국내중독현황 (Statistics of Poison Exposure in Korea)

  • Objective: This study was conducted for the nationwide statistical survey of poison exposure to provide the rationale for establishing and developing the poison control center (PCC) in Korea. Design: Study group for Korea PCC in National Medical Center reviewed the medical literature on poison exposure of Korea from death reports of National Statistical Office (NSO), the toxic exposure statistics from the report of National Health Insurance Corporation (NHIC), and poison related data from 119 ambulance services (FD) for the purpose of obtaining the poison and its related data. We also conducted questionnaire from the expert who work in emergency medicine department at the designated 320 emergency medical centers in Korea for the preparedness and acknowledgement about necessity of PCC and their need for that. Results: We reviewed the reliable data from the death report of NSO, poison exposure data from NHIC, and running report from FD. Poisoning death occured at home ($36.7\%$) and hospital ($46.3\%$). Poisoning are more common in rural area than the city area. Patients were seen more frequently in the local clinics than in any hospital. The drugs ($45.7\%$) and pesticide ($18.1\%$) are common poison. Common place to poison exposure were residential area ($39.9\%$), industry ($9\%$). mass residential area ($7\%$). and farm ($6\%$). The education level were primary school ($33.2\%$), high school ($23.7\%$), and middle schol ($21.3\%$) in order. We have to provide the poison guideline for lay public to understand easily, and for medical experts. The medical facilities need to be invested and have more interest for toxicology. All medical staff who work in the designated emergency medical center want PCC to establish. They want to have poison information from hospital ($91.3\%$), regional poison information center ($45.0\%$), regional poison control center ($52.5\%$), nationwide poison information center ($48.8\%$), nationwide poison control center ($46.25\%$), as a role of poison control center. They also want that pcc have poison epidemiologic study and statstics, training program for the experts, registration of rare case of posion on website, reflection of policies to activities for antidote production etc., speedy consultation system for poison analysis, public education, establishment of both regional and national pee, etc. Conclusion: Poison center must be established to provide poison information for all the public and medical experitise, focusing rural area and private clinic, to detoxify, to reduce the cost, time, morbity, and mortality through the whole country.

Appraisal of Guidelines for Research & Evaluation (AGREE) II를 이용한 교통사고 상해증후군의 국내·외 기개발 임상진료지침의 평가 (Appraisal of Guidelines for Research & Evaluation II Appraisal of Clinical Practice Guidelines for Traffic Injuries)

  • Objectives This study was aimed to evaluate clinical practice guidelines (CPGs) of traffic injuries, which has already been developed at domestic or outside of country, and to explore the Korean medical treatments included in the CPGs. Methods Twelve electronic databases (PubMed, Cochrane library, China National Knowledge Infrastructure [CNKI {Chinese Academic Journals, CAJ}], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], KoreaMed, Korean Medical Guideline Information [KoMGI), National Guideline Clearinghouse [AHRQ], Core Outcome Measures in Effectiveness Trials Initiative Website [COMET], Turning Research into Practice [TRIP], The National Institute for Health and Care Excellence [NICE], and Medical Research Information Center [MedRIC]) up to July 2021 were searched. Only systematically developed CPGs for traffic injuries were selected and appraised. The appraisal was conducted by using Appraisal of Guidelines for Research & Evaluation (AGREE) II tool. Results Six CPGs were included and evaluated. All CPGs were appraised as highly recommended because they exceeded 60% in more than 4 among 6 domains of AGREE II, including domain of 'rigor of development', and 30% in the rest. Recommendations related to Korean medicine treatments such as on manual therapy related to Chuna were given in 6 CPGs, 4 for acupuncture, 1 for Qigong and 1 for Taping. Conclusions The 6 CPGs were developed up to now through a systematic development methodology and there were many recommendations for Korean medical treatments related to manual (Chuna) treatment, acupuncture and Qigong. However, the evidence for the side effects and risk factors of Korean medical treatments was scantly reflected in CPGs. Therefore, it is considered that balanced CPG with benefits and risks should be developed, covering Korean medical diagnosis, treatment and prognosis.

기후변화 대응을 위한 RCP 시나리오 기반 국내 열지수와 불쾌지수 예측 (Future Prediction of Heat and Discomfort Indices based on two RCP Scenarios)

  • There has been an increasing need to assess the effects of climate change on human health. It is hard to use climate data to evaluate health effects because such data have a grid format, which could not represent specific cities or provinces. Therefore, the grid-format climate data of South Korea based on RCP (Representative Concentration Pathway) scenarios were modified into area-format climate data according to the major cities or provinces of the country, up to the year 2100. Moreover, heat index (HI) and discomfort index (DI) databases were developed from the modified climate database. These databases will soon be available for experts via a Website, and the expected HI and DI of any place in the country, or at any time, can be found in the country's climate homepage (http://www.climate.go.kr). The HI and DI were analyzed by plotting the average indices every ten years, and by comparing cities or provinces with index level changes, using the geographic information system (GIS). Both the HI and DI are expected to continually increase from 2011 to 2100, and to reach the most dangerous level especially in August 2100. Among the major cities of South Korea, Gwangju showed the highest HI and DI, and Gangwon province is expected to be the least affected area in terms of HI and DI among all the country's provinces.

해외투자(海外投資)와 지속가능발전 원칙 - 적도원칙(赤道原則)(Equator Principles)을 중심으로 - (How to Reflect Sustainable Development in Overseas Investment including Equator Principles)

  • The Equator Principles are a set of voluntary environmental and social guidelines for ethical project finance. These principles commit banks and other signatories to not finance projects that fail to meet these guidelines. The principles were conceived in 2002 on an initiative of the International Finance Corporation and launched in 2003. Since then, dozens of major banks have adopted the Principles, and with these banks among them accounting for more than three quarters of all project loan market volume the Principles have become the de facto standard for all banks and investors on how to deal with potential social and environmental effects of projects to be financed. While regarding the Principles an important initiative, NGOs have criticised the Principles for not producing real changes in financing activities and for allowing projects to go through that should have been screened out by the Principles, such as the Sakhalin-II oil and gas project in Russia. In early 2006, a process of revision of the principles was begun. The Equator Principles state that endorsing banks will only provide loans directly to projects under the following circumstances: - The risk of the project is categorized in accordance with internal guidelines based upon the environmental and social screening criteria of the International Finance Corporation (IFC). - For all medium or high risk projects (Category A and B projects), sponsors complete an Environmental Assessment, the preparation of which must meet certain requirements and satisfactorily address key environmental and social issues. - The Environmental Assessment report addresses baseline environmental and social conditions, requirements under host country laws and regulations, applicable international treaties and agreements, sustainable development and use of renewable natural resources, protection of human health, cultural properties, and biodiversity, including endangered species and sensitive ecosystems, use of dangerous substances, major hazards, occupational health and safety, fire prevention and life safety, socio-economic impacts, land acquisition and land use, involuntary resettlement, impacts on indigenous peoples and communities, cumulative impacts of existing projects, the proposed project, and anticipated future projects, participation of affected parties in the design, review and implementation of the project, consideration of feasible environmentally and socially preferable alternatives, efficient production, delivery and use of energy, pollution prevention and waste minimization, pollution controls (liquid effluents and air emissions) and solid and chemical waste management. - Based on the Environmental Assessment, Equator banks then make agreements with their clients on how they mitigate, monitor and manage those risks through an 'Environmental Management Plan'. Compliance with the plan is required in the covenant. If the borrower doesn't comply with the agreed terms, the bank will take corrective action, which if unsuccessful, could ultimately result in the bank canceling the loan and demanding immediate repayment. - For risky projects, the borrower consults with stakeholders (NGO's and project affected groups) and provides them with information on the risks of the project. - If necessary, an expert is consulted. The Principles only apply to projects over 50 million US dollars, which, according to the Equator Principles website, represent 97% of the total market. In early 2006, the financial institutions behind the Principles launched stakeholder consultations and negotiations aimed at revising the principles. The draft revised principles were met with criticism from NGO stakeholders, who in a joint position paper argued that the draft fails by ignoring the most serious critiques of the principles: a lack of consistent and rigorous implementation.

교육효율성 측면에서의 지방자치단체 간 교육격차의 비교·분석 : 자료포락분석(DEA) 기법의 적용 (An Analysis of the Educational Gap between Local Governments : An Application of Data Envelopment Analysis(DEA))

  • 본 연구는 교육의 투입요소와 산출요소를 포괄적으로 고려할 수 있는 자료포락분석(DEA) 기법을 적용하여, 전국 250개 지방자치단체의 소재지별 및 규모별 교육격차를, 교육효율성의 측면에서 비교 분석해 보고자 하는 것이다. 분석을 위한 투입요소로는 교사1인당 학생수, 학급당 학생수, 직원1인당 학생수, 학생1인당 발전기금을 선정하였으며, 산출요소로는 대학진학률과 취업률을 선정하였다. 분석에 필요한 자료는 학교알리미를 통해서 수집하였으며, 대상학교는 대학진학률과 취업률을 산출요소로 사용하기 때문에 초등학교와 중학교는 제외하고 고등학교만을 대상으로 하였다. 연구결과, 지방자치단체의 소재별로 수도권과 비수도권을 비교해 본 결과, 교사1인당 학생수, 학급당 학생수, 직원1인당 학생수 등 주요 투입요소의 측면에서 수도권 지방자치단체 관내 학교의 과밀도가 높게 나타났다. 반면에 산출요소인 대학진학률과 취업률은 비수도권 지방자치단체 관내 학교들이 보다 높게 나타났으며, 효율성 점수 역시 BCC모형과 CCR모형 모두에서 비수도권 지방자치단체 관내 학교들이 수도권 지방자치단체 관내 학교들보다 높게 나타났다. 다음에 지방자치단체 규모별 교육효율성의 차이를 분석한 결과는 인구가 적은 농촌형 지방자치단체가 대도시형 지방자치단체에 비해서 투입요소의 과밀도가 낮으며, 대학진학률과 취업률은 높게 나타났다. 그리고 교육효율성 측면에서도 대도시형이나 도농복합형보다 농촌형 지방자치단체 소재 학교들의 교육효율성이 높게 나타났다.
