A Comparison of the Performances of Confrontation Naming Test and Verbal Fluency Task in Patients with Prodromal Alzheimer's Disease and Mild Alzheimer's Disease (노인성 알츠하이머병 위험군과 초기 알츠하이머병 환자의 이름대기와 구어유창성 능력의 비교)

  • We identified the characteristic impairmants of linguistic semantic memory in patients with prodromal Alzheimer's disease(AD) and mild AD. To elucidate the earliest changes of semantic language function in subjects with AD, performances on confrontation naming test and verbal fluency task were compared among patients with AD patients (n=20), mild AD patients (n=27) and healthy elderly controls (n=20). Tasks in this study included the confrontation naming test of Test of Lexical Processing in Aphasia(TLPA/Japanese) and one-minute verbal fluency task (semantic/ phonetic categories). The results were as follows: 1) Performances of the prodromal AD group showed the comparable to those of the control group on the confrontation naming test, 2) In the semantic/phonetic verbal fluency tasks, the performances of the control group were better than those of the prodromal AD and mild AD groups, but no significant differences were shown between the prodromal AD and the mild AD group.

Effects of a Sea Breeze Walking Program on Respiratory Health in Patients with Metabolic Diseases (대사질환자의 해풍 걷기 프로그램이 호흡건강에 미치는 영향)

  • Purpose : This study aims to evaluate the effectiveness of a sea breeze walking program by analyzing the metabolic disease-related, immune-inflammation-related, and respiratory health-related variables of the test subjects associated with improved respiratory health. Methods : In the experiment, 30 patients with metabolic diseases were divided into an experimental group (n=15) and a control group (n=15). The experimental group walked on the Namparang-gil, Geoje trail, while the control group walked on the Hoeya-cheon, Yangsan trail. Both groups participated in the same walking program for two hours, twice a week for four weeks. Thereafter, the metabolic disease-related, immune inflammation-related, and respiratory health-related variables were measured and compared between the two groups. Results : After the four-week sea breeze walking program, in terms of changes in the metabolic disease-related variable, a statistically significant interactive effect was found in waist circumference (p<.001). The experimental group showed a significant decrease in waist circumference after the program. After the four-week sea breeze walking program, the control group showed a statistically higher increase in lactic acid (p.<05), whereas the experimental group exhibited a decrease in lactic acid. For the respiratory health-related variables, no statistically significant differences were found after the sea breeze walking program. However, the experimental group showed an increase in FEV1,while the control group showed a decrease in FEV1. For the maximum oxygen intake, no statistically significant interactive differences were found but there was a statistically significant effect in time (p<.05). The two groups exhibited an increase in maximum oxygen intake. Conclusion : After the sea breeze walking program, positive physical changes were observed in the metabolic disease-related and immune inflammation-related variables.

The Clinical Experience of 1000 Cases in Open Heart Surgery (개심술 1000예의 임상적 고찰)

  • From May 1977 to April 1990, 1000 cases of open heart surgery were performed under the cardiopulmonary bypass. There were divided into group A[500 cases, from May 1977 to June 23rd 1986] and group B[500 cases, from June 23rd 1986 to April 1990] which were analyzed, summarized, and compared. Of 1000 cases of open heart surgery, 677 cases[67.7%] were congenital heart diseases, and 323 cases[32.3%] were acquired heart diseases. In 677 cases of congenital heart disease, there were 545 cases with acyanotic congenital heart anomaly[A: 279, B: 266] and 132 cases with cyanotic congenital heart anomaly[A: 87, B: 45]. In 323 cases of acquired heart disease, most cases were valvular heart disease[315 cases, A: 133, B: 182] in which 357 valves were implanted. The operative mortality of congenital R acquired heart disease was 7.8% & 8.0%, and then overall mortality was 7.9%. The operative mortality was markedly decreased. [about half] from 10.0% of A group to 5.6% of B group.

Effects of a Nurse Presence Program on Suicide Prevention for Elders with a Chronic Disease (간호사의 함께있음 프로그램이 만성질환 노인의 자살예방에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of a nurse presence program on suicide prevention for elders with chronic disease. Method: The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Twenty subjects in the control group received no intervention and nineteen subjects in the experimental group received a nurse presence program. Result: There were more significant decreases in suicide ideation, and the cortisol level and increases in life satisfaction in the experimental group compared to the control group. Conclusion: According to the above results, a nurse presence program for elders with a chronic disease decreased stressful events like suicide ideation and increased self esteem through therapeutic interaction. These findings suggest that this program can be used as an efficient independent nursing intervention for elders in a critical situation.

  • The survey was conducted to investigate several factors affecting the disease outcome with 116 infants aged 10 to 24 months residing in Kunsan city, Cheonbuk province . General characteristics, weaning practice , nutrient intake and the actual state for affecting disease were studied. Among many factors, mother's educational status was found to be the most influencing factor for affecting the disease outcome analyzed by oneway ANOVA. The exposure index disease, cold and diarrrhea, against mother's education were analyzed to find out the major factors for disease outcome. The education group up to middle school graduates, mother's job , nutrients supplements, feeding method, sex of baby were the factors, for the high school graduates, job , nutrients supplements were the causes, and the group graduated from the college the above grade mother's health state was the most important factor for the baby exposing to the disease. The disease outcome decreased when the bottle feeding was replaced by breast feeding, sufficient nutrients supplementation was recommended , and health care for mother during pregnancy was strongly advised.

Study on the Obesity and Blood parameters Differences between Fire/Heat and Qi-deficiency Pattern Identification/Syndrome Differentiation among Acute Stroke Patients (급성기 중풍환자에서 비만 및 혈액지표의 기허 및 화열 변증의 차이에 대한 고찰)

  • Object : In the present study, we investigated the obesity and blood parameters between Qi-deficiency and Fire/Heat pattern identification/syndrome differentiation (PI/SD) in acute stroke patients. Materials and Methods : A total of 391 stroke patients within 7 days after onset were consecutively recruited from 12 hospitals across South Korea from Nov. 1st, 2006 to Jun. 31st, 2009. They were diagnosed as Fire/Heat or Qi-deficiency among five PI/SD subtypes by two independent stroke experts. We investigated the differences of obesity and blood characteristics between Fire/Heat and Qi-deficiency by statistical analyses. Results : In male subjects, obesity was significantly associated with Fire/Heat PI/SD. The averaged mean BMI ($24.13kg/m^2$) and waist circumference(89.34cm) of the Fire/Heat group were higher than those of the Qi-deficiency group ($22.60kg/m^2$ and 83.43 cm, respectively). The number of obese patients was larger in the Fire/Heat group than in the Qi-deficiency group (p = 0.001). Hyperlipidemia was also related with Fire/Heat. However, obesity was not associated with PI/SD in female subjects where the number of hyperlipidemic patients was higher in the Qi-deficiency group. Among blood parameters, the levels of triglycerides and fasting blood sugar were higher in the Fire/Heat group compared with the Qi-deficiency group in male subjects. However, total cholesterol of the Qi-deficiency group was higher than in the Fire/Heat group among female subjects. Conclusion : This study shows that obesity and hyperlipidemia are significantly difference between Qi-deficiency and Fire/Heat. We suggests that PI/SD may be associated with clinical characteristics and large population study between PI/SD and clinical characteristics including blood parameters are needed.

The relation of Dampness-Phlegm Diagnosis and interanl carotid artery stenosis by carotid artery sonography in cerebral infarction patients (경동맥 초음파를 이용한 뇌경색 환자의 내경동맥 협착도와 습담변증(濕痰辨證)의 관련성 연구)

  • Objectives : This study was aimed to clarify the relationship between the dampness-phlegm diagnosis and internal carotid artery stenosis by measuring carotid artery sonography in cerebral infarction patients. Methods : One hundred eighty subjects were recruited from the patients admitted to the Department of Internal Medicine at Kyunghee university oriental medical center from September 2008 to July 2010. We assessed one hundred eighty patients' carotid artery sonography data and diagnosed dampness-phlegm by oriental medical diagnosis. then, analyzed their characteristics, risk factor, lifestyle, metabolic syndrome, body mass index, Waist/Hip ratio(W/H ratio) and dampness-phlegm diagnosis. Results : On the demographic variables of the patients, age, smoking, W/H ratio and dampness-phlegm group were significantly higher in severe internal carotid artery(ICA) stenosis group than in the control group. According to the significant difference in dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by ICA stenosis. As a result, sputum, bowel sound, chest discomfort, slippery pulse were significantly higher in the severe ICA stenosis group than in the control group. In multivariate analysis, dampness-phlegm group showed close relationship with severe ICA stenosis group. Conclusion : According to the analysis, significance between dampness-phlegm diagnosed patients group and severe ICA stenosis were clarified. These results can be utilized in the future as a basis material.

The Study on the Effect of Acupuncture Treatment in Patients with Idiopathic Parkinson's Disease (특발성 파킨슨 환자에서 경혈(經穴)에 따른 침(鍼) 치료 효과의 비교 연구)

  • Objectives : This study was designed to evaluate the effect of acupuncture with various scales on symptoms of Idiopathic Parkinson's disease. Methods: Subjects were voluntarily recruited through newspapers and internet advertisement. All the subjects were confirmed as idiopathic parkinson's disease by a neurologist. The acupuncture therapy was performed twice a week for 4 weeks by oriental medical doctor at Kyung-hee University hospital. Patients were randomly assigned to three groups : sham acupuncture group, acupuncture treatment group and Sasang-Constitution acupuncture treatment group. Acupun cture points used in acupuncture treatment group were $GB_{34}$, $LR_3$, and $ST_{36}$, which were proven to be effective in rat Parkinson's disease study. Acupuncture points used in sham treatment were non-acupoints near $GB_{34}$, $LR_3$, and $ST_{36}$. In Sasang-Constitution acupuncture treatment, we classfied Sasang constitution of patients by QSCCII. Acupuncture was applied based on theory of Korean medicine. The patient's symptoms were assessed at baseline and after 4 weeks of treatment by one evaluator with Unified Kingdom Parkinson's Disease Rating Scale (UPDRS), modified Hoehn-Yahr(H-Y) stage, and Schwab & England activ ity of daily living and freezing of gait questionnaire (FOGQ). Results : The results were as follows ; 1. UPDRS IV score differences between sham group and Sasang-Constitution acupuncture treatment were statistically significant(p=0.001). 2. UPDRS total score differences between Sasang-Constitution acupuncture treatment and acupuncture treatment group, Sasang-Constitution acupuncture treatment and sham group were statistically significant(p=0.041, p=0.014). 5. FOGQ score differences between Sasang-Constitution acupuncture treatment and acupuncture treatment group, Sasang-Constitution acupuncture treatment and sham group were statistically significant(p=0.001, p=0.001). Conclusion : The results suggest Sasang-Constitution acupuncture treatment can be applicable to improve symptoms in patients with idiopathic Parkinson's disease.

Plasma Total Homocysteine, Folate and Vitamin $B_{12}$ Concentrations in Patients with Coronary Stenosis (관상동맥협착 환자의 혈장 총 Homocysteine, 엽산 및 비타민 $B_{12}$ 농도)

  • Recently elevated plasma homocysteine concentration is considered an independent risk factor for atherosclerosis and thrombosis with coronary artery disease. Folate and vitamin $B_{12}$ are cofactors and closely related with metabolism of homocysteine. The purpose of this study is to evaluate the correlation between homocysteine and folate and vitamin $B_{12}$ in patients with ischemic heart disease. Twenty-six patients, in whom coronary angiographic finding revealed more than $50\%$ of stenosis at least in one coronary vessel were enrolled as the patient group, and thirty subjects, in whom angiographic finding revealed in not significant stenosis, but complained of chest pain, were selected as the control group. Fasting venous blood was obtained and measured the concentration of plasma total homocysteine, folate and vitamin $B_{12}$ by high performance liquid chromatography and fluorescence detection method. We examined the correlation between homocysteine and folate and/or vitamin $B_{12}$ in the control group and the patient group, respectively. Compared with the control group, the patient group had relatively higher plasma total homocysteine concentration ($10.7\pm4.2\;vs\;9.6\pm3.5$ umol/L), but showed no significant difference. Folate and vitamin $B_{12}$ concentration are low in the patient group, but showed no significant difference between patient and control group. Plasma total homocysteine concentration showed negative correlation with folate and vitamin $B_{12}$ in both the control group and the patient group, and showed significantly negative correlation in patient group {r = -0.550 (p < 0.01) vs r = -0.609 (p < 0.01)}. We knew that the plasma total homocysteine concentration were relatively elevated in patient group compared with the control group. Because plasma total homocysteine concentrations are closely negative correlated with folate and vitamin $B_{12}$ in the patient group, folate and vitamin $B_{12}$ supplement can lower the mortality and morbidity of ischemic heart disease. The Effects and Safety of Cardiotonic Pills with Pletaal on Post-Reperfusion Syndrome of Arteriosclerotic Occlusive Disease(Pilot Study)

  • Objectives: The patients who had arteriosclerotic occlusive disease were treated by reperforating procedures or vessel replacing operations. We divided them by two groups. one(control group) is treated by Pletaal(cilostazole), the other(CP group) is by Pletaal with cardiotonic pills(CP). Methods : Control group was treated by Pletaal, CP group was treated by Pretaal with CP for 8 weeks. We primarily evaluated the outcomes by visual analogue scale(VAS) of pain, coldness, numbness, and edema, secondarily hematologic tests. Results : Coldness, numbness and edema were reduced at both groups. In CP group, the VAS dropped more sharply than control group, but not significant. Otherwise, there was significant reduction of VAS on pain. In hematologic tests, there were no abnormal results of all items. Conclusion : The therapy of Pletaal with CP is effective to relieve PRS specially in pain and safety on hematologic tests.
