A Study on the Current Status of Prescribed Drugs in Oriental Health Insurance and their Improvement (한방건강보험 약제 투약 실태 및 활성화 방안 연구)

  • Objective : To investigate the current status of prescription drugs in Oriental medical institutes and to draw up a future plan for the revitalization of Oriental medical health insurance, this survey has been performed. Method : The survey has been made with 321 doctors working at Oriental medical institutes in Daegu and Kyungbuk areas for a period of 3 month from June 1, 2010 until September 1, 2010. Result : 1. When it comes to the current status of the use of herbal drugs in Oriental Health insurance, most of doctors surveyed prescribe insurance drugs, and they prescribe insurance drugs to patients, who are less than 20% of total patients visiting their clinics. 2. The awareness of Herbal Health Care Drugs is investigated. When it comes to the understanding of the difference between insurance drugs(powder type drugs) and granular type drugs, doctors admit that they differ only in one aspect, whether or not their being covered by health insurance. Based on the survey results on the understanding of insurance coverage of granular type drugs, doctors, even though they long for granular type drugs to be accepted as insurance drugs, are worrying whether the number of outpatients might dwindle due to increased insurance co-payments. They also point out that the biggest obstacles in the expansion of the granular type drugs as insurance drugs are the lack of understanding of the government and the objection of the Health Insurance Review and Assesment service (HIRA) for fear of increased insurance claims. 3. Upon investigation on Oriental medicine doctors' understandings of herbal pharmaceutical industry, it is found that doctors' responses on pharmaceutical industry are not all positive ones('new product development and neglect of R&D infrastructure' and 'smallness of industry'). When it is investigated what area needs the greatest improvement in herbal pharmaceutical industry, 'securing sufficient capital, good manufacturing, and strengthening quality control', is the highest. 4. When it is asked what are the most needed in order to improve herbal health insurance medicine, responses such as 'the increase in the accessibility to and the utilization of Oriental medical clinics through the diversification of the means of prescriptions', 'the improvement of insurance benefits(cap adjustments)', 'increase the proportion of high quality medicinal plants', 'the ceiling of co-payments(deductible) at 20,000 won or more', 'expansion of the choices of formulations', 'formulational expansions of tablets and pills', and finally 'admittance and expansion of granular type drug as insurance drug' are the highest. 5. Upon investigating the general characteristics of the current status of the usage of Oriental health care herbal drugs, the followings are observed. First, the frequency of use of health insurance drugs by the doctors who use health insurance with general characteristics shows similar differences in case of total monthly sales amount (p<0.001), average number of daily patients (p<0.05). Secondly, as to the willingness of the expanded usage of insurance drugs, similar differences are observed in case of total monthly sales amount (p<0.05). 6. Upon investigating the general characteristics of the perception of Herbal health care drugs, the followings are observed. First, inspecting general characteristics and insurance claims due to increased co-payments(deductible amount) reveals similar differences in case of working period (p<0.01) and in case of total monthly sales amount (p <0.01). Secondly, inspecting general characteristics and the obstacles that hinder granular type drugs from being accepted as health care insurance drugs shows similar differences in case of working period (p<0.05). 7. Upon investigating the general characteristics of the understanding of Oriental Herbal pharmaceutical companies, the followings are observed. First, opinions on the general characteristics of pharmaceutical companies, when examined with variance analysis, shows similar differences in case of total monthly sales amount (p<0.05). Secondly, when opinions are examined on general characteristics and the problems of herbal pharmaceutical companies, similar differences are found in case of working period (p<0.01) and in case of total monthly sales amount (p<0.001). Lastly, opinions on the general characteristics and reforms of pharmaceutical companies, similar differences are observed in case of working period (p<0.001). 8. Upon investigating the general characteristics of the improvement of insurance Herbal drugs, the followings are observed. First, regarding general characteristics and insurance benefits, similar differences are observed in case of working period (p<0.05), in case of total monthly sales amount (p<0.05), and in case of average number of daily patients (p<0.01). Secondly, opinions on the general characteristics and the needs for the improvement of Herbal insurance drugs are examined in 5 different aspects, which are the approval of granular type drugs as insurance drugs, the expanded practices of the number of prescription insurance drugs, the needs of a variety of formulations, the needs of TFT of which numbers of Oriental medical doctors are members for the revision of the existing system, and the needs of adjusting the current ceiling of the fixed amount and the fixed rate. When processed by the analysis of variance, the results show similar differences in case of average number of daily patients (p<0.01). Conclusion : From the results of this study the first measures to take are, to reform overall insurance benefit system, including insurance co-payment system(fixed rate cap adjustment), to expand the number of the herbal drugs to be prescribed matching with insurance benefit accordingly, and to revitalize herbal medicine insurance system through the change of various formulations. In addition, it is recommended to improve the effectiveness of herbal medicine by making plans to enhance the efficacy of herbal medicine and by enabling small pharmaceutical companies to outgrow themselves.

The Critique of Hallyu, or K-Entertainment as a Gendered Meta-narrative -Focusing on Female Fans, Girl Groups, and Young Women (젠더화된 메타서사로서 한류, 혹은 K-엔터테인먼트 비판 -여성 팬, 걸 그룹, 그리고 여성 청년을 중심으로)

  • The present study examines the transnational "Hallyu" (the Korean wave) phenomenon after the 1990s in the context of a solidarity movement of East Asian women. It also focuses on the passion for the world stage given the cultural industry was supported by the government as a "chimney-less factory" during the IMF financial crisis. Over the past twenty years and through Hallyu 1.0, Hallyu 2.0, and Hallyu 3.0, "K-entertainment" has been advocated, as a concept that encompasses K-drama, K-pop, etc. in the cultural industry. Furthermore, everything Korean, through K-culture, is being put at the forefront. However, there is insufficient discussion regarding the actions of the women who led the Korean wave. This paper examines the female fans and girl groups who played leading roles in the rise of popular culture and its transnational prominence within the context of the female agency and female labor involved. The lack of acknowledgment of their roles is linked to the current erasure of the discussion on the female youth. Discussion on "woman" is still limited to the domain of reproduction in the generational discussion that has replaced the existing nation-state or class led discussions in the current era of neoliberalism. However, since The reboot or the popularity of feminism in recent years, the interest in the female narrative, in works such as 'Kim Ji-young, Born 1982' has been expanding beyond East Asia to the rest of the world. Just as Hallyu was created by women in the beginning, there is a new trend in which women across national borders are joining in solidarity. As such, the present study attempts to prove that the female fan, girl group, and female youths must be one meta-narrative through a feminist reading, rather than individuals with separate identities.

A Study on the Analysis of Sexual Health Education for Korean Adolescents (청소년대상 인쇄매체 성교육자료분석)

  • The Purpose of this study is to offer basic data when the new printed media for sex education are developed, by grasping the problem through analyzing the data of the printed media for sex education that are produced by the government and composed of relatively inclusive contents. The subjects of analyzed data were 10 printed media books connected with sex education. This study focused on analysis for the printed media. therefore the method of analysis used effectively for the study of the mass media was used. The SAS program was used for analyzing the frequency of data and processing with percentage. The result of this study is as follows. 1. The subject of sex education that the printed media intend for was classfied the data for students and for teachers and the general. 2. According to the result of analyzing the writers of the printed media for sex education, classified by occupation, research workers accounted for $30.2\%$ as 23 persons, and the rest were distributed as 19 professors $(25\%)$, 13 teachers $(17.2\%)$, 10 workers in related institutions of various kinds $(13.1\%)$, 6 doctors $(10.6\%)$ and 3 writers $(3.9\%)$. Examing the organization of writers of the printed media for sex education for students, it was composed of sequence as research workers $30.5\%$. workers in related institutions of various kinds $19.6\%$, teachers $19.5\%$, professors $13\%$, doctors $10.8\%$ and writers $6.5\%$. And that of writers of the printed media for sex education for teachers and the general was shown as professors $43.2\%$ (13 persons), research workers $30.1\%$ (9 persons), teachers $13.2\%$ (4 persons) and doctors $10.2\%$ (3 persons). 3. Seeing the result of analyzing for the printed media, classified by themes, it was shown as 314.1 pages $(28.5\%)$ in a field of development of men, 195.9 pages $(17.6\%)$ in a field of sex-health, 141.7 pages $(12.9\%)$ in a field of relationship. 74.1 pages $(6.7\%)$ in a field of a sex act. 51.7 pages $(4.6\%)$ in a field of a personnal technology and 313.4 pages $(28.4\%)$ in the others. As are suit of the analysis classified by themes about the printed media for sex education for students, it was distributed as the content about sexual abuse $12.3\%$ (60.2 pages), the anatomical physiology of reproduction $12.1\%$ (59.3 pages). a friendship $8.5\%$ (41.6 pages). the reproduction $7.2\%$ (35.5 pages). a sexual disease $6.8\%$ (33.2 pages) and the puberty $4.3\%$ (20.9 pages). Examing the result of the analysis about contents classified by themes of the printed media for sex education for teachers and the general. it was indicated as 67.1 pages $(10.9\%)$ on the 'puberty', 37.9 pages $(6.2\%)$ on the anatomical physiology of reproduction, 36.6 pages ($6.0\%$ on a sexual disease, 23.2 pages $(3.8\%)$ on dating, 21.7 pages $(3.5\%)$ on a human sexual reaction. 21.0 pages $(3.4\%)$ on a sexual role. 20.1 pages $(3.3\%)$ of the reproduction. The suggestion according to the upper result is as follows. 1. It is needed to expand the range of analysis into the data. besides the printed media. into the data for sex education using such media as a video. a slide, a CD-ROM, on internet, etc. 2. It is necessary to develop the ideal program for sex education of teenagers and analyze the effect. on the basis of analyzing data about data sex education. 3. It is needed that practice workers for nursing or teachers for nursing as well as workers in educational field or medical experts participate in producing data for sex education.

A Study on Development and Site selection of an AIRFIELD (경비행장 개발 및 입지선정에 관한 연구)

  • As of end of 2014, the population engaging in aviation activities for leisure has reached approximately 13 million, where approximately 356 cases involve a general aircraft, 200 cases involve light aircraft, and 636 cases involve an ULM. The industry for leisure has become a very promising industry in line with rapidly rising living standards which are expected to further increase in the future. The demand for such services is expected to increase over time. The purpose of this paper is to review the development and site selection of airfields in anticipation of these developments in the industry. While the government also has experience in the review of airfield location and candidate sites, it is not the government that carries out the actual construction. As such, the feasibility of the site needs to be verified in terms of actual construction. This study identified factors for Site Selection of factors through a review of related documents and existing research reports. A questionnaire was also used to collect the views of experts in the field, which was then analyzed. The Research model was confirmed in the layered form for an AHP analysis. The factors for Site Selection were identified as the technical / operational factors and economic / political elements for a two-stage configuration. The third step consisted of technical and operational elements. The final step is was constructed a total of 11 elements (weather, surface conditions, obstacle limitation surface, airspace conditions, operating procedures, noise problems, environmental issues, availability of facilities, construction and investment costs, contribution to the local economy, accessibility, demand / the proximity of demand). The surveys are conducted for more than 10 General and light aircraft pilots, professionals, and instructor. The analysis results showed a higher level in the technical / operating elements (73.2%) in the first step, while the next step sawa higher level of the operational elements (30.9%) than the other. The factors for Site Selection were any particular elements did not appear high, the weather conditions (17.5%), noise problems (19.8%), the proximity of demand (6%), accessibility (5.7%), environmental issues (11.1%), availability of facilities (8%), airspace conditions (7.9%), obstacle limitation surface (12%), construction and investment costs (4.2%) and to operating procedures (4.9%), contribution to the local economy (3.8%).

A Multicenter Study to Identify the Respiratory Pathogens Associated with Exacerbation of Chronic Obstructive Pulmonary Disease in Korea

  • Background: Although respiratory tract infection is one of the most important factors triggering acute exacerbation of chronic obstructive pulmonary disease (AE-COPD), limited data are available to suggest an epidemiologic pattern of microbiology in South Korea. Methods: A multicenter observational study was conducted between January 2015 and December 2018 across 28 hospitals in South Korea. Adult patients with moderate-to-severe acute exacerbations of COPD were eligible to participate in the present study. The participants underwent all conventional tests to identify etiology of microbial pathogenesis. The primary outcome was the percentage of different microbiological pathogens causing AE-COPD. A comparative microbiological analysis of the patients with overlapping asthma-COPD (ACO) and pure COPD was performed. Results: We included 1,186 patients with AE-COPD. Patients with pure COPD constituted 87.9% and those with ACO accounted for 12.1%. Nearly half of the patients used an inhaled corticosteroid-containing regimen and one-fifth used systemic corticosteroids. Respiratory pathogens were found in 55.3% of all such patients. Bacteria and viruses were detected in 33% and 33.2%, respectively. Bacterial and viral coinfections were found in 10.9%. The most frequently detected bacteria were Pseudomonas aeruginosa (9.8%), and the most frequently detected virus was influenza A (10.4%). Multiple bacterial infections were more likely to appear in ACO than in pure COPD (8.3% vs. 3.6%, p=0.016). Conclusion: Distinct microbiological patterns were identified in patients with moderate-to-severe AE-COPD in South Korea. These findings may improve evidence-based management of patients with AE-COPD and represent the basis for further studies investigating infectious pathogens in patients with COPD.

The Comparison of Risk-adjusted Mortality Rate between Korea and United States (한국과 미국 의료기관의 중증도 보정 사망률 비교)

  • The purpose of this study was to develop the risk-adjusted mortality model using Korean Hospital Discharge Injury data and US National Hospital Discharge Survey data and to suggest some ways to manage hospital mortality rates through comparison of Korea and United States Hospital Standardized Mortality Ratios(HSMR). This study used data mining techniques, decision tree and logistic regression, for developing Korea and United States risk-adjustment model of in-hospital mortality. By comparing Hospital Standardized Mortality Ratio(HSMR) with standardized variables, analysis shows the concrete differences between the two countries. While Korean Hospital Standardized Mortality Ratio(HSMR) is increasing every year(101.0 in 2006, 101.3 in 2007, 103.3 in 2008), HSMR appeared to be reduced in the United States(102.3 in 2006, 100.7 in 2007, 95.9 in 2008). Korean Hospital Standardized Mortality Ratios(HSMR) by hospital beds were higher than that of the United States. A two-aspect approach to management of hospital mortality rates is suggested; national and hospital levels. The government is to release Hospital Standardized Mortality Ratio(HSMR) of large hospitals and to offer consulting on effective hospital mortality management to small and medium hospitals.

Change of Smoking Behavior by Male White-collar Workers after a Tobacco Price Increase (담배가격 인상에 따른 사무직남성근로자들의 흡연행태 변화)

  • Objectives: The purpose of this study is to examine the effect of the raise of cigarette prices by KRW 2,000 at the beginning of 2015 on the change in smoking behavior among male office workers, and to analyze the correlation of various factors including their work behaviors and socio-economic factors with their smoking rate. Methods: In this research, a follow-up observation panel was constituted with 420 smokers as targets from among male office workers at a bank located in Daegu, South Korea. A cross-analysis and ANOVA analysis were carried out in order to examine whether changes in smoking status, amount of smoking, stop-smoking motivation, and reasons for smoking cessation failure after the passage of time since the cigarette price hike were statistically significant. The level of statistical significance was P < 0.05. Results: After the cigarette price hike, among the 420 smokers who were the target of the panel the rate of smoking cessation declined at the time-point of the survey to 15.5%, 12.4%, 8.5%, and 5.7% after one month, three months, six months, and 12 months, respectively. As a result of a follow-up observation of 65 smokers who stopped smoking immediately after the price hike, the actual non-smoking rate declined to 15.5%, 8.3%, 4.4%, and 3.1% after one month, three months, six months, and 12 months, respectively. One (1) year after the cigarette price hike, the non-smoking rate among the 420 smokers reached as low as 3.1% (13 persons). The most important reason for the failure of the attempts to quit smoking was stress for more than 60% of the smokers who attempted to stop. Conclusions: It seems that a powerful anti-smoking policy by the state targeting the nation's workers is necessary. For companies, mediation for workers' job stress can become a strategy for the success of non-smoking attempts. The government seems to require a practical policy to reduce the smoking rate by actively carrying out social, economic, and scientific research to come up with a reduction method for the cigarette hazard, an effective price hike policy, and other non-price policies.

A Research on Current Farm Management and Marketing Situation of Korean Native Chickens (재래닭의 경영 및 판매 실태에 관한 조사 연구)

  • The purpose of this research was to get basic information for the development of Korean native chicken industry by reviewing the current native chicken farm management and marketing situation of native chicken products(meat and eggs). The research was carried on the basis of the farm field survey covering 210 native chicken feeders out of 9 different local areas, and the results were as follows ; 1. Average raising size of native chicken flocks of sample farms was 1,787 heads and about 50% of those farms raised less than 500 heads chickens for self-sufficiency or on the side. 2. Most farmers made the decision to start on feeding native chickens in small scale with small amount of capital without sound feeding program, and their decision was mainly influenced by recommendation of mass-media( 19.5%) and neighbors (17.2%). 3. The average income per farm earned by raising the native chickens was 13,719 Won, and income per head of chicken was 8,800 Won. 4. About 40% of feeders expressed that the poor marketing management and lack of capital were the bottleneck to native chicken farm management. 5. About 70% of feeders evaluated the prospect of native chicken industry positively and so, about 60% of feeders hoped to expand the raising size in the future. 6. Most farmers directry made a bargain with marketer including middleman and enduser in selling the chicken products because there was not established special marketing system for native chicken products. 7. The sales age of native broiler was about 16~20 weeks and average body weight of broiler was 1.5~2.0 kg. And farm recieved price was not decided on the basis of each body weight or meat quality but only number of heads. 8. The average first egg-laying age of chickens was about 165 days and average annual laying rate was only about 56%. 9. In order to develop the successful Korean native chicken industry, followings are recommended ; 1) Reducing the production costs and increasing the productivity of native chickens should be carried out through technological research and development for sound feeding program of native chickens and sufficient fund supply. 2) Orderly native chicken marketing and pricing system should be established to give good vision about native chickens to farmers and to delight the consumers. 3) The measures for product differentiation including meat quality and nutritional value of native chicken products against other improved chickens should be actively taken by feeders and government.

An Exploration on Food Waste Management of Local Governments (전국 지방자치단체의 음식물쓰레기 관리 분석)

  • This research is to explore food waste management across local governments. In particular, pubic administration on food waste, food waste management (from generation to disposal) and civil complaints in jurisdiction are examined. In doing so, a self-administered questionnaire survey was conducted among civil officers in charge of food waste management, and all the collected responses were statistically analyzed. The main results were as follows: public spending on food waste management was a little larger in metropolises than in provincial cities, and the largest food waste source was identified as households (in housing). While regular collection of food waste by trucks was the most common transport method adopted by local governments, resource recovery for compost/fertilizer production was widely used. Also, most of the respondents agreed that the current approach to food waste handling practices are necessarily replaced with more advanced technology converting waste into energy or fuel. Further, it's found that the civil complaints on food waste management were largely categorized into 3 groups - food waste handling, civil service and food waste retrieval. Therefore, the findings indicate that the development and application of no-food waste or waste-to-resource systems are effective in housing estates where large amount of food waste is generated and eliminated.

Ecological Characteristics of Actinomycetes from Mercury and Chrome Polluted Soil (수은, 6가크롬 오염토양으로부터 분리된 방선균군의 생태학적 특성)

  • Ecological characteristics of microbial populations inhabiting heavy metal polluted soil were investigated. The samples were collected from 293 sites around an factory and industry at Gyeoungsangbuk-do. We measured the contents of seven heavy metal elements (Cd, Cu, As, Hg, Pb, $Cr^{6+}$, CN), seven sites have been seriously contaminated by mercury and chrome. A quantitative evaluation of microbial populations in mercury and chrome contaminated soil was examined by using plate count method. Bacterial numbers in polluted soil samples ranged from $7.4X10^5\;to\;9.3X10^7\;cfu\;g^{-1}$, about $10\sim100$ fold less than the count for the unpolluted soil. Moulds were not detected in chrome polluted soil. The log values of actinomycetes of each contaminated soil samples were log ranged from 6.18 to 7.52. The ratio of actinomycetes was similar to unpolluted soil. The investigation showed actinomycetes to be the major microbial population inhabiting the mercury and chrome polluted soil. Thirty-one isolates among the total isolates were examined for antibacterial activity. These isolates were identified based on a phylogenetic analysis using 16S rRNA gene nucleotide sequences, they were categorized in three major phylogenetic groups, belong to the Streptomyces (6 strains), Saccharopolyspora (3 strains), Nocardiodes (1 strain). On the phylogenetic tree, the clade consisting of five isolates were distantly related to all of the established Streptomycetes genera, indicating the possibility as members of new species.
