The Effects of an Advanced Cardiac Life Support Training via Smartphone's Simulation Application on Nurses' Knowledge and Learning Satisfaction (스마트폰 어플리케이션을 활용한 전문심폐소생술 시뮬레이션 재학습이 간호사의 지식 및 교육 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to examine how an Advanced Cardiac Life Support (ACLS) simulation application via smartphone effects nurses' ACLS knowledge and learning satisfaction. Methods: The participants were selected from nurses in medical Intensive Care Unit (ICU), surgical ICU and emergency room. The experimental group consists of fifty nurses who were self-learned with ACLS simulation application via smartphone and the control group of seventy-one nurses who used traditional learning materials. Outcome variables included nurses' knowledge and learning satisfaction which were collected before and after the intervention. Results: The scores of ACLS knowledge were higher in the control group compared to the experimental group (p=.001) while learning satisfaction showed no statistical difference (p=.444). In learning satisfaction, the experimental group showed higher interest than the control group (p=.019) while the control group rated higher on the item, 'the contents of education was reliable' (p=.007). Conclusion: ACLS knowledge score was graded higher in control group that used traditional learning method than the experimental group that used the smartphone application. This study showed that training with the new material significantly increased nurses' interest in ACLS education. Hence, more applications for smartphones should be developed to provide self-learning environment for nurses and improve care quality.

Clinical Implications of Microsatellite Instability in Early Gastric Cancer

  • Purpose: We aimed to evaluate the clinical characteristics of microsatellite instability in early gastric cancer. Materials and Methods: The microsatellite instability status of resected early gastric tumors was evaluated using two mononucleotide repeat markers (BAT25 and BAT26) and three dinucleotide repeat markers (D5S346, D2S123, and D17S250). Tumors with instability in two or more markers were defined as microsatellite instability-high (MSI-H) and others were classified as microsatellite stable (MSS). Results: Overall, 1,156 tumors were included in the analysis, with 85 (7.4%) classified as MSI-H compared with MSS tumors. For MSI-H tumors, there was a significant correlation with the female sex, older age, tumor location in the lower gastric body, intestinal histology, lymphovascular invasion (LVI), and submucosal invasion (P<0.05). There was also a trend toward an association with lymph node (LN) metastasis (P=0.056). In mucosal gastric cancer, there was no significant difference in MSI status in tumors with LN metastasis or tumors with LVI. In submucosal gastric cancer, LVI was more frequently observed in MSI-H than in MSS tumors (38.9% vs. 25.0%, P=0.027), but there was no difference in the presence of LN metastases. The prognosis of MSI-H tumors was similar to that of MSS tumors (log-rank test, P=0.797, the hazard ratio for MSI-H was adjusted by age, sex, pT stage, and the number of metastatic LNs, 0.932; 95% confidence interval, 0.423-2.054; P=0.861). Conclusions: MSI status was not useful in predicting prognosis in early gastric cancer. However, the frequent presence of LVI in early MSI-H gastric cancer may help guide the appropriate treatment for patients, such as endoscopic treatment or limited LN surgical dissection.

  • Purpose: This study was conducted to develop and evaluate an oral care protocol for cancer patients receiving chemotherapy. Methods: The participants in this study were cancer patients in Severance hospital in Seoul. A total of 31 patients were assigned to the experimental group, and 29 patients to the control group. Data were collected from August 1 to October 5, 2010. The protocol included definition and symptom of oral mucositis, self-check method of oral status, prophylactic method of oral mucositis (oral care, eating habits, and gargling) and management of oral mucositis. Oral Assessment Guide (OAG) was used as the measurement tool of oral mucositis in this study, Oral Care Performance Scale was used as the measurement tool of oral care performance status. Data were analyzed with a $x^2-test$ and t-test, and repeated measures ANOVA, using SPSS/WIN 18.0 program. Results: Patients in the experimental group receiving the oral care protocol had a significant difference (t=-2.938, p=.005) in the oral care performance compared to the control group. However, there was no significant difference (F=1.255, p=.274) in the oral mucositis status between two groups. Conclusion: This study showed that the oral care protocol could improve oral care performance status for the patients under chemotherapy.

  • e-Health system is the use of information and communication technologies to provide health care information and services to stakeholders. It pursues an improvement of service quality of health care, increases of accessability of information, reduction of medical cos and enforcement of competitiveness in health care industry. There are lots of difficulties in implication of internet technology to health care process. This is because that the characteristics of health care industry is very closed and specific and also the internet technology has the very user-oriented characters. So it is necessary to have the system independent conceptual design method to combine these two characteristics. In this paper a new modified conceptual design methodology will be proposed for the successful implementation of e-health system in korean health care industry. The proposed methodology will be verified its validity and feasibility through AHP method. And the results of this research will be implied as the basis for the development and revitalization of e-Health system in Korea.

  • Concerns about a global economic recession are rising following the coronavirus disease 2019 (COVID-19) pandemic. Accordingly, government entities, which are committed to overcome two barriers to severe inflation and economic recession, are showing high interest in spending management so as not to undermine fiscal soundness. Since the health care sector especially accounts for a large proportion of fiscal expenditure, it should be managed in a manner that the expense is appropriately spent. The National Health Insurance System and Healthcare System have secured international competitiveness and reliability by effectively responding to the COVID-19 pandemic. Likewise, considerable efforts should be made to reorganize the welfare and healthcare systems so that they can be sustainable during the post-COVID-19 era and the recession.

Healthcare System Needs to be Reinforced toward the Environmental Changes (변화에 부응하는 보건의료정책 전환이 필요하다)

  • 2022 was a time when the global healthcare system was challenged and has grown in response to the coronavirus disease 2019 pandemic. In addition, various issues accumulated in the process of quantitative growth have emerged in the Korean healthcare system. For instance, problems of sustainability in health insurance finances and removing bubbles from excessive medical utilization should be urgently resolved as tasks that occurred in the process of expanding the coverage of the health insurance system. The deficit of applicants for the pediatrician residency program suggests that expanding health insurance coverage alone has limitations in providing essential medical services. There is a need to incentivize doctors who provide essential medical care services. In 2023, we hope that efforts to enhance and internally reinforce the healthcare system will be concentrated.

Development and Reconsideration of Korea Healthcare System (보건의료체계의 발전과 성찰)

  • During last 65 years, Korea has achieved very rapid economic growth and social reformation including healthcare system. Many foreigners have praised that Korea healthcare system is very good in the respect of ease accessibility to healthcare under the lowest cost among the industrialized countries. Whole population are covered by the National Health Insurance. Also utilizations of healthcare among different income classes are even. However Korea healthcare system faced with several challenges, in terms of the an aging population and a rise in chronic disease problem, new threats of communicable disease due to globalization, the rapid increase of healthcare expenditure and high financial burden of patients even though they are insured. To cope with these challenges, we need reconsider the healthcare system as followings; to set up ideology of healthcare as normative public goods, to rebuild paradigm of healthcare for 21 century, to reform public health for strengthening health promotion, to develop new method for healthcare management including quality improvement and consumer responsiveness, to build new governance for health and to view new perspective on healthcare as a kind of industry.

  • Having gone through socioeconomic changes from an agricultural to a post-industrial society, mental health system accordingly has been changed. In Korea where the socioeconomic change has been so rapid, the mental health system and law have been behind the socioeconomic system. Post-industrial society needs more humanized and advanced mental health system, however, Korean mental health law reflects ideology of Korean society in industrial age. This paper attempts to assess the significance Korea's Mental Health Law against the backdrop of socioeconomic changes. A substantial part of the report is devoted to identifying discrepancies between Korea's Mental Health Law and the perspectives of post-industrial ideology and areas for improvement. Improvement in mental health law should take place in line with the changes occurring in socioeconomic environments, the social concept of family, and the public awareness of human rights. Korea's mental health law should be changed in a way to improve hospitalization procedure for people with mental illness, promote the opening of mental health facilities and improve the quality of the lives of the mentally ill. Further changes are deemed necessary in the public and media view of mental illness. Also, the national budget will have to be increased with a view to raise the social rights of those with mental illness to receive quality rehabilitation services.

  • This paper discussed environmental health policies for the past and coming decade by reviewing the First Comprehensive Environmental Health Plan (2011~2020) and introducing the Second Comprehensive Environmental Health Plan (2021~2030). The major achievement of the First Comprehensive Environmental Health Plan was the establishment of receptor-oriented environmental health policies. However, the main limitations were insufficient policy support for relief and/or recovery from environmental pollution damage and low public awareness of environmental health policies. The Second Comprehensive Environmental Health Plan presents the following major policy tasks: establish an omnidirectional environment health investigation and monitoring system, provide customized environmental health services, improve the environmental health damage relief and recovery system, and promote regional environmental health policies. The Second Plan has a clear distinction from the First Plan in that it expands the field of environmental health from the prevention and management of environmental risk factors to proactive damage response and recovery, which will effectively contribute to alleviating the burden of environmental disease.
