An Analysis of Comparison between Autogenes Training and Imaginary Meditation by Heart Rate Variability and SCL-90-R (심박변이도 및 SCL-90-R 측정을 통한 자율훈련법과 과일 명상의 효과 비교 분석)

  • Objective: There are many meditation methods used in clinical field. But the effect of meditation may be different to people's mind, and meditation itself. Now we investigate this subject with two different meditation methods which has been analyzed by heart rate variability. Method: We tried to check people's heart rate who educated meditation, before educated and during the meditation. The meditation was educated differently for each group. One is the auto genes traning, and the other is the imaginary meditation by thinking about eating and smelling favorite fruits. All of them are used to oriental psychiatry clinic patients. And we simultaneously conduct people's emotional state by SCL-90-R. Result: The result in Autogene traning group has significant difference in total power and HRV-index score between before and during the training but in imaginary meditation group has no significance: Autogene traning group has a significance in low frequency score and has increased tendency with increasing tendency of Heart rate index and Total power. Through the SCL-90-R score, the higher depression t-score, the anxiety t-score, and the hostility score is, the HRV scores such as total power, low frequency, and HRV-index much increase. and so on. Conclusion: We can guess that the Autogenes training has more significant effect than the imaginary meditation. And we can also guess that as the emotional problem is getting more severe, the effect of meditation would be much better. And we finally suggest that we should make a standard guideline of imaginary meditation, cause the imaginary meditation has so many variation.

Development and Effectiveness of a Profanity Intervention Application (여자 중학생 대상 욕설 중재 앱 프로그램 개발 및 효과)

  • Purpose: The purpose of this study was to investigate the effects of a profanity intervention application on interpersonal relationship ability, aggression, and empathic ability among female middle school students. Method: The subjects of the present study were freshmen students at a girls' middle school in Gyeonggi-do. The profanity intervention application was implemented from November 20th to 27th, 2017. The pre- and post-tests were conducted one week before and two weeks after the intervention, respectively. The study was designed as a randomized controlled study and the collected data were analyzed using SPSS/WIN 23.0 (Chicago, IL). A homogeneity test between the two groups was performed using $x^2$ test, Fisher's exact test, independent t-test, and ANCOVA. Results: The two groups were found to be homogeneous in the pre-test. After the implementation of the profanity intervention application, aggression significantly decreased (F=20.17, p<.001) and interpersonal reactivity (F=5.92, p=.023) and empathy (F=11.10, p=.003) significantly increased in the experimental group compared to the control group. Among the factors related with aggression, verbal aggression, anger, and hostility significantly decreased in the experimental group. Meanwhile, among the factors of interpersonal reactivity, empathic concern and perspective-taking significantly increased and, among the factors of empathy, cognitive empathy significantly increased in the experimental group compared to the control group. Conclusion: Based on the findings of the present study, various intervention programs that can consistently improve the moral and ethical sensitivity of students should be presented starting from elementary school to prevent verbal violence in adolescents.

Perceived Family Support and Mental Health of Middle school students (중학생이 지각한 가족지지와 정신건강)

  • This study was conducted to identify the relationship between the perceived family support and mental health of middle school students and provide basic data to develop a nursing program for prevention of mental disease and mental health promotion in adolescent. The subjects for this study were 427 students in their first, second and third year of two middle school in K city. The data were collected from December 12th to 19th, 2001. The instruments used for this study were the family support scale by Kang and the mental health assessment scale was the Symptom Check List-90-R. The data were analyzed by frequency, percentage, mean, standard deviation, Pearson Correlation Coefficient, ANOVA, and Duncan test with SPSS program. The results were as follows: The mean score of the perceived family support was 42.26 and the mean score of mental health was 52.24. As a subarea of mental health was analyses, the scores of obsessive-compulsive reaction and hostility were respectively higher but the scores of phobia were lower than others. Family support was significantly different according to student's grade, religion and economic status, and educational level, the relationship between them, and living in parents. the characteristic influence on the level of mental health were sex, grade, and economic status of students and the relationship between them of parents. There was a significant correlation between the perceived family support and the level of mental health was revealed a significant correlation(r=-.35(P=0.001)). In conclusion, this study was found that family support was an important factor for promoting mental health of middle school students. In order to increase mental health of middle school students effectively, family support must be increased. Efforts to promote mental health of students are required in home and school. School authorities have to search a plans like "family support-class program" as one countermeasure to increase family support. teachers have to make educational programs to promote mental health constantly with participating parents.

The Effect of Seizure-Related Variables on Interictal Psychiatric Symptoms in the Intractable Epileptic Patients (난치성 간질환자에서 간질관련요인이 발작간 정신증상에 미치는 영향)

  • The aim of this study is to find the relationship between interictal psychiatric symptoms and seizure-related variables such as structural changes, regional interictal perfusion changes, the number of interictal epileptic discharges and the presence of accompanying generalized tonic clonic seizure(GTC). The subjects were 75 patients (47 males : mean age $28.3{\pm}7.7$) with intractable epilepsy, Interictal psychiatric symptoms were rated by Symptom Checklist-90-R(SCL-90-R). Each of 4 seizure variables was measured by MRI, $^{99m}TC-HMPAO$ SPECT and the prolonged EEG monitoring. The mean SCL-90-R subscale T-scores and the ratio of the patients with definite psychiatric symptoms(T-score > 65 at any one subscale of SCL-90-R) were compared among the groups with different seizure variables. Demographic variables and clinical variables were not statistically different among the groups with different seizure variables. The patients with right mesial temporal sclerosis had higher mean SCL-90-R scores for obsessive compulsive symptom, interpersonal sensitivity, depression, hostility and psychotic symptoms than the patients with left mesial temporal sclerosis, and they also showed higher ratio of definite psychiatric symptoms. The presence of interictal epileptic discharges was related with higher T-scores of paranoid subscale. The areas of hypoperfusion, the presence of the GTC, and other clinical seizure variables showed no significant influences on the mean SCL-90-R subscale T-scores. from these results, the authors speculated that interictal subictal epileptic dischrages may be related with psychiatric symptoms of the intractable epileptic patients, especially with right mesial temporal sclerosis.

  • The research focus on participation and competence in sports and their difference in mental health. Moreover, it focus on how the sports participation and spots competence in health affect their mental health. For the research, 448 high school students were collected and the one-way ANOVA and the multiple regression analysis was carried on. As a result, first of all, according to the participation and competence in sports and their mental health, the sophomores and juniors were stronger than seniors and in case of depression which is in the lowest of the mental scale, the juniors showed the highest. Secondly, sports participation did not affect the mental health of students but sports competence show positive effects on all the lowest scale in the mental health such as insecurity, depression and hostility.

Perioperative Comprehensive Supportive Care Interventions for Chinese Patients with Esophageal Carcinoma: a Prospective Study

  • Objective: To assess the effects of perioperative comprehensive supportive care interventions on outcome of Chinese esophageal cancer patients in a prospective study. Methods: 60 patients with primary esophageal carcinoma were randomized into an intervention group (IG, n=31) and a control group (CG, n=29). The Chinese version of symptom checklist-90 (SCL-90) was adopted to assess their psychological status. The interventions, including health education, psychological support, stress management, coping strategies and behavior training, were carried out in 3 phases (preoperative, postoperative I and postoperative II), and psychological effects were thereafter evaluated accordingly before surgery, and 1 week, 4 weeks and 24 weeks post-surgery. Medical costs were estimated at discharge. Survival of patients was estimated each year post-surgery. General health status and satisfaction-with-hospital were surveyed by a follow-up questionnaire 4 years post-surgery. Results: All the subjects demonstrated higher scores in the preoperative phase than the normal range of Chinese population concerning 7 psychological domains including somatization, obsessive-compulsive, depression, anxiety, hostility, phobic anxiety and paranoid ideation. Although no significant difference was observed between the two groups at admission, the scores of IG, which tended to decrease at a faster rate, were generally lower than those of CG at weeks 1, 4 and 24 post-surgery. The length of hospital stay and medical costs of IG were significantly less than those of CG and satisfaction-with-hospital was better. However, there was no significant difference in 4-year survival or health status between two groups. Conclusions: Appropriate perioperative comprehensive supportive care interventions help to improve the psychological state of Chinese patients with esophageal carcinoma, to reduce health care costs and to promote satisfaction of patients and their families with hospital.

The Relationship of Stress Perception with Serum Cholesterol in Patients with Essential Hypertension (본태성 고혈압 환자들에서 스트레스지각과 혈청 Cholesterol 간의 관계)

  • This study was aimed at investigating the relationship of stress perception and psychopathology with biological variables such as systolic and diastolic blood pressure. serum total cholesterol high density lipoprotein and triglyceride in 50 adult patients with essential hypertension and 50 normotensive individuals. Both of the groups were matched to each other regarding age, sex, the level of education and BMI(body mass index). Stress perception and psychopathology between the two groups were compared using GARS and SCL-90-R scales. Biological variables such as blood pressure(systolic. diastolic), serum total cholesterol, high density lipoprotein and triglyceride were measured in all the subjects. The results were as follows ; 1) Hypertensive patients scored significantly higher than normotensive individuals in stress perception related to unusual happenings, change or no change in routine and overall global area, respectively. 2) Regarding psychopathology, hypertensive patients scored significantly higher than normotensive individuals in depression. hostility and interpersonal sensitivity. respectively. 3) Systolic blood pressure had a significantly positive correlation with scores of stress perception related to sickness and injury, and change or no change in routine, respectively. Diastolic blood pressure had a significantly positive correlation with scores of stress perception related to change or no change in routine. 4) In hypertensive patients. the level of serum total cholesterol had a significantly positive correlation with scores of stress perception related to unusual happenings. However, the level of serum triglyceride had a significantly negative correlation with scores of stress perception related to changes in relationships, sickness or injury, and change or no chang in routine. 5) There were no significant correlations between scores of stress perception and biological factors in the normotensive group. 6) The level of serum triglyceride correlated negatively with phobic anxiety in hypertensive group. whereas it correlated positively with PSDI in normotensive group. In conclusion, these results suggest that hypertensive patients have higher level of stress perception and psychopathology than normal individuals. In hypertensive patients, their stress perception is suggested to be correlated with serum cholesterol.

The Relationship between Epileptic Focus and Psychiatric Symptoms of the Refractory Epileptic Patients (난치성 간질환자의 간질초점 위치와 정신증상과의 관련성)

  • The prevalence rate of psychiatric symptoms of the refractory epileptic patients was evaluated according to the location of the epileptic focus. The subjects were 91 patients admitted to Epilepsy Monitoring Unit of Seoul National University Hospital. The psychiatric symptoms were assessed by Korean version of Symptom Checklist-90-R(SCL-90-R). The locus of epileptic focus was assessed by clinical features, 2-hour interictal EEG, long-term video-EEG monitoring, brain MRI, interictal and ictal brain SPECT, and interictal brain PET The subjects were divided into three groups according to the epileptic focus, non-temporal(N=29), left temporal (N=26), and right temporal(N=32). There were no statistical differences in demographic and seizure-related variables among groups. The number of patients with $T-score {\geq} 65$ at any subscale of the SCL-90-R were compared by $X^2-test$ among groups. The mean T-scores of each subscale of the SCL-90-R were compared by oneway-ANOVA among groups. The prevalence rate of psychiatric symptoms of the refractory epileptic patients was 38.5%. There was no statistical difference in the prevalence rate of psychiatric symptoms among groups. However, the patients with non-temporal or right temporal epileptic foci showed statistically significant higher mean T-scores of interpersonal sensitivity, depression, hostility, and phobic subscales than the patients with left temporal epileptic foci. These results suggest that the epileptic focus plays an important role in the production of interictal psychiatric symptoms of the refractory epileptics.

Relationships between Parenting Behavior, Parenting Efficacy, Adaptation Stress and Post Traumatic Stress Disorder among Mothers who Defected from North Korean (북한 이탈 어머니들의 자녀양육행동과 양육효능감, 적응스트레스 및 외상 후 스트레스 증상)

  • Purpose: The purpose of this study was to investigate the relationships between parenting behavior, parenting efficacy, adaptation stress and PTSD (Post Traumatic Stress Disorder) among mothers who have defected from North Korean. Methods: A descriptive study was conducted to illustrate the parenting behavior, parenting efficacy, adaptation stress and PTSD and report the relationships among them. Data were collected between August and November of 2009 in Hanawon and 105 mothers who met eligibility criteria participated in the study. Results: Ignorant/controlling parenting behavior of parenting showed the highest score in our study. The level of parenting efficacy was $2.72{\pm}0.36$ and adaptation stress level was $3.35{\pm}0.47$ showing perceived hostility the highest and homesickness the lowest. Participants with moderate to severe PTSD accounted for 79.5% of the mothers, and 75% of them reported symptoms for 3 months or more. Authoritative parenting behavior showed negative relationships with parenting efficacy, cultural crisis and fear, adaptation stress and PTSD. Conclusion: Mothers who have defected from showed negative parenting behaviors and high adaptation stress level and PTSD while parenting efficacy was high. These findings indicate that parenting intervention programs which can guide these mothers towards positive parenting behaviors need to be developed.

Comparison between Major Depressive Disorder & Hwabyung (Culture Bound Syndrome) using the SCL-90-R (SCL-90-R을 이용한 화병과 주요우울장애의 비교)

  • Hwabyung is a Korean culture bound syndrome and translated as anger syndrome at DSM-IV. And it is caused by the suppression of anger and has characteristics of chest discomfort, burning up as physical symptoms, and mortification, chagrin as psychological symptoms. It is very common in the field of psychiatry in Korea with Major Depressive Disorder (MDD). * Objective : 95 patients with MDD or Hwabyung, who visited the department of psychiatry in Korea University Medical Center or oriental medical hospital of Kyunghee Medical Center from May 2003 to August 2003, were selected for the study to compare between Major Depressive Disorder and Hwabyung. * Methods : The Structured Clinical Interview Schedule for DSM-IV (SCID) and Hwa-Byung Diagnostic Interview Schedule (HBDIS) were conducted for diagnosis. And Symptom Check List-90-Revised (SCL-90-R) was carried out for comparison between MDD and Hwabyung. * Result : Among the 95 patients, MDD group was 23 persons, Hwabyung group was 36 persons, and MDD plus Hwabyung group was 36 persons and it occupied 37.9%. MDD plus Hwabyung group had higher scores than any other groups in the whole dimensions of SCL-90-R. Hwabyung group had higher scores than MDD group in somatization, anxiety, hostility, Global Severity Index(GSI) and positive symptom total (p<.05) of SCL-90-R, but two groups had no difference in obsessive-compulsive, interpersonal-sensitivity, depression, phobic anxiety, paranoid ideation, psychoticism, positive symptom distress index of SCL-90-R. * Conclusion : These result showed that Hwabyung is similar to MDD at the point of depression and psychoticism but more severe at somatization, anxiety and anger than MDD.
