Characteristics of Sediment Transport under External Force in Haeundae Beach (해운대 해빈에서 특정 외력하의 표사이동 특성)

  • The width of Haeundae beach has been decreasing annually due to the loss of sand from land, high waves, reflected waves, etc.. The accurate prediction of wave-induced currents is indispensible to analyze the beach deformation due to the sediment transport. In the this study, Numerical experiments were performed with seasonal representative wave on the basis of a long term and comprehensive survey data. In summer, we found the deposition of Mipo by longshore current eastly by ordinary and S waves, and in winter, the deposition of Dongback-island by longshore current westly by ordinary and E waves. In addition, rip current occurs in the middle of Haeundae beach by 50 year return period wave and current westly along the coast by ESE directional wave.

A Prospective Randomized Trial Comparing Manual Needle Aspiration to Closed Thoracostomy as an Initial Treatment for the First Episode of Primary Spontaneous Pneumothorax

  • Background: Variation exists in the initial treatment for the first episode of primary spontaneous pneumothorax (PSP), and no definitive consensus exists due to a lack of high-quality evidence. This study examined the outcomes of needle aspiration and closed thoracostomy in first episodes of PSP requiring intervention. Methods: This study was a randomized, prospective, single-center trial conducted between December 2015 and August 2016. Patients of all ages with a documented first episode of PSP who were unilaterally affected, hemodynamically stable, and had a pneumothorax measuring over 25% in size were included. Patients with underlying lung disease, severe comorbidities, bilateral pneumothorax, tension pneumothorax, recurrent pneumothorax, traumatic pneumothorax, and pregnancy were excluded. Patients were randomly assigned to the needle aspiration or closed thoracostomy group using a random number table. Results: Forty patients with a first episode of PSP were recruited, and 21 and 19 patients were included in the needle aspiration group and the closed thoracostomy group, respectively. The hospital stay of each group was $2.1{\pm}1.8days$ and $5.4{\pm}3.6days$, respectively (p<0.01). However, no significant differences were found in the success rate of initial treatment or the 1-month and 1-year recurrence rates. Conclusion: Needle aspiration is a favorable initial treatment in patients experiencing a first episode of PSP.

Optimal harvest and efficient use of septal cartilage in rhinoplasty

  • Background: Nasal septal cartilage is used to obtain favorable aesthetic and functional outcomes in rhinoplasty, but is often difficult to harvest or the harvested amount is insufficient. Therefore, the objective of this study is to introduce how to harvest septal cartilage optimally without losing and use harvested cartilage efficiently. Methods: From March 2015 to January 2020, we tried to harvest as much septal cartilage as possible while maintaining the L-strut in 30 patients. A spreader flap and septal rotation suture were used instead of a spreader graft. Also in patients who needed a spreader graft and septal extension graft, a spreader graft was used on one side and a one-piece spreader graft combined with a septal extension graft was performed on the other side. For tip plasty, a columella septal suture was performed first. Postoperative patient satisfaction was assessed using the Rhinoplasty Outcome Examination questionnaire. Results: No serious complications were observed. The patient satisfaction score was 50% or above in 27 patients (90%) and less than 50% in only three patients (10%). The average score was 81.5 points. Conclusion: For septal cartilage deficiency, a spreader flap, the septal rotation suture, or onepiece spreader graft combined with a septal extension graft was used. The nasal tip was sufficiently rotated using the columellar septal suture technique first. These techniques made it possible to obtain good aesthetic outcomes using only septal cartilage, without harvesting other cartilage.

Clinical association between serum cholesterol level and the size of xanthelasma palpebrarum

  • Background: Xanthelasma palpebrarum (XP) is a benign periorbital lesion. The relationship between xanthelasma lesion size and serum cholesterol levels has been poorly studied. In this study, we investigated this relationship in the context of the clinical etiology of XP. Methods: We retrospectively reviewed medical records and pathology reports of all patients treated for XP at our hospital between June 2014 and June 2021; the data were used to analyze lesion size, underlying disease, serum cholesterol, and disease recurrence. Results: The mean values for patient age, serum cholesterol, and lesion size were 53.0 years, 235.0 mg/dL, and 69.2 mm2, respectively. XP mainly occurred in women (64.7%); furthermore, the incidence of XP and lesion size was greatest among patients in their 5th decade of life (41.2%). There was no statistically significant relationship between xanthelasma lesion size and serum cholesterol level. Conclusion: This study compared lesion size with various clinical features in XP patients. In patients who underwent surgery for XP, serum cholesterol levels tended to be higher than those in the general population. However, the trend between the size of XP and serum cholesterol level was unclear. Therefore, if a patient with XP visits the hospital for surgery, it is recommended to check the lipid profile to confirm underlying dyslipidemia regardless of the size.

A numerical study on rip currents at the Haeundae coast changed after the beach nourishment (양빈 후 지형변화에 따른 해운대 이안류 수치모의)

  • The Haeundae beach and coast suffered synthetical dramatic changes especially in the nearshore topography by the beach nourishment project (i.e., 2013-2015). A previous study showed the rip current characteristics were changed according to the topographical change in terms of their magnitudes and likelihoods through numerical simulations using the FUNWAVE model. The magnitude and likelihood of rip currents decreased because the surfzone width decreased just after the beach nourishment. In this study, however, numerical simulations of the Haeundae rip currents were performed by using the 2017- and 2020-surveyed topographies changed for several years after the beach nourishment. From the simulation results using the topographies surveyed before and after the beach nourishment, it was found that the magnitude and likelihood of rip currents increased due to its increasing surfzone width in 2017 and 2020 and they might be increasing larger than those before the beach nourishment.

Are there network differences between the ipsilateral and contralateral hemispheres of pain in patients with episodic migraine without aura?

  • Background: We aimed to identify any differences in the structural covariance network based on structural volume and those in the functional network based on cerebral blood flow between the ipsilateral and contralateral hemispheres of pain in patients with episodic migraine without aura. Methods: We prospectively enrolled 27 patients with migraine without aura, all of whom had unilateral migraine pain. We defined the ipsilateral hemisphere as the side of migraine pain. We measured structural volumes on three-dimensional T1-weighted images and cerebral blood flow using arterial spin labeling magnetic resonance imaging. We then analyzed the structural covariance network based on structural volume and the functional network based on cerebral blood flow using graph theory. Results: There were no significant differences in structural volume or cerebral blood flow between the ipsilateral and contralateral hemispheres. However, there were significant differences between the hemispheres in the structural covariance network and the functional network. In the structural covariance network, the betweenness centrality of the thalamus was lower in the ipsilateral hemisphere than in the contralateral hemisphere. In the functional network, the betweenness centrality of the anterior cingulate and paracingulate gyrus was lower in the ipsilateral hemisphere than in the contralateral hemisphere, while that of the opercular part of the inferior frontal gyrus was higher in the former hemisphere. Conclusions: The present findings indicate that there are significant differences in the structural covariance network and the functional network between the ipsilateral and contralateral hemispheres of pain in patients with episodic migraine without aura.

A Case Report of Complex Korean Medical Treatments, Including Shihogayonggolmoryo-tang, for Sleep Disorders Accompanied by a Herniated Intervertebral Disc (한방병원에 입원한 추간판탈출증을 동반한 수면장애 환자에 대한 시호가용골모려탕을 포함한 복합 한의치료 1례 증례보고)

  • Objectives: We describe the clinical course in a 53-year-old female patient admitted to a Korean Medicine Hospital with insomnia disorder accompanied by a herniated intervertebral disc and managed with Korean medical treatments. Methods: The patient was treated with herbal medicines, including Shihogayonggolmoryo-tang, acupuncture, pharmacopuncture, and Chuna manual therapy for 33 days. The therapeutic effect was evaluated based on the Insomnia Severity Index, Numerical Rating Scale score, Oswestry Disability Index, and European Quality of Life 5 Dimensions Scale. Results: Post-treatment, the Insomnia Severity Index decreased from 21 to 6, the Numerical Rating Scale score decreased from 7 to 3, the Oswestry Disability Index decreased from 60 to 18, and the European Quality of Life 5 Dimensions Scale increased from 0.35 to 0.85. Conclusions: Korean medical treatments improved the symptoms of this patient with insomnia disorder accompanied by a herniated intervertebral disc. Further clinical research on patients with insomnia disorder is needed.

Treatment of a Carotid-Cavernous Sinus Fistula via the Superior Ophthalmic Vein Approach: A Case Report (상안정맥을 이용한 해면정맥동루의 색전술 치험례)

  • Purpose: Arteriovenous fistulas that involve the cavernous sinus often produce ophthalmologic symptoms and signs. Transvenous endovascular access is the method of choice for a carotid-cavernous sinus fistula. The superior ophthalmic vein is a safe and reasonable alternative route for the transvenous embolization of carotid-cavernous sinus fistula. We report a case of the embolization of a carotid-cavernous sinus fistula using the superior ophthalmic vein approach. Methods: A 58 year old female had conjunctival congestion, periocular pain and diplopia with a 2 month duration. Diagnostic orbital CT, brain MRI and cerebral angiography revealed a carotid-cavernous sinus fistula. The fistula occlusion was treated by coil embolization using the superior ophthalmic vein approach. Results: The initial presenting symptoms, conjunctival congestion, periocular pain and diplopia, decreased after surgery. Coil embolization via the superior ophthalmic vein approach was difficult because of the venous tortuosity and friability. During the follow up period, the patient was in a good condition without complications. Conclusion: Surgical exposure of the superior ophthalmic vein provides direct venous access to the cavernous sinus as well as an effective and safe treatment approach. The cooperation of the plastic surgeon and interventionist is a factor in successful treatment.

  • Based on the previous studies that argued Brand Personality Scale(BPS) developed by J.Aaker(1997), the results are not shown very suitable for all cases. Authors found out more suitable Brand personality Scale for Marine Tourism Destination. This study then is to verify a newly developed Marine Tourism Destination BPS, to apply the multiple regression to BPS and images for Haeundae, and then to confirm the relationship between a newly developed BPS and Images proven in the previous study.

Case Report of Patients Diagnosed with Epiphora Improved by Traditional Korean Medical Treatment and Auricular Acupuncture (이침치료를 동반한 한방치료로 호전된 유루증 환자 치험 1례)

  • Objectives : The purpose of this study is to report the effect of traditional Korean medical treatment and auricular acupuncture on the patient suffered from epiphora. Methods : One patient suffering from epiphora was treated with traditional Korean medical treatment including auricular acupuncture. HRQL(Health-related quality of life) and VAS(Visual Analogue Scale) were measured and compared to evaluate its treatment effectiveness. Results : We attained results which show recovery from watery eye, pain, eye strain in symptoms and improvement on quality of life. Conclusions : In this study, traditional Korean medical treatment can be effective in obtunding symtoms and improving quaility of life for patients suffering from epiphora.
