The Analysis of Attributive Level of District Image for City Image - Focus on Busan City - (도시 이미지에 대한 지구 이미지의 기여수준 분석 - 부산시를 중심으로 -)

  • This article statistically analyzed contributive levels of district image based on an effect and a similarity index through the evaluation of citizens and suggested the efficient management system of a city image according to the results. For this study, Busan City was selected as a case city by the preceding literature and was investigated concerning district image and city image through a questionnaire. The new evaluation method for analysis of a city image was presented in this process. The results of this research are as follows: 1. Busan City has a substantial positive and culturally unique image, and each of its districts have other image characteristics. for example, the CBD district has a positive image, and the sea shore district has a busy and prosperous image, but the backward sea shore district has an image of stagnancy. 2. The image of Yeonje-gu has the largest effect on the image of Busan. Next in influence are Jung-gu, Saha-gu, Suyoung-gu, respectively. The effect index is closely connected with the variance of evaluative adjectives. 3. Busanjin-gu and Haeundae-gu have similar images to Busan City. Next in similarity are Nam-gu, Jung-gu, Youngdo-gu, Suyoung-gu, respectively. The similarity index is closely connected with the correlation of evaluative adjectives. Busan City and its districts can establish their image strategies with the above analyzed results. This study is meaningful in that a statistical evaluative method was proposed. With continued follow-up research, this study may serve as a systematic and logical model to improve the urban landscape and image.

A Study on the Users' Awareness of Waterfront in Japan - Focusing on Yokohama Minatomirai21 area - (일본 워터프런트 공간에 있어서 수변공간의식에 대한 연구 ~요코하마 미나토미라이21지구를 대상으로~)

  • Using a questionnaire, this study examined the users' consciousness through the human five senses on the waterside space in the district of Minato Mirai 21, Yokohama located in the east of Kanagawa prefecture, which is well known for its waterfront in Japan. Among the users, the results of the questionnaire showed 52.2% of male, and 29.6% accounted for people in their twenties as the dominant groups. Out of the nineteen questions regarding the consciousness on the waterside space, 'feel the sea breeze' accounted for 237(78.7%) while 'the sea is visible' accounted for 217(72.1%). Then, the contents of the nineteen questions were re-classified into such seven items as the human five senses(sight, hearing, touch, taste, and smell), activities, and the existence of facilities, discovering the consciousness of the users. The results demonstrated that touch(22.9%) and sight(20.8%) respectively were the most influential items.

A Study on the use of Cinnamomum cassia in Dongeuibogam Prescription (동의보감 처방 중 계류(桂類) 약재(藥材) 활용에 대한 고찰)

  • Objectives : To study the use of Cinnamomum cassia among the 394 prescriptions listed in Donguibogam Methods : After selecting a total of 394 prescriptions, 284 prescriptions with Cinnamon Cassia as the protagonist were analyzed and summarized in the table. In particular, the investigation was conducted by comparing the Cinnamomi Ramulus and Cinnamomi Cortex. Results : The Cinnamomum cassia is used in 52 fields of treatment such as common cold & flu diseases, abscess and stroke, etc. Prescriptions containing Cinnamomi Ramulus were most commonly used for cold disease. And prescriptions containing Cinnamomi Cortex were most commonly used for wind diseases. Prescriptions that utilize Cinnamomum cassia are used in the treatment of feeling of cold, numbness, low back pain, cough and so on. Prescriptions containing Cinnamomi Ramulus mainly treat cold disease. And prescriptions containing Cinnamomi Cortex treat mainly abdominal pain and diarrhea. 14 prescriptions of the same name are included with Cinnamomum cassia. The other unclassified Cinnamomum cassia is considered suitable for use with Cinnamomi Cortex. Cinnamomum cassia is only used as a raw material. Conclusions : According to the classification of Cinnamomum cassia, further study of prescription utilization is needed.

Analysis of Radiological and Clinical Results in Treatment of Open Segmented Tibia Fractures: A Comparison between Intramedullary Nailing and Minimal Invasive Plate Osteosynthesis

  • Purpose: The purpose of this study was to compare the data comprehensively including not only the clinical and radiographic outcomes but some parameters related to operation between the minimally invasive plate osteosynthesis (MIPO) technique and intramedullary nailing (IMN) for treatment of segmental tibia shaft fractures. Methods: We conducted a retrospective study of 31 patients (mean age, 49.3 years, range, 27-74 years), with a mean follow-up of 14.1 months (range, 12-19 months) with acute segmental tibial fractures (AO 42-C2) who underwent either surgical treatment of MIPO or IMN. In accordance with the Gustilo-Anderson classification, 11 were type I, 5 were type II, and 15 were type III. Initial compartment syndrome was confirmed in 2 cases. Results: There were no statistically significant differences in terms of the patient demographic data between the two groups. The time to definitive fixation was longer in the MIPO group (mean $13.7{\pm}10.9days$; range, 2-27) than in the intramedullary group (mean $5.4{\pm}9.6days$; range, 0-35) with statistically significant difference (p=0.002). Bony union was observed in most of cases but except 5 cases of nonunion were diagnosed (3 in the MIPO vs 2 in IMN, p=0.188). The average bone healing time was 27.1 weeks (10 to 56 weeks) in MIPO group and 23.2 weeks (13 to 66 weeks) in IMN group, respectively (p=0.056). Overall complications were 5 cases in MIPO group and 2 cases in the IMN group. Difference in LEFS was not statistically significant between both groups (p=0.824). Conclusion: This study showed that segmental tibia shaft fractures treated with both MIPO and intramadullary nailing was challenging with relatively high complication rate. A well planned sequential strategy with keeping the soft tissue and personality of fracture in mind is utmost significant as much as the choice of surgical modalities.

  • Synovial sarcoma is a malignant soft tissue tumor that most commonly occurs in the extremities of young and middle-aged adults, in the vicinity of large joints. Although synovial sarcoma is frequently associated with joints, it may arise in unexpected sites, such as the mediastinum, heart, lung, pleura, or chest wall. Primary synovial sarcoma of the pleura is rare. To date, nearly 36 cases of primary synovial sarcoma of the pleura have been reported since Gaertner et al. published the first case in 1996. The oncologic characteristics, treatment, and prognosis for pleural synovial sarcomas are not well defined because of a paucity of data. However, a multimodal approach, including surgical resection, chemotherapy, and radiotherapy, has generally been suggested. We report the outcome of one patient with primary pleural synovial sarcoma treated with radical resection and adjuvant treatment.

A Study on Flood Area Typology Using the Inundation Trace Map - A Case Study of Busan Metropolitan City - (침수흔적도를 활용한 침수지역 유형화에 관한 연구 -부산광역시를 사례로-)

  • Recently, expected casuality or property damage, such as flood damage, environmental destruction, etc, is increased because of the local heavy rain or flood by climatic changes. Especially, there are lots of property damages caused by increase of flood frequency and expansion of flood area because impervious rate is increased by urban sprawling development. Therefore, this research intends to analyze reasons of flood damage by each form and suggest protective measures through the typology of flood area for Haeundae-gu, Suyoung-gu, Saha-gu in Busan Metropolitan City with Busan's inundation trace map which has made from 2009. To achieve them, flood area was divided into water flooding type, river flooding type and seawater flooding type. And, protective measures focusing on previous prevention gave the relationship by dividing into managing system aspect, infrastructure building aspect and system aspect, so it approached flooding problems more objectively and scientifically. And, this research intends to suggest vitalizing measures for the utilization of inundation trace map in the future.

A case of pseudohypoaldosteronism type 1 with a mutation in the mineralocorticoid receptor gene

  • Pseudohypoaldosteronism type 1 (PHA1) is a rare form of mineralocorticoid resistance characterized in newborns by salt wasting with dehydration, hyperkalemia and failure to thrive. This disease is heterogeneous in etiology and includes autosomal dominant PHA1 owing to mutations of the NR3C2 gene encoding the mineralocorticoid receptor, autosomal recessive PHA1 due to mutations of the epithelial sodium channel (ENaC) gene, and secondary PHA1 associated with urinary tract diseases. Amongst these diseases, autosomal dominant PHA1 shows has manifestations restricted to renal tubules including a mild salt loss during infancy and that shows a gradual improvement with advancing age. Here, we report a neonatal case of PHA1 with a NR3C2 gene mutation (a heterozygous c.2146_2147insG in exon 5), in which the patient showed failure to thrive, hyponatremia, hyperkalemia, and elevated plasma renin and aldosterone levels. This is the first case of pseudohypoaldosteronism type 1 confirmed by genetic analysis in Korea.

Identification and Characterization of Alginate Lyase Producing Pseudomonas sp. N7151-6 (Alginate Lyase 생산 균주 Pseudomonas sp. N7151-6의 분리 및 특성)

  • A Gram-negative, alginate lyase-producing bacterium was isolated from the Haeundae Coast, Korea. The isolated strain N7151-6 produced alginate lyase. The optimal temperature and pH for growth were found to be $30^{\circ}C$ and pH 8.0, respectively. This strain can be grown at the NaCl concentration of 0-7% (w/v). Analysis of 16S rDNA sequence and physiological profiling indicated that the strain N7151-6 belonged to Pseudomonas sp. The enzyme alginate lyase produced by Pseudomonas sp. N7151-6 was partially purified by ultrafiltration (MWCO= 30 kDa). The optimum pH and temperature for the activity of the purified enzyme were found to be 7.0 and $30^{\circ}C$, respectively. The enzyme was stable at the pH range of 5.0-9.0 and temperature range of $23-30^{\circ}C$. The total activity of alginate lyase produced was reached about 110 unit/L.

Intra-Arterial Thrombolysis Using Double Devices: Mechanicomechanical or Chemicomechanical Techniques

  • Objective : To optimize the recanalization of acute cerebral stroke that were not effectively resolved by conventional intraarterial thrombolysis (IAT), we designed a double device technique to allow for rapid and effective reopening. In this article, we describe the feasibility and efficacy of this technique. Methods : From January 2008 to September 2009, twenty patients with acute cerebral arterial occlusion (middle cerebral artery : n=12; internal carotid artery terminus : n=5; basilar artery : n=3) were treated by the double device technique. This technique was applied when conventional thrombolytic methods using drug, microwires, microcatheters and balloons did not result in recanalization. In the double device technique, two devices are simultaneously placed at the lesion (for example, one microcatheter and one balloon or two microcatheters). Chemicomechanical or mechanicomechanical thrombolysis was performed simultaneously using various combinations of two devices. Recanalization rates, procedural time, complications, and clinical outcomes were analyzed. Results : The initial median National Institute of Health Stroke Scale (NIHSS) was 16 (range 5-26). The double device technique was applied after conventional IAT methods failed. Recanalization was achieved in 18 patients (90%). Among them, 55% (11 cases) were complete (thrombolysis in cerebral infarction 2B, 3). The median thrombolytic procedural time including the conventional technique was $135{\pm}83.7$ minutes (range 75-427). Major symptomatic hemorrhages (neurological deterioration ${\geq}4$ points in NIHSS) developed in two patients (10%). Good long term outcomes (modified Rankin Scale ${\leq}2$ at 90 days) occurred in 25% (n=5) of the cases. Mortality within 90 days developed in two cases (10%). Conclusion : The double device technique is a feasible and effective technical option for large vessel occlusion refractory to conventional thrombolysis.

A Case of Cauda Equina Syndrome in Early-Onset Chronic Inflammatory Demyelinating Polyneuropathy Clinically Similar to Charcot-Marie-Tooth Disease Type 1

  • To present a case of cauda equina syndrome (CES) caused by chronic inflammatory demyelinating polyneuropathy (CIDP) which seemed clinically similar to Charcot-Marie-Tooth disease type1 (CMT1). CIDP is an immune-mediated polyneuropathy, either progressive or relapsing-remitting. It is a non-hereditary disorder characterized by symmetrical motor and sensory deficits. Rarely, spinal nerve roots can be involved, leading to CES by hypertrophic cauda equina. A 34-year-old man presented with low back pain, radicular pain, bilateral lower-extremity weakness, urinary incontinence, and constipation. He had had musculoskeletal deformities, such as hammertoes and pes cavus, since age 10. Lumbar spine magnetic resonance imaging showed diffuse thickening of the cauda equina. Electrophysiological testing showed increased distal latency, conduction blocks, temporal dispersion, and severe nerve conduction velocity slowing (3 m/s). We were not able to find genetic mutations at the PMP 22, MPZ, PRX, and EGR2 genes. The pathologic findings of the sural nerve biopsy revealed thinly myelinated nerve fibers with Schwann cells proliferation. We performed a decompressive laminectomy, intravenous IgG (IV-IgG) and oral steroid. At 1 week after surgery, most of his symptoms showed marked improvements except foot deformities. There was no relapse or aggravation of disease for 3 years. We diagnosed the case as an early-onset CIDP with cauda equine syndrome, whose initial clinical findings were similar to those of CMT1, and successfully managed with decompressive laminectomy, IV-IgG and oral steroid.
