A successful management after preterm delivery in a patient with severe sepsis during third-trimester pregnancy

  • A 33-year-old woman visited the emergency department presenting with fever and dyspnea. She was pregnant with gestational age of 31 weeks and 6 days. She had dysuria for 7 days, and fever and dyspnea for 1 day. The vital signs were as follows: blood pressure 110/70 mmHg, heart rate 118 beats/minute, respiratory rate 28/minute, body temperature $38.7^{\circ}C$, and oxygen saturation by pulse oximetry 84% during inhalation of 5 liters of oxygen by nasal prongs. Crackles were heard over both lung fields. There were no signs of uterine contractions. Chest X-ray and chest computed tomography scan showed multiple consolidations and air bronchograms in both lungs. According to urinalysis, there was pyuria and microscopic hematuria. She was diagnosed with community-acquired pneumonia and urinary tract infection (UTI) that progressed to severe sepsis and acute respiratory failure. We found extended-spectrum beta-lactamase producing Escherichia coli in the blood culture and methicillin-resistant Staphylococcus aureus in the sputum culture. The patient was transferred to the intensive care unit with administration of antibiotics and supplementation of high-flow oxygen. On hospital day 2, hypoxemia was aggravated. She underwent endotracheal intubation and mechanical ventilation. After 3 hours, fetal distress was suspected. Under 100% fraction of inspired oxygen, her oxygen partial pressure was 87 mmHg in the arterial blood. She developed acute kidney injury and thrombocytopenia. We diagnosed her with multi-organ failure due to severe sepsis. After an emergent cesarean section, pneumonia, UTI, and other organ failures gradually recovered. The patient and baby were discharged soon thereafter.

Development of MgH2-Ni Hydrogen Storage Alloy Requiring No Activation Process via Reactive Mechanical Grinding

  • $MgH_2$ was employed as a starting material instead of Mg in this work. A sample with a composition of 94 wt% $MgH_2-6$ wt% Ni (called $MgH_2-6Ni$) was prepared by reactive mechanical grinding. The hydriding and dehydriding properties were then examined. An $MgH_2-Ni$ hydrogen storage alloy that does not require an activation process was developed. The alloy was prepared in a planetary ball mill by grinding for 4 h at a ball disc revolution speed of 250 rpm under a hydrogen pressure of about 12 bar. The sample absorbed 3.74 wt% H for 5 min, 4.07 wt% H for 10 min, and 4.41 wt% H for 60 min at 573 K under 12 bar $H_2$, and desorbed 0.93 wt% H for 10 min, 1.99 wt% H for 30 min, and 3.16 wt% H for 60 min at 573 K under 1.0 bar $H_2$. $MgH_2-6Ni$ after reactive mechanical grinding contained ${\beta}-MgH_2$ (a room temperature form of $MgH_2$), Ni, ${\gamma}-MgH_2$ (a high pressure form of $MgH_2$), and a very small amount of MgO. Reactive mechanical grinding of Mg with Ni is considered to facilitate nucleation, and to reduce the particle size of Mg. $Mg_2Ni$ formed during reactive mechanical grinding also increases the hydriding and dehydriding rates of the sample.

  • Cryogenic propellant rockets utilize a natural circulation loop of cryogenic fluid to cool the engine inlet temperature before launch. The geometric information about the circulation system, such as length and diameter of the pipes and the heat input to the system, defines the mass flow rate of the natural circulation loop. We performed experiments to verify the natural circulation mass flow rate and compared the results with the analytical results. The comparison of the mass flow rate between experiments and numerical simulations showed a 12% offset. We also included a prediction of the natural circulation flow rate in the low-gravity section and in the acceleration section in the upper stage of the launch vehicle. The oxygen tank should have 100 kPa(a) of pressure in the acceleration section to maintain a high flow rate for the natural circulation loop. In the low-gravity section, there should be an optimal tank pressure that leads to the maximum natural circulation flow rate.

Thermodynamic Calculation and Observation of Microstructural Change in Ni-Mo-Cr High Strength Low Alloy RPV Steels with Alloying Elements (압력용기용 Ni-Mo-Cr계 고강도 저합금강의 합금원소 함량 변화에 따른 미세조직학적 특성변화의 열역학 계산 및 평가)

  • An effective way of increasing the strength and fracture toughness of reactor pressure vessel steels is to change the material specification from that of Mn-Mo-Ni low alloy steel(SA508 Gr.3) to Ni-Mo-Cr low alloy steel(SA508 Gr.4N). In this study, we evaluate the effects of alloying elements on the microstructural characteristics of Ni-Mo-Cr low alloy steel. The changes in the stable phase of the SA508 Gr.4N low alloy steel with alloying elements were evaluated by means of a thermodynamic calculation conducted with the software ThermoCalc. The changes were then compared with the observed microstructural results. The calculation of Ni-Mo-Cr low alloy steels confirms that the ferrite formation temperature decreases as the Ni content increases because of the austenite stabilization effect. Consequently, in the microscopic observation, the lath martensitic structure becomes finer as the Ni content increases. However, Ni does not affect the carbide phases such as $M_{23}C_6 $ and $M_7C_3$. When the Cr content decreases, the carbide phases become unstable and carbide coarsening can be observed. With an increase in the Mo content, the $M_2C$ phase becomes stable instead of the $M_7C_3$ phase. This behavior is also observed in TEM. From the calculation results and the observation results of the microstructure, the thermodynamic calculation can be used to predict the precipitation behavior.

Fabrication and Mechanical Properties of WC-Mo2C-Co Hard Materials by the Pulsed Current Activated Sintering Method (펄스 전류 활성 소결법을 이용한 WC-Mo2C-Co 소결체 제조 및 기계적 특성 평가)

  • The pulsed current activated sintering method (PCAS) is a new rapid sintering method that was developed recently for fabricating ceramics and composites. This method combines a high temperature for a short time with pressure application. In this work, PCAS was used to fabricate $WC-5wt%Mo_2C-5wt%$ Co hard material using WC, $Mo_2C$, and Co. The $WC-Mo_2C-Co$ was almost completely dense with a relative density of up to 100% after the simultaneous application of a pressure of 60 MPa and electric current for 11 min without grain growth. The average grain size of WC that was produced through PCAS was about $0.5-0.6{\mu}m$. The vickers hardness and fracture toughness of the $WC-5wt%Mo_2C-5wt%$Co hard materials were about $2453.5kg/mm^2$ and $7.9MPa{\cdot}m^{1/2}$, respectively, for 60 MPa at $11200^{\circ}C$.

Investigation of Optimum Condition of Heat Treatment and Flow to Improve H2S Adsorption Capacity for Practical use of an Activated Carbon Tower (활성탄 흡착탑의 실용화를 위한 최적 유동특성 선정 및 열처리 조건에 따른 황화수소 포집능 향상 연구)

  • This study was conducted to improve the operating conditions of an adsorption tower filled with potassium impregnated activated carbon for high hydrogen sulfide capture capacity. Heat treatment modified the surface properties of activated carbon, and ultimately determined its adsorption capacity. The activated carbon doped with potassium showed 57 times more adsorption at room temperature than that of using the raw adsorbent. It is believed that uniform pore formation and strong bonding of the potassium on the surface of carbon contributed to the chemical and physical absorption of hydrogen sulfide. The SEM analysis on the surface structure of various commercial carbons showed that the modification of surface properties through the heat treatment generated the destruction of pore structures resulted in the decrease of the absorption performance. The pressure drop across the activated carbon bed was closely related with the grain size and shape. The optimum size of irregularly shaped activated carbon granules was 2~4 mesh indicating economical feasibility.

  • H2 generation from renewable sources is crucial for ensuring sustainable production of energy. One approach to achieve this goal is biohydrogen production by utilizing renewable resources such as biomass and microorganisms. In contrast to commercial methods, biohydrogen production needs ambient temperature and pressure, thereby requiring less energy and cost. Biohydrogen production can reduce greenhouse gas emissions, particularly the emission of carbon dioxide (CO2). However, it is also associated with significant challenges, including low hydrogen yields, hydrodynamic issues in bioreactors, and the need for H2 separation and purification methods to obtain high-purity H2. Various technologies have been developed for hydrogen separation and purification, including cryogenic distillation, pressure-swing adsorption, absorption, and membrane technology. This review addresses important experimental developments in dense polymeric membranes for biohydrogen purification.

Fabrication of Bi2Te2.5Se0.5 by Combining Oxide-reduction and Compressive-forming Process and Its Thermoelectric Properties (산화물환원과 압축성형 공정에 의한 Bi2Te2.5Se0.5 화합물의 제조와 열전특성)

  • We report the effect of plastic deformation on the thermoelectric properties of n-type Bi2Te2.5Se0.5 compounds. N-type Bi2Te2.5Se0.5 powders are synthesized by an oxide-reduction process and consolidated via spark-plasma sintering. To explore the effect of plastic deformation on the thermoelectric properties, the sintered bodies are subjected to uniaxial pressure to induce a controlled amount of compressive strains (-0.2, -0.3, and -0.4). The shaping temperature is set using a thermochemical analyzer, and the plastic deformation effect is assessed without altering the material composition through differential scanning calorimetry. This strategy is crucial because the conventional hot-forging process can often lead to alterations in material composition due to the high volatility of chalcogen elements. With increasing compressive strain, the (00l) planes become aligned in the direction perpendicular to the pressure axis. Furthermore, an increase in the carrier concentration is observed upon compressive plastic deformation, i.e., the donor-like effect of the plastic deformation in n-type Bi2Te2.5Se0.5 compounds. Owing to the increased electrical conductivity through the preferred orientation and the donor-like effect, an improved ZT is achieved in n-type Bi2Te2.5Se0.5 through the compressive-forming process.

Prediction Skill of East Asian Precipitation and Temperature Associated with El Niño in GloSea5 Hindcast Data (GloSea5의 과거기후 모의자료에서 나타난 El Niño와 관련된 동아시아 강수 및 기온 예측성능)

  • In this study, we investigate the performance of Global Seasonal Forecasting System version 5 (GloSea5) in Korea Meteorological Administration on the relationship between El $Ni{\tilde{n}}o$ and East Asian climate for the period of 1991~2010. It is found that the GloSea5 has a great prediction skill of El $Ni{\tilde{n}}o$ whose anomaly correlation coefficients of $Ni{\tilde{n}}o$ indices are over 0.96 during winter. The eastern Pacific (EP) El $Ni{\tilde{n}}o$ and the central Pacific (CP) El $Ni{\tilde{n}}o$ are considered and we analyze for EP El $Ni{\tilde{n}}o$, which is well simulated in GloSea5. The analysis period is divided into the developing phase of El $Ni{\tilde{n}}o$ summer (JJA(0)), mature phase of El $Ni{\tilde{n}}o$ winter (D(0)JF(1)), and decaying phase of El $Ni{\tilde{n}}o$ summer (JJA(1)). The GloSea5 simulates the relationship between precipitation and temperature in East Asia and the prediction skill for the East Asian precipitation and temperature varies depending on the El $Ni{\tilde{n}}o$ phase. While the precipitation and temperature are simulated well over the equatorial western Pacific region, there are biases in mid-latitude region during the JJA(0) and JJA(1). Because the low level pressure, wind, and vertical stream function are simulated weakly toward mid-latitude region, though they are similar with observation in low-latitude region. During the D(0)JF(1), the precipitation and temperature patterns analogize with observation in most regions, but there is temperature bias in inland over East Asia. The reason is that the GloSea5 poorly predicts the weakening of Siberian high, even though the shift of Aleutian low is predicted. Overall, the predictability of precipitation and temperature related to El $Ni{\tilde{n}}o$ in the GloSea5 is considered to be better in D(0)JF(1) than JJA(0) and JJA(1) and better in ocean than in inland region.

A Study on the Diffuser Design of Exhaust Pipes for the Infra-Red Signature Reduction of Naval Ship (함정 적외선 신호 감소를 위한 폐기관의 디퓨져 설계에 관한 연구)

  • In modern naval ships, an infrared signature suppression (IRSS) system is used to reduce the metal surface temperature of the heated exhaust pipe and high-temperature exhaust gases generated from the propulsion system. Generally, the IRSS systems used in Korean naval ships consist of an eductor, mixing tube, and diffuser. The diffuser reduces the temperature of the metal surface by creating an air film due to a pressure difference between the internal gas and the external air. In this study, design variables were selected by analyzing the shapes of a diffuser designed by an advanced overseas engineering company. The characteristics of the design variables that affect the performance of the IRSS were investigated through the Taguchi experimental method. A heat flow analysis technique for IRSS systems established in previous studies was used analyze the performance of the diffuser. The performance evaluation was based on the area-averaged value of the metal surface temperature and exhaust gas temperature at the outlet of the diffuser, which are directly related to the intensity of the infrared signature. The results show that the temperature of the exhaust gas was significantly affected by changes in the diameter of the diffuser outlet, and the temperature of the diffuser's metal surface was significantly affected by changes in the number of diffuser rings.
