  • Objective : This study was designed to investigate causes, symptoms and treatments for the diabetes insipidus with oriental medical literatures. Methods : We surveyed the oriental and western medical books from to recent published bocks that have articles on diabetes insipidus Results and Conclusions : The symptoms of diabetes insipidus are polyuria and polydipsia, these are similar symptoms of Pae-so(肺消), Gouk-so, So-gal(消渴), Sang-so(上消), Sin-so(腎消), So-sin(消腎), Ha-so(下消) and Sin-jung(腎中) of kinds of So-gal(消渴) in the oriental medicine. The major causes of diabetes insipidus are the deficiency of the yin of the kidney, the heat of the heart and the dryness of the pulmonary. The methods of treatment are tonifying the yin of the kidney(滋補腎陰), tonifying qi and arrest discharges(益氣固澁), removing heat to moisten dryness (淸熱潤燥) and promoting fluid production to quench thirst(生津止葛). The herbal-medications for treatment are So-galbang and Yidongtang(消渴方合二冬湯), Ryoumijihawangtang gamibang(六味地黃湯加味方).

A Clinical Case Report of Endoscopic Reflux Esophagitis with Gastric Dysmotility Treated with Korean Traditional Medical Therapy (위 운동성 장애가 동반된 역류성 식도염 환자에 대한 한방단독 치험 1례)

  • Reflux esophagitis is a common disease in developed nations. We describe the case of a female patient with endoscopic reflux esophagitis complicated by gastric dysmotility. Both electrogastrography and enterotachography were performed to detect gastric myoelectrical activity and pyloric sphincter function and evaluate gastric motility. The patient was treated only with herbal medications and general acupuncture, with electrical stimulation of the ST.36 (Zusanli) point, in addition to moxibustion therapy. After each primary and secondary treatment, the therapeutic effect was immediately evaluated. At the final follow-up 5 mon after the end of the secondary treatment, the patient’s general condition was assessed, in addition to the mucosa of the esophagus. At follow up, all the patients’ symptoms had disappeared, and the mucosa of the esophagus had returned to normal. We attributed these therapeutic effects to improved gastric dysmotility. To confirm the usefulness of this treatment method, studies of larger numbers of patients with reflux esophagitis treated with Korean traditional medicine are needed.

Four Cases of Dizziness Alleviated by Korean Medicine Treatment with Gami-banhabakchulchunma-tang (가미반하백출천마탕 및 한의치료로 호전된 어지럼 환자 4례)

  • Objectives: The purpose of this case report was to show the effects of Korean medicine treatment (KMT) with Gamibanhabakchulchunma-tang(GBBCT) on dizziness. Methods: This case report describes four patients with dizziness. The patients were treated with KMT, including GBBCT and acupuncture, while continuing to take the same medications they were already taking to treat underlying diseases. A numerical rating scale (NRS) was used to evaluate the effectiveness of treatment for dizziness. Results: After the treatment, the NRS score for dizziness decreased in all cases. Conclusions: These four cases showed that KMT with GBBCT may be effective for the treatment of dizziness. GBBCT can be used to treat dizziness, regardless of diagnosis.

Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital (동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.

Improved Symptoms of Peripheral Facial Nerve Palsy in ChAdOx1 nCoV-19 Vaccine Recipients Following Complex Korean Medicine Treatment

  • Peripheral facial nerve palsy is a neurological condition/disease characterized by unilateral paralysis of the facial muscles. We report the effects of complex Korean medicine treatment in 2 patients diagnosed with Bell's palsy who developed symptoms 3-4 days following vaccination with ChAdOx1 nCoV-19 and who received oral steroids and antiviral medications. Complex Korean medicine treatments including acupuncture, electroacupuncture, bee venom acupuncture, cupping therapy, herbal medicine, and physical therapy were performed. Symptom improvement was evaluated daily using the Numerical Rating Scale, House-Brackmann Grade, and Yanagihara's Score. Patient 1 was hospitalized for 22 days and Patient 2 visited the outpatient clinic 20 times. In both cases, postauricular pain caused by Bell's palsy disappeared, and symptoms of facial paralysis improved following completion of treatment. These findings could suggest that combined Korean medicine treatment may improve symptoms of peripheral facial nerve palsy.

A Case Study of an In-Patient with Delirium Treated with Shihogayonggolmoryo-tang-gami (시호가용골모려탕가미로 호전된 입원 환자의 섬망 치험 1례)

  • Objective: In this case study, we describe the use of Shihogayonggolmoryo-tang-gami to treat an in-patient with delirium. Methods: An in-patient with symptoms of delirium was treated with Korean medicine therapies, including the herbal medication Shihogagyonggolmoryo-tang-gami, for 7 days. To evaluate its therapeutic effect, we used the Nursing Delirium Screening Scale (Nu-DESC) and Memorial Delirium Assessment Scale (MDAS). Results: After the treatment, Nu-DESC and MDAS scores improved. Of note, instances of the patient calling out improved, in addition to the patient's sleeping pattern. Although all the symptoms of delirium did not disappear completely, Shihogayonggolmoryo-tang-gami had therapeutic effects. Conclusion: These results suggest that Shihogayonggolmoryo-tang-gami may have a beneficial effect on delirium, without using other antipsychotic medications.

Case of Stage IV Renal Cancer Patient Whose Metastatic Lymph Node and Severe Back Pain Improved after Integrative Treatment (한양방 통합 치료로 통증감소 및 전이 림프절 축소가 관찰된 4기 신장암 환자 1례)

  • Two years ago, a 75-year-old female was diagnosed left renal cancer and went on radical nephrectomy. Recently, metastatized lesions on lumbar spine and various lymph nodes including paraaortic lymph node of which diameter was about 5.5 cm was identified. She suffered from extreme low back pain despite using opioid agents of fentanyl patch and oral oxycodone. During about one month, Korean medicine therpies including herbal medicine(Yang-He tang), acupuncture with her conventional medications were co-administered. However her pain didn't improved and gradual cough and dyspnea developed. So she transferred to the hemato-oncology part for 2 weeks under the diagnosis of everolimus-induced noninfectious pneumonitis. After quitting everolimus her pneumonitis was improved and she came back our ward and started another herbal medicine, Ai-Tong-Ning tang for about 2 months. During this period, her pain was well managed without oral opioid agents and her paraaortic lymph node was regressed to about 2.2cm in diameter.

Complementary and Alternative Medicine Treatment for Burns and Sequela : A Scoping Review of Randomized Controlled Trials (화상 및 후유증의 보완 대체 의학 치료 : 무작위 대조 시험에 대한 주제범위 문헌고찰)

  • Objectives : In order to investigate the current status of recent clinical evidence related to complementary and alternative medicine(CAM) treatment for burns and to inform research and treatment strategies for future, we publish a scoping review(ScR) of randomized controlled trials(RCTs). Methods : The research question of the ScR was "Are there any RCTs of CAM treatment for burn?". RCTs published from 2000 to 2022 were identified in 7 databases(PubMed, Cochrane, CNKI, OASIS, RISS, KCI, KMbase) in March 2023. Data were tabulated and analyzed descriptively with respect to the research questions. Results : 41 RCTs were included. 21(51.2%) were conducted in China, 13(31.7%) in Iran. The main treatment criteria were herbal medicine in 28 cases, acupuncture in 9 cases, chuna therapy in 4 cases, and psychotherapy in 3 cases. Among the herbal medicine, there were 19 topical medications, 5 injections(intravenous pharmacopuncture), 4 aerosol drugs(aromatherapy), and 1 oral drug. Among the acupuncture, there were 4 plum blossom needles(seven-star needles), 2 wrist-ankle acupunctures, 2 press needles, and 1 electroacupuncture. CAM treatments were effective in treating burns. It reduced pain and pruritus at the burn site, helped recovery and management of the donor site, reduced anxiety and pain during dressing change, improved hematological problems and vital signs, and finally lowered the mortality rate. CAM treatments also lowered health care costs. Conclusions : CAM treatments for burns is prospective, and that it deserves to make high-quality studies including additional large-scale RCTs.

Elevated Levels of Serum Lipids Accelerate Biological Vascular Aging evaluated by the Second Derivative of Photoplethysmogram Waveform in the Middle-Age Adults (중년 성인남녀의 혈중 지질농도가 가속도맥파에 미치는 영향)

  • Objective : There is accumulating data that cardiovascular diseases, including hypertension, cerebro-vascular accident, coronary heart disease, relate to atherosclerotic changes. Increased serum levels of lipids could play a role in these circumstances. the SDPTG(the second derivative of photoplethysmogram waveform) is noninvasive methods for evaluating the pulse wave and is correlated with age and other risk factors for atherosclerosis. Methods : To study the effects of serum lipid levels on the SDPTG in the middle-age adults, data on serum lipid levels and the SDPTG were collected in 80 healthy middle age with varying serum lipid concentrations. Biological vascular age was assessed using the SDPTG. the SDPTG consists of an a, b, c, and d wave in systole and an e wave in diastole; an SDPTG aging index(Al) was calculated as (b-c-d-e)/a. Results : Serum triglyceride, total cholesterol, total lipid, phospholipid, HDL cholesterol levels were respectively and significantly correlated with SDPTG Al. Serum lipid levels and blood pressure are age-dependent and the SDPTG is blood pressure-dependent. Restricting analysis to age and blood pressure yield strong positive correlations between serum triglyceride, total lipid levels and SDPTG Al, but no correlations between serum total cholesterol, phospholipid levels and SDPTG Al. Conclusion : The results of this study suggest that preventive acupuncture programs or herbal medications for reducing serum lipid, especially triglyceride, levels in middle-age should include a evaluation of biological vascular aging with the second derivative of photoplethysmogram waveform.

Secondary adrenal insufficiency caused by sorafenib administration in a patient with hepatocellular carcinoma

  • Sorafenib (Nexavar) has been regarded as a treatment for unresectable hepatocellular carcinoma (HCC), with side effects that include hand-foot skin reaction, diarrhea, rash, fatigue, hypertension, nausea, anorexia, weight loss, and alopecia. Thyroid disorder, such as endocrine side effect, has also been reported. However no case involving adrenal insufficiency has been reported. Here, we report a case of adrenal insufficiency which occurred after taking sorafenib in a patient with HCC. A 56-year-old man visited our hospital due to right upper quadrant abdominal pain and he was diagnosed as multiple disseminated and unresectable HCCs with portal vein invasion; therefore transarterial chemoembolization was performed and sorafenib administration was started. Two months later, he was admitted to the hospital complaining of severe fatigue. The laboratory results showed cortisol of <$0.2{\mu}g/dL$ and adrenocorticotropic hormone of <1.00 pg/mL. The patient had no history of taking steroids or herbal medications. Secondary adrenal insufficiency was diagnosed and prednisolone 10 mg per day was started immediately; as a result, fatigue remarkably improved. This may be the first report indicating a possible association between sorafenib and adrenal insufficiency and it implies that the possibility of adrenal insufficiency should be considered in patients taking sorafenib who complain of severe fatigue.
