Study on the Obesity and Blood parameters Differences between Fire/Heat and Qi-deficiency Pattern Identification/Syndrome Differentiation among Acute Stroke Patients (급성기 중풍환자에서 비만 및 혈액지표의 기허 및 화열 변증의 차이에 대한 고찰)

  • Object : In the present study, we investigated the obesity and blood parameters between Qi-deficiency and Fire/Heat pattern identification/syndrome differentiation (PI/SD) in acute stroke patients. Materials and Methods : A total of 391 stroke patients within 7 days after onset were consecutively recruited from 12 hospitals across South Korea from Nov. 1st, 2006 to Jun. 31st, 2009. They were diagnosed as Fire/Heat or Qi-deficiency among five PI/SD subtypes by two independent stroke experts. We investigated the differences of obesity and blood characteristics between Fire/Heat and Qi-deficiency by statistical analyses. Results : In male subjects, obesity was significantly associated with Fire/Heat PI/SD. The averaged mean BMI ($24.13kg/m^2$) and waist circumference(89.34cm) of the Fire/Heat group were higher than those of the Qi-deficiency group ($22.60kg/m^2$ and 83.43 cm, respectively). The number of obese patients was larger in the Fire/Heat group than in the Qi-deficiency group (p = 0.001). Hyperlipidemia was also related with Fire/Heat. However, obesity was not associated with PI/SD in female subjects where the number of hyperlipidemic patients was higher in the Qi-deficiency group. Among blood parameters, the levels of triglycerides and fasting blood sugar were higher in the Fire/Heat group compared with the Qi-deficiency group in male subjects. However, total cholesterol of the Qi-deficiency group was higher than in the Fire/Heat group among female subjects. Conclusion : This study shows that obesity and hyperlipidemia are significantly difference between Qi-deficiency and Fire/Heat. We suggests that PI/SD may be associated with clinical characteristics and large population study between PI/SD and clinical characteristics including blood parameters are needed.

  • Object : The aim of this study was to examine the differences of symptom improvement between different diagnosis classification groups in acute cerebral infarction patients. Methods : We studied inpatients within a month after the onset of cerebral infarction who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2012. We compared the improvement of Motricity Index and Scandinavian Stroke Scale score between Fire-heat group(n=20), Yin deficiency group(n=31), Dampness-phlegm group(n=30), and Qi deficiency group(n=13). Results : Yin deficiency group patients with cerebral infarction showed the most improvement in MI and SSS scores, and patients in Fire-heat group showed the poorest improvement in MI and SSS scores. There was a significant difference between the two groups, but there were no significant differences between all four diagnosis classification groups. Conclusions : This study provides evidence that diagnosis classification could be considered as an important factor in predicting the prognosis of acute cerebral infarction.

  • Object : The purpose of this study is to observe the correlation between change in pattern identification symptoms and scandinavian stroke scale in ischemic stroke patient who had receive traditional Korean medical treatment and conservative treatment. Methods : 43 subjects were recruited from patients with ischemic stroke within 30 days of onset. We chose the subjects who had at least one follow up session and had checked the score between last follow up session and first session in pattern identification and scandinavian stroke scale. We also assessed the correlation between pattern identification and scandinavian stroke scale. Results : There were significant negative correlation between pattern identification and scandinavian stroke scale in Fire-heat pattern and positive correlation in Dampness-phlegm pattern. Conclusions : This study provides evidence that collaborative treatment maybe effective in improving neurologic symptoms in ischemic stroke patients diagnosed as Fire-heat pattern. Further studies with larger scale and longer observation period, more neurologicscales scales, control group would be required.

Immediate Effect of Sustained Stretching Exercises with Far Infrared on the Ankle Range of Motion and Muscle Tone in Patients with Stroke

  • Purpose: The spasticity of stroke patients decreases the ankle range of motion and increases the gastrocnemius muscle tone. This study examined the effects of stretching exercise and far infrared irradiation on the ankle function in stroke patients with spasticity. Methods: This study was conducted on 20 stroke patients admitted to Jesaeng General Hospital, who were divided into a study group (stretching exercise with far infrared) and control group (stretching exercise only). The dorsiflexion range of motion was measured using a smartphone and the medial gastrocnemius muscle tone and stiffness were measured using a Myoton pro. Results: With the exception of the non-paretic gastrocnemius muscle tone in the control group, the medial gastrocnemius muscle tone and stiffness decreased significantly in both groups. In both groups, the dorsiflexion range of motion increased significantly. In addition, the experimental group had a significantly higher dorsiflexion range of motion than the control group. On the other hand, there was no significant difference between the two groups in terms of the medial gastrocnemius muscle tone and stiffness. Conclusion: For stroke patients with spasticity, stretching exercises increased the ankle's range of motion and decreased the gastrocnemius muscle tone. The addition of heat therapy further increased the ankle's range of motion. On the other hand, as the sample size was small, future studies should include more subjects.

Study of the Specific Indicators Calculation of Fire-Heat Pattern Identification and Yin Deficiency Pattern Identification in Stroke (중풍 화열 변증과 음허 변증을 구분하는 특정 지표 산출에 대한 연구)

  • This study was done to calculate the specific indicators of Fire-Heat Pattern Identification and Yin Deficiency Pattern Identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. The subjects were 764 hospitalized patients with stroke, and a list of registry was made for each of them. Frequency and Correlations among Fire-Heat and Yin Deficiency indicators were studied. Fire-Heat pattern group included 175 patients out of 764, Yin Deficiency pattern group included 103 patients out of 764. Among the indicators of Fire-Heat pattern, those, in order of highest frequency, were 'heat vexation and aversion to heat', 'reddened complexion', 'thirst'. Among the indicators of Yin Deficiency pattern, those, in order of highest frequency, were 'dry mouth', 'red tongue', 'fine and rapid pulse'. Among the Fire-Heat pattern indicators, 'yellow fur' and 'thirst', 'heat vexation in the chest' and 'vexation and insomnia' showed most significant correlation. Among the Yin Deficiency pattern indicators, 'aphtha or tongue sore' and 'heat in the palms and soles', 'red tongue' and 'dry fur' showed most significant correlation. 'Reddened complexion', 'dry eyes', 'surging and parid pulse', 'heat vexation and aversion to heat' are classify between Fire-Heat pattern and Yin Deficiency pattern. 'Surging and parid pulse', 'Reddened complexion' are mostly representative of Fire-Heat pattern and 'fine and rapid pulse' are mostly representative of Yin Deficiency pattern.

  • Recently, summer high temperature events caused by climate change and urban heat island phenomenon have become a serious social problem around the world. Urban areas have low albedo and huge heat storage, resulting in higher temperatures and longer lasting characteristics. To effectively consider the urban heat island measures, it is important to quantitatively grasp the impact of urban high temperatures on the society. Until now, the study of urban heat island phenomenon had been carried out focusing only on the effects of urban high temperature on human health (such as heat stroke and sleep disturbance). In this study, we focus on the effect of urban heat island phenomenon on air pollution. In particular, the relationship between high temperature phenomena in urban areas during summer and the concentration of photochemical oxidant is investigated. High concentrations of ozone during summer are confirmed to coincide with a day when the causative substances (NO2,VOCs) are high in urban areas during the early morning hours. Further, it is noted that the night urban heat island intensity is large.. Finally, although the concentration of other air pollutants has been decreasing in the long term, the concentration of photochemical oxidant gradually increases in Daegu.

Study of The Diagnostic Indicators of Deficiency of Eum Pattern Identification In Stroke Patients (중풍환자의 음허변증 진단지표에 관한 연구)

  • The purpose of this study was to investigate that which symptoms are adequate indicator of the deficiency of Eum pattern in the stroke patients. In the time period Dec. 2006 to Aug. 2007, 479 patients with a first-ever stroke admitted in the department of Internal Medicine of 12 Oriental Medical Hospitals were included. Patients were hospitalized within 1 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Eum-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Deficiency of Eum group included 65 case, Non Eum group 414 case out of 479 patients. dryness of mouth, short and rapid purse, white face and reddish zygoma, mirror-like tongue were higher among Eum group. Eum and Non Eum patients do not significantly differ in reddened tongue, dryness in tongue, night sweat, palpitation, afternoon tidal heat, palmar heat, sores of the mouth or tongue. This study was insufficiency because sample size is very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the Stroke.

  • Objectives : This study was to investigated the correlation between average temperature & precipitation and pattern identification of the patients with stroke. Methods : 115 stroke patients within 30days of onset were registered during the study period. Pattern identification of stroke was classified into Fire and heat, Turbid phlegm and others. We took the mean of average temperature according to pattern identification and analyzed pattern identification into two groups according to climatological normal of average temperature in Seoul. We also analyzed pattern identification into two groups according to existence of precipitation. Results : Heat and fire, and other type of pattern identification had high frequency when average temperature was higher than 12.2℃. And there was no correlation between precipitation and pattern identification of stroke.

The Effect of Moxibustion at Chonjung(CV17, Shanzhong) on Patients with Dysphagia after Stroke (중풍환자(中風患者)의 연하장애(嚥下障碍)에 전중혈 구치료(灸治療)가 미치는 효과(效果))

  • Objectives: Dysphagia is a common in stroke patients. Dysphagia often affects the rehabilitation of stroke patients by increasing the risk of nutritional deficits and aspiration pneumonia. Despite the proliferation of physical therapies including swallowing training, much controversy remains regarding the application and benefit of them. Therefore, in this study, the clinical effect of moxibustion at Chonjung(CV17, Shanzhong) on post-stroke dysphagia were assessed using Swallowing Provocation Test(SPT). Methods: Dysphagia subjects were selected by Dysphagia Screening Test. Swallowing function was tested by Swallowing Provocation Test(sec). Direct moxibustion was applied to the acupoint, Chonjung, five times and Swallowing Provocation Test was performed before and after 30 minute. The Latency Time of Swallowing Reflex (LTSR) was checked by SPT. To find factors related with improving swallowing function, Cold-Heat and Excess-Deficiency Diagnosis were considered. Results: A total of 42 patient were included, but two of them were excluded due to severe coughing. Overall, the swallowing reflex improved significantly. In subgroup analysis on brain lesion, non-brain stem lesion patients significantly improved. Moxibustion was more effective in the cold group than in the heat group, but there were no differences between the Excess and the Deficiency groups. Conclusions: The result of this clinical study suggest that moxibustion at Chonjung(CV17, Shanzhong) is an effective treatment for the dysphagia patients after stroke, especially in non-brain stem lesion and the cold diagnosed patients.

  • The simulation of power cycle and unsteady gas exchange processes in the inlet and exhaust systems of the single-cylinder 2-stroke cycle compression ignition engine was studied in this paper. In power cycle process, the single-zone model proposed by Whitehouse and Way was used, and the convective and radiative heat transfer from cylinder contents to surroundings was considered. To solve the equations for gas exchange process, the generalized method of characteristics including area change, friction, heat transfer and entropy gradients was used. Also with the path line calculation, the entropy change along the path line and the variation of specific heat due to the change of temperature and the composition of cylinder gas were considered. As a result of the simulation, the change of pressure and temperature in the cylinder against the crank angle, the rate of net heat release, and the change of properties at each point in the inlet and exhaust pipe against the crank angle were obtained. The engine performances under various operating conditions were also calculated.
