Drug-induced liver injury

  • Drug-induced liver injury (DILI), including herbal and dietary supplement hepatotoxicity, is often passed lightly; however, it can lead to the requirement of a liver transplant or may even cause death because of liver failure. Recently, the American College of Gastroenterology, Chinese Society of Hepatology and European Association for the Study of the Liver guidelines for the diagnosis and treatment of DILI have been established, and they will be helpful for guiding clinical treatment decisions. Roussel Uclaf Causality Assessment Method scoring is the most commonly used method to diagnose DILI; however, it has some limitations, such as poor validity and reproducibility. Recently, studies on new biomarkers have been actively carried out, which will help diagnose DILI and predict the prognosis of DILI. It is expected that the development of new therapies such as autophagy inducers and various other technologies of the fourth industrial revolution will be applicable to DILI research.

천연물제제 마루플랜트$-AMP-365^{TM}$의 항염증 및 면역활성에 미치는 영향 (Anti-inflammatory and Immunomodulatory Effects of the Botanical Product $AMP-365^{TM}$)

  • The effect of aqueous extracts of medicinal plants $AMP-365^{TM}$ was tested for immune system regulating activity based on anti-inflammatory activity, anti-oxidant, macrophage proliferation and T-lymphocyte proliferation activity. $AMP-365^{TM}$ dose-dependently increased proliferation of RAW264.7 macrophage cells and its nitric oxide production as well as DPPH radical scavenging activity. On the other hand, T-lymphocyte proliferation activity was decreased on dose-dependent manner. Passive cutaneous anaphylaxis reaction was alleviated by 49% by administering 250 mg/kg of $AMP-365^{TM}$. The results suggest that $AMP-365^{TM}$ can be beneficial in the treatment of immediate allergic reactions as an adjuvant supplement material.

A Survey for Health-related Factors of Middle School Students in Daejeon

  • Objective: To investigate the situation of health-related main factors of Korean middle school adolescents. Methods: Data were collected via a self-reporting questionnaire from 2,254 students (650 boys and 1,604 girls), and their sleeping, exercise, dietary pattern, stress, physical problems, and taking of food supplements were analyzed. Results: Korean middle school students sleep about 7 hours per day, and they exercise less than 4 hours per week. Around 13% of the students skip breakfast nearly every day. 35% of the students were under severe stress, and 38% have reported at least one symptom of physical distresses including gastrointestinal disorders or headaches. 31% of the students took a functional supplement, especially one with vitamins as the most favored one, and ginseng and herbal drugs coming second and third. Conclusion: This result first reports a general feature of health-associated factors in middle school students. This study in the future will be basic information to develop medical supports for adolescents using traditional Korean medicine.

  • Phytoestrogen has been used as supplement in order to treat osteoporosis. The representative phytoestrogen, isoflavones, are daidzein, genistein and formononetin which were present highly in legumes. We have studied the quantitative analysis of isoflavones in KUNBO by HPLC. KUNBO, a mixed herbal extract including Astragali Radix and Rhynchosiae nulubilis Semen (Leguminosae), etc., is a nutraceutical candidate for type I osteoporosis. olumn used in HPLC was LUNA 5${\mu}$ C18 (250 ${\times}$4.6mm) (Phenmenex Co.m Torroance, CA, U.S.A.). Potentials of Phyllanthus amarus, Viscum album and Moringa oleifera supplements to mitigate heat stress in female rabbits in humid tropics

  • Global warming is a key challenge subjecting animals to heat stress conditions resulting in multiple physiological alterations in tropical climate. Dietary approach seems to be the more friendly approach to curb the adverse effects of heat stress in rabbits. Some herbs have been categorized to have high potential for promotion of immune responses for amelioration of heat stress. Thus, this research aims to evaluate the potential of Mistletoe (Viscum album), Moringa (Moringa oleifera) and Phyllanthus (Phyllanthus amarus) leaf meal as herbal supplements for the alleviation of heat stress in female rabbits by measuring improvement in sex and stress hormonal responses in serum biochemistry. 80 Rabbit does were exposed to 4 dietary groups supplemented with each of Mistletoe, Moringa, Phyllanthus and a control in an 84-day trial at the summit of thermal stress in South west Nigeria. Growth indices were monitored throughout the study, blood samples were compiled at the end of the trial to assess serum biochemistry, stress and sex hormonal responses of the Does using standard protocols. The results revealed that final weight and weight gain of Does fed on Phyllanthus were significantly (p < 0.05) higher (11.46% and 14.25%, respectively) than Does on control. The herbal supplements enhance glucose, protein, albumin and globulin, reduced cholesterol, and creatinine of Does under heat stress conditions. Among the herbal treatment groups, mistletoe, moringa and phyllanthus had 12.42%, 18.39% and 16.90%, respectively, lower corticosterone than control groups which had 39.76ng/ml. Triiodothyronine of Does fed control were significantly (p < 0.05) lower than Does on Moringa oleifera and Phyllanthus amarus supplements. Estradiol and Follicle stimulating hormone of rabbit Does fed on moringa supplement were significantly (p < 0.05) higher than other treatments. In conclusion, the herbal supplements tend to mitigate the detrimental outcome of thermal stress on Does by suppressing stress hormones. Moringa oleifera and Phyllanthus amarus enhanced sex hormones while Phyllanthus amarus confered growth promoting effects on the Does.

Preliminary Results of Extraction, Separation and Quantitation of Arsenic Species in Food and Dietary Supplements by HPLC-ICP-MS

  • Various extraction procedures were investigated using reference materials and samples to evaluate extraction efficiency and effectiveness. Inductively coupled plasma mass spectrometry (ICP-MS) was used to measure total arsenic and to quantitate arsenic species when coupled to an HPLC (high pressure liquid chromatography). Arsenic species were extracted from rice flour (NIST SRM 1568a) with water/methanol mixtures using accelerated solvent extraction (ASE). Total arsenic extraction efficiency ranged from 42 to 64%, for water and various methanol concentrations. From spinach (NIST SRM 1570), freeze-dried apple, and rice flour (NIST SRM 1568a), arsenic species were extracted with trifluoroacetic acid (TFA) at 100 ${^{\circ}C}$. Total arsenic extraction efficiency was 90% for spinach, 75% for freeze-dried apple, and 83% for rice flour. Enzymatic extraction with alpha-amylase and sonication resulted in extraction efficiency of 104% for rice flour, 98% for freeze-dried apple, and 7% for spinach. Chromatograms of arsenic species extracted by the optimum extraction methods were obtained, and the species were quantified. Arsenite (As(III)), arsenate (As(V)), dimethylarsinic acid (DMA), and monomethylarsonic acid (MMA) were found in the apple sample, and DMA and As(V) in the rice flour sample. As(V) and MMA were found in three herbal dietary supplement samples.

난소절제 쥐에서 칼슘섭취수준에 따른 망간의 보충이 골격상태 및 칼슘평형에 미치는 영향 (The Effects of Manganese Supplementation on Bone Status and Calcium Balance in Ovariectomized Rats according to the Calcium Intake Levels)

  • The purpose of this study was to investigate the effect of manganese (Mn) supplementation on bone status and calcium balance in ovariectomized rats according to the calcium intake levels. Total of 50 Sprague Dawley female rats (6 weeks) were divided into 5 groups and bred for 12 weeks: sham operated control group (SACa), OVX Ca deficiency group (OLCa) with Ca deficiency diet (0.1% Ca modified AIN-93N diet), OVX Ca deficiency & Mn supplement group (OLCaMn), OVX adequate Ca group (OACa; 0.5% Ca AIN-93N diet) and OVX adequate Ca & Mn supplement group (OACaMn). BMD (bone mineral density) of the femur was increased by Mn supplementation in OVX adequate Ca group. However, BMDs of spine, femur and tibia were lowered in OLCa compared to the OLCaMn group. Bone strength of tibia in OLCaMn group was significantly lower than OLCa group. Serum ALP (alkaline phosphatase) and CTx (C-telopeptide of collagen cross-links) levels were significantly higher in ovariectomized rats than those in the sham group, but they were not changed by Mn supplementation. Ca retention rate and Ca absorption rate did not differ among the experimental groups. Urinary Ca excretion was increased by Mn supplementation in Ca deficiency rats. In summary, Mn supplementation resulted in positive effects on bone mineral density ovariectomized rats with which intake adequate Ca. However, Mn supplementation on Ca deficiency ovariectomized rats resulted in decrement of BMO and bone strength by increasing Ca excretion. Therefore, it is encouraged to consider calcium intake levels in supplementation of manganese in order to prevent postmenopausal osteoporosis and to keep bone healthy. A comparative study on immune-stimulatory and antioxidant activities of various types of ginseng extracts in murine and rodent models

  • Background: Ginseng (Panax ginseng) is a widely used traditional herbal supplement that possesses various health-enhancing efficacies. Various ginseng products are available in market, especially in the Korean peninsula, in the form of drinks, tablets, and capsules. The different ginseng types include the traditional red ginseng extract (RGE), white ginseng, and black red ginseng extract (BRGE). Their fermented and enzyme-treated products are also available. Different treatment regimens alter the bioavailability of certain compounds present in the respective ginseng extracts. Therefore, in this study, we aimed to compare the antioxidant and immune-stimulating activities of RGE, BRGE, and fermented red ginseng extract (FRGE). Methods: We used an acetaminophen-induced oxidative stress model for investigating the reduction of oxidative stress by RGE, BRGE, and FRGE in Sprague Dawley rats. A cyclophosphamide-induced immunosuppression model was used to evaluate the immune-stimulating activities of these ginseng extracts in BALB/c mice. Results: Our results showed that most prominently, RGE (in almost all experiments) exhibited excellent antioxidant effects via increasing superoxide dismutase, catalase, and glutathione peroxidase activities in the liver and decreasing serum 8-hydroxy-2'-deoxyguanosine, aspartate aminotransferase, and lactate dehydrogenase levels compared with the groups treated with FRGE and BRGE. Moreover, RGE significantly increased the number of white blood cells, especially T and B lymphocytes, and antibody-forming cells in the spleen and thymus, and it also activated a number of immune cell subtypes. Conclusion: Taken together, these results indicate that RGE is the best supplement for consumption in everyday life for overall health-enhancing properties.

Simultaneous determination of illegal galactagogue adulterants in supplement diets by LC-MS/MS

  • Recently, for successful lactation, many breastfeeding mothers seek various products, including herbal medicine, dietary supplements, and prescribed medicines, to improve milk production. As demand for galactogogues grows, it is highly possible that pharmaceutical galactogogues may be adulterated with illegal products to maximize their efficacy. For continuous control and supervision of illegal products, we developed and validated a simple and sensitive LC-MS/MS method capable of simultaneously determining five galactogogues. Chromatographic separation was conducted using an Agilent Poroshell $120SB-C_{18}$ column with a mobile phase consisting of 20 mM ammonium formate (pH 5.4) and 100 % acetonitrile. The total run time was 13 min per analyte. The proposed method was performed according to the guidelines of the International Conference of Harmonization and it produced reliable results. This method showed high sensitivity and specificity, with a limit of detection (LOD) and limit of quantitation (LOQ) of 0.01-0.82 ng/mL and 0.02-2.45 ng/mL, respectively, for the solid- and liquid-type samples. Specificity was evaluated by analyzing matrix-blank samples spiked with the target compounds at LOQ levels, which provided a good separation of all peaks without interference. Additionally, the repeatability and intermediate precision were typically <15 %, whereas the recovery was 80-120 % of the values obtained using blank samples. Thus, we concluded that this method could be used for the identification and quantification of galactogogues in food or herbal products.

뜰보리수 추출물을 첨가한 혼합음료 이화학적 특성과 제조조건의 최적화 (Optimization of Manufacturing Condition and Physicochemical Properties for Mixing Beverage added Extract of Elaeagnus multiflora Thunb. Fruits)

  • 뜰보리수를 이용한 가공식품을 개발하기 위한 연구의 일환으로 뜰보리수 추출물과 현미식초를 혼합한 음료를 개발하기 위하여 혼합비에 따른 이화학적 품질 특성을 분석하여 혼합조건을 최적화 하고자 하였다. 중심합성계획법에 의하여 설계된 실험조건에 따라 제조된 혼합음료의 당도는 9.77-11.5 $^{\circ}Brix$, pH 는 3.0정도 이었다. 음료의 색도는 뜰보리수의 추출물의 함량에 따라 약간 증가하였으며, 총산의 함량은 현미식초의 혼합정도에 따라 증가하였다. 총페놀성 물질의 함량은 각 조건에서 0.28에서 0.84 mg% 사이의 함량을 보여 혼합조건에 따른 차이를 보였다. 뜰보리수 음료의 최적 혼합조건을 찾기 위하여 반응표면분석법에 의해 수립된 2차 회귀모형에 적합시켜 얻은 결과, 측정된 이화학적 특성을 나타내는 변수 모두 전반적으로 높은 결정계수값을 가졌다. 각 반응변수에 미치는 독립변수의 영향은 뜰보리수 추출물의 함량은 모든 이화학적 품질특성에 영향을 미치는 요인이었으며, 특히 당도, pH, 총산과 총페놀 함량에는 아주 높은 유의수준의 상관관계를 나타내었다. 혼합음료의 총산의 함량을 최대로 하는 조건은 정상점이 0.88%일때 뜰보리수 추출물이 19.2%, 현미 식초가 7.6% 첨가하는 조건이었다. 혼합음료의 당도를 최대로 하는 조건은 뜰보리수의 함량을 24.7%, 현미식초를 4.9% 첨가할 경우 11.5 $^{\circ}Brix$ 로 최대 함량을 나타내는 것으로 나타났으며, 총 페놀 함량을 최대로 하는 조건은 뜰보리수 추출물 25.0%, 현미식초 4.3%를 혼합할 경우 14.47 mg%로 가장 높은 함량을 나타내었다.
