantly(p<0.05) decreased compared with NAME treated group at 6 weeks. These results suggest that Evodiae Fructus is applicable to the treatment of hypertension and vascular hypertrophy.

Ginseng Total Saponin Attenuate Cardiac Hypertrophy Induced by Homocysteine in Rats

  • Recent studies have shown that Panax ginseng has a variety of beneficial effects on the cardiovascular system. Homocysteine (Hcy), which is derived from methionine, has been closely associated with the increased risk of cardiovascular diseases. In the present study, whether the in-vivo long-term co-administration of ginseng total saponins (GTS), active ingredients of Panax ginseng, with L-methionine (Met) inhibits methionine-induced hyperhomocysteine (HHcy) and H-Hcy-induced cardiovascular dysfunctions was investigated, and it was found that the plasma Hcy level, which was measured after 30 and 60 days, in the GTS+Met co-administration group was more significantly reduced than in the Metalone-treatment group. The left-ventricle (LV) wall thickness of the heart was likewise examined in each treatment group, and it was found that the co-administration of GTS with Met significantly reduced the Met-induced LV wall thickness. The results of the study indicate that the in-vivo long-term co-administration of GTS with Met not only inhibited H-Hcy induced by long-term Met-alone administration but also attenuated the H-Hcy-induced cardiovascular dysfunctions in rats.

  • Authors have obserbed a rare case of unilateral hypertrophy occured in the left mandibular condyle of 25 years old man. In the serial roentenograms, Authors have drawn following conclusions. 1. The patients face is markedly asymmetrical. This asymmetry consisted of an elongation and widening of the left mandibular ramus, neck and head of condyle which pushed the chin to the other side. 2. Left mandibular angle is flattened and mandibular inferior border is lower than right. 3. In the relationship of the left posterior teeth, severe mesioocclusion is occured.

Effects of providing procedural information to patients undergoing prostate biopsy on anxiety, depression and sleep quality (전립선 조직검사 사전정보교육이 전립선비대증 환자의 불안, 우울 및 수면의 질에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of providing prostatic hypertrophy patients with information about an upcoming prostate biopsy procedure on their anxiety, depression, and sleep quality. Methods: Sixty-two participants were divided equally into an experimental and control group. Experiments were conducted from July 31, 2015, to March 30, 2016. After providing information, we evaluated anxiety, depression, and sleep quality using structured questionnaires. Data were analyzed using chi-square tests, Fisher's exact tests, t-tests, and ANCOVA using SPSS. Results: The experimental group demonstrated significantly lower levels of anxiety and depression than the control group. The experimental group also demonstrated significantly higher sleep quality. Conclusion: Information on an upcoming prostate biopsy improved psychological outcomes in patients with prostatic hypertrophy. This education should be incorporated into nursing practice.
