A Study on the Introduction of Natural Elements in Public places of Nursing Homes - Focusing on ensuring the amenity of mediation, interaction, and rest places - (노인요양시설 공용공간의 자연요소 도입에 관한 연구 - 매개, 교류, 휴식공간의 쾌적성 확보를 중심으로 -)

  • As an aging society accelerates its pace for the present, providing a pleasant environment to nursing homes is such an environmental requirement with which elderly people could improve their quality of life and even acquire the effects of healing. As an optimal element of ensuring amenity, nature is a general hospital that heals human beings physically and mentally. In this vein, the investigator drew out the frame of analysis, namely emotional/healing/relating amenity, by means of theoretical examination of such natural elements as water, light, soil/stone, and plant. And on the basis of the drawn amenity, the investigator examined the application of natural elements to study subjects in order to analyze the ensured amenity. Study findings show that the existing nursing homes have ensured healing amenity sufficiently, but they are unsatisfactory in ensuring emotional and relating amenity. Based on interview survey, the investigator suggests that the introduction of natural elements into nursing homes such public space as mediation space, interaction space, and rest places to ensure emotional/healing/relating amenity must be ideally equipped with the formation of outdoor forest in which water, light, soil/stone, and plant get joined together, rather than Introducing a single element respectively. In addition, the plan of construction of nursing homes must take residing elderly people's emotional/healing/relating amenity into account on the basis of physical amenity.

A Case of Creation of Mediastinal Tracheostoma with Unilateral Manubrial Resection as Salvage Operation (구제수술에서 일측 복장뼈자루 절제를 통한 전종격동 기관절개술의 1례)

  • A 65-year-old male visited hospital in the state of tracheostomal recurrence 1 year after total pharyngolaryngectomy. Extensive recurrence around stoma or paratracheal, superior mediastinal area is challenging in treatment, especially in securing airway. We performed mediastinal tracheotomy through resection of unilateral manubrium, some part of sternal side of clavicle and $1^{st}$ rib as the salvage operation for recurrent laryngeal cancer. This procedure could be risky, thus very careful patient selection is required because of a tortuous postoperative course. We would like to present the case that anterior mediastinal tracheostomy could be needed as appropriate palliative means of airway construction in the patients with recurrent laryngeal cancer with lower neck extension with literature review.

  • 본 논문은 R-CDM 의료영상정보를 기반으로 ELK Stack 기술을 적용하여 획득한 데이터의 분석 결과를 시각화하기 위한 시스템에 대해 기술한다. 제안한 시스템은 의료 빅데이터의 검색, 수집 그리고 분석 결과를 모니터링 할 수 있으며, 특히 대량의 데이터의 변화와 데이터간의 차이를 확인할 수 있다. 본 연구에서 제안한 시스템은 수집된 의료영상 빅데이터에 대해 적용하여 현황과 처리결과 그리고 실시간 분석결과에 대한 모니터링을 통해 관리의 효율성을 높여 실시간 검색 및 분석 서비스 분야에 기여할 것으로 기대된다.

  • 최근 의료서비스에 ICT 기술이 도입되면서 의료서비스 패러다임이 병원중심에서 환자 중심으로 변화되고 있다. 특히, 사물인터넷 기술은 스마트헬스케어 서비스를 현실화하고 있다. 이로인하여 병원이 아닌 곳에서도 환자의 상태를 확인할 수 있고 이에 대한 적절한 조치를 취할 수 있는 다양한 서비스를 개발 연구가 활발히 진행되고 있다. 그러나 의료 사각지대의 환자는 여전히 신속하게 조치를 못하는 실정이며, 관리가 되고 있지 않아 사회적인 문제로 이슈화되고 있다. 본 논문은 의료사각지대의 환자에 대한 의료영상 진단 지원을 위한 클라우드환경 기반의 Connected Radiology Care System을 제안하고자 한다.

A Study on the Estimation of Temperature, Humidity and Cooling Load (온도, 습도 및 냉방부하 예측에 관한 연구)

  • The peak demand of electricity in summer season mainly comes from the day time cooling loads. Ice thermal Storage System (ITSS) uses off-peak electricity at night time to make ice for the day time cooling. In order to maximize the use of cold storage in ITSS, the estimation of day time cooling load for the building is necessary. In this study, we present a method of cooling load estimation using 5 years of normalized outdoor temperature, relative humidity, and the building construction data. We applied the hourly-based estimation to a general hospital building with relatively less sudden heat exchange and the results are compared with the measured cooling load of the building. The results show that the cooling loads estimation depends on the indoor cooling design temperature of the building.

Development and Validation of an Instrument to Measure Nursing Information Literacy Competency

  • Purpose: The purpose of this study was to develop an instrument for measuring nursing information literacy competency, and then to examine the validity and reliability of the instrument. Methods: The developmental process of the instrument includes construction of a conceptual framework, generation of initial items, verification of content validity, preliminary study, extraction of final items, and psychometric testing. Its content validity was verified by three experts from nursing and nursing informatics. Its construct, convergent, and discriminant validity was examined in confirmatory factor analysis. Finally, its criterion validity was measured with Pearson's correlation. The tool's reliability was examined by Cronbach's ${\alpha}$. The participants include 382 nurses from four hospitals and one university hospital. Results: Twenty seven items in total were selected for the final scale, and the results of the confirmatory factor analysis were supported with acceptable model fit, which were named competency for identifying problem, potential sources for information, searching fine information, evaluating information, acquising and managing of information, using information ethically, and integrating new information. The convergent, discriminant and criterion validities were also supported. The Cronbach's ${\alpha}$ coefficient was .93. Conclusion: The instrument is valid and reliable to comprehensively assess nurses' information literacy competency, and to provide a basic direction for developing nursing information literacy program.

BIM based Construction Project Case Analysis for Facility Life Cycle Management from the Perspective of the Client (발주처 관점의 시설물 생애주기 관리를 위한 BIM 기반 건설 프로젝트 사례 분석)

  • From the perspective of the client, the value of information for use in facility management is very low at the point of use. This is because the facility owner, who is a non-professional, cannot accurately request necessary information from the perspective of long-term facility life cycle management. The purpose of this study is to define the information necessary for facility management and to analyze the project operation method to procur this information to the client using BIM. To this end, this study analyzed the role of client and contractor, project delivery process, standards, and submissions in the BIM-based procurement area through USACE Air Force Hospital project case analysis. As a result of the analysis, if the client, who is the subject of facility operation, defines in advance the type and input method of necessary information in the operation stage, the client can receive the BIM optimized for operation from the contractor. The results of this study can be used to establish standards for receiving information necessary for establishment of procurement process and facility management of public institutions considering BIM introduction.

Abusive Supervision and Work Engagement: The Mediating Effects of Psychological Contract Breach (상사의 비인격적 감독이 부하직원의 직무 열의에 미치는 영향과 심리적 계약 위반의 매개 효과)

  • Purpose - This study seeks to deepen our understanding of abusive supervision and its negative consequences on subordinates. Specifically, it examines the underlying psychological mechanism between abusive supervision and subordinates' work engagement, in particular with its focus on the mediational role of psychological contract breach. Design/methodology/approach - Following the two rounds of pilot interviews, this study collected and analyzed 182 survey data from one construction company and one hospital - the sectors in which abusive supervision is known to be frequent, with one-month interval to avoid common methods variances. Findings - We have two key findings. First, abusive supervision is negatively related to subordinates' work engagement. Second, the psychological contract violation mediates the relationship between abusive supervision and subordinates' work engagement. Research implications or Originality - This study enriches extant research on abusive supervision by deepening our understanding of the underlying mechanism in which abusive supervision causes negative outcomes. On a practical note, companies need further efforts to address abusive supervision, for example, by introducing sophisticated HR practices, including accessible grievance procedures and more serious disciplinary actions, and/or by offering leadership educations to supervisors.

  • Due to the development of technology and medical care following the 4th industrial revolution, the medical paradigm is shifting towards patient-centered medical services. Based on the development of smart home technology, the residential environment is changing into a residential space that cares for and heals the lifestyles and the healthcare of families. As lifestyle changes, the concept of supporting smart home care based on the residential environment is making it possible to build a smart home IoT service design with enhanced accessibility and convenience for medical appointments and well-being lifestyle care. This paper is a study on user-centered health care smart home IoT service design suitable for family members based on the health care, beauty care, exercise care, and customized diet care beyond the conventional concept of health care monitoring. Based on the analysis, this paper proposes a personal care coordinate smart home service design in a human-centered wellness clinic care smart home service design environment. Human-centered wellness clinic smart home IoT service design is meaningful in presenting a vision for research on smart home service design that links hospital-linked and care-linked service industries, which should be considered from the smart home construction planning stage.

Discretized solenoid design of a 1.5 T and a 3.0 T REBCO whole-body MRI magnets with cost comparison according to magnetic flux

  • Rare earth barium copper oxide (REBCO) materials have shown the possibility of high-temperature superconductor (HTS) magnetic resonance imaging (MRI) magnets due to their elevated transition temperature. While numerous MRI magnet designs have emerged, there is a growing emphasis on estimating the cost before manufacturing. In this paper, we propose two designs of REBCO whole-body MRI magnets: (1) 1.5 T and (2) 3.0 T, the standard center field choices for hospital use, and compare their costs based on conductor usage. The basis topology of the design method is based on discretized solenoids to enhance field homogeneity. Magnetic stress calculation is done to further prove the mechanical feasibility of their construction. Multi-width winding technique and outer notch structure are used to improve critical current characteristic. We apply consistent constraints for current margins, sizes, and field homogeneities to ensure an equal cost comparison. A graph is plotted to show the cost increase with magnetic flux growth. Additionally, we compare our designs to two additional MRI magnet designs from other publications with respect to the cost and magnetic flux, and present the linear relationship between them.
